Jurnal Plastik Rekonstruksi is an open access, peer reviewed, international journal focusing on global plastic and reconstructive surgery. JPR publishes on all areas of plastic and reconstructive surgery, including basic science/experimental studies pertinent to the field and also clinical articles on such topics as: breast reconstruction, head and neck surgery, pediatric and craniofacial surgery, hand and microsurgery, wound healing, and cosmetic and aesthetic surgery. Clinical studies, experimental articles, ideas and innovations, and techniques and case reports are all welcome article types.

Abstract

Introduction: In reconstructive surgery, free flaps are a superior method for resurfacing defects. While free flap viability is typically monitored by subjective clinical examination, lactate and glucose levels in free flaps, which can affect tissue metabolism during ischaemia and reperfusion, can help predict viability. This study aims to review previous research and provide a theoretical basis for using artificial intelligence in lactate and glucose measurement as a means of assessing flap viability.Method: The primary databases used to retrieve the key medical literature presented in this study were book references and Google Scholar, PubMed and Science Direct, using search terms related to the topic. Only articles written in English and published less than ten years ago were included.Results: Lactate levels detect perfusion impairment earlier than clinical signs or other biochemical markers while glucose monitoring can indicate underlying metabolic dysregulation or physiological stress, helps early detection of complications. Combining lactate and glucose measurements enhances diagnostic accuracy and allows for timely interventions for flap viability. Studies confirm this dual monitoring is a practical, unbiased, and has the potential to be developed into an artificial intelligence tool to improve patient outcomes.Conclusion: Lactate and glucose measurements in free flap monitoring have distinct benefits. Lactate detects ischaemia and reflects tissue metabolism, while glucose monitors energy metabolism and systemic health. Combining these leading to improved flap survival rates. With accessible tools, this approach improves patient care and outcomes in reconstructive surgery.

Abstract

Introduction: Managing burn injuries is a significant clinical challenge in promoting wound healing and minimizing complications. Advances like amniotic membrane as a biological dressing have been introduced to improve wound healing. This study analyzes the efficacy of amniotic membrane in burn care and its impact on wound healing outcomes.Methods: We conducted a systematic review in Pubmed, Cochrane, and ScienceDirect, using “Amnion”, “Burns”, and “Wound healing” as keywords. The inclusion criteria are studies assessing the application of amniotic membrane on burn wounds. The outcome measures were mean healing time, wound healing rate, incidence of wound infection, dressing renewal frequency, pain score, and LOS.Results: We identified eleven trials (n=971) ranging from the year 1989 to 2023, containing eight RCTs, and three NRCTs. The pooled RR showed statistically significant differences between amniotic membrane group and control group in mean healing time (RR -4.52 [95% CI; -6.93, -2.11]; p=0.0002), wound healing rate (RR 1.60 [95% CI; 1.09, 2.33]; p=0.02), incidence of wound infection (RR 0.48 [95% CI; 0.30, 0.77]; p=0.002), and dressing renewal frequency (RR -1.64 [95% CI; -2.48, -0.79]; p=0.0002).Conclusion: This meta-analysis indicates that amniotic membrane is advantageous as a biological dressing for burn patients.

Abstract

Background: Pressure injury (PI) is chronic wound over bony prominence that should be managed to avoid morbidity and increased costs. Braden scale (BS) has been used worldwide for almost forty years as PI's standard risk assessment tool. This study aimed to identify issues among nurses regarding the implementation of the PI risk assessment tool in a referral hospital in rural Indonesia.Methods: We performed two cycles of participatory action research (PAR) to initiate the BS implementation and to establish follow-ups accordingly in inpatient units. The PAR ensured the active roles of the 12 nurses and midwives who initially implemented the tool. The transcribed focused-group discussion (FGD) recordings were analysed following six steps of thematic analysis.Results: There were three main themes emerged. The themes were the nurses' understanding of the need for PI risk assessment, the increasing workload when implementing PI risk assessment, and the importance of the primary nurse role. The themes reflects the need for nurse practitioners to further implement the tool by seeking confirmation from the primary nurse's group. The ease and trialability of the BS enhanced the nurses' compliance with the new tool. The nurse manager and leader needed to concentrate on perceived innovation attributes to implement the PI risk assessment tool further.Conclusion: The PAR cycles have recruited early adopters and suggested implementing BS to assess PI risk. The role of hospital managers and leaders in ensuring continuity of implementation is crucial.

Abstract

Breast augmentation, a commonly sought cosmetic procedure, has shifted towards autologous fat grafting (AFG) as an alternative to implants, primarily due to long-term complications associated with implants. This review aims to provide a comprehensive summary of the autologous fat grafting (AFG) application in breast augmentation surgery. A systematic search strategy was developed for the MEDLINE database and additional articles were identified by reviewing the references. AFG involves harvesting, processing, and injecting the patient's fat into the breast, providing natural results and fewer complications compared to implants. The procedure's success depends on the surgeon's skill. There are challenges and considerations associated with AFG, including volume limitations, variable retention rates, and potential complications such as infections and cyst formation. Uncertainties in fat retention rates influenced by various factors like fat content, processing methods, injection techniques, and patient characteristics, emphasizing the need for further research and careful patient selection.

Abstract

Synovial sarcoma, although rare, is commonly diagnosed in patients under 20 years of age. This case presents the case of a 21-year-old female with synovial sarcoma affecting the 1st to 3rd metatarsal and tarsal bones. She underwent extensive tumor resection, followed by fibula free flap. The fibula was reconstructed into an L-shape to provide structural support. A bypass was performed because of compromised blood flow to the first digit, but the digit failed. Skin necrosis occurred after tumor dissection, necessitating a second reconstruction with an anterolateral thigh flap (ALT) after six weeks. At the years follow-up, the reconstruction was stable, and radiation therapy further improved the outcomes. The patient’s gait was satisfactory, with special shoes for weight bearing. This case demonstrates the potential of limb-sparing surgery with free fibula flaps for synovial sarcoma resection, which offers promising functional and psychological benefits.

Abstract

Nutrition plays a crucial role in the wound-healing process. Nutrition deficiencies can hinder the wound healing process and prolong the recovery period turning the wound into a chronic wound. One nutrient that is rarely discussed is BCAA (Branched Chain Amino Acid). BCAA are essential amino acids that are believed to accelerate the wound healing process. This study aims to investigate the effects of BCAA on the wound healing process. Studies have shown that BCAAs can increase serum albumin levels by preventing the formation of mRNA-PTB (Polypyrimidine Tract-Binding Protein) in liver cells, which activates and accelerates the wound healing process. Administering BCAA as a single treatment is not optimal; therefore, combined therapy with other essential amino acids is necessary. BCAAs can be administered orally, enterally, parenterally, and topically.

Abstract

Introduction: Van der Woude syndrome (VWS) is a rare genetic disorder characterized by cleft lip and/or palate and congenital lower lip pits. It is an autosomal dominant condition with high penetrance, affecting 0.5-2% of all cleft lip/palate cases. VWS is caused by mutations in the IRF6 gene and is associated with hypodontia and dental anomalies. Lip pits, which can cause aesthetic or functional problems, may intermittently drain saliva. VWS is diagnosed using genetic testing and bioinformatics tools, providing more accurate predictions. Treatment requires multidisciplinary care, including surgical removal of lip pits and cleft correction. This study describes a rare case of VWS family, including management strategies in low resource settings.Case presentation: A 42-year-old man from a non-consanguineous marriage presented with four out of five children affected by VWS, characterized by cleft lip and palate. All four children underwent labioplasty. The family history revealed no congenital defects in the father's and the first wife’s lineage, except for the second wife.Conclusion: Early identification of familial patterns and clinical presentation is essential in Van der Woude Syndrome, as it significantly impacts the management and quality of life of affected individuals, particularly in resource-limited settings.

Abstract

Background:Secondary lymphedema with a predominant composition of fat is not effectively managed with physiological techniques. Instead, it necessitates debulking liposuction that reduces extremity circumference and improves lymph flow. This study aims to discover the effectiveness of improving lymphatic flow through indocyanine green lymphography examination.Methods: This retrospective cohort study involving secondary fat–predominant lower extremity lymphedema patients who underwent debulking liposuction in the Division of Reconstructive Plastic and Aesthetic Surgery, Department of Surgery, CMGH, in December 2022 - August 2024. The variables studied were the subject's lower extremity circumference and degree of leg dermal backflow before and after debulking liposuction.Results: Among 15 subjects, pre- and postoperative debulking liposuction measurements of inferior extremity circumference demonstrated significant reduction at 10 cm and 25 cm above the ankle, the patella, 10 cm and 25 cm above the knee. Postoperatively, 5 subjects with leg dermal backflow stage IV turned to stage II (1), III (4), while 10 subjects with the leg dermal backflow stage V turned to stage IV (9).Discussion: Reduction in the circumference of the inferior extremities after debulking liposuction at almost all measurement points except for the foot and ankle circumference. This related to the anatomy of lymphatic flow. Debulking liposuction showing a significant improvement in lymphatic flow, it clinically proven by the reduction dermal leg backflow degree after procedure.Conclusion: Debulking liposuction in secondary lower extremity lymphedema with a predominant composition of fat effectively reduces extremity circumference and improves lymphatic flow as evidenced by indocyanine green lymphographic examination.

Abstract

Background : Anthropometry is a non-invasive, inexpensive, and objective method for evaluating orofacial morphology. This method has clinical applications in myofunctional assessment and therapy. Photogrammetry is a reliable and accurate method for measuring certain facial dimensions. In Indonesia, to date there has been no research that has conducted a photogrammetric evaluation of lip anthropormetry. Method : This study was conducted at the Faculty of Medicine, Udayana University which began in May to October 2024 with research subjects, namely 90 first-year undergraduate medical students, Faculty of Medicine, Udayana University. Data in the form of anthropometry, race, history of congenital abnormalities, facial trauma, siliconoma, age, gender and smoking were recorded and then analyzed. Result : The results of the analysis of lip anthropometry based on gender showed a significant difference in lower lip height, where in women the average was 3.25 (0.36) and in men 3.01 (0.30), with a p value of 0.001. Likewise, the lower lip height was found to be significantly different with a p value = 0.003. Upper vermilion height in women was 1.51 (0.35) while in men it was 1.36 (0.25), these results were statistically significant (p value = 0.041). However, the comparison of lower vermillion did not show any significant difference between the two groups. Cutaneous lower lip and philtrum width were also significantly different with p values of 0.004 and 0.015, respectively. Conclusion: The results of this study are expected to provide an overview of the analysis of anthropometric measurements of the lips using the photogrammetry method as basic data for measuring ideal lip anatomy in the field of facial reconstruction in Indonesia. Abstract

Background: Plastic surgery spans beyond aesthetics, yet public perception often limits it to cosmetic procedures. This study explores public knowledge, preferences, and perceptions of plastic surgery in Indonesia. Method: A cross-sectional study was conducted between June–July 2024 using a structured, 20-item online questionnaire covering demographics, knowledge, preferences, and perceptions. Data from 488 participants were collected and analyzed using SPSS (version 29, IBM Corp., Armonk, NY, USA). Results : A total of 488 participants were recruited; most recognized aesthetic surgery (92%), burns (85%), facial fractures (89%), and clefts (87%) as part of plastic surgery but were less aware of its role in penile reconstruction (55%) and finger amputation (38%). Most respondents rejected misconceptions about plastic surgery and expressed a high regard for appearance (89%), and 57.4% of all respondents had an aesthetic consultation. Despite this, trust in Indonesian plastic surgeons achieving global standards remained limited, with only 66% expressing trust. Conclusion: Although the Indonesian public generally aware of the basic roles of plastic surgery, significant knowledge gaps exist for specialized procedures. Despite valuing appearance, hesitation to seek services and the lack of trust in local surgeons highlight the need for public education and trust-building initiatives. The Indonesian public showed good understanding of plastic surgery, with gaps in knowledge of its more specialized procedures, and a lack of trust in local plastic surgeons.
