Abstract

 Background: Health service quality and patients satifaction are two important determinants for a hospital competitiveness. Patients always expect that the hospital provide good quality and satisfying service. This study aimed to determine the organizational factors that affect medical service quality and patient satisfaction at Dr. Moewardi Hospital, Surakarta.Subjects and Method: This was an analytical observational study using cross sectional design. This study was conducted at Dr. Moewardi Hospital, Surakarta, Indonesia, in November-December 2016. A total of 120 patients at Dr. Moewardi Hospital, were selected by stratified random sampling. The independent variables were personnel welfare, availability of facilities, and regulation of health care. The dependent variables were the quality of inpatient medical service and patient satisfaction. The data were collected by a questionnaire and analyzed by path analysis.Results: Personnel welfare (b=-0.13; p = 0.595) was not associated with the quality of medical service and patient satisfaction. Availability of facilities (b= size was 2.82; p = <0.001) had positive effect on the quality of medical care and patient satisfaction. Path analysis showed goodness of fit of the model, with CMIN = 4.35, p= 0.113, NFI = 0.99, CFI = 0.99, and RMSEA = 0.09.Conclusion: Personnel welfare and availability of facilities have positive effects on the quality of inpatient medical service and patient satisfaction.Keywords: Personnel welfare, availability of facility, quality of inpatient medical service, patient satisfaction, hospital.Correspondence: Riska Rosita. Abstract

Background: The performances of the doctors and the nurses are especially important in accomplishing health care in a continuous and effective way. It is important to examine all organizational, individual, social factors that especially affect the performances of the doctors and nurses holistically. The purpose of this study was to assess factors affecting work performance among nurses in delivering health service for the national health insurance patientsat Dr. Moewardi Hospital, Surakarta.Subjects and Method: A cross-sectional study was carried out in Dr. Moewardi Hospital, Surakarta, Central Java. A sample of 150 nurseswho provided health services to the National Health Insurance patientswas selectedfor this study by proportional random sampling. The dependent variable was work performance. The independent variables were work satisfaction, work motivation, and supervision. The data were collected by questionnaire and analyzed by path analysis run on Stata 13.Results: Work performance was affected by work satisfaction (b=0.88; 95%CI=0.05 to 1.71; p=0.037), work motivation (b=0.81; 95%CI=-0.06 to 1.68; p=0.070), and supervision (b=0.97; 95%CI=0.18 to 1.76; p=0.016). Work performance was also indirectly affected by motivation. Conclusion: Work performance is affected by work satisfaction, work motivation, and supervision. Abstract

Background: Data from the Ministry of Social Services Republic of Indonesia Social Ministry in 2009 showed there were 1,541,942 people with disability in the Indonesian population. Problems often arise regarding access to public-transportation services for people with disability. Despite the existence of UU No 4 in 1997 and UU No 25 in 2009 on accessibility for people with disability, public transport seemed to be poor for them. This study aimed to determine the public transport accessibility for people with disabilities and to provide alternative solutions.Subjects and Methods: This was a qualitative explorative study. Data were obtained from people with physical and mental disability, and provider (Division Head, Surakarta Public Transport Service Bureau). Data were analyzed using triangulation.Results: The local government of Surakarta has provided some transport facilities. However, these transport facilities are in low standard and non-accessible to people with disability. Transportation accessible to people with disability is not of priority. According to the provider this was because of limited budget. Consultation and coordination amongst the providers were on going in order to provide better transportation system accessible to people with disability.Conclusion: Transportation system was not accessible to people with disability. Abstract

Background: Vaccine stimulates immune system form to the infants to prevent the disease and death. However, the coverage of complete basic vaccine (IDL) has not reached the target yet in Kupang regency, Nusa Tenggara Timur. The study aims to analyze the factors that effect a completeness status of the effect basic vaccine for infants aged 0-24 months at the Oemasi regency, Kupang, East Nusa Tenggara.Subject and Method: The study was observational study using cross-sectional approach. The study was conducted at Oemasi community health center, Kupang regency, Nusa Tenggara Timur. The total of the population were 527 infants aged between 0-24 months. The sample were 60 infants by using quota sampling technique. The independent variable were maternal age, education, and working time, family income, and number of family members. The dependent variable was complete basic vaccine status. The data were collected using questionnaire and analyzed by multiple logistic regression.Result: The maternal education level increased the infants’ possibility to get complete basic vaccine and stasticially significant (OR= 11.47; CI 95%= 1.28  up to 102.63; p= 0.029). Parents’ income increased infants’ possibility to get complete basic vaccine, however it was not statistically signicant. Family member ≥5 people and mother’s working time ≥7 hours/day decreased infants’ possibility to get complete basic vaccine, however it was not statistically significant. Mother’s age did not influence the infants getting complete basic vaccine. Conclusion: mother’s education is a factor which has the strongest effect to increase infants’ possibility to get complete basic vaccine. Keyword: complete basic vaccine, mothers’ education level, family income, family members                                                                                             Correspondence: Frans Salesman. Abstract

Background: The quality of hospital care is an important element in the implementation of healthcare service. The quality which is perceived is an assessment and a form of evaluation of healthcare users. Improvement on healthcare service is continually conducted to enhance the quality of service and patients’ satisfaction. The study aimed to explain the influence of patients’ personal factors, doctors, payment methods and types of class toward the quality and satisfaction of inpatient care.Subjects and Methods: This was an analytic observational study with cross-sectional design. This study was conducted at Dr. Moewardi Hospital, Surakarta, from March to April 2017. A total of 144 subjects were selected by stratified random sampling. Exogenous variables in this study were family income, level of education, length of stay, doctor’s salary, the surgeon, type of insurance, and type of class. Endogenous variables were patient’s satisfaction and quality of service. The data were collected by a set of questionnaires and secondary data of doctor’s working period and salary. Data analyzed by path analysis.Results: Patient’s satisfaction were affected by family income (b=-0.08; SE=0.48; p=0.093), level of education (b=-0.44; SE= 0.27; p= 0.102), length of stay (b=0.19; SE=0.99; p=0.059), doctor’s salary (b= 0.02; SE=0.01; p=0.060), doctors working period (b=0.99; SE=0.44; p=0.024), type of insurance (b= 0.72; SE=0.32; p=0.027), type of class (b= 2.11; SE=0.38; p<0.001), and quality of health services (b=0.16; SE=0.51; p=0.002). Quality of health services were affected by family income (b=-0.15; SE=0.07; p=0.039), length of stay (b=0.37; SE=0.15; p=0.017), doctors working period (b= 0.13; SE=0.68; p=0.056), insurance types (b= 1.04; SE=0.50; p= 0.036), and type of class (b=2.24; SE=0.59; p<0.001).Conclusion: Patient’s satisfaction are affected by family income, level of education, length of stay, doctor’s salary, doctors working period, type of insurance, type of class and quality of health services. Abstract

Background: Primary care doctors and other staff working in primary care require comprehensive and accurate data on patients at the point-of-care if they are to provide high quality health services to their patients. Electronic patient records are an effective method of achieving this objective, by dispensing with the need to use difficult to access, and often illegible, paper-based records. This study aimed to analyze the effect of accreditation status on job performance of the electronic medical record clerks at community health center.Subjects and Method: This was an analytic observational study with a cross-sectional design. The study conducted for 25 puskesmas (community health centers), in Yogyakarta, from March to June 2018. A sample of 200 patient medical records was selected by simple random sampling. The dependent variables were completeness and accuracy of medical record. The independent variables were tenure and multi task as level 1 in the multilevel model (MLM). Accreditation status of puskesmas was taken as level 2 in the MLM. The data were collected by questionnaire and analyzed by MLM.Result: Longer tenure (b= 1.03; 95% CI= 0.30 to 1.77; p= 0.006) and single task occupation (b= 4.21; 95% CI= 6.23 to 2.20; p<0.001) were positively associated with completeness of electronic medical record. Likewise, longer tenure (b= 0.54; 95% CI= 0.079 to 1.01; p= 0.022) and single task occupation (b= 2.14; 95% CI= 3.42 to 0.87; p= 0.001) were positively associated with accuracy of medical record medical record. Accreditation status of puskesmas showed negligible contextual effect on completeness (ICC= 0.36%) and accuracy (ICC= 1.17%) of the electronic medical record.Conclusion: Longer tenure and single task occupation are positively associated with completeness and accuracy of medical record. Abstract

Background: Human resources (HR) quality is a requirement of absolute attainment of development in all fields. Nutritional status of being one of the very important factors in the quality of human resources is mainly associated with intelligence, productivity and creativity. To achieve an optimum between flower growing by giving breast milk to infants from birth in the early minutes of life until the age of 6 months exclusive breast milk given without other food. This study was aimed to determine the cause of the low coverage of breast milk exclusively and the factors that affected it.Subject and methods: This was a case study with qualitative design, was carried out in July-August 2016 in Cilacap, Central Java. A total sample of parents with infants aged 7-24 months of age, the mother/mother-in-law and health workers. The data was collected by interviews, observation and documents, and analyzed with data presentation, data reduction, and the withdrawl of the conclusion.Results: Pre-disposing Factors good motivation, attitudes, education and employment of the informant was indeed affect Exclusive breast feeding in the process. Enabling Factors are readily available and easily accessible due to the support of the party giver services though in terms of infrastructure there are still shortcomings in various things. While Reinforcing factors demonstrates the role of health workers and the people closest to either mother or mother-in-law strongly affect the behavior ofthe mother in giving breast feedingexclusively. Restricting factors namely wrong beliefs about the incessant promotion of baby food, infant formula and health problems in the mother and the baby. In addition, the existence of the recording and reporting of less fit so that it affects the actual coverage figures in society.Conclusion: the cause of the low coverage of exclusive breast feeding include knowledge of mother/mother-in-law limited about breast milk exclusively, the role of health workers that is not optimal, the existence of a mistaken beliefs and culture in the community about BREAST MILK exclusively and recording and reporting factors that still need to be addressed so that Exclusive breast feeding data in accordance with the conditions in the community.Keywords: exclusive breast feeding, coverageCorrespondence: Pemi Sunarsih Ruhyana. Abstract

Background: Community health center (puskesmas) management information system or often referred to as SIMPUS is a local health system aimed at providing health information at primary health care level. P Care (Primary Care) application is an information system that is designed to serve BPJS patients. The bridging system links SIMPUS with P care application. These three systems have been used in all puskesmas in Sukoharjo district since 2017. This study aimed to evaluate the implementation of community health center management information system (Simpus), Primary Care (P Care), and bridging data system, in Sukoharjo District, Central Java.  Subjects and Method: This was a qualitative study. The study was conducted in Sukoharjo, Kartasura, Bendosari, Mojolaban community health centers, in Sukoharjo District, Central Java, from October to December, 2017. Four community health centers were selected randomly from 13 community health centers in Sukoharjo. The dependent variables were information service, performance, efficiency, and security. The independent variable was health information system (SIMPUS, P care, and bridging system). The data were collected by in-depth interview, direct observation, and document review. Data credibility was checked by triangulation method.Results: Three health information systems (SIMPUS, P care, and bridging system) have been implemented in Sukoharjo District. However, their performance is yet to be improved. Loading time was lengthy and error often occured. Patients often complained of the lengthy service. Some items in the information system form were left blank. Data entry occasionally was inconsistent with examination outcome. Security system of the health information existed, but sometimes unauthorized individuals can access and use that information. There was a lack of human resources and hardware. The health information system had not met the information need.Conclusion: Three health information systems (SIMPUS, P care, and bridging system) have been implemented in Sukoharjo District. Abstract

Background: Nowadays patients demand quality and safe hospital care. Quality of care depends on the roles of the government as quality of care regulator, hospital management, health care providers, patients, and the community.  The study aimed to examine the effects of patient and physioterapist characteristics on perceived quality of physiotherapy care.Subjects and Method: This was an observational analytic study with cross-sectional design. The study was conducted at Dr. Moewardi Hospital, Surakarta, Central Java, from June to July, 2017. A total sample of 122 physiotherapy patients were selected for this study using exhaustive sampling. The dependent variable was quality of physiotherapy care. The independent variables were patient’s education and income, duration of treatment, insurance status, years of services, physiotherapist training, and salary. The data were collected using a set of questionnaire and analyzed by multiple linier regression.Results: The quality of physiotherapist care was affected by patient’s income (b= -0.18; 95% CI= -0.35 to 0.001; p= 0.052), patients education (b= -3.32; 95% CI= -6.59 to 0.04; p= 0.047), duration of treatment (b= -0.07; 95% CI= -0.14 to -0.01; p= 0.020), insurance status (b= 3.41; 95% CI= 0.15 to 6.68; p= 0.041), years of services (b= 0.55; 95% CI= 0.15 to 0.97; p= 0.010), physiotherapist training (b= 0.90; 95% CI= 0.09 to 1.71; p= 0.030), and salary (b= 0.38; 95% CI= -0.12 to 0.77; p= 0.061).Conclusion: Quality of physiotherapist care is affected by patient’s income, patients education, duration of treatment, insurance status, years of services, physiotherapist training, and salary.Keywords: quality of care, physiotherapy, insurance status, duration of treatment, years of servicesCorrespondence: Afif Ghufroni. Abstract

Background: Circumcision is one of the surgical practices often performed by nurses in rural and urban areas. Minister of Health Decree No. 1239 of 2001 on the registration and practice of nurses has not provided clarity on the limits of authority and legal protection for certain nurses. Many nurses provide services in the community that are not in accordance with the rules and authority.Subjects and Method: This was a normative study with statute, case study, and conceptual approach. Legal materials were collected using document review. Legal materials were analyzed by describing the problem under study.Results: Article 65 of Law Number 36 the Year 2014 on Health Personnel provides delegation of authority from the doctor to the nurse if the doctor cannot perform a medical service, provided that the physician is obliged to provide clear written authority delegation to perform the medical service. Regulation of the Minister of Health of the Republic of Indonesia No. 17 of 2013 on Amendment to Regulation of the Minister of Health No. Hk.02.02/Menkes/148/I/2010 Concerning Permits and Implementation of Nurse Practices states that medical service can only be delivered based on written request from the doctor. Regulation of the Minister of Health of the Republic of Indonesia Number 17 the Year 2013 Concerning Amendment to Regulation of the Minister of Health No. Hk.02.02/Menkes/148/I/2010 Concerning Permission and Implementation of Nurse Practice Article 3a authorize Nurses to undertake nursing practice in health-care facilities outside of the solo practice.Conclusion: Legal construction for nurses who practice circumcision provides legal certainty in the rules relating to solo practice so that there is a law protection. Ministerial Regulation does not have binding power.
