Abstract

Introduction: Maxillofacial injuries are common cause of trauma in emergency department. The goals in the treatment of maxillofacial injury are to restore premorbid occlusion. This study aimed to evaluate the characteristics and outcome of the maxillofacial injuries patient in Sanglah General Hospital Denpasar Bali.Methods Retrospective study was held in Sanglah General Hospital from January 2012 to November 2018. The samples consisted of 241 patients. Age, gender, type of the fractures and surgical management were evaluated.Results Maxillofacial injuries mostly occurred in male, aged 18-40 years old, the mandible was most frequently involved facial bone.  Internal fixation was the golden standard of the treatment chosen (83.73%) followed by Arch bar (16.27%).Conclusion Road traffic accidents are the main reason for maxillofacial injury in Sanglah General Hospital Denpasar Bali. Precise diagnosis and proper treatment result in optimal occlusion and integrity nervous system, both internal fixation and arch bar were given good outcome.

Abstract

Background: Pharyngitis infection issues by Streptococcus pyogenes bacteria have been increasing worldwide, and antibiotic resistance to the bacterium treatment also continues to escalate. Young papaya leaves (Carica papaya L.) have a bactericidal and bacterostatic effect (include papain, flavonoids, alkaloids (karpain alkaloids), saponins, glycosides, phenols (tacophenol), and tannins) that can be an option for treatment of pharyngitis infections other than drugs. This study aims to determine the effectiveness of ethanol extract of young papaya leaves (Carica papaya L.) in inhibiting the growth of Streptococcus pyogenes bacteria through in vitro testing.Methods: This study was a true-post experimental study using control group design. The sample was divided into 2 groups: positive and negative control groups as well as treatment groups with concentration variants of 25%, 50%, 75%, and 100%. Data were analyzed using SPSS version 17 software for windows.Results: The result obtained from this study showed that papaya leaf extract (Carica papaya L.) was proven to significantly inhibit the growth of Streptococcus pyogenes bacteria at concentrations of 25% to 100% with an average diameter of the inhibition zone of 6.4 mm to 7.4 mm (P<0.05).Conclusion: This evidences that the ethanol extract of young papaya leaves (Carica papaya L.) with various concentrations does exhibit an influence on the growth of Streptococcus pyogenes bacteria.

Abstract

Background: Numerous data records that the number of total hip joint replacement is increasing in many parts of the world which also indicates an increase demand for total hip joint replacement and revision surgery. The purpose of this paper is to review the literature regarding the size of femoral head and its relation to the volumetric wear rate of the polyethylene lining of the hip’s acetabular component.Methods: A systematic literature search performed to identify relevant publications for this review in five databases: University of Melbourne Science, Medline/ Pubmed, Cochrane Library, and Ebsco. The search was completed on 6 October 2012 and again on 24 October 2012. The following keywords and phrases were used: total hip joint replacement, femoral head size, wear, polyethylene and arthroplasty.Results: The six studies are all prospective cohort studies with a follow-up time ranging between three to ten years at the minimum. One study conducted by Hammerberg et al. did not mention the minimum length of their follow up however they include a mean follow up period which is 3.6 years. Two studies did not mention the number of patients they are following up. Every other studies mention how many participants included in their study, but these two studies mention how many hips undergo the total hip joint replacement surgery. The number of patients mentioned in the studies vary from the minimum of 42 patients up until 200 patients. The number of hips in each study ranged from 45 hips to 385 hips.Conclusion: This review concludes that the use of larger femoral head size hastens the wear rate of the polyethylene lining of the acetabulum component.

Abstract

Background: Child abuse and neglect is any form of physical, emotional, sexual abuse, or neglect of children. Child abuse causes various consequences that are harmful to the child and the future of the nation. The purpose of this study was to determine the characteristics of children who experience child abuse at Sanglah Hospital Denpasar.Methods: A cross sectional study design was conducted using secondary data taken from medical records of the Forensic Medicine Department, Sanglah General Hospital Denpasar from January 2015 to December 2017. Data related to age, gender, origin, types of abuse, and perpetrator’s house were analyzed as percentage using SPSS version 17 software. Ethical clearance was carried out prior study conducted. Result: From 2015 to 2017, 173 subjects were recorded as child abuse. The age group of 13-15 years and 16-18 years is the highest age group that experiences child abuse which is 28.3% and 33.5%. Subjects of female sex were 56.0%. The highest distribution of subject origin comes from Denpasar area 20.8% and the highest education level of parents is 45.0% senior high school. Subjects who experienced physical violence and threats were 93.6%, sexual violence was 50.8%, and 1.7% experienced neglect. The perpetrators of child abuse are mostly people who are known to the victim. The perpetrator's or subject's house is the place where most physical, emotional, or sexual violence occurs, which is 72.2%, 69.7%, and 89.7% respectively.Conclutions: Most cases of Child abuse occur in the age group of 13-15 years and 16-18 years. Child abuse is more common in women; most of the perpetrators are people who are recognized. Home is the most common place of physical, emotional, or sexual violence.

Abstract

Introduction: Colorectal cancer is a major cause of morbidity and mortality throughout the world. To identify the presence of colorectal cancer, screening that is recommended can be carried out. Screening is an important process in every disease including colorectal cancer Method: This is crossectional-descriptive research. 100 samples that met inclusion criteria were included in the research. Samples obtained using consecutive sampling technique. The samples were interviewed to obtain the data used to fill out the questionnaireResult: 74% of the student has answered that a history of personal cancer are risk factors of colorectal cancer and also 88% of students have answered family history also a risk factor for colorectal cancer. The students have less knowledge on risk factors such as Inflammatory Bowel Disease (64%) and eating spicy food (60%). 87% of students agree that knowledge on colon and rectal cancer is important to prevent colorectal cancer.Conclusion: Good knowledge of colorectal cancer risk factors among students in medicine faculty, Udayana University.

Abstract

Introduction: Nutrition is the consumption of food, considered about the body’s dietary essentials. Excellent nutrition is a sufficient, well-balanced diet joined with regular physical activity which is a foundation for satisfying health. On the other hand, poor nutrition can lead to low immunity, amplified susceptibility to diseases, impaired physical and mental development, and decreased productivity.Method: This cross-sectional study was conducted on students of the medical faculty in University Udayana. There were 100 students were participated in this study, and they were divided into two groups. Of the total study participants, 50 were male, and 50 were female. They were asked to fill up a self-administered questionnaire. The variables that will be seen in this research is as follows; age, gender, weight, height, nutrition status, eating habit and knowledge.Result: Majority of the respondents had fair to adequate knowledge of nutrition. However, the students have a poor eating habit.Conclusion: Though the overall knowledge and nutrition status among the students were good, few minorities of the students held some misconceptions. On the other hand, the students have poor eating habits.

Abstract

Introduction: a Tension-type headache (TTH) is a headache that is commonly happened in almost two-thirds of the world population, especially in medical students. Psychosocial factors, such as anxiety, is often contributed to the incidence of a tension-type headache.Method: The research was using a cross sectional design study to prove the correlation of anxiety disorders and TTH in medical students. Samples were collected by a consecutive method from medical students on second semester, fourth semester, and sixth semesters of Udayana University study year 2017/2018. TTH was diagnosed based on Konsensus Nasional IV PERDOSSI 2013 and anxiety was scored by Hamilton Rating Scale for Anxiety (HARS). Lambda test was used to prove the correlation between anxiety disorders and TTH.Result: There were 300 subjects with dominant age was between 19-20 years old (60.7%) and mostly woman (56.7%). The proportion of TTH was 85.3% while anxiety disorders were 66.7%. The incidence of TTH was found more commonly happened in a subject with anxiety disorders (94.5%) and statistically significant (p=0.001) with weak correlation (r=0.153).Conclusion: These data show a correlation of anxiety disorders and TTH with weak correlation.

Abstract

Background: Penile injection of paraffin is one of procedure that people think can increase penile size. Nevertheless, many people do not know if paraffin injection causes significant adverse effects such as penile paraffinoma. A surgical procedure by wide excision of skin and subcutaneous tissue infiltrated by paraffin continued with appropriate phalloplasty could be done as definitive therapy with the good aesthetic and functional result. This case report aims to present penile parafinoma or sclerosing lipogranuloma due to subcutaneous penile injection of paraffin, its management and the result after surgical procedure.Case: A 40-year-old man came to the Urology Polyclinic, Mangusada Badung Regional Hospital who presented with penile swelling and pain for 3 days. There was a history of subcutaneous penile injection of paraffin about one-year ago by non-medical personnel. On examination of genital organ, there was swelling on entire penile skin with tenderness and induration. A surgical procedure was performed using a two-step cutaneous plasty technique with scrotal flaps on the shaft of the penis. After two months, the surgical procedure gave an excellent penile aesthetic and functional result.Conclusion: Penile parafinoma is a lipogranulomatous inflammatory reaction due to injection of paraffin substance, which cases are still found today, especially in Indonesia. Reconstructive surgery is the definitive therapy to overcome aesthetic or functional problems.

Abstract

Introduction: Neuroendocrine tumors are malignancies that need special attention because they provide clinical manifestations that are not typical of excess gastrointestinal endocrine production. Case reports of neuroendocrine tumors are needed to be able to recognize the symptoms and determine the appropriate therapy.Case series: The first patient was 49 years old Balinese female, consulted from internist with right lower-quadrant tenderness with fever for three days before. The patient also had diarrhea for one week before. The patient has done an appendectomy, and the pathological result of the appendix specimen was Neuroendocrine Appendix. The second patient was 62 years old Balinese male, complained about intermittent diarrhea for two months. Patient has done laparotomy exploration and colostomy because of a transverse colon tumor. The pathological result of the specimen from transverse colon tumor was mixed adeno-neuroendocrine carcinoma. Both patients have been treating with hormonal therapy (somatostatin analogs) long-acting release (LAR); the second patient also had combined with folfox IV chemotherapy.Conclusion: The choice of therapy with somatostatin analogs in the first case and surgery in the second case gave a good response after treatment. However, in both cases, it does not guarantee the cure of this disease.

Abstract

Background: Phosphodiesterase type-5 (PDE5) inhibitors was the first choice for the oral treatment of erectile dysfunction because of its ability to prevent endothelial dysfunction and restored erectile function properly. However, the inflammatory parameter such as hs-CRP can be used to assess the performance of PDE5 inhibitors as well as clinical questionnaires, IIEF-5, and EHS. So that, this study aims to determine the effect of PDE5 inhibitors administration such as sildenafil citrate 50 mg per day can decrease the value of hs-CRP, as well as increased the IIEF-5 and EHS at Dr. Soetomo General Hospital.Methods: A true clinical experimental studies used pretest and posttest control group design were carried out among 20 patients during 4 weeks. In the next 2 weeks, the crossover was conducted among respondents for the washout period whereas divided into 2 groups, control (K1 and K2) and treatment (P1 and P2). Data were analyzed using Statistical Package for the Social Sciences (SPSS) version 16.0 software for Windows.Results: The average age of group K1 and P2 was 51 ± 12.07 years old whereas 48.60 ± 15.40 years old in group P1 and K2. The research was conducted in Andrology Clinic of Doctor Soetomo Hospital in Surabaya during June to November 2017. The research data were not normally distributed, then analyzed by two-tail T-group tests and Wilcoxon test at significance level α=0.05. The results showed that PDE5 inhibitors did not decrease the hs-CRP significantly (p>0.05), but increased EHS (p<0.05) and increased IIEF-5 (p<0.05) significantly.Conclusion: This study concluded that the administration of sildenafil citrate per day by oral increased the EHS and IIEF-5, but not able to reduced hs-CRP in patients with erectile dysfunction significantly.
