Abstract

Background Inflammation is a local protective response by tissue damage. Tamarillo fruit peel ethanol extract has flavonoid as anti-inflammatory, anti-oxidant, inhibit cyclooxygenase and lipoxygenase as well as leukocyte accumulation. This study aims to determine anti-inflammatory effect of Tamarillo fruit peel ethanol extract on carrageenan-induced buccal mucosa of rats. Methods Extraction was done by maceration with 70% ethanol. The 30 rats divided into five groups, namely positive control (diclofenac sodium), negative control (NaCl), and extract doses of 70, 140 and 280 mg/kgBW. Carrageenan was injected as inflammation inducer in the right buccal mucosa. Macroscopic and microscopic observations were performed at three times, 24, 48, and 72 hours after carrageenan injection. Results At 24 hours, inflammatory cells of 70 mg/kg extract were 37, at 140 and 280 mg/kgBW were 18; positive and negative control as 35 and 405. At 48 hours, inflammatory cells of positive control were 22; extract of 70, 140, and 280 mg/kgBW were 21, 17, and 16 while negative control was 26. At 72 hours, positive and negative controls were 18 and 27. Extract of 70, 140, and 280 mg/kgBW had 20, 12 and 9 cells. Extract groups had no significant differences with the positive control (p>0.05) except for the 48th hours of 70 mg/kgBW however the extract groups showed significance with negative control (p<0.05). The lowest number cells (6) was at 280 mg/kgBW on 72 hours. Conclusion Tamarillo fruit peel ethanol extract is effective anti-inflammatory agent especially dose of 280 mg/kgBW on 72 hours.

Abstract

Background Malignancies of ampulla of vater and pancreas are often difficult to distinguish. Malignancies in ampulla of vater include malignancies in major duodenal papilla, distal common bile duct and pancreatic duct opening (Wirsung). The origin of the primary tumor, histopathological diagnosis and staging play a role in therapy and prognosis. WHO Classification of tumors of the digestive system, 2019 made changes in tumor staging following the system of the American Joint Committee on Cancer (AJCC), 8th edition. This study was conducted to determine types of pancreatic and ampullary malignancies, demographic distribution, location, profile, histomorphology and tumor staging changes in the 7th edition of the AJCC to 8th edition of the AJCC in the WHO Classification of tumors of the digestive system, 2019. The aim to determine clinicopathological profile of malignancies of ampulla of vater and pancreas as well as tumor staging changes based on AJCC edition 8. Methods Observational descriptive study on pancreatic and ampulla vater malignancy resection preparations at Department of Anatomic Pathology FKUI/RSCM, period January 2015 to December 2019 that met the inclusion criteria. Demographic evaluation, tumor location, histopathological profile and patient staging changes were carried out based on the 8th edition of AJCC. Results There were 27 cases of adenocarcinoma in ampulla of vater, 10 cases of pancreatic ductal adenocarcinoma, 1 case of mucinous cystic neoplasm with adenocarcinoma focus, 1 case of serous cystadenocarcinoma, 7 cases of PanNET, 2 cases of PanNEC, 3 cases of SPN, 1 case of MiNEN. Changes in pT were present in 29 cases. Changes in pN were present in 6 cases. Conclusion The diagnosis and pathological staging are in accordance with the 2019 WHO digestive system tumor classification. Tumor differentiation, presence or absence of lymphovascular invasion, perineural invasion and lymph node metastases and incision margin status are important histopathological parameters.

Abstract

Background Meningiomas are the most common primary tumors of the central nervous system. About one-fifth of meningiomas tend to recur. The World Health Organization (WHO) histopathological grade was the most useful morphological predictor of recurrence. Fibrotic tumor vessels (FTV) were detected in nearly half of WHO grade I meningiomas and correlated with vascular density and increased risk of recurrence. Fibulin-2 is an extracellular matrix glycoprotein whose expression increases in traumatic central nervous system injuries and can be used as a marker to differentiate the histopathological grade of meningiomas. This study aimed to determine the association of fibulin-2 expression with histopathological grade and FTV in meningiomas. Methods This research was an observational cross-sectional study. The study sample was 36 cases of meningioma in three Anatomical Pathology laboratories in West Sumatra on period January 2019 to December 2020. Samples obtained from resected meningioma cases were then reevaluated on Hematoxylin Eosin (HE) slides to determine the histopathological grade and FTV. Van-Gieson staining was performed to confirm FTV. Fibulin-2 expression in tumor cells was analyzed using immunohistochemical staining. Bivariate statistical analysis using the Chi-Square test with p<0.05 was considered significant. Result High-risk meningiomas (WHO grade II and III) showed high fibulin-2 expression (62.5%), whereas low-risk meningiomas (WHO grade I) were more abundant with low fibulin-2 expression (75%). The presence of FTV at low fibulin-2 expression was 58.3%. Statistical analysis showed no significant association between fibulin-2 expression with histopathological grade (p=0.077) and FTV (p=1,000). Conclusion In summary, the study showed that fibulin-2 expression was not associated to histopathological grade nor to FTV in meningiomas.

Abstract

Background Matrix metalloproteinase-9 (MMP-9) is a protease that plays an important role in various biological processes of breast cancer. This marker may be useful in determining the prognosis and become a promising therapy, especially in breast carcinoma with an aggressive subtype including HER2 enriched and triple negative. Both subtypes have high MMP-9 expression, but it is necessary to differentiate them because they have different biological and molecular aspects. This study aims to compare the density of MMP-9 between triple negative and HER2 enriched breast carcinoma subtypes and their relationship with clinicopathological characteristics. Methods This is a cross sectional study using 32 samples of paraffin blocks and slides of breast carcinoma patients with triple negative and HER2 Enriched subtypes at the Department of Anatomical Pathology Faculty of Medicine Sriwijaya University/Dr. Mohammad Hoesin Hospital Palembang for the period January 1, 2016 to December 31, 2019. Each sample was stained with MMP-9 antibodies immunohistochemistry (polyclonal, Abcam). Comparative analysis of MMP-9 density between the two subtypes was carried out using the median value based on the Mann-Whitney analysis. MMP-9 density was categorized as high and low based on the ROC curve and analyzed by using chi-square test to assess its association with clinicopathological characteristics. Results Mann-Whitney test showed that the MMP-9 density was higher in the HER2 enriched subtype (median 12.0) than triple negative (median 8.8) with a p value of 0.632. The relationship distribution of high MMP-9 density was found in the category of age group >40 years old (60.0%), positive lymphovascular invasion (86.7%) and histological grade III (80.0%) with p values of 0.798, 0.402 and 0.691. Conclusion MMP-9 density in HER2 enriched subtype was higher than triple negative subtype, but statistically not significant.

Abstract

Background Breast cancer is the most prevalent malignancy among women and is the second largest contributor to cancer-related deaths globally. Women with a high body mass index (BMI) are at a greater risk of developing breast cancer and an increased risk of lymph node metastasis. This retrospective study aimed to determine the relationship between BMI and the clinical pathological profile of invasive breast carcinoma (IBC) patients. Method This study was conducted from 2019 to 2022. Data were collected through the archives of the Department of Anatomic Pathology FKUI/RSCM (Faculty of Medicine, University of Indonesia/Cipto Mangunkusumo Hospital). Clinical pathological data were collected. The histological type, grade, lymphovascular invasion, and lymph node metastasis of the tumors were re-evaluated. The chi-square hypothesis test was used to determine the relationship between BMI and clinical pathological profile. The results were considered statistically significant with a p <0.05. Result A total of 151 cases of IBC with lymph node dissection from 2019 to 2022 were included in this study, all of which were female, with the majority being >50 years old (55%). The most common tumor size and histological type were T2 (39,7%) and invasive ductal carcinoma NST (74,8%), 49% of cases were grade 3 tumors. Most IBC cases had an obese BMI (49%), 63.1% with lymph node metastasis had an overweight-obese BMI. Statistical analysis was performed between BMI and lymph node metastasis status, and p-value=0.025 was obtained. The relationship between tumor size and lymphovascular invasion with lymph node metastasis was also found to be statistically significant, as indicated by a p-value of 0.000. Conclusion Cases with lymph node metastasis had the highest rate of overweight-obesity, and a significant relationship was found between BMI and lymph node metastasis status. A significant relationship was also found between tumor size and lymphovascular invasion with lymph node metastasis.

Abstract

Malignant Peripheral Nerve Sheath Tumor (MPNST) is a type of spindle cell sarcoma with approximately 5% of all sarcomas. Its diagnosis is challenging due to the absence of specific immunohistochemical markers. Recently, H3K27me3 was discovered as a potential specific immunohistochemical marker to differentiate MPNST from other sarcomas and distinguish between low and high-grade MPNST. Therefore, this research aims to investigate the use of the H3K27me3 as a potential specific marker for Malignant Peripheral Nerve Sheath Tumor (MPNST). A cross-sectional analysis was conducted on 50 cases of sarcomas, including 13 MPNST, 14 synovial sarcomas, 13 dermatofibrosarcoma protuberans (DFSP), and 10 leiomyosarcomas originating from the Department of Anatomical Pathology, Faculty of Medicine, Universitas Indonesia, Cipto Mangunkusumo Hospital (FMUI-CMH) from January 2013 to December 2021. H3K27me3 images were obtained and categorized as complete loss when more than 95% of the tumor cells showed loss of nuclear staining. The results found in MPNST showed a loss of H3K27me3 expression, which is statistically significant compared to other sarcomas mimicking MPNST (p=0.021), indicating its potential as a diagnostic marker. There is a difference in the expression of H3K27me3 between the high and low-grade MPNST but it is not statistically significant (p=0.105). This showed that H3K27me3 loss of expression can be used to diagnose MPNST, especially high-grade MPNST, and differentiate it from other sarcomas mimicking MPNST.

Abstract

Ureteropelvic junction obstruction (UPJO) is an abnormality in the canal that connects the renal pelvis to the ureter, leading to impairment of urinary flow. The use of maternal ultrasound has increased the number of UPJO during the antenatal period. Histopathological features in UPJO are important for establishing the diagnosis and understanding the pathogenesis of the condition. The current study aims to explain the irregularity of the muscularis propria and a decreased Cajal cells in the affected segment of the UPJO.

Abstract

Background Plasma cells are important prognostic factors in various malignancies including Classical Hodgkin lymphoma (CHL). In the CHL microenvironment, plasma cells are the main prognostic factors, but research of relationship between plasma cell density in the microenvironment and CHL stage is still very limited. This study aims to determine the relationship between plasma cell density in the microenvironment and the CHL stage. Methods A case series procedure was conducted. The samples were paraffin blocks of CHL patients at Anatomic Pathology Department of Faculty of Medicine Universitas Sriwijaya/Mohammad Hoesin General Hospital. Among 47 cases of CHL diagnosed from 1st Januari 2018 to 30th November 2022, only 30 cases were included. Immunohistochemistry was performed by using specific antibody of plasma cells (anti-CD138 antibody). The number of plasma cells in each sample was calculated based on the expression of CD138 in the cell membrane and cytoplasm with weak-strong intensity. Then the cut-off point value was determined based on the ROC curve to classify the high and low plasma cell densities. The density of plasma cells was correlated to clinicopathological characteristics including age, sex, tumor location, CHL subtypes and stage. Statistical analysis was performed using Spearman test of SPSS with significant consideration if p<0.05. Results A high proportion (>10,9) of tumour-associated plasma cells of the tumor microenvironment was found in CHL advanced stage. Spearman test resulted in a significant correlation between density of plasma cell with advanced stage classical Hodgkin lymphoma (p=0.000). Conclusions There is significant relationship between high density of plasma cells with CHL stage.

Abstract

Background Cervical squamous cell carcinoma (CSCC) represents 70-80% of all cervical cancer cases. This cancer ranks 4th most common death cause due to malignancy in women. Cervical SCC patients were found at an older age and were detected at an advanced stage. High risk human papillomavirus (HPV) infection is main cause of SCC. Protein cyclin-dependent kinase inhibitor 2A also called p16 is a well-known tumor suppressor protein involved in cell cycle regulation, senescence, apoptosis, cell invasion and angiogenesis. This p16 protein can be used as surrogate marker for HPV infection. From several previous studies, there were differences between p16 expression relationship with differentiation degree and cervical SCC tumor stage results. Relationship research between p16 expression and differentiation degree and tumor stage of cervical SCC patients has never been conducted in West Sumatra. Methods This research was retrospective observational cross-sectional study with 35 cervical SCC cases that have been diagnosed at Anatomical Pathology Laboratory, RSUP Dr. M. Djamil Padang period January 2018-December 2019. Samples were obtained by simple random sampling from paraffin blocks derived from tumor tissue. The histopathological slide was reassessed to assess differentiation degree and tumor stage data were obtained from the patient's medical record. p16 expression in tumor cells was seen by immunohistochemical examination. To determine the correlation, bivariate statistical analysis was performed using the chi-square test with significance p<0.05 level. Results p16 high expression was found in 22 cases, tended to be higher in degree of well differentiation (40.9%) and moderate differentiation (50.0%) and was higher at an advanced stage (90.9%). Statistically, there is no correlation between p16 expression and differentiation degree with p value=0.138. There was no correlation between p16 expression and tumor stage with p value=0.337. Conclusions This study concluded that p16 expression was not associated with differentiation degree and tumor stage in cervical SCC.

Abstract

ABSTRAKLatar belakangEpidermal Growth Factor Receptor (EGFR) berperan penting pada karsinogenesis beberapa karsinoma termasuk karsinoma prostat.Overekspresi EGFR pada adenokarsinoma menyebabkan sel tumor menjadi resisten terhadap terapi hormonal. Tujuan penelitian iniuntuk mengetahui hubungan antara ekspresi EGFR dengan derajat histopatologik dan invasi perineural pada adenokarsinomaprostat.MetodePenelitian ini merupakan penelitian observasional dengan desain cross sectional dengan sampel penelitian sebanyak 60 kasusadenokarsinoma prostat yang telah didiagnosis di 5 Laboratorium Patologi Anatomik di Sumatera Barat periode Januari 2015 sampaiDesember 2017. Sampel diperoleh dari blok parafin yang berasal dari jaringan hasil operasi transuretral resection of prostat (TURP)dan prostatektomi kemudian dilakukan reevaluasi terhadap derajat histopatologik dan invasi perineural. Ekspresi EGFR pada seltumor dilihat dengan pewarnaan imunohistokimia. Analisis statistik menggunakan uji Chi-Square.HasilEkspresi EGFR positif ditemukan pada 38 kasus (63,33%) dan kasus terbanyak dengan EGFR positif adalah poorly differentiated(57,9%). Kasus adenokarsinoma prostat dengan invasi perineural paling banyak ditemukan dengan ekspresi EGFR positif (42,1%).Analisis statistik menunjukkan terdapat hubungan bermakna antara ekspresi EGFR dengan derajat histopatologik (p=0,003) dan tidakterdapat hubungan yang bermakna antara ekspresi EGFR dengan invasi perineural (p=0,129).KesimpulanTerdapat hubungan yang bermakna antara ekspresi EGFR dengan derajat histopatologik pada adenokarsinoma prostat dan tidakterdapat hubungan antara ekspresi EGFR dengan invasi perineural.
