Abstract

Background: Absorption of the expenditure budget is one of the performance assessments that is still a challenge in the Regional Public Service Agency (BLUD) of Health Center in Gresik Regency. The purpose of this study is to explore the evaluation of the level of absorption of the budget of the regional public service agency of the Health Center in Gresik Regency.Subjects and Method: This qualitative study was conducted in Health Center in Gresik Regency in 2 to 28 october 2022. The informants in this study were the owner or person in charge of the clinic with a total of 24 (twenty-four) people with triangulation of sources. The qualitative data and information were analyzed using source triangulation.Results: In the Resource Indicator which includes knowledge, attitudes and workload at the BLUD Health Center, it is known that the informant's knowledge regarding budgeting and BLUD absorption is quite sufficient. In addition, with the change in the status of the Health Center to BLUD, the workload of health workers has also increased. All informants have a good attitude and commitment in implementing BLUD Health Center. The unavailability of regulations related to HR management, accounts payable, investment management and cooperation management in the context of financial independence at the BLUD Health Center in Gresik Regency. There has never been any monitoring assistance from the planning process to the absorption of the BLUD budget at the Gresik Regency Health Center by the Gresik Regency Regional Financial Revenue Management Agency (BPPKAD).Conclusion: It is necessary to immediately issue regulations in the form of Decrees and Regent's Regulations related to the management of Human Resources, accounts payable, investment management, and the management of cooperation with the BLUD Health Center in Gresik Regency.Keywords: Budget, BLUD & Public Health CenterCorrespondence: Eni Wahyuni. Abstract

Background: Quality of service is one of the elements that can affect patient satisfaction in the laboratory. Ba'a Rote District hospital, is the only hospital in Rote Ndao District which has a laboratory unit to support efforts to diagnose disease and restore health. Measurement of patient satisfaction can be used as a tool for evaluating the quality of health services. This study aims to determine the effect of the quality of health services on the level of satisfaction of outpatients in the Ba'a Rote Hospital laboratory in 2022.Subjects and Method: A cross-sectional study was conducted at the Rote Ndao Hospital Laboratory, East Nusa Tenggara, Indonesia, from February to September 2022. A total of 85 subjects were selected for this study. The dependent variable is satisfaction. The independent variable is the tangible dimension, the empathy dimension, the reliability dimension, responsiveness and assurance. The data were collected using a questionnaire, and analyzed using a logistic regression test.Results: Reliability dimension (aOR= 5.30; 95% CI= 1.90 to 13.30; p= 0.001), responsiveness dimension (aOR= 6.49; 95% CI=2.37 to 17.77; p<0.001), assurance dimension (aOR= 33.58; 95% CI = 8.70 to 129.57; p< 0.001), empathy dimension (aOR= 12.53; 95% CI=4.37 to 35.94; p<0.001), and tangibles dimension (aOR= 9.25; 95% CI=3.31 to 25.84; p<0.001) significantly influence patient satisfaction.Conclusion: The tangible dimension, the empathy dimension, the reliability dimension, the responsiveness dimension, the assurance dimension influence patient satisfaction. Abstract

Background: Diabetes mellitus is a chronic metabolic disorder or disease with numerous etiologies characterized by high blood glucose levels accompanied by lipid, protein, and carbohydrate metabolism disorders. Type 2 diabetes is the most common type of diabetes with about 90% of all diabetes cases. The International Diabetes Federation (IDF) estimates that the number of diabetics in the world can reach 783.7 million people while in Indonesia it is estimated to reach 28.57 million people in 2045. This number is an increase of 46% compared to 536.6 million in 2021. The study aims to discover how much influence internet-based self-management has on blood glucose control in type-2 diabetes mellitus patients.Subjects and Method: It was a meta-analysis study using PRISMA flowchart guidelines with the PICO formulation. P = type 2 diabetes mellitus patients. I = Internet-based self-management. C= No internet-based self-management. O= HbA1C. This study was conducted by searching for articles obtained from the MEDLINE/PubMed, Google Scholar, ProQuest, Science Direct, and Spinger Link databases with keywords and operators “Boolean electronic health record" OR "computerized" OR "healthcare system information" OR "mobile health" OR "mobile app" OR "Telehealth" AND "self-management" AND "Diabetes" OR "diabetes mellitus" OR " type 2 diabetes mellitus" OR "Type 2 diabetes. Based on the database, 10 articles met the inclusion criteria. The analysis was performed using Revman 5.3 software.Results: There were 10 articles from Asia, America, and Europe. A meta-analysis using Randomized Controlled Trials indicated that internet-based self-management significantly reduced HbA1c levels by 0.45 units (SMD= -0.45; CI 95%= -0.57 to -0.33= p<0.001).Conclusion: Internet-based management can reduce HbA1c levels of type 2 diabetes mellitus patients.Keywords: internet-based self-management, hba1c, type 2 diabetes mellitusCorrespondence:Moh Maulana. Abstract

Background: To improve services for members of the Social Security Administrative Body at the Hingk Community Health Center in West Papua Province, it is highly hoped that the leadership of the Community Health Center can improve services for participants of the Health Social Security Administering Body. This study aims to obtain information about the services of members of the Health Social Security Administration at the Hingk Community Health Center, West Papua Province, Indonesia.Subjects and Method: This study uses a qualitative descriptive approach. In this study, the authors used two key informants and main informant to dig for indepth information related to health services in terms of waiting time, health service facilities and payment mechanisms for members of the Social Security Administering Body in the working area of the Hingk Health Center. The instrument used by researchers in collecting data is the researchers themselves. The types of data and data sources that the researchers will collect in this study consist of primary data and secondary data. Data collection techniques were carried out through observation, in-depth interviews and documentation. While the stages of qualitative data analysis were carried out by researchers with three stages of the process, namely data reduction, data categorization, and data interpretation.Results: The results of the study illustrate that the level of service for members of the Social Security Administering Body at the Hingk Community Health Center in West Papua Province, waiting time, and facilities are still relatively low for services for participants of the Health Social Security Administering Body. This was revealed during in-depth interviews that most of the patients of the Health Social Security Administering Body waited a long time to be examined, and patients were dissatisfied with the facilities provided by health workers.Conclusion: The long waiting time for patient examinations would cause complaints from patients and the lack of availability of facilities provided by the Public Health Center would cause patients to feel less satisfied with the services provided.Keywords: waiting time, service facilities, financing mechanismsCorrespondence:Nur Annisa. Abstract

Background Consistent application of self-management is important in efforts to improve the quality of life and health status of hypertensive patients. Electronic health is here to facilitate self-management of hypertensive patients and provide health information to health workers in real time and promote lifestyle modifications. The purpose of this study was to analyze the influence of electronic health-based self-management on blood pressure control in hypertensive patients.Subjects and Method: : This study is a meta-analysis using PICO namely Population: Hypertensive patients; Intervention: Self-management based on electronic health; Comparison: No self-management based on electronic health; Outcome: Systolic and diastolic blood pressure. This study uses articles from several databases, i.e Pubmed, Google Scholar, Science Direct and the Cochrane Library using the following keywords “Hypertension” OR “Hypertensive” OR “Blood Pressure” OR “High Blood Pressure” AND “Self Management” OR “Self Care” OR “Self Monitoring” AND “Electronic Health” OR “eHealth” OR “Telemedicine” OR “Telehealth” OR “mHealth” OR “Mobile Health” OR “Mobile Application” OR “Message Text” OR “Phone Calls”. Articles included in the research are articles that use English with an RCT study design whose publication is within the period of 2012-2022. The steps in the systematic review are carried out based on the PRISMA flow diagram guidelines. Quantitative analysis was carried out using Review Manager (Revman) 5.3.Results: There were 9 articles from South Africa, America, South Korea, Spanish, England, Scotland, and Honduras, with a sample size of 3,071 hypertensive patients concluded that electronic health-based self-management could reduce systolic blood pressure by 0.38 units and was statistically significant (SMD= -0.38; 95% CI = -0.55 to -0.20; p<0.001 ). The meta-analysis synthesized 9 studies with a total sample size = 2.095 hypertensive patients concluded that electronic health-based self-management could reduce diastolic blood pressure by 0.21 units and was statistically significant (SMD= -0.21; 95% CI = -0.29 to -0.12; p< 0.001).Conclusion: Electronic health-based self-management can reduce systolic and diastolic blood pressure control in hypertensive patients.Keywords: Self management, electronic health, blood pressure, hypertensionCorrespondence:Aulia Siti Nur Rahmah. Abstract

Background: The phenomenon of the limitations of the community health center in managing budget expenditures can be seen when the beginning of the year cannot be realized. The limitations on spending on goods and services are also faced by community health centers every year. Changing the status of the community health center to regional public service agency (RPSA) allows the community health center to improve the quality of public health services. This study aimed to explore the evaluation of RPSA at community health center. Subject and Method: This was a qualitative study, with interviews with key informants. Discussions and interviews were conducted at community health center in the working area of Mojokerto district health office, Mojokerto, East Java, Indonesia from August to September, 2022. Participants: interview involved 27 informants, and 1 triangulated informant. The transcribed data were analyzed using content analysis.Results: A total of 27 Community Health Centers were ready to implement the Community Health Center RPSA because they have met the technical and administrative requirements for implementing Community Health Centers as RPSA. Government support is still lacking, seen from the absence of regional regulations after the establishment of the BLUDs that help the flexibility of the community health center. The support from the Mojokerto District Health Office is still considered low because 27 community health center stated that they had never been visited by the Health Office during their monitoring activities at the community health center.Conclusion: A total of 27 Community Health Centers are ready to implement the Community Health Center RPSA. Abstract

Background: The application of stress management at this time really needs a smartphone application to prevent the occurrence of severe mental disorders, reduce stress levels and use this application more effectively. The purpose of this study is to conduct a meta-analysis with the aim of studying and estimating the effectiveness of mobile-based stress management applications on stress and depression in workers.Subjects and Method: This study is a meta-analysis using PICO, Population: Workers, Intervention: Using a mobile-based stress management application, Comparison: Not using a mobile-based stress management application, Output: Stress and Depression. The process of searching for articles between 2012-2022 uses 5 databases: PubMed, Google Scholar, ProQuest, Science Direct and Scopus. The keywords used are “stress” OR “depression” AND “apps” OR “digital health” OR “mobile health” OR “message text” OR “phone calls” OR “website” OR “email” AND “employee” OR “ worker”. Inclusion criteria: article must be a full paper with Randomized Controlled Trial (RCT), article using English, population namely workers, mobile-based stress management application intervention, reported results are stress, depression, include research results number of respondents, average -mean score and standard deviation (SD). Articles that met the requirements were analyzed using the RevMan 5.3 application. Results: There were 14 articles with a Randomized Controlled Trial (RCT) research design originating from Germany, Spain, Switzerland, England, America, Thailand, Taiwan, Korea, Vietnam which were carried out by meta-analysis. The size of the stress sample is 4,865 workers. The meta-analysis shows that workers who use the mobile-based stress management application have 1.08 units lower stress than do not use the mobile-based stress management application, and it is statistically significant (SMD = -1.08; 95% CI = -1.70 to -0.45; p= 0.007). The sample size for depression is 3,983 workers. Workers using the mobile-based stress management application had depression 0.47 units lower than those not using the mobile-based stress management application, and it was statistically significant (SMD = -0.47; 95% CI = -0.85 to -0.10; p = 0.01).Conclusion: Mobile-based stress management application use reduces stress and depresssion in workers.Keywords: stress, depression, stress-based mobile application, worker, employee, meta analysisCorrespondence:Niken Yuliani Untari. Abstract

Background: A pediatric emergency is a clinical situation with life-threatening risks without prompt and adequate management. The Public Safety Center (PSC) is the spearhead of the service to get a quick response and function as a service provider for emergency patients and to coordinate with the intended health facility. The purpose of this study was to analyze the factors that influence the implementation of the use of PSC services using the TAM and UTAUT methods in pediatric patients at RSUD dr. Iskak Tulungagung. Subjects and Method: Observational research was conducted at RSUD Dr. Iskak, Tulungagung, East Java from August to October 2022. A total of 130 children were selected for this study. The dependent variable is the implementation of the public safety center (PSC). The independent variables are the Technology Acceptance Model (TAM) and the Unified Theory of Acceptance and Use of Technology (UTAUT). Data were collected using a questionnaire, and analyzed using path analysis. Results: Perceived benefits (p < 0.001), perceived convenience (p < 0.001) have a significant effect on PSC implementation. Work expectations (p= 0.068), business expectations (p= 0.436), and facilitating conditions (p= 0.106) are related to PSC implementation but not statistically significant.
