Jurnal Plastik Rekonstruksi is an open access, peer reviewed, international journal focusing on global plastic and reconstructive surgery. JPR publishes on all areas of plastic and reconstructive surgery, including basic science/experimental studies pertinent to the field and also clinical articles on such topics as: breast reconstruction, head and neck surgery, pediatric and craniofacial surgery, hand and microsurgery, wound healing, and cosmetic and aesthetic surgery. Clinical studies, experimental articles, ideas and innovations, and techniques and case reports are all welcome article types.

Abstract

Background: Neoadjuvant chemotherapy (NAC) is a crucial component in the management of tongue-based locally advanced squamous cell carcinoma (LASCC) due to its ability to decrease tumor mass and facilitate free tissue transplantation. The utilization of adjuvant radiation and chemotherapy has been observed to decelerate the progression of post-operative tumor growth. Probability of free flap failure is elevated. Post-operative complications and the long-term functional outcomes of speaking, salivation, and swallowing determine the quality of free flaps.Methods: A retrospective review of tongue LASCC patients who underwent NAC followed by glossectomy and free tissue transfer from 2015 to 2018. Tongue functional outcomes of speaking, swallowing, and salivation were assessed using FACE-Q scale in 3 times follow-up period.Result: This study included 7 patients who underwent tongue reconstruction. Treatment modalities were based on tumor presentation, with 4 patients (57.1%) receiving Taxane, 5-Fluorouracil, and Paclitaxel/Doclitaxel (TPF), and 3 patients (42.1%) receiving Paclitaxel and 5-Fluorouracil (PF). Tongue reconstruction utilized the radial forearm free flap in 5 patients (71.4%) and the anterolateral thigh free flap in 2 patients (28.6%). Intraoperatively, NAC had no impact on the integrity of small and reliable donor vessels. Three patients died from tumor metastases after the second follow-up assessment, while one patient was lost to follow-up. The study found no significant association between chemotherapy dosage and free flap vitality (P = 0.629). FACE-Q assessments revealed moderate to low scores in speaking, eating, and drinking outcomes.Conclusion: Tongue reconstruction after NAC in LASCC patients remains a challenging procedure for surgeons. While the consideration of surgical difficulties due to damage of the donor vessels demands a more structured pre-operative plan. No correlation between the exposure of NAC or AR to free flap complication as well as functional outcome.

Abstract

Background: Free flap is tissue transplantation by including its feeding artery from one body part to another. Intravascular thrombosis is the main obstacle commonly faced in the anastomosed part. Yet, postoperative extremity mobilization has widely become the main question related to flap failure. Only a few studies explained the best time to perform lower extremity mobilization post free flap surgery to maintain adequate flap viability. This study aimed to find the effectiveness of the dangling protocol in reducing lower extremity free flap surgery complications.Methods: A literature search was conducted through PubMed, Cochrane, and Embase in March 2022 using “Free tissue flap” or “free flap” AND “dangling protocol” or “early ambulation” AND “postoperative complication” keywords. Title and abstract screening, full-text reading, and eligibility assessment were performed to find related evidence regarding our clinical question.Results: The two relevant articles are a systematic review and a clinical trial study. Both studies showed 94-100% flap viability after applying early dangling protocol as early as three days post lower leg free flap surgery with level evidence of IIB up to IV without any incidence of flap necrosis.Conclusion: Early dangling protocol can be implemented safely after free flap surgery of the lower extremity without postoperative complications such as total or partial necrosis.

Abstract

Introduction: Facial contour deformity is a very challenging problem for aesthetic and reconstructive surgeons. Assessment of facial deformities is important for developing a subsequent reconstruction plan. Various techniques have been described for the repair of tumor-related defects of facial soft tissues. This study aimed to describe methods of Facial Soft Tissue Reconstruction after Tumor Resection at Soedarso General Hospital, West Kalimantan.Method: A retrospective descriptive study using data such as age, gender, type of tumor, size of the defect, and type of reconstruction done, which were collected from medical records of all patients with malignant facial soft tissue tumors who have underwent tissue reconstruction after tumor resection at Soedarso General Hospital, West Kalimantan, between 2017 and 2022.Result: A total of 48 patients were included. 33 patients underwent rotational flap only, 8 patients underwent rotational flap and skin graft, 1 patient underwent transposition flap, 6 patients underwent skin graft. 39 patients had BCC, 7 patients had SCC, 1 patient had MM, 1 patient had Meibom gland Adenocarcinoma. 25 patients were female, 23 patients were male. 41 patients had age 60 years old and older, 7 patients had age younger than 60 years old. 8 patients with defect size larger than 4x4x3 cm, 40 patients with defect size smaller than 4x4x3 cm.Conclusion: Knowledge of distribution patterns of facial soft tissue tumors will help to correctly choose the right options in facial reconstruction. Unawareness can lead to inadequate treatment with serious consequences for the affected patient.

Abstract

Cyanoacrylate, a compound formed from formaldehyde and alkyl cyanoacetate, serves as a potent adhesive in daily life, cosmetics, medicine, and industry. Variants like methyl 2-cyanoacrylate and ethyl 2-cyanoacrylate are common for daily and industrial use, with butyl 2-cyanoacrylate preferred in surgery as an adhesive for its low toxicity. While typically safe for skin, frequent contact can lead to dermatitis, paronychia, and onycholysis. This case report highlights a superficial burn in a two-year-old child caused by cyanoacrylate adhesive. Such burns are rare but require attention. Treatment involves gently separating the adhesive from the skin using soapy water, followed by the use of acetone, petroleum jelly, 0.9% NaCl solution, or 5% sodium bicarbonate solution, followed by the use of pumice stone or nail file to remove residue. Preventive measures include careful glue placement to limit access, especially by children. Caution is crucial when handling cyanoacrylate adhesive near cotton or wool due to its adhesive properties. In summary, cyanoacrylate offers versatility but requires caution to prevent dermatological issues and burns, especially when catalysed by cotton.

Abstract

Cleft lip and/or palate (CL/P) is a common congenital craniofacial abnormality with significant psychosocial implications. This review synthesizes recent research on the psychological landscape of children and adolescents with CL/P, emphasizing the impact on emotional well-being, social functioning, and quality of life. Psychological distress, including anxiety and lower self-esteem, is prevalent among individuals with CL/P, persisting into adulthood and influencing various aspects of mental health. While some studies suggest resilience or underreporting of psychological difficulties, others highlight enduring challenges in appearance satisfaction and emotional well-being. Factors such as familial support and satisfaction with appearance play crucial roles in positive adjustment. Social functioning and peer acceptance present significant challenges, with many children experiencing isolation and stigma. However, some demonstrate adaptive social behaviours, suggesting protective factors. Quality of life is adversely affected, emphasizing the need for holistic interventions addressing physical and psychosocial aspects. While treatments positively impact perceived quality of life, specific cleft types and comorbidities like obstructive sleep apnea further influence outcomes. Overall, understanding the multifaceted challenges of CL/P is essential for implementing targeted interventions and improving overall well-being. Further research is needed to assess the long-term effectiveness of interventions and identify novel approaches for enhancing mental health in this population.

Abstract

Background: Morphometry is a quantitative approach to finding information about variations in the shape of organisms. The maxilla has a central position and plays a vital role in supporting the viscerocranium. Morphometric variations are beneficial in reconstructive procedures and are an essential reference in plastic surgery. However, this study is currently limited. This study aims to determine variations in the maxillary morphometry of patients at Mohammad Hoesin General Hospital that may become a reference for maxillary reconstruction. Method: This descriptive observational study with a cross-sectional approach aims to identify variations in maxillary morphometry in patients treated at RSUP Mohammad Hoesin Palembang. The study is directed at describing a community or society to create an objective picture or description of a condition. It continues by analyzing and comparing maxillary morphometry based on gender. Results: In this study, the results were obtained from as many as 96 samples (male, n = 60, female, n = 36). Variable of male and female were assessed, namely intercanine width (3.58 ± 0.27cm; 3.44 ± 0.18cm), intermolar width (5.64 ± 0.43cm; 5.47 ± 0.27cm), arch length (3.31 ± 0.38cm; 3.32 ± 0.31cm, and palate height (2.46 ± 0.28cm; 2.35 ± 0.22cm) significant results were obtained that the dimensions of the male maxilla were more significant than those of females. Conclusion: There are variations in maxillary morphometry based on gender. Males have larger inter canine width, intermolar width, and palate height morphometry than females.

Abstract

Introduction : Lower extremity defect poses a significant challenge in reconstructive surgery due to the limited usability of soft tissue. The keystone flap which utilizes fasciocutaneous perforators, has emerged to be a potential technique in fixing soft tissue defects. However, scientific evidence, specifically on its effectivity and complication rate in lower extremity defect is lacking. Therefore, this systematic review aims to assess the outcome and potential complications correlated to the use of Keystone Flap in repairing lower extremity defect. Method : This systematic review was done in accordance to the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA). Literature searching was conducted using four databases, which is PubMed Central, Embase, Scopus and Cochrane Library. Result : A grand total of sixteen studies were included in the review. Among it, fourteen studies were case series. The others were cohort prospective and single arm interventional studies. Out of the 662 keystone flaps used, a total of 129 complications (19.5%,) were recorded, with the majority being infection or cellulitis (5%, n=33), delayed healing (4.5%, n=30), wound dehiscence (4.3%, n=28), and partial flap loss or necrosis (2%, n=13). Conclusion: The keystone flap could be an alternative for lower extremity defect with a realtively low complication rate.

Abstract

Introduction : Postoperative feeding after palatoplasty should be initiated immediately to maintain nutritional intake while preserving the palatal sutures to avoid trauma or tension that can lead to wound rupture. Currently, there is no consensus on post-palatoplasty preferred feeding method. This study aims to compare bottle versus syringe feeding in post-palatoplasty patients.Method : A systematic search was conducted in five databases namely PubMed, Embase, Scopus, ProQuest, and CINAHL, with the following keywords; palatoplasty, cleft palate repair, bottle feeding, syringe feeding, outcome, complication, growth, and nutrition. The selected studies were critically appraised using the Oxford CEBM (Centre for Evidence-Based Medicine) critical appraisal tools.Results: Two studies were included, with a total of 162 subjects. Both studies found that the formation of postoperative wound dehiscence and weight gain did not significantly differ between bottle and syringe groups. However, faster weight gain was observed in the bottle group. Habits and cultural factors could influence the outcomes of these studies.Conclusion: Bottle feeding is not significantly inferior to using syringe in terms of short-term postoperative complications, and can be used as an alternative feeding method for post-palatoplasty patients.

Abstract

Introduction: Cleft lip and cleft palate are the most common birth defects affecting newborns, yet their variability across each geographic origin is not clearly understood. This study aims to investigate the CL/P prevalence, profile, and corrective surgery timing in Surabaya, Indonesia.Method: This retrospective research used a descriptive method by collecting medical data records from the Surabaya CLP Center Foundation from January 2021 to December 2023, which related to sociodemographic problems, clinical features, and management timing were then statistically analyzed.Results: A total of 495 patients underwent cleft surgery, of which 59.4% were male and 40.6% were female. The most common type of Craniofacial Cleft observed is Combination Cleft Lip and Palate “CLP” (71.1%). CLP is more common in male patients than in female patients (43.4% vs. 27.7%); while CL is more common among males (14.1% vs. 9.7%) and CP is more common in females (3.2% vs. 1.8%). Significantly, complete clefts (72.9%) were more common than the incomplete clefts (27.1%). Unilateral clefts were more common than bilateral clefts (76.2% vs 18.8%). It was observed that only 22% of patients had a family history of CL/P. Overall, 74,9% of patients underwent corrective surgery within the right period.Conclusions: The data shows that most patients tended to seek medical advice and treatment at appropriate times, suggesting a high awareness of CLP as the result of proactive education and social outreach by the charity institution. Further investigations using data from other institutions are suggested to conclude the CLP management in Surabaya entirely.

Abstract

Introduction : Meningoencephalocele is one form of neural tube defect that forms a sac containing brain tissue, meninges, and cerebrospinal fluid (CSF). It can cause structural deformities and many other clinical symptoms that come after. Many medical consultations are needed for the case. The collaboration between neurosurgeon and plastic surgeon plays a significant role in managing this case.Case Report : A case of a 10-year old boy with an apple-size lump in his nasofrontal region, nasal congestion, and central visual field disturbance. The patient has not undergone an examination or surgery in a long time because he lives in a rural area. Radiological results show herniaton brain tissue, meninges, and CSF protruding through the defect in the frontal bone of the cranium that suggest meningoencephalocele as a diagnosis.Discussion : The definitive management for this case is through surgery. Selecting a surgical technique for meningoencephalocele is essential for optimizing the outcomes and minimizing the risks. The Chula technique approach was chosen for the implementation of this surgery. In this case, we describe the modified Chula technique with stages adjusted according to the patient’s condition. Plastic surgery complements many aspects of reconstruction and restores aesthetic subunits. Managing this case was a challenge until the recovery period. The surgery was successfully performed, and patient went home without any serious complications.Summary: Caution is necessary in handling surgery and involves aesthetic creativity within it. Long-term monitoring is necessary for post-operative results since many clinical aspects and social developments must be regularly evaluated.
