Abstract

Background: Thalassemia is a genetic disease that results in abnormal formation of globin chains. Patients with thalassemia led to thyroid dysfunction. Levels of Thyroid Stimulating Hormone (TSH) and Free T4 (FT4) were used to determine the function of the thyroid particularly in a repeated transfusions of thalassemia patients. This study aim to analyze the correlation of ferritin with TSH and FT4 levels due to repeated transfusion in thalassemia patients.Methods: Retrospective study of analytic observational design, a cross sectional study of 33 thalassemia patients in Kariadi Hospital Semarang during the period from September to Oktober 2018. Correlation analysis with Spearman test. Ferritin levels, TSH an FT4 were used Elisa method. The data were analysed with the SPSS program using the Spearman correlation test Results: No Correlation analysis between Ferritin levels with TSH levels (r=-0,087; p=0,630)  and there is weak positive correlation between ferritin levels with FT4 levels (r=0,390; p=0,025)Conclusion: There is no correlation between ferritin levels and TSH levels, but there is correlation between ferritin levels and FT4 levels, but Further research is needed to analyze the factors affecting thyroid dysfunction and required additional screening for other thyroid function.

Abstract

Background: Coronary artery disease (CAD) is a narrowing of blood vessels due to the formation of atherosclerotic plaques which can cause disruption of oxygen supply to the heart. High sensitivity troponin I (hs-TnI) is a diagnostic tool for detecting heart disorders with minimal injury to the heart muscle.Aim: The study aimed to compare and observe the relationship of troponin I (TnI) and hsTnI with /RSUP dr.Wahidin  Sudirohusodo Makassar.Methods: The design of the study was cross-sectional study, using primary data of blood samples from 31 suspected CAD patients who underwent coronary angiography examination at RSUP Dr.Wahidin Sudirohusodo Makassar. Samples were taken from a population of subjects who met the inclusion criteria by using consecutive sampling technique in the period of September 2018. Statistical analysis were done using descriptive statistical calculations, frequency distribution and, Spearman’s correlation test. The test results were considered significant if p<0.05.Results: CAD was more common in men at the age of 50-69 years. Data analysis showed a significant correlation between TnI, hsTnI, and the degree of stenosis (p<0.001). The correlation coefficient of TnI and degree of stenosis was 0.707, hsTnI with degree of stenosis was 0.877, and TnI with hsTnI was 0.804.Conclusion: Higher levels of TnI and hsTnI are related to the severity of stenosis in CAD.

Abstract

Background: Pancreatic cancer is a cancer of the thirteenth sequence of events worldwide and the fourth most common cause of death by some cancers. Amylase levels can increase if there is damage to pancreatic cells. Therapeutic ERCP procedure is expected to reduce amylase levels that are elevated in pancreatic cancer. This study aimed to evaluate the preand post ERCP serum amylase in pancreatic cancer patients.Method: Observational retrospective using data from pancreatic cancer patients in January 2017 - January 2018 at the Laboratory of Clinical Medicine and Mechanical Installation of Dr. Wahidin Sudirohusodo Hospital, Makassar. Data were analysed using SPSS version 22 software for windows.Results: Total of 39 pancreatic cancer subject aging 31–93 years were found, most of them were women. PreERCP amylase levels with a value of 167.5 ± 235.6 as against of 137.5 ± 159.4 in post ERCP. Some subject showed a decreased level and some showed an increased level. Statistical tests did not show significant results (Wilcoxon-signed rank test p> 0.05).Conclusion: Serum amylase levels that have increased therapeutic post ERCP can be information for clinicians to pay more attention to ERCP cannulation techniques in order to avoid post ERCP complications.

Abstract

Background: Acquired hemophilia A (AHA) is a rare acquired bleeding disease, characterized by autoantibody against factor VIII. Bleeding usually in the skin and mucosal. AHA frequently in the elderly, in male and female and is associated with several conditions, such as autoimmune, pregnancy, drug reactions, solid tumor and monoclonal gammopathy. Half of patients are idiopathic. AHA is diagnosed in patients without previous personal or family bleeding history in which prolonged aPTT is not corrected after mixing and accompanied by a decrease in FVIII levels. Specific antibody that plays a role in inhibiting FVIII activity can be seen from the immonotyphing examination. Waldenstrom’s Macroglubulinemia (WM) is a one of disorders that can cause AHA.Case Description: A 56-year-old man came with a weak and pale 10 days of hospitalized. Physical examination shown anemia, gum bleeding, lien palpable in Shuffner-1. Laboratory shown bisitopenia and leukocytosis, prolonged aPTT is not corrected after mixing with decrease FVIII levels. (BMP) support Chronic Lympocytic Leukemia/Lymphoma/ Smoldering Myeloma. SPE M-Spike with Immunotyphing examination of IgM lamda monoclonal. History, physical examination, laboratory, BMP, SPE, and immunotyping shown monoclonal gammophaty which cause Acquired Hemophilia A. Bethesda assay examination recommended if FVIII therapy is not responding. Lymphoplasmasitoid cells infiltration and monoclonal IgM production establishes diagnosis of WM.Conclusion: Spontaneous hemorrhage without prior history with prolonged aPTT is not corrected after mixing and followed by a decrease in FVIII leves can lead to (by the presence inhibitor factors) coagulation disorders.

Abstract

Background: Atherosclerosis is a complex process. It involve many pathological event including endotelial dysfunction, abnormal lipid metabolism, trombosit activation, trombosis, oxidative stres, activation smooth muscle in blood vessel. Recently, it is known that inflamation plays a role in every steps of atherosklerosis formation. Diabetes Mellitus (DM) is the main risk factor of cardiovascular disease. The epidemiology showed that cardiovasculer mortality 2 to 3 times higher compared to population without DM. This study aims to know the difference in lipid profile between type 2 diabetics’ patients with and without hypertension.Method: This is a cross sectional study, which was held in Patology Clinic Department FK-USU / H. Adam Malik Hospital, from Agustus 2017 to November 2017. Target population is type-2 DM. Data were analyzed using SPSS version 17 for windows.Result: It found 72 subject is 36 people as patient DM type 2 with hypertension and 36 people as control. According to statistic analysis found significant difference between total cholesterol, triglyceride, HDL and LDL in patients with DM type 2 and hypertension compared with patients with DM type 2 without hypertension (p < 0,05).Conclusion: In this study we found there are significant positive increased level of total cholesterol, trigliseride, HDL and LDL and hypertension on type-2 DM population.

Abstract

Background: Thyroid Cancer is the most common endocrine cancer. The majority of thyroid cancer (80-85%)  is well differentiated. Surgery is the treatment of choice for Well-Differentiated Thyroid Cancer (DTC). This research aims to evaluate the quality of life of patients with thyroid cancer who have undergone total thyroidectomy compared to the general population and also assess the influencing factors.Methods: This research includes 50 patients suffering from DTC after total thyroidectomy and 50 participants were selected to represent the general population who are then asked to fill out a detailed questionnaire EORTC QLQ 30 Indonesia Version including demographic data and clinical data as the TSH levels, the type of histopathology and interval of time since diagnosis, taken from the patient's medical record. Data were analyzed using independent t-test and linear regression in STATA 14 for windows.Results: It is evident that the quality of life of patients with DTC after total thyroidectomy is significantly lower compared to the general population (P<0.05). In addition, several factors such as age, educational level and income of the family give a strong influence as for gender, marital status, employment status, and the time interval since diagnosis gives a mild influence to the quality of life (P<0.05). However, TSH levels and type of histopathology was not proven to affect the quality of life (P>0.05)Conclusion: There are several factors related to quality of life among patients with DTC such as age, educational level and income of the family, gender, marital status, employment status, and the time interval since diagnosis

Abstract

Background: This study aimed to investigate the role of neutrophil to lymphocyte ratio (NLR) as non-specific and independent laboratory biomarkers when associated with Global Registry of Acute Coronary Events (GRACE) mortality risk score in predicting the morbidity and mortality of patient with acute myocardial infarction (AMI).Methods: A cross-sectional study design among 103 AMI patients who met inclusion criteria were enrolled in this study. NLR was the ratio of absolute count between neutrophil and lymphocyte. In addition, the GRACE score was a valid instrument in evaluating acute coronary syndrome (ACS). Data were analyzed using SPSS software version 17.Results: Correlation test showed a weak positive correlation between NLR value and GRACE risk score (r=0,388; p<0,05). Bivariate comparative analysis showed that there was a significant difference in NLR mean in GRACE score group (F (2,100) =5,215, p=0,007). Using the least significance different, it was found that there was a significant difference of NLR mean between low and high GRACE score group {(p=0,003; 95% CI =-3,556 – (-0,736)}.Conclusion: Our study suggests that there was an increase in NLR value proportional to the higher GRACE stratification risk score and there was a significant difference between NLR values in low and high GRACE groups.

Abstract

Background: N-terminal-proBNP (NT-proBNP) is one of the most important biomarkers in diagnosing heart failure (HF). The excretion of NT-proBNP is affected by the decline of kidney function, thus the interpretation of serum levels of NT-proBNP in chronic kidney disease (CKD) patients to diagnose HF needs to be assessed carefully. Therefore, the purpose of this study is to investigated the difference between NT-proBNP levels in CKD patient with and without heart failure.Method: This observational analytic with cross-sectional study examined primary data of the serum level of NT-proBNP in predialyzed CKD patients with and without HF in outpatient clinic in Dr. Soetomo hospital, Surabaya between August and September 2018. Kidney function in predialyzed patients were classified into five stages using the Cockcroft Gault formula of estimated glomerular filtration rate (eGFR). Patients with eGFR <60mL/min/1.73m2 were included in this study. The differences between NT-proBNP levels in predialyzed CKD patients with and without heart failure were then analyzed using univariate and bivariate analysis.Results: The study population consisted of 50 predialyzed CKD patients which include 25 patients with HF and 25 patients without HF. The NT-proBNP mean in HF patients was [23,082 pg/mL (22,413)] and in patients without HF was [815 pg/mL (651)]. There was significant difference in NT-proBNP level between patients with HF and without HF (p<0,001).Conclusion: This study showed that there was significant increase in NT-proBNP level of predialyzed CKD patients with HF compare to patients without HF.

Abstract

Introduction: Dysfunctional uterine bleeding (DUB) is a major cause of a significant burden of health care for women, their families, and the community as a whole, leading to high morbidity for women who experience it. The purpose of this study is to determine the prevalence, characteristics of patients, and histopathological description in patients with DUB. Method: This study uses a secondary data analysis method in the medical record data of patients with DUB who has been histopathologically examined at the Anatomy Pathology Laboratory, Sanglah General Hospital Denpasar in the period 2014 to 2018.Result: From the period 2014 to 2018, 24 cases of DUB were found, the highest number was found in the pre menopause age (41-50 years) in 15 cases (62.5%), based on the type of estrogen related bleeding hormonal disorders in 17 cases (70.8%), and 7 cases of progesterone related bleeding (29.2%). The most histopathological type in the estrogen related bleeding group was 13 disordered proliferative phases (54.2%). In the group with progesterone related bleeding, the highest histopathological picture was irregular shedding in 5 cases (20.8%).Conclusion: Estrogen related bleeding pattern still dominating the histopatological pattern in the dysfunctional uterine bleeding at Pathology Anatomy Laboratory, Sanglah General Hospital period 2014-2018.

Abstract

Background: Sub-optimal nutritional status in Indonesia is still a continuing problem. There is no significant change in the prevalence of sub-optimal nutritional status. First 1000 days of life is a golden period of brain development. Failure of providing adequate nutrition can lead to the deviation of child development.Objective: To investigate the association between wasting nutritional status and delayed child developmentMethods: A cross sectional study was conducted in this study. Fifty eligible subjects were taken from April 2018 to July 2018 using consecutive sampling methods. Characteristics such as child age, sex, exclusive breastfeeding status, birth weight, birth height, current weight, current height, nutritional status, prematurity, and mother’s working status recorded. Child nutritional status was recorded and assessed with the WHO growth chart. Child developmental status was measured with the pre-screening development questionnaire (KPSP). Score ≤ 8 was assumed as a deviation in child developmental status.Results: Wasting nutritional status is significantly associated with child development (p=0.041, CI -0.013-0.096). Prevalence ratio is 3.5 shows that wasting nutritional status is a risk factor for delayed child development. Domain for child developmental delay was 54,5% has language & speech delay, 36,4% has a gross motor delay, and 9,1% has social & independency delay.Conclusion: There is an association between wasting nutritional status and delayed child development. The most often child developmental delay found is speech delay.
