Jurnal Kesehatan Reproduksi first printed version was published in 2014 with ISSN 2302-836X. In 2016, we also have an online journal version with ISSN 2621-461X. Currently, we already use the Online Journal System, requiring all authors to submit their papers online. Afterwards, authors, editors and reviewers will be able to monitor the manuscript processing. This journal is published annually every April, August and December.

Abstract

Background: Secretome is defined as a set of bioactive molecules produced by cells in the extracellular space, which include but are not limited to proteins, nucleic acids, proteasomes, exosomes, microRNAs, and membrane vesicles. Secretome has received considerable attention in wound care, both in vitro and in vivo studies have demonstrated the ability of secretome to aid the wound healing process by accelerating angiogenesis, reducing inflammation, and stimulating fibroblast and keratinocyte proliferation.Objective: This study aims to determine the effect of administering secretome on post-laparotomy wound healing in female Wistar rats.Methods: A post-test control group design laboratory experimental study compared the REEDA scale and modified VSS score as indicators of post-laparotomy wound healing process in female Wistar rats. A sample of 30 rats was divided into three treatment groups. Group A was the control group that was not given injection, group B was given phosphate buffer saline (PBS) injection, and group C was given subcutaneous injection of UCMSC secretome. The injection treatment was carried out on the 3rd day post-laparotomy according to the wound healing phase. The development of the wound healing process was documented and observed through REEDA scale assessment on days 6, 9, 12, 15, 18, and 21 of the study and scar tissue was carried out on the 21st day of the study using the modified VSS score.Results: The Kruskal Wallis test showed significant results on the REEDA scale on day 6 (p = <0.01), day 9 (p = 0.002), day 12 (p = <0.01), and day 15 (p = 0.008). Significant Post Hoc tests on days 6, 9, 12, and 15 showed that the UCMSC secretome had the most effect on reducing the REEDA scale. On the VSS score, the Kruskall Wallis test showed significant results (p = 0.005). Post Hoc tests on the modified VSS score showed that the UCMSC secretome had the most significant effect on the modified VSS score.Conclusion: UCMSC secretome has a decreasing effect on the REEDA scale and Modified VSS score on the post-laparotomy wound healing process in female Wistar rats so that it can be considered as a therapeutic option for post-laparotomy wound healing.

Abstract

Latar Belakang: Masa prakonsepsi merupakan periode penting dalam persiapan kehamilan yang sehat. Angka Kematian Ibu (AKI) di Kalimantan Barat tahun 2023 mencapai 165 per 100.000 kelahiran hidup dan Angka Kematian Bayi (AKB) sebanyak 862 kasus. Tingginya angka kematian ini salah satunya disebabkan oleh kurangnya persiapan kehamilan yang optimal. Pengetahuan tentang kesehatan prakonsepsi berperan penting dalam membentuk kesiapan kehamilan pasangan usia subur. Studi pendahuluan di TPMB Hj Nuripah S.ST., Bdn menunjukkan 85% WUS belum mengetahui tentang kesehatan prakonsepsi dan 80% belum melakukan persiapan kehamilan secara optimal.Tujuan: Penelitian ini untuk mengetahui hubungan antara hubungan pengetahuan ibu tentang kesehatan prakonsepsi dengan kesiapan kehamilan di TPMB HJ. Nuripah S.ST., Bdn.Metode: Penelitian ini menggunakan pendekatan Cross sectional dengan populasi adalah wus yang berkunjung ke TPMB bulan januari sampai dengan Maret 2025 rata-rata sebanyak 79 orang.  Jumlah sampel 48 responden. Teknik pengambilan sampel simple random sampling. Instrumen penelitian menggunakan kuesioner.Hasil: Sebagian besar responden memiliki pengetahuan rendah dan kesiapan kehamilan yang belum optimal. Uji Chi-Square menunjukkan adanya hubungan signifikan antara pengetahuan wanita usia subur tentang kesehatan prakonsepsi dengan kesiapan kehamilan (p = 0,005).Kesimpulan: Hasil penelitian ini menyarankan program promosi kesehatan prakonsepsi melalui penyuluhan rutin kepada Wanita Usia Subur, menyediakan media edukasi yang mudah dipahami, meningkatkan kualitas pelayanan konseling prakonsepsi, serta melakukan monitoring dan evaluasi berkala terhadap pengetahuan dan kesiapan kehamilan WUS di wilayah kerjanya.

Abstract

Latar Belakang: Anemia pada calon pengantin (catin) merupakan masalah kesehatan yang perlu mendapat perhatian serius karena dampaknya terhadap kesehatan ibu dan janin saat kehamilan. Data Puskesmas Kalasan menunjukkan prevalensi anemia pada catin mencapai 62% pada periode Januari–Mei 2025. Studi pendahuluan terhadap 10 catin menunjukkan 70% belum memiliki pengetahuan yang memadai tentang anemia.Tujuan: Penelitian ini bertujuan mengetahui hubungan antara tingkat pengetahuan tentang anemia dengan perilaku pencegahan anemia pada catin di Puskesmas Kalasan.Metode: Metode yang digunakan adalah cross sectional dengan total sampling sebanyak 37 responden berusia 17–37 tahun, instrumen berupa kuesioner tervalidasi (Cronbach’s Alpha: 0,884 untuk pengetahuan; 0,846 untuk perilaku), dan analisis bivariat menggunakan Fisher’s Exact Test.Hasil: Hasil penelitian menunjukkan sebagian besar responden berpengetahuan baik (59,5%) dan berperilaku positif (70,3%). Nilai p = 0,025 (p < 0,05) menunjukkan adanya hubungan yang signifikan antara tingkat pengetahuan dengan perilaku pencegahan anemia. Nilai Odds Ratio sebesar 7,238 (95% CI: 1,25–41,91) artinya catin berpengetahuan kurang berisiko 7,238 kali lebih besar untuk berperilaku negatif.Kesimpulan: Disarankan agar puskesmas memperkuat program edukasi prakonsepsi dan optimalisasi distribusi tablet tambah darah.

Abstract

Hyperemesis gravidarum is a severe form of nausea and vomiting during pregnancy that leads to persistent fluid and electrolyte imbalances, with hypokalemia being among the most clinically significant complications. Hypokalemia disrupts normal cardiac membrane excitability and can precipitate lethal arrhythmias, including ventricular tachycardia, ventricular fibrillation, and torsade de pointes — rhythms that carry a substantial risk of sudden cardiac death if not promptly identified and treated. These life-threatening arrhythmias arise from hypokalemia-induced hyperpolarization of the cardiomyocyte resting membrane potential, which paradoxically heightens ventricular automaticity and creates a vulnerable electrophysiological substrate susceptible to malignant dysrhythmias. A 33-year-old multiparous woman at 29 weeks of gestation presented to the emergency unit with a three-day history of generalized weakness, fatigue, persistent nausea, vomiting exceeding five episodes per day, diffuse muscle cramping, and abdominal discomfort. On examination, she was tachycardic with an irregular pulse of 100 beats per minute and tachypneic at 24 breaths per minute. The initial electrocardiogram demonstrated ventricular bigeminy, and brief self-terminating runs of non-sustained ventricular tachycardia were subsequently observed — findings that signaled an evolving arrhythmogenic state with potential for progression to fatal ventricular arrhythmia. Laboratory investigations confirmed severe hypokalemia with a serum potassium level of 1.7 mEq/L. Two-dimensional echocardiography revealed no structural cardiac abnormalities with a preserved left ventricular ejection fraction of 60%. The patient was managed with intravenous potassium chloride replacement, empirical magnesium sulfate supplementation, Bisoprolol, and Methyldopa in the Intensive Care Unit. Serial electrocardiograms documented progression from ventricular bigeminy to trigeminy before complete resolution following serum potassium normalization to 3.4 mEq/L. This case demonstrates that hypokalemia secondary to hyperemesis gravidarum can induce lethal ventricular arrhythmias, and that favorable outcomes are achievable through early recognition and systematic management.

Latar Belakang: Estimasi berat janin yang akurat merupakan komponen penting dalam manajemen persalinan untuk menurunkan morbiditas dan mortalitas perinatal.Tujuan: Membandingkan akurasi rumus Johnson Toshach dengan pemeriksaan ultrasonografi dalam memprediksi berat badan lahir bayi spontan pervaginam.Metode: Penelitian cross-sectional dilakukan terhadap 99 ibu hamil yang melahirkan spontan pervaginam di Rumah Sakit Pupuk Kaltim Bontang periode Februari-April 2025. Data taksiran berat janin menggunakan rumus Johnson Toshach dan ultrasonografi dibandingkan dengan berat lahir aktual menggunakan uji Wilcoxon Signed-Rank Test. Akurasi dinilai dengan Mean Absolute Error (MAE) dan Mean Absolute Percentage Error (MAPE).Hasil: Ultrasonografi menunjukkan tidak ada perbedaan signifikan dengan berat lahir aktual (p=0,553) dengan selisih rata-rata 24,17 gram, sedangkan rumus Johnson Toshach menunjukkan perbedaan signifikan (p=0,000) dengan selisih 134,53 gram. Nilai MAE ultrasonografi (239,8 gram) lebih rendah dibanding Johnson Toshach (243,2 gram). Nilai MAPE keduanya kurang dari 10% (7,94% vs 7,83%).Kesimpulan: Ultrasonografi lebih akurat dibanding rumus Johnson Toshach dalam memprediksi berat lahir bayi, meskipun keduanya tergolong sangat akurat secara klinis.

Abstract

Kehamilan pada pasien dengan penyakit serebrovaskular seperti penyakit moyamoya, merupakan kondisi berisiko tinggi yang memerlukan penatalaksanaan multidisiplin. Pasien wanita usia 36 tahun, G3P2A0, usia kehamilan 22 minggu, datang ke IGD dengan keluhan perut terasa kencang dan keram sejak subuh. Riwayat obstetri anak pertama lahir spontan tahun 2010 dan anak kedua dengan sectio caesarea tahun 2012. Pasien memiliki komorbiditas penyakit moyamoya dan telah menjalani operasi revaskularisasi pada Maret 2025. Pemeriksaan fisik dalam batas normal tanpa defisit neurologis. Pemeriksaan obstetri tinggi fundus sesuai usia kehamilan dan terdapat his. Ultrasonografi menunjukkan kehamilan tunggal intrauterin 22 minggu, janin hidup dengan denyut jantung 120 kali per menit, taksiran berat 500 gram, plasenta posterior, dan cairan ketuban cukup. Mempertimbangkan risiko maternal, dilakukan tindakan sectio caesarea disertai tubektomi bilateral. Bayi lahir dengan berat 500 gram dan meninggal 10 menit pascakelahiran. Kondisi ibu pascaoperasi stabil dan pemulihan berjalan baik. Kehamilan dengan penyakit moyamoya meningkatkan risiko komplikasi neurologis akibat perubahan tekanan darah selama kehamilan dan persalinan. Penelitian sebelumnya menunjukkan bahwa revaskularisasi sebelum atau selama kehamilan dapat menurunkan risiko kejadian serebrovaskular, meskipun tidak sepenuhnya menghilangkan risiko tersebut dan sectio caesarea sering dipilih untuk menghindari lonjakan tekanan intrakranial akibat mengejan. Pada kasus ini, riwayat revaskularisasi, usia maternal lanjut, kontraksi aktif usia 22 minggu, dan potensi risiko perdarahan serebral menjadi dasar pertimbangan terminasi operatif disertai sterilisasi definitif. Pada usia gestasi previabel dengan risiko maternal tinggi, keselamatan ibu menjadi prioritas utama dalam pengambilan keputusan klinis. Penatalaksanaan yang tepat dan pemantauan ketat pascaoperasi berperan penting dalam mencapai luaran maternal yang optimal.

Abstract

Adolescent pregnancy is a global public health concern associated with increased maternal and neonatal morbidity and mortality, particularly among very young adolescents, whose physical, psychological, and social readiness for pregnancy is often inadequate. Postpartum contraception is a critical strategy to prevent rapid repeat pregnancies. A 15-year-old unmarried primigravida subsequently delivered a live infant. Due to her young age and high risk of repeat pregnancy, postpartum contraceptive counseling was provided. After a comprehensive explanation was given to the patient and her parents, informed consent was obtained from the parents, and a postpartum IUD was inserted to prevent subsequent pregnancy. The procedure was performed without complications, and the patient was scheduled for regular follow-up and psychosocial support. Adolescent mothers (aged 10–19 years) face higher risks of preeclampsia, anemia, and maternal mortality, while their infants are at increased risk of low birth weight, preterm birth, and stillbirth. Recurrent pregnancies with short interpregnancy intervals may further increase these risks. In Indonesia, adolescent reproductive health services are regulated under Government Regulation No. 28 of 2024, which implements the Health Law No. 17 of 2023 and includes access to contraception as part of adolescent reproductive health services. Postpartum IUD insertion in an adolescent mother is clinically appropriate, ethically acceptable, and legally permissible with parental consent, adequate clinical documentation and careful legal consideration to ensure patient safety and protect healthcare providers. Nevertheless, attention to the patient’s religious and sociocultural background remains important to ensure that contraceptive counseling is respectful, patient-centered, and culturally sensitive.

Abstract

Preeclampsia remains a leading cause of maternal morbidity and mortality, particularly in low-resource settings where access to early screening tools is limited. Urinary miRNA-1-3p have emerged as promising biomarkers for preeclampsia due to their stability in maternal urine during the first trimester, weeks before the onset of clinical symptoms. It reflects early molecular and pathological changes in preeclampsia. Recent advances in nanotechnology have improved miRNA detection using electrochemical biosensors that are suitable for POC testing. The incorporation of gold nanoparticles, further enhances biosensor performance. This study aims to describe the potential of this novel biosensor for detecting miRNA-1-3p in maternal urine as an early antenatal screening tool for preeclampsia. There was elevated levels of miRNA 1-3p in maternal urine during first trimester in women with PE with p value 0.035, showing its potential as non-invasive biomarkers. This biosensor can detect miRNA 1-3p through probe hybridization and produces a measurable electrochemical signal. The incorporation of gold nanoparticles significantly enhances the biosensor sensitivity by about 80% by increasing electroactive surface area and enabling the detection of very low miRNA concentrations with low LOD by about 0.35 fM. Moreover, the gold nanoparticle enhance the biosensor specifity by reducing non-specific binding. This novel biosensor offers non-invasive, rapid, low cost, and portable tool, making it suitable for POC testing in low resource settings. This novel biosensor has strong potential to become a transformative as an early antenatal screening tool for preeclampsia in low-resource settings, may contribute to better maternal health outcomes.
