Abstract

Introduction: Relapse pulmonary tuberculosis (TB) is a TB patient who has previously received TB treatment, and has been declared cured or complete treatment, was re-diagnosed with positive smear TB based on swab examination or culture. In Indonesia there are many cases of repeat treatment. 70% of them are relapse cases. So that research on the characteristics of pulmonary TB patients relapse is important. Method: This research is a cross sectional descriptive study where the source of the data comes from secondary data, namely medical records of patients with pulmonary TB patients relapse in pulmonary outpatient clinics at Sanglah General Hospital period May 2017 until September 2018.Result: The results of this study showed that there were 40 patients proven to have pulmonary TB relapse. Most of the patients have ages of 50 to 59 years (25.0%), male sex (55.0%), last education in high school (57.5%), as many as 27.5% of them work as entrepreneurs and 27.5% do not work , the range of BMI in relapse pulmonary TB patients is 13-31 kg / m2 to 28.04 kg / m2 with an average of 19.54 + 3.03 kg / m2, the most clinical complaints are cough (80%), the most common comorbidities are pneumonia (15%) and based on thorax X-ray results found that the most finding was a picture of pleural effusion (22.5%).Conclusion: Based on demographic characteristics, relapse pulmonary TB patients were dominated by age + 50 years with male sex, highschool education status, dominant in patients who did not work and had a low BMI. Based on clinical symptoms, coughing up phlegm, coughing up bleeding and chest tightness are common symptoms. Based on the accompanying comorbidities, pneumonia and heart failure that most often accompanying disease. The most common X-ray image found in relapse pulmonary TB patients is the presence of pleural effusion.

Abstract

Introduction: Indonesia is the biggest contributor of medical tourists to Malaysia and Singapore in recent years, mostly from Medan. Literature reviews and experts opinion of community medicine revealed that poor doctors’ communication skills in Indonesia lead to decreased patients satisfaction and lead to the number of patients seeking treatment abroad.Aim: This study aimed to analyze the relationship between physician-patient communication on the level of patient satisfaction in Central Public Hospital Haji Adam Malik, Medan.Method: This research uses a combination of descriptive-analytic study with cross sectional design. Statistical analysis was performed by chi-square test. The sample selection of research done by consecutive sampling. Measurement doctor-patient communication and patient satisfaction rate is determined by using modified CAT (Communication Assessment Tool) questionnaire.Result: Ten outpatient clinics of Central Public Hospital Haji Adam Malik (10 patients per outpatient clinic) participated in the field test. The average mean score was 4.086 across all modified CAT items (SD = 0.609, range 2.23-5.00). The average proportion of satisfaction rate was 81.7% (S.D. = 12.19, range 44.62-100%). There was a significant relationship between physician-patient communication with patient satisfaction (p <0.05).Conclusion: Assessment doctor-patient communication in Outpatient Clinics of Central Public Hospital Haji Adam Malik categorized as good. Patient satisfaction rate in Outpatient Clinics of Central Public Hospital Haji Adam Malik categorized as satisfied. Effective physician-patient communication associated with patient satisfaction with the physician.

Abstract

Introduction: Several major subjects contained in the current curriculum in high school that make students must bring a lot of book. This causes the bag used by high school students become heavier, which can cause musculoskeletal disorders in high school. This research aims to find the relationship between the weight of bags used to school with musculoskeletal disorders in high school students.Method: This research is an analytic cross-sectional design of the high school students from October to December 2015. The level of musculoskeletal disorders was obtained using a Nordic Body Map questionnaire. Pearson Correlation test used to find out the relationship between weight bag with musculoskeletal disorders.Result: The result was found that the weight of bags have a relationship with musculoskeletal disorders in high school students. Results of the analysis of 91 samples showed a p-value = 0.02, which means that there is a statistically significant relationship (p<0.05).Conclusion: Weight bag has a relationship with musculoskeletal disorders in high school students, where the increasing weight bag will affect the severity of musculoskeletal disorders that appear on the subject.

Abstract

Background: Higher education changes status from student into college-student. In Indonesia, it is regarded more than just student, which make college student have more higher demands. Academic demands they face become their stressor. And this stressor itself can trigger sleep disturbance, especially in medical students.Objective: This research was aimed to determine the proportion of sleep disturbance among medical students in the first and seventh semester at the Medical Faculty of Udayana University.Methods: A cross-sectional descriptive study was conducted to know the proportion of sleep disturbance among medical students in first and seventh semester at Medical Faculty of Udayana University, as much as 140 college students used as samples and chosen by stratified random sampling method and using Pittsburgh Sleep Quality Index (PSQI) as an instrument for sleep disturbance measurement. Data were analyzed using SPSS version 17 for Windows.Results: The results of the study found that 81 students experience sleep disturbance, and found more in student in the first semester at about 33,5% than in seventh semester student which only approximately 24,2 %. Other results based on ages and sex concluded students with the age of 18 experience sleep disturbance the most and dominantly by female students.Conclusion: Most first semester students experience sleep disorders where the highest rate is at the age of 18 years and female sex

Abstract

Background: Diabetes Mellitus Type II (DM-2) is a type of hyperglycemic state which is related to metabolic disorders caused by mutations in several genes. Faal hemostasis is a function of the body that aims to maintain the blood that has been thinned so that the blood flowing in the veins and could be interfere with blood glucose level. This study aims to describe the faal hemostasis examination in patients with DM-2 in Sanglah Hospital.Methods: A cross-sectional study was conducted among patients with diabetes mellitus type II (DM-2) who were hospitalized in Sanglah hospital at the period started from January 1st to December 31st, 2015. The sample selection were using consecutive sampling with a total of twenty eight samples. Data were analyzed using SPSS version 17 for WindowsResults: The study found that males were predominant in this study (53.6%). In addition, 51-60 years age group were also predominant (39.3%), following by history of hypertension (82.1%), 10-14 seconds of Prothrombin Time (PT) (67.9%), 30-40 seconds of Activated Partial Thromboplastin Time (APTT) (53.5%), and bleeding time (BT) > 1.3 minutes (67.9%).Conclusion: Several faal hemostasis assessments were predominant in patients with DM-2 such as 10-14 seconds of PT, 20-40 seconds of APTT, and bleeding time more than 1.3 minutes.

Abstract

Introduction: One of the problems of excess nutrients that occur in children and received special attention in the world of health is obesity. Obesity itself can be defined as the accumulation of excess fat raises the risk of health problems. The prevalence of obesity in children in the world between 1980 and 2013 increased by 47.1%. Childhood obesity has both immediate effects and long-term health. This study aims to determine the prevalence and characteristics of obesity in children at elementary school.Method: This research is a descriptive cross sectional study, conducted in Saraswati V Denpasar elementary school, November - December 2016, by taking samples of obese children in Saraswati V elementary school Denpasar 2016.Result: From the results, 17.2% children with obese, 18.2% children with overweight, 54.6% children with normal BMI, and 10.0% children with underweight from total students many as 1003 children. While the characteristics of obese children obtained: 62.2% of obese children aged <9 years, 73.4% of obese children of boy, 86.7% of obese children have birth weight ≥2500 gram, 62.9% do breakfast, 98,6% escorted to school, 67.8% had obese father and 34.3% had obese mothers, 99.3% had educated father and 97.2% had educated mother.Conclusion: The high prevalence of obesity, which is 17.2%, is a focus that should be of a better concern than parents, and the government.

Abstract

Latar Belakang: Indonesia memiliki angka beban stroke terbanyak kedua setelah Mongolia yaitu sebanyak 3.382,2/100.000 orang berdasarkan DALYs (disability-adjusted life-year). Berdasarkan profil kesehatan provinsi Bali, tipe stroke yang memiliki tingkat prevalensi yang tinggi adalah stroke iskemik. Strategi praktis dalam mengatasi beban akibat stroke harus difokuskan pada pencegahan dan penanganan berdasarkan faktor risiko. Tujuan penelitian ini adalah untuk mengetahui hubungan antara faktor-faktor risiko dengan tingkat risiko penderita stroke iskemik yang rawat inap di RSUD Klungkung Bali periode Oktober 2017 sampai dengan September 2018.Metode: Penelitian ini merupakan penelitian analitik observasional dengan desain cross sectional. 65 Subyek penelitian yang sesuai dengan kriteria inklusi, diambil dari data rekam medis pasien stroke iskemik yang rawat inap di RSUD Klungkung. Variabel yang diteliti dalam penelitian ini yaitu usia, jenis kelamin, tekanan darah, kadar kolesterol, riwayat merokok, indeks massa tubuh (IMT), aktivitas fisik, riwayat diabetes, riwayat fibrilasi atrium, dan riwayat stroke dalam keluarga.Hasil: Dari total 65 pasien stroke iskemik yang rawat inap di RSUD Klungkung, paling banyak berada pada kelompok usia ≥ 55 tahun – 64 tahun dengan usia rerata 61,45 ± 12,016 tahun. Pasien berdasarkan jenis kelamin didominasi oleh laki-laki dengan perbandingan kejadian stroke iskemik antara laki-laki dan perempuan sebesar 2,4:1. Hasil penilaian tingkat faktor risiko dengan Stroke Risk Scorecard (SRS), 51 orang (78,5%) memiliki risiko tinggi, 9 orang (13,8%) memiliki risiko sedang, dan 5 orang (7,7%) memiliki risiko rendah.Simpulan: Uji statistik chi-square menunjukkan adanya hubungan yang signifikan antara tingkat risiko stroke iskemik dengan indeks massa tubuh, tekanan darah, riwayat merokok, riwayat diabetes, dan kadar kolesterol (p<0,005).Introduction: Indonesia is the second highest country of stroke burden after Mongolia, which are 3,382.2 / 100,000 people based on DALYs (disability-adjusted life-year). Based on the health profile of the province of Bali, the type of stroke that has high prevalence rate is ischemic stroke. Practical strategies in overcoming the burden of stroke must be focused on prevention and treatment based on risk factors. The study aims to determine the relationship between risk factors and the level of risk of ischemic stroke patients hospitalized at RSUD Klungkung Bali.Methods: This study is observational analytic study with a cross sectional design that includes 65 patients taken from the medical records. The variables studied are age, gender, pressure blood, cholesterol levels, smoking history, body mass index (BMI), physical activity, history of diabetes, history of atrial fibrillation, and history of stroke in the family. Data analysis techniques used were univariate analysis and bivariate analysis.Result: The most characteristics based on age group is ≥ 55 years - 64 years with an average age of 61.45 ± 12.016 years. Patients are dominated by men (2,4:1). The assessment of the level of risk factors in each sample using the Stroke Risk Scorecard (SRS), 51 people (78.5%) had a high risk, 9 people (13.8%) had moderate risk, and 5 people (7.7%) has a low risk of ischemic stroke.Conclusion: The chi-square statistical analysis showed a significant relationship between the risk level of ischemic stroke with body mass index, blood pressure, smoking history, history of diabetes, and cholesterol levels (p<0,005).

Abstract

Background: Asphyxia is a serious problem in newborns because it can cause high levels of mortality and mortality. Deaths because of asphyxia are around 38% and are 3rd rank in neonatal deaths. Asphyxia is the most common cause of seizures in newborns with the incidence of 1-3 per 1,000 births. Seizure after asphyxia can occur impaired cognitive and developmental disorders in children. From the relationship, well and proper management of infants with asphyxia can reduce the risk of death and permanent disability due to seizures.Methods: Observational analytic research with cross sectional study design to find out the relationship between asphyxia and seizure in neonates. The sample of this study is 718 neonates from 0 to 28 days with asphyxia in perinatology and NICU of Wangaya General Hospital Denpasar city from July 2017-July 2018.Result: The prevalence of neonates with asphyxia are 214 and neonates with seizure are 8 from 214. From this study found a relationship between asphyxia and occurrence of seizure with p=0,000.Conclusion: There is a significant relationship between asphyxia and the occurrence of seizures in neonates.

Abstract

Background: Low birth weight babies (LBW) are still a problem. The causes of LBW are multifactorial, one of which is exposure to toxic substances (cigarettes). Exposure to cigarette smoke can disrupt fetal development during the womb. Based on studies, active and passive smokers have almost the same level of risk. This study aimed to find out about a smoker's husband as a risk factor for the occurrence of low birth weight babies in neonates at Perinatology Room of Wangaya Hospital, Denpasar.Methods: Analytical observational research with case-control research design. The study sample was all neonates who were treated in the Perinatology Room of Wangaya Hospital from May to August 2018 and fulfilled the inclusion and exclusion criteria. Based on the calculation of the sample size, 72 neonates were obtained. The sample is determined by consecutive sampling. In this study, data were obtained from medical records (documentation) and questionnaires.Result: Samples were divided into two groups, namely the case group and controls where each group consisted of 36 samples. As many as 23 of the 36 neonates who were smokers' husbands suffered from low birth weight babies (63.88%), while 13 others did not suffer from low birth weight babies (36.11%). The results of hypothesis testing using the chi-square test, the value of p = 0.03 (p < 0,05, OR = 2,78, CI = 0,091-0,267).Conclusion: Smokers' husbands were significantly a risk factor for the occurrence of low birth weight babies in neonates who were treated in the Perinatology room of Wangaya Hospital, Denpasar.

Abstract

Background: Stress can cause mental and physiological changes in a person's body, by activating the HPA axis and the autonomic nervous system. As a result, catecholamines and cortisol will be secreted and cause changes to the additional energy reserves by the body to deal with stressors and the emergence of introseptive stimuli. This energy can be depleted, causing a person to no longer have the energy to continue their activities, while introseptive stimuli cause fatigue and decreased motivation.Methods: This analytic observational study was designed in the form of a cross sectional study of questionnaire data to measure stress and fatigue, that is distributed to 300 students of education programs for doctors in 2nd, 4th, and 6th semester of Faculty of Medicine Udayana University in 2018. General Health Questionnaire - 12 (GHQ-12) and the 30 item Fatigue Questionnaire was used to measure stress and fatigue.Result: 262 samples met the inclusion and exclusion criteria, 23.7% of students experiencing stress, 1.1% experienced very high fatigue, 12.2% experienced high fatigue, 56.9% experienced moderate fatigue, and 29.8 % experience low level fatigue. Stress has a significant effect with a significance value is 0.000.Conclusion: Stress has a relationship with fatigue in students of the Faculty of Medicine Universitar Udayana, in Bachelor of Medicine and Professionals Doctor Study Program.
