Abstract

In the era of BPJS Kesehatan (national health insurance), business competition among hospitals is intensifying, with hospitals striving to offer unique and excellent services while upholding the principles of healthcare ethics. Hospitals can optimize the existing infrastructure and tools to outperform their competitors. This study aims to determine the effect of the 7P marketing mix strategy (product, price, place, promotion, people, process, and physical evidence) on patient loyalty. This study is a quantitative analytic study with a cross-sectional design conducted at Royal Prima Medan Hospital (RPMH). This study enrolled 398 outpatients who completed valid and reliable questionnaires using an online application. Data were analyzed using binomial logistic regression tests to determine the factors most influencing patient loyalty. The Prima Indonesia University Health Research Ethics Committee has ethically cleared this study. This study found that over 70% of respondents had a good perspective on all aspects of the health decision for early warning, and RPMH successfully activated 73.12% of decisions. Bivariate analysis showed that all aspects of the 7P marketing mix were associated with patient loyalty (p<0.001). The study concluded that the 7P marketing mix strategy has a significant and positive influence on patient loyalty, with the most influential factor being people or human resources (p < 0.001; OR: 33.628). It is highly recommended that hospitals emphasize training in their human resources to nurture patient loyalty to the hospital.

Abstract

Reproductive health is a critical aspect of adolescent development, yet gender disparities persist in access to information and services, particularly in Indonesia. This study explores how Public Health students at Universitas Jambi perceive gender-based inequalities in adolescent reproductive health, aiming to understand how future health professionals interpret and respond to these challenges. A qualitative descriptive approach was employed, involving three focus group discussions with 24 undergraduate students (14 female, 10 male) who had completed relevant coursework and participated in health promotion activities. Thematic analysis revealed three key themes: gendered access and assumptions, socio-cultural and structural barriers, and institutional roles and responsibilities. Participants described how reproductive health education and services disproportionately focus on girls, with boys often excluded due to cultural norms and institutional biases. Socio-cultural taboos, shame, and fear further limited access for both genders, though in distinct ways: boys faced invisibility and judgment, while girls experienced stigma and moral scrutiny. Institutions such as schools, families, and healthcare providers were seen as both barriers and potential agents of change, often reinforcing gendered norms through selective education and biased service provision. These findings underscore the need for more inclusive and gender-equitable reproductive health education that actively engages all adolescents. Addressing these disparities requires rethinking institutional practices and cultural narratives that marginalize male adolescents while overburdening females with sole responsibility for reproductive health. By incorporating gender equity into academic and community-based health initiatives, Public Health students can be better prepared to advocate for and implement inclusive reproductive health programs.

Abstract

The nasal is a fundamental anatomical feature of humans and serves as a benchmark for beauty ideals. Rhinoplasty is a popular procedure worldwide. References to anatomical elements are crucial when discussing the distinct nasal shapes of each race, particularly in the context of Indonesia. Research is scarce on the nasal morphology of the Javanese ethnic group, resulting in a lack of precise quantitative measurements for their specific nasal form. The study conducted was a cross-sectional investigation with a sample of 50 individuals, consisting of 20 males and 30 females of Javanese ethnicity. The analysis was performed to determine the precise quantities of the three variables under investigation, specifically intercanthal distance, alar width, and base of nasal width. The mean figure at the intercanthal was 2.38 cm, alar base 2.92 cm, and base of nasal width 3.46 cm. The T-test results are significant for the base of nasal width variable at a p-value of 0.002. The acquired data were subsequently juxtaposed with data from various races in Indonesia and other regions, serving as a benchmark for rhinoplasty operations specifically for individuals of Javanese descent in Indonesia. The data reveal a significant gender gap in the Javanese base of nasal width. A study reveals that Javanese nasals are shorter and wider than those of other ethnicities, suggesting that rhinoplasty may be recommended over non-surgical methods.

Abstract

Storage procedures are the main factor that influences the quality of platelet concentrate. In vitro, platelet concentrate is stored in a platelet agitator at a temperature of 20ºC-24ºC for 5 days. This study aims to determine the difference in the number of platelets in platelet concentrate with and without the agitation process. This type of research is quasi-experimental using a time series design with an independent t-test. The research was conducted in April 2024. Samples of 16 bags of platelet concentrate with and without the agitation process which were stored on the 0th, 1st, 2nd, 3rd, 4th, and 5th days were checked for platelet counts. In the normality test of the platelet examination data, the p-value was <0.05, so it was stated that the data was not normally distributed so it had to be continued with the Mann-Whitney U test. The research results showed that the p-value on days 0, 1, 2, and 3 was >0.05, indicating no significant difference in the number of platelets between the groups with and without the agitation process. Meanwhile, on days 4 and 5, a p-value <0.05 was found, and there was a difference in the number of platelets with and without the agitation process. It can be concluded that agitation plays an important role in maintaining platelet stability after the third day of storage, indicating that the agitation process is necessary to preserve platelet quality during longer storage periods.

Abstract

Chemotherapy becomes the primary treatment for cancer patients, but it causes various side effects and leads to anxiety. Many efforts have been made to address anxiety, one of which is self-hypnosis performed independently by the patient. This study aims to determine the effectiveness of self-hypnosis in reducing the anxiety levels of cancer patients undergoing chemotherapy. This study applied a quasi-experimental design with a one-group pretest-posttest approach. A total of 40 cancer patients undergoing chemotherapy at Persahabatan Hospital were selected using purposive sampling. The intervention was self-hypnosis, administered for three consecutive days. Anxiety levels were measured using the HARS scale before and after the intervention. Data were analyzed using univariate and bivariate analysis, including normality testing and the dependent t-test. The results show that in the intervention group, there was an average decrease of 10.83 from 31.78 to 20.95. The results of the dependent T-test showed a p-value of 0.015; there is a significant difference in the average anxiety levels before and after the intervention. The implications of this study on healthcare are to enhance the role of nurses in providing independent nursing care. Nurses can better understand the adaptation processes that occur in cancer patients, where patients must undergo a long series of treatments, thus having to adapt from their previous environment, where they could move freely, to a limited one due to declining physical conditions and dependence on healthcare services to improve their health. Relaxation through self-hypnosis can restore patients' motivation to recover and become more optimistic about undergoing treatment.

Abstract

Sports significantly contribute to students' physical and mental development, enhancing fitness and social skills. However, high-intensity training, such as sprinting, often results in Delayed Onset Muscle Soreness (DOMS), which can impair athletic performance and motivation. Massage manipulation therapy is a physiotherapeutic approach aimed at preventing and alleviating DOMS symptoms while increasing the Range of Motion (ROM) and reducing muscle pain in sprinter athletes at SMAN 91 Jakarta. This quasi-experimental study employed a pre-test and post-test design with three groups: ESK (effleurage, shaking, kneading, n=15), ESH (effleurage, shaking, hacking, n=16), and a control group (n=13). Interventions were administered once weekly over four weeks. ROM was measured using a goniometer, and muscle soreness was assessed via the Visual Analog Scale (VAS). Data were analyzed using the Kolmogorov-Smirnov and Shapiro-Wilk tests for normality, the Friedman test, and further analysis using the Kruskal-Wallis test for within-group differences. The ESK group showed a significant reduction in muscle soreness from a mean of 3.48 to 1.91 (p<0.005), while the ESH group exhibited a significant improvement in ROM from a mean of 1.56 to 3.06 (p<0.005). The control group showed no statistically significant changes in either variable. Massage manipulation therapy was effective in reducing muscle pain and enhancing ROM. ESH was more effective for improving ROM, whereas ESK was more effective for reducing pain. These findings emphasize the importance of combining multiple Massage techniques for comprehensive physiotherapy benefits in school-based athletic training.

Abstract

Stunting remains a significant public health issue due to its long-term effects on child development, especially in low- and middle-income countries. This study aims to explore the impact of stunting on multiple developmental domains, including gross motor skills, fine motor skills, speech and language, socialization, and overall child development, using the Developmental Pre-Screening Questionnaire (KPSP) as a developmental assessment tool. We hypothesized that children with stunting would exhibit significantly lower developmental scores across all domains compared to non-stunted children, even after controlling for age and number of siblings. A comparative cross-sectional design was employed, and data were analyzed using independent t-tests, Multivariate Analysis of Covariance (MANCOVA), and Analysis of Covariance (ANCOVA). MANCOVA revealed significant effects of both stunting and age on most developmental domains, with stunting having a powerful impact on fine motor skills (F=6.424, p=0.018, η²=0.211), speech and language skills (F=11.924, p=0.002, η²=0.332), and overall KPSP scores (F=46.874, p<0.001, η²=0.661). ANCOVA confirmed the unique effect of stunting after adjusting for age. These findings underscore the critical need for early nutritional interventions, as stunting not only affects physical growth but also hampers essential developmental milestones. Addressing stunting could significantly improve long-term developmental outcomes and reduce health disparities among children in affected populations.

Abstract

Tuberculosis (TB) infection can cause changes in hematological parameters such as neutrophils, monocytes, and lymphocytes. The neutrophil-to-lymphocyte ratio (NLR) and monocyte-to-lymphocyte ratio (MLR) are used as indicators of inflammation and immune response in infectious diseases, including TB. Diabetes mellitus (DM) as a comorbidity can exacerbate inflammation, potentially affecting NLR and MLR values in TB patients. This study aimed to determine the NLR and MLR values in patients with pulmonary TB and pulmonary TB with DM. This research used a quantitative observational design with a cross-sectional approach, involving 72 respondents from several primary health centers (Puskesmas) in Semarang City, divided into two groups of 36 individuals each. The sampling technique used was purposive sampling. The results showed that the mean NLR value in pulmonary TB patients was 2.20, and in pulmonary TB with DM patients was 2.89, with a significant difference (p=0.002). Meanwhile, the mean MLR value in pulmonary TB patients was 0.34 and in pulmonary TB with DM patients was 0.33, with no significant difference (p=0.752). These findings indicate that NLR is more sensitive than MLR in reflecting the inflammatory status and DM complications in TB patients. NLR can be used as an additional marker in the clinical monitoring of TB patients, especially those with DM comorbidity.

Abstract

Chronic kidney disease is a significant public health problem worldwide. Hemodialysis is the most common form of kidney replacement therapy in the world. Hemodialysis therapy is associated with deterioration in health-related quality of life. However, limited studies focused on health-related quality of life subscales among older persons with hemodialysis in the Indonesian population, especially with long-term follow-up. This study aims to explore the six-month longitudinal changes in health-related quality of life subscales among older persons with hemodialysis. This prospective cohort study evaluated 119 older adults undergoing hemodialysis using purposive sampling between March and September 2024 at PKU Yogyakarta Hospital, Indonesia. Outcomes status was evaluated with health-related quality of life at 1-, 3-, and 6-months. The Indonesian version of SF-36 assessed health-related quality of life. Data were analyzed using general estimating equations. In general, the health-related quality of life among older persons with hemodialysis was decreased. The longitudinal changes of eight subscales significantly deteriorated from the first month to the third month and remained consistently worse during the third to the sixth months (p=.001, respectively). Most deterioration was found in role limitation due to physical problem (β=-94.4; p=.001), general health (β=-70.5; p=.001), and vitality (β=-66.9; p=.001). Social function showed the better domain during the 6 months (β=-36.6; p=.001) than other health-related quality of life subscales. Different aspects of health-related quality of life deteriorate differently among older persons with hemodialysis. Specific attention needs to be paid to high-risk Indonesians among older persons with hemodialysis health-related quality of life, especially those who are older.

Abstract

The E-Kescatin application was developed as an innovation in premarital health services to support stunting prevention efforts through a digital approach. However, the effectiveness of utilization of this application still shows variations between regions and is not optimal at the implementation level. This study aims to explore the contextual factors that influence the effectiveness of the E-Kescatin application utilization in Pringsewu District, Lampung Province. The research used a qualitative approach with an exploratory study design. Informants were selected purposively, consisting of midwives, brides-to-be, Health Office officers, and MCH program managers. Data collection was conducted through in-depth interviews and focus group discussions. Data analysis was done thematically with an interpretative approach. The results identified five main themes that influence the effectiveness of app utilization, namely: (1) accessibility of technology and digital infrastructure; (2) digital competence of health workers; (3) perceived benefits from the user side; (4) policy and regulatory support at the local level; and (5) cross-sector integration in the implementation of premarital services. It is concluded that the effectiveness of E-Kescatin application utilization is influenced by contextual factors that include technical, social, and institutional aspects. Therefore, improving the success of application implementation requires a collaborative and adaptive approach to local conditions. The findings of this study provide a significant contribution to strengthening digital transformation policies in the field of reproductive health.
