Abstract

Background: The emergence of multidrug-resistant (MDR) Enterobacteriaceae, particularly Escherichia coli and Klebsiella pneumoniae, poses a significant global public health threat. Although these bacteria are common commensals of the human gastrointestinal tract, they may serve as reservoirs of antimicrobial resistance genes that can be transferred to pathogenic strains. Correctional facilities are considered high-risk environments for the transmission of MDR organisms due to overcrowding, limited healthcare access, and close living conditions. This study investigated the fecal carriage of MDR coliforms among persons deprived of liberty (PDLs) in a correctional facility in Negros Occidental, Philippines. Methods: A total of 132 fecal specimens were collected and processed using the modified Landman technique. Isolates were identified through conventional microbiological methods and subjected to antimicrobial susceptibility testing, phenotypic resistance detection, and Multiple Antibiotic Resistance Index (MARI) analysis. Results: Fifty-three coliform isolates were recovered, including 38 E. coli and 15 K. pneumoniae. High rates of resistance were observed to co-amoxiclav and piperacillin/tazobactam, whereas all isolates remained susceptible to carbapenems and amikacin. Four isolates were identified as AmpC producers and three as extended-spectrum β-lactamase (ESBL) producers. The overall MDR prevalence was 7.57%, with E. coli and K. pneumoniae being the predominant MDR organisms. Although the mean MARI was 0.16, ten isolates exhibited MARI values exceeding 0.20, indicating exposure to high-risk antibiotic environments. Conclusion: The presence of MDR coliforms among PDLs highlights correctional facilities as potential reservoirs for antimicrobial resistance. Strengthening antimicrobial stewardship, routine surveillance, infection prevention measures, and policy-driven interventions is essential to limit the emergence and spread of MDR pathogens in correctional settings.

with fatal infections worldwide, including uropathogenic E. coli (UPEC), which causes Urinary Tract Infections (UTIs). UPEC has a specific set of virulence genes, one of which is the type 1 pilus (fimH adhesin), which mediates bacterial adhesion to the bladder epithelium, resulting in bacterial growth and colonization. fimH also has potential as a vaccine target for UTI to reduce the high rate of multidrug resistance. The aim of this research is  (1) to determine the resistance level of E. coli isolates from UTI patients to amoxicillin, ciprofloxacin, and gentamicin antibiotics, and (2) to evaluate the presence of the fimH gene encoding the adhesion factor in these isolates. Methods: This study is an experimental laboratory study. The determination of antibiotic resistance levels to amoxicillin, ciprofloxacin, and gentamicin uses the CLSI’s disc diffusion methods. The presence of the fimH gene (gene ID: 948847) was determined by Polymerase Chain Reaction (PCR). Results: The tested UPEC showed the greatest resistance to amoxicillin (92%), followed by ciprofloxacin (88%) and gentamicin (56%). Additionally, molecular identification showed that 49 of 50 isolates (98%) were positive for fimH. Conclusion: Escherichia coli isolated from patients with urinary tract infections showed the highest resistance to amoxicillin, followed by ciprofloxacin and gentamicin. At the same time, the fimH gene encoding the adhesion factor was also highly prevalent.

Abstract

Background: Dengue fever remains a major public health challenge in tropical and subtropical countries, including Indonesia. Community-based prevention efforts, particularly the implementation of the 3M Plus strategy, are essential for controlling dengue transmission. Knowledge and attitudes are considered important determinants influencing individuals’ engagement in dengue prevention practices. This review aimed to synthesize current evidence regarding the relationship between knowledge, attitudes, and dengue prevention measures. Methods: A literature review was conducted in accordance with the PRISMA 2020 guidelines. Articles were retrieved from PubMed, ScienceDirect, Google Scholar, and OpenAlex databases using relevant keywords related to knowledge, attitudes, and dengue prevention. Eligible studies were full-text articles published between 2020 and 2025 that examined the association between knowledge, attitudes, and dengue preventive behaviors. A total of 25 studies met the inclusion criteria and were analyzed using narrative synthesis. Results: Most studies reported a significant positive association between knowledge, attitudes, and dengue prevention practices. Individuals with higher levels of knowledge and more positive attitudes were more likely to engage in preventive measures, including environmental management, eliminating mosquito breeding sites, and participating in vector control activities. However, several studies indicated that knowledge and attitudes alone were insufficient to ensure consistent preventive behavior. Factors such as social support, resource availability, environmental conditions, and the role of health workers also influenced the adoption of dengue prevention measures. Conclusion: Knowledge and attitudes play a significant role in promoting dengue prevention practices. Effective dengue control programs should combine health education with community empowerment and environmental support to achieve sustainable behavioral change and reduce dengue transmission.

Abstract

Background: Substandard workplace environmental conditions may increase the risk of Acute Respiratory Infections (ARIs) among healthcare workers. Environmental factors such as temperature, humidity, and ventilation can influence the growth and transmission of respiratory pathogens. This study aimed to analyze the association between environmental conditions and ARI incidence among outpatient healthcare workers in 2025. Methods: A cross-sectional study was conducted using environmental monitoring data on temperature, humidity, and ventilation, along with ARI incidence data obtained from medical records between August and September 2025. Total sampling was applied, involving 85 outpatient healthcare workers. Data were analyzed using univariate and bivariate analyses with the chi-square test. Results: Among the participants, 60 workers (70.6%) experienced ARIs. Inappropriate environmental conditions were identified for temperature among 43 workers (50.6%), humidity among 58 workers (69.7%), and ventilation among 66 workers (77.6%). Bivariate analysis showed a significant association between humidity and ARI incidence (p<0.001), with inappropriate humidity increasing the risk of ARIs sixfold (OR=6.00; 95% CI: 2.16–16.64). No significant associations were found between temperature (p=0.082) or ventilation (p=0.814) and ARI incidence. Conclusion: Humidity was significantly associated with ARI incidence among outpatient healthcare workers. Inappropriate humidity may promote the growth and survival of respiratory pathogens, thereby increasing the risk of infection. In contrast, temperature and ventilation were not significantly associated with ARIs, possibly due to limited environmental variation and the use of air-conditioning systems.

Abstract

Background: Leptospirosis is a zoonotic disease caused by bacteria from the genus Leptospira. It has a wide variety of clinical manifestations and could cause high morbidity if not diagnosed correctly. According to the Indonesian Health Profile Report in 2022, there were 1,419 cases of leptospirosis reported, with 139 deaths. Case Presentation: This study reports a case of leptospirosis in a 48-year-old male who presented with the main complaint of seizure. Prior medical history showed that he had a prolonged fever for more than 7 days. He worked as a farmer and lived in flood-prone areas. Physical examination showed icteric sclerae, tenderness in the upper right abdominal region, and pain in both calves. Laboratories showed acute liver injury, with highly elevated liver enzymes (ALT: 1408 U/dL, AST: 595 U/dL), without other severe manifestations, such as acute kidney injury or hemorrhage manifestations. The serology test showed a positive IgM antibody to Leptospira. The patient was treated with cefotaxime intravenously and other supportive therapies. After 10 days, the patient was discharged with improvement in clinical symptoms and laboratory parameters. Conclusion: Given its unusual presentations, diagnosing leptospirosis can be challenging, especially in tropical regions. This report aims to describe unusual findings in leptospirosis and to raise awareness of its possible etiology in a patient with acute liver injury, particularly in a high-risk individual in a tropical region.
