Abstract

Breast cancer is a malignancy which invaded breast tissue in the form of ductal or lobular. One most therapy which is given is neoadjuvant chemotherapy. Neoadjuvant Chemotherapy can reduce tumor size so that surgerycan be performed with good breast removal with Modification of Radical Mastectomy (MRM) and Breast Conservative Therapy (BCT). This purpose from this research is to find out neoadjuvant chemotherapy response in Locally Advanced Breast Cancer Patients which has received chemotherapy treatment in RSUP dr Mohammad Hoesin Palembang. This observational descriptive study was conducted at RSUP Mohammad Hoesin Palembang in the period between October until November 2019. The sample of this study was locally advanced breast Cancer patients who underwent chemotherapy that met the inclusion and exclusion criteria. The data were obtained by interviews and observed medical records from the patients which were then analyzed by univariate analysis using SPSS version 25. In this study there were 34 locally advanced breast cancer patients who fulfilled the inclusion and exclusion criteria. 24 of 34 patients (70,6%) received positive response and 10 of 34 patients (29,4%) received negative response.

Abstract

Introduction. Exam management is crucial in the implementation of evaluation in educational institutions. This is because the test implementation process is one of the determinants of the quality of an institution. This study aims to assess the optimization of examination management in the health analyst department of the Health Polytechnic of the Ministry of Health in Maluku. Methods. This study used a descriptive research design. This research was conducted in the health analyst department of the Maluku Health Polytechnic Ministry of Health in July - August 2019. The sample in this study were all employees and students in the Maluku Health Poltekkes Department of Health analyst with the sampling technique using total sampling. A Likert scale questionnaire was used as an instrument in the study. Results. There are respondents with more job distribution, namely 45 students (80.4%). Respondents who rated the application and website development category as very satisfied were 30 respondents (53.5%), and satisfied were 26 respondents (46.5%). Respondents who assessed the optimal management of the examination category were very satisfied with 38 respondents (67.8%), and satisfied with 18 respondents (32.2%). Conclusion. There is no relationship between the implementation of the SOP for exam management with optimal exam management, and there is a relationship between making applications and websites with optimal exam management.

Abstract

Background: Frozen shoulder is a condition of the shoulder joint that experiences inflammation, pain, adhesions, atrophy and shortening of the joint capsule resulting in limited motion. In frozen shoulder patients, the limited range of motion of the shoulder joint can affect and reduce functional ability. This study aims to analyze the correlation between the limited area of motion of the shoulder joint with the functional ability of frozen shoulder patients at the Medical Rehabilitation Installation Dr. Mohammad Hoesin Palembang. Methods: This study was an observational analytic study, correlation test, with a cross sectional design. There were 29 frozen shoulder patients who met the inclusion criteria in the Medical Rehabilitation Installation Dr. Mohammad Hoesin Palembang in November 2018 was taken as a sample using consecutive sampling techniques. Functional ability was assessed using the quickDASH questionnaire and the area of motion was measured using a goniometer, then analyzed. Results: The results of the correlation test showed significant results between functional abilities and the area of motion of the shoulder joints. Active flexion (p = 0.000; r = -0.669), active extension (p = 0.004; r = -0.520), active abduction (p = 0.000; r = -0.663), active adduction (p = 0.022; r = -0.423 ), passive flexion (p = 0.001; r = -0.589), passive extension (p = 0.002; r = -0.543), passive abduction (p = 0.000; r = -0.676), passive adduction (p = 0.038; r = -0.388). Conclusion: There is a significant correlation between limited joint motion and functional ability in frozen shoulder patients at the Medical Rehabilitation Installation of Dr. Mohammad Hoesin Palembang.

Abstract

Background: Osteoarthritis (OA) is a degenerative disease that most often causes functional disabilities. The complaints that most often cause functional disabilities in people with OA are pain and stiffness. The severity of OA can be seen radiologically. The aim of this study was to assess the correlation of radiological severity of OA (Kellgren and Lawrence) with functional disabilities (WOMAC score). Method: This type of research is an analytic observational study with a cross sectional design. The population who became the respondents of this study were patients suffering from knee osteoarthritis at the Medical Rehabilitation Installation of Dr. Mohammad Hoesin Hospital, Palembang in November 2018 who met the inclusion criteria. Samples were taken by consecutive sampling technique. The functional ability of the patient will be assessed using the WOMAC score and the severity of OA radiologically interpreted based on the Kellgren and Lawrence criteria. Result: Statistical test used Somer's correlation test. From 32 respondents' data, it was found that there was no significant correlation between the radiological severity of OA and functional disability (p = 0.097). Conclusion: There is no significant correlation between radiological severity of OA (Kellgren and Lawrence) with functional disabilities (WOMAC score).

Abstract

Background: Low back pain (NPB) is the first disease that causes disabilities at all ages, and its incidence increases with age. A number of previous studies have reported that hydrotherapy has an effect on improving low back pain and increasing flexibility.Therefore, this study aims to determine the effect of hydrotherapy on pain intensity and lumbosacral flexibility in low back pain patients at dr. Mohammad Hoesin Palembang. Methods: This study is a preexperimental study with a one group pretest-posttest design with a sample size of 35 NPB patients. Inclusion criteria were NPB patients who did not undergo operative action, had no contraindications to undergo hydrotherapy, and were > 29 years old. The exclusion criteria were patients with communication disabilities and a history of low back pain due to infection, tumor and metastases. Subjects dropped out of the study if they underwent hydrotherapy less than three times or withdrew. Pain intensity and lumbosacral flexibility data were obtained before and after the subject underwent hydrotherapy once a week for one month. Results: There was a decrease in pain intensity from5.57 to 4.00 (p <0.001) and an increase in lumbosacral flexibility from 48.10 to 48.73 (p = 0.014) in low back pain patients undergoing hydrotherapy. Conclusion: There is an effect of hydrotherapy on reducing pain intensity and increasing lumbosacral flexibility in low back pain patients undergoing hydrotherapy at the Medical Rehabilitation Installation Dr. Mohammad Hoesin Palembang,

Abstract

Background. Typhoid fever is a common health problem in developing countries. Antibiotics are used to treat typhoid fever which is caused by a bacterial infection. Selection and use of appropriate and rational antibiotic therapy can determine a success in treatment to avoid bacterial resistance and minimize drug side effects. This study aims to determine the rationality of the use of antibiotics in adult patients diagnosed with typhoid fever in the Inpatient Installation of X Hospital in Yogyakarta. Method. Non-experimental research with descriptive observational research design and retrospective data collection. The sample of this study was inpatients with a diagnosis of typhoid fever and was recorded at the X Hospital Medical Records Installation in Yogyakarta for the period January 2016 - December 2017 which was included in the inclusion criteria. Result. Data taken came from 75 medical records that were included in the inclusion criteria. Patients were dominated by female patients as many as 64% and the adult age range was 18-30 years. The single most widely used antibiotic was levofloxacin in 27 cases (36%). The use of antibiotics with the right indication was 75 patients (100%), the right type was 75 patients (100%), the exact duration of administration was 64 patients (85.33%), the right dose was 73 patients (97.33%), the right interval was 73 patients (97.33%) and the right route of administration were 75 patients (100%). Conclusion. The rationality of using antibiotics is good with accuracy> 75%.

Abstract

Background: Knee pain is a major source of disability, and the most frequently complained of disease in people aged 50 years and over, this complaint is often associated with osteoarthritis. Complaints of knee pain in OA patients are strongly associated with decreased stability of the knee joint. As the main stabilizer in the knee joint, extensor muscle strength plays a major role in the pain complaints that occur, so that one of the recommended physiotherapy measures for knee osteoarthritis patients in reducing pain complaints is to exercise the knee extensor muscle strengthening in the form of pedaling a bicycle. The pedaling movement is a work activity carried out by pedicab drivers. Therefore, this study was conducted to determine the incidence of knee pain complaints in pedicab riders in the District of Kemuning, Palembang. Method: This type of research is descriptive with cross sectional study design. The study population was pedicab drivers who operated in the Kemuning District of Palembang. Primary data were collected by conducting interviews to determine whether there were complaints of pain in the knee and assessed by the KOOS Score and Numeric Rating Scale (NRS), as well as measurements of body weight and height. Results: The results showed that 8 pedicab drivers (17.8%) complained of knee pain and 37 pedicab drivers (82.2%) did not complain of pain in their knees. Conclusion: The incidence of knee pain complaints among pedicab drivers in the Kemuning sub-district of Palembang is lower than that of pedicab drivers who do not complain of pain in their knees.

Abstract

Background The quality of life of the elderly is influenced by several factors such as physical health, psychological health, and good social relationships. Physical health is related to daily living activities that a person does in his daily life including ambulation, eating, dressing, bathing, brushing teeth and making up. At old age, a person will experience a decrease in physical condition which will affect the value of fulfilling one's daily living activities. This study aims to analyze the relationship between the level of independence of daily living activities and the level of quality of life of geriatric patients. Methods: This study was an observational analytic study with a cross sectional design. The samples were geriatric patients in Dr. Mohammad Hoesin Palembang in September-October 2019. The sample of this study was 55 respondents. Results: From 55 Geriatric patients in Dr. Mohammad Hoesin Palembang found 60% male respondents and 40% female respondents, the 60-74 years age group as much as 74.5%, the 75-90 year age group as much as 25.5%, the 50-60 year age group as much as 26%. Chi-square test results showed a significant relationship between the level of Activity Daily Living and the level of quality of life of geriatric patients (p value = 0.000). The results of the odds ratio in geriatric patients state that the probability of improving the quality of life is 27.7 times greater in patients who have an independent daily living activity level than in patients who have a total dependent daily living activity level. In the analysis, it was also found that the level of daily living activity of geriatric patients is a protective factor for the quality of life of geriatric patients. (95% CI = 0.006-0.206). Conclusion: There is a significant relationship between the level of daily living activity and the quality of life of geriatric patients in , dr. Mohammad Hoesin Palembang.

Abstract

Most of the livestock in Indonesia are still conventional or traditional farms, where the quality of the seeds, the use of technology and the skills of farmers are still relatively low, thus affecting the productivity and genetic quality of livestock. Therefore it is necessary to have knowledge about livestock reproduction that discusses fertilization, cleveage and implantation. Fertilization or fertilization (singami) is the fusion of two gametes which can be a nucleus or nucleus cells to form a single cell (zygote) or fusion of the nucleus. the process starts with the preparation of ovum cells and spermatozoa; penetration; core incorporation; and early zygote cleavage. Fertilization phase is the meeting between sperm cells and ovum cells and will produce zygote. Zygote will perform cell division (cleavage). The zygote then undergoes growth and development through stages, namely division, gastrulation, and organogenesis. Implantation or also known as oxidation is the process of implanting the embryo, which is the result of conception, into the uterine wall (endometrium) to further develop.

Abstract

In the world, the incidence of nosocomial infections in hospitals had increased. There had been an increase in the incidence of infections caused by Enterobacteriaceae, one of them is Klebsiella pneumonia, which resistant to carbapenem in the worldwide. The consequences of increased rates of resistance to many drugs pose a high need for the discovery of new types of antibiotic drugs. Rhodomyrtus tomentosa (aiton) hassk has an antibacterial effect that has long been used by Indonesians as a traditional drug. This study aims to find out the effectiveness of karamunting leaf fraction as an antimicrobial in carbapenemase resistant K.pneumonia bacteria and to find out the minimum inhibitoryconcentration (MIC) and minimum bactericidal concentration (MBC) of the active compound of karamunting leaves as antimicrobial in Carbapenemase resistant Klebsiella pneumonia (CRKP) bacterial isolates. This research was an experimental laboratory research in vitro that exploratory analytical. The results of the study obtained the value of MICethyl acetate fraction started at a concentration of 125 μg/ml and n-hexan fraction at a concentration of 4000 μg/ml. MBC value was 125 μg/ml for ethyl acetat fraction and n-hexan fraction of karamunting leaves was 8000 μg/ml. From the bacterial activity test obtained at a concentration 32000 μg/ml of n-hexan fraction, the average diameter of the inhibitory zone 7.80 ± 1.30 mm and ethyl acetate fraction at a concentration of 4000 μg/ml can inhibit the growth of CRKP bacteria with an average of inhibition zone diameter was 9.40 ± 1.67 mm. From the results of the analysis using the Independent T Test and mann whitney test obtained a probability value between all groups with positive control was <0.05. It can be concluded that the ethyl fraction of acetate and n-hexan leaves of karamunting leaves contains active compounds that can interfere with the integrity of CRKP bacterial cell walls or membranes so that the bacteria can die.
