Abstract

The novel coronavirus, which is now known as COVID-19, has caused a global pandemic. It is known that the main transmission routes of the novel coronavirus are through droplets, close human-to-human contact, and contact with inanimate surfaces contaminated by the virus. Thus, it is important to find out how long SARS-CoV-2 can remain viable on different surfaces. A study by van Doremalen et al. (2020) found that SARS-CoV-2 can remain viable for up to 72 hours on plastic and stainless steel surfaces, 4 hours on copper, and 24 hours on cardboard. Chin et al. (2020) reported that SARS-CoV-2 could persist on wood and cloth for 2 days, and could last for 4 days on glass and banknotes. These findings indicate that viral contamination of object surfaces is an important and dangerous factor in spread of disease, emphasizing the urgent need for prevention strategies against transmission of infection through contact with inanimate surfaces.

Abstract

Introduction: Endometriosis in bowel is rare condition, about 12% of endometriosis cases. Most of bowel endometriosis rising in the rectosigmoid (90% of bowel endometriosis). Incidence of caecal endometriosis is very low (<5% of bowel endometriosis) and almost never causing acute small bowel obstruction(0.1-0.7%). The aim of this paper is to show that although  bowel obstruction caused by caecal endometriosis is difficult to diagnose as it is rare, and may require laparotomy to make definite diagnosis, but it should be considered in infertile female patient. Case: 37 years old woman infertile woman with intestinal obstruction with pre-operative diagnosis total acute small bowel obstruction caused by right colonic mass, with sepsis as the complication. Before the acute small bowel obstruction she complained of chronic right lower quadrant pain with chronic constipation alternate with chronic diarrhea, symptoms that happened both in bowel endometriosis and colorectal malignancy. She also complained of  chronic pelvic pain and dysmenorrhea. She has been married for 10 years with no child. The patient was never diagnosed with endometriosis and never seek medical attention for the infertility and the chronic pelvic pain. The patient underwent Abdominal CT Scan, with result:  massive small bowel obstruction, and caecal mass that causing acute small bowel obstruction. Diagnosis of Acute small bowel obstruction due to right colonic mass was made and emergency exploratory laparotomy was performed the patient. Methods: During the laparotomy, mass at caecum and ileocaecal that causing massive small bowel obstruction was found and standard right hemicolectomy and temporary mucousfistula ileostomy was performed. Result: The patient recovered well, discharged on 7th day post op. The pathology examination showed  ectopic endometriosis lesions in caecum and ileocaecal valve. The histopathology also confirmed with the immunohistochemistry, in which positive ER, PR, CD 10 and CD7 was found the ileocaecal and caecal mass. In the second operation,  reanastomosis of the ileum was done 3 months after the first operation. The chronic pelvic pain is decreasing dramatically after the first and second operation. Conclusion: Although bowel obstruction caused by caecal endometriosis is an extremely rare cause of intestinal obstruction but it   should be considered as a cause in infertile female patient to reduce morbidity and mortality, to reduce stoma creation and to promote resection completeness.

Abstract

Salmonella infections including Non-typhoidal Salmonella (NTS) and enteric fever are important global public health problem, causing approximately 94 million human cases of gastroenteritis with 150,000 deaths annually around the globe. The aim of this study was to determine risk factors for NTS bacteremia patients compared to Typhoidal patients in Indonesia, area with high incidence of enteric fever. This retrospective descriptive study was conducted in Siloam Hospital in Karawaci, Indonesia, from January 2011 to December 2015. Logistic regression model was used to determine independent predictors of NTS bacteremia including demographic and epidemiologic characteristics, clinical presentations, and laboratory results. Out of 129 positive isolates for Salmonella with complete medical records, 18 (13.9%) were positive for NTS. Patients with NTS bacteremia were more likely to belong in the age group below 5 or above 60 year-old, more frequent to have anemia and abnormal leucocyte count. The susceptibility patterns against antimicrobial of NTS bacteremia and Typhoidal bacteremia were similar. In logistic regression analysis, age below 5 or above 60 year-old, hemoglobin level below 12 g/dL and leucocyte count below 4,000/µL or above 12,000/µL were independent risk factors for NTS bacteremia.

Abstract

Introduction: Pancreatic injury is a rare case, caused by blunt or sharp trauma. Difficulty in making diagnose on pancreatic trauma cases are associated with high mortality, and the treatment can be either operative or conservatively. However, It is still unclear which treatment is more favorable.Case: We present 2 cases of Grade III pancreatic injury with stable hemodynamic who suffered bicycle accident. First case, 12-year-old boy complaining severe pain on the upper left abdomen (VAS 9-10) and get worsening by time, with vomiting. The patient underwent distal Pancreatectomy-Splenectomy. Second case, 8-year-old boy complaining of pain on the upper left abdomen (VAS 6-7) without extension on whole abdominal region with vomiting and fever. The patient was treated conservatively. In both cases, patient was discharged with improvement. However, about 3 months later patients who were treated conservatively developed into a pseudocyst. Conclusion: The selection of management in grade III pancreatic injury can be operative or conservative depending on clinical findings such as hemodynamic condition and the quality of abdominal pain. But the occurrence of pseudocysts pancreas is another surgical challenge.

Abstract

Atopic dermatitis (AD) or atopic eczema is one of the most common forms of pediatric dermatological skin conditions, primarily beginning in childhood with a variable natural course characterized by intense pruritus and eczematous lesions. This condition requires multifactorial interplay and risk factors to explain the pathogenesis. We hereby report a case of a two-month-old male who presented with widespread erythematous lesions, excoriations, and dry skin since one day before admission. The antenatal history showed use of antibiotics during pregnancy, familial history of atopy, and non-exclusive breastfeeding which are all known risk factors. Dermatological examination revealed skin phototype III with diffuse erythematous patches scattered on the face, trunk, upper and lower limbs, and groin followed with excoriations and palmar hyperlinearity. Further laboratory and histopathological examinations were not conducted. The patient was treated with topical corticosteroid and emollients, resulting in a notable improvement without any further flares. A thorough history taking and physical examination must be conducted to establish the diagnosis based on the Hanifin and Rajka criteria and identify risk factors such as genetic factors, impaired skin barrier, and environmental factors and microbial exposure, which may play a protective or harmful role in AD development. Holistic treatments consist of pharmacological and non-pharmacological treatments, with significant emphasis on education to the patient and caregiver(s) to improve quality of life and prevent exacerbations and infectious complications while minimizing potential medication side effects.

Abstract

Introduction: Sleep is one of the basic human needs, where human uses one third of their time only for sleeping. Some contributing factors towards bad sleep quality are stress, depression, surrounding noise, and the use of a bright light. It is known that white lights cause the activation of melanopsin, causing circadian cycle shift, whereas using yellow light does not. The aim of this research was to analyze whether there is an effect of using either white or yellow light towards sleep qualityMethod: This is RCT on 81 subjects. Sleep quality was measured using PSQI questionnaire on 3 measurements with each measurement taken every week. Result of the analyzed data using the General Linear Model – Repeated Measurement (GLM-RM) method Result: There were significant differences on PSQI score between group that used white light during sleep (7.6 ± 2.3) in comparison to the control group (5.2 ± 1.8) (p = 0.02), but result also showed that there were no significant difference between the group that used yellow light (6.1 ± 1.7) during sleep in comparison with the control group (p = 0.14). Conclusion: There is a significant difference between the uses of white lights during sleep towards sleep quality, but there has not been any significant difference between the uses of yellow light during sleep towards sleep quality

Abstract

Intra-Uterine Fetal Demise (IUFD) is defined as death of human conception at age of 20 weeks’ gestation or older or with a minimum 500-g birthweight before complete delivery from the mother and induced termination involved. In 2015, Indonesia has contributed a stillbirth rate of 13 out of 1,000 total births in which 17.1% of the cases were caused by congenital anomalies. Fetal Hydrops as a pathological condition in which there is an accumulation of fluid in fetal soft tissues and serous cavities. With the advancements of sonographic technology, identification of fetal hydrops has become uncomplicated. However, what remains a challenge is to investigate etiology and determine management. In order to plan proper management, the etiology of fetal hydrops must first be determined to predict the prognosis of fetal hydrops. In Indonesia; limited facilities and experts combined with high costs in etiology determination and management have complicated the matter. Furthermore, the strong influence of several Eastern communities’ norms and religious views have further complicated both physicians and patients in decision making. In this report, we present a case of late intra-uterine fetal demise with fetal hydrops, whom was admitted on her 35 weeks age gestation. We performed elective Caesarean Section in order to deliver the stillborn fetus, with no significant post-operative complication. Unfortunately, this condition was actually diagnosed earlier during 20th weeks of gestation, hence advised to continue the pregnancy without further evaluation and information to the mother regarding the hydrops condition.
