Abstract

Background: Hospitals in the VUCA era face a dual imperative: maintaining social missions while ensuring financial sustainability. This complexity is exacerbated by the JKN shift to INA-CBGs, digital disruptions, and Law No. 17 of 2023 reinforcing corporate liability. Objective: This study analyzes challenges in strategy development and internal/external barriers to formulate recommendations for Indonesian hospital managers. Method: A Systematic Literature Review (SLR) with a descriptive-analytical approach was conducted on literature (2019–2025) from Google Scholar, PubMed, and SINTA, utilizing thematic synthesis and the Fishbone framework. Results: Findings reveal multidimensional internal barriers, including HR shortages, cultural resistance, and budget constraints. JKN implementation mandates a strategic shift from revenue generation to cost containment. While digital transformation is crucial for competitiveness, it faces infrastructure and cost hurdles. Furthermore, regulatory changes demand prioritized risk management due to heightened corporate liability. Conclusion: Strategic success depends on integrating operational efficiency, technological adaptation, and legal compliance. Adopting adaptive leadership, Lean Management, and robust risk management is recommended to maintain organizational competitiveness and sustainability

Abstract

Paracetamol is an analgesic–antipyretic drug commonly formulated as a tablet dosage form. Binders play an important role in determining the physical and mechanical quality of tablets. This journal review aims to evaluate the effect of various types of binders on the physical and mechanical properties of paracetamol tablets based on studies published within the last decade. The results indicate that the type and concentration of binders influence tablet hardness, friability, and disintegration time. Excessive binder concentration may adversely affect tablet performance by increasing tablet hardness and prolonging disintegration time, while natural binders show potential as alternative binding agents in paracetamol tablet formulations.

Abstract

Electronic Medical Records are implemented to ensure the security, confidentiality, integrity, and availability of data in accordance with data and information security principles. This study aims to explore the security and data protection aspects of EMR at Sungai Mesa Public Health Center in Banjarmasin. This is a qualitative research with a case study design, using interviews and observations. The subjects consisted of four individuals: the Head of the Health Center, a Doctor, and Registration Officers. The results show that confidentiality is ensured through login systems with personal usernames and regularly updated passwords, with access restricted based on roles. Integrity is maintained through the "Edit" feature, allowing data modification according to authority. Availability is supported by the "Search" feature, though there are obstacles such as internet disruptions, power outages, and the absence of an independent backup system. Legally, there are SOPs on data misuse sanctions, but no training or socialization has been conducted. In conclusion, confidentiality and integrity are well-implemented, while availability still faces technical challenges.

Abstract

Ventilator-Associated Pneumonia (VAP) is a nosocomial infection that frequently occurs in patients treated in the Intensive Care Unit (ICU), particularly among stroke patients with comorbid diabetes mellitus who require mechanical ventilation. Decreased level of consciousness, endotracheal tube insertion, impaired protective airway reflexes, and hyperglycemic conditions increase the risk of oropharyngeal microorganism colonization that may lead to VAP. One of the nursing interventions recommended in the VAP prevention bundle is oral hygiene using antiseptic agents. This study aimed to analyze the effectiveness of oral hygiene using 1% povidone-iodine in preventing Ventilator-Associated Pneumonia in stroke patients with diabetes mellitus receiving mechanical ventilation in the ICU. This study employed a descriptive case study design involving two patients treated in the same hospital ICU: one patient diagnosed with recurrent ischemic stroke and diabetes mellitus, and one patient diagnosed with ischemic stroke and diabetes mellitus. The nursing intervention consisted of scheduled oral hygiene using 1% povidone-iodine performed three times daily and combined with the VAP prevention bundle, including effective suctioning, 30° semi-Fowler positioning, and continuous clinical monitoring. Evaluation was conducted through clinical observation, including oral cavity hygiene, secretion characteristics, respiratory rate, oxygen saturation, random blood glucose levels, and the Modified Clinical Pulmonary Infection Score (MCPIS). The results demonstrated improved airway clearance in both patients, indicated by reduced secretion production, decreased rhonchi, stabilized respiratory rate, and increased oxygen saturation. Throughout the observation period, MCPIS values in both patients remained ≤5, and no clinical signs of VAP were identified. Therefore, oral hygiene using 1% povidone-iodine is effective in preventing Ventilator-Associated Pneumonia in stroke patients with diabetes mellitus receiving mechanical ventilation in the ICU and is recommended for integration into ICU nursing standard operating procedures.

Abstract

Radiology services in hospitals are an integral part of the healthcare system, with CT-Scan being one of the primary modalities. The CT-Scan room requires a highly controlled environment, specifically a temperature range of 20–24°C and humidity level between 40–60%, to ensure optimal equipment performance and image quality. Excessive humidity can lead to condensation, which risks damaging expensive electronic and optical components and may compromise imaging results. Therefore, an Internet of Things (IoT)-based temperature and humidity monitoring system with real-time alarm features has been developed for the CT-Scan room at the Radiology Installation of Sultan Suriansyah Regional Public Hospital. This device is designed to issue alerts when humidity reaches or exceeds 60%, and enables remote, real-time monitoring through a web-based interface. Test results indicate that the system operates stably and accurately, transmitting online data with less than ±5% deviation compared to a reference device (thermophygrometer). The system has proven effective in maintaining the environmental conditions of the CT-Scan room, improving operational efficiency, and supporting the reliability of medical equipment. With IoT integration, monitoring can be conducted anytime and anywhere, strengthening preventive maintenance and enhancing the quality of radiology services.

Abstract

RSUD dr. H. Moch. Ansari Saleh Banjarmasin, the number of patient referrals received through SISRUTE at Unit 119 has been operational since August 1, 2024, as part of strengthening digital-based emergency services. The utilization of SISRUTE, which is integrated with the Electronic Medical Record (ERM) system, supports the rapid and accurate exchange of clinical patient information. Based on data from August 1, 2024, to the end of December 2025, a total of 459 patients were referred through SISRUTE, where the referral data was documented in the ERM and used as a basis for the continuity of medical services at the receiving healthcare facility. These data indicate that the utilization of SISRUTE plays a vital role in supporting the smooth process of emergency patient referrals and enhancing coordination and service efficiency between healthcare facilities. This study is a qualitative research. Qualitative research in this study was used to identify the factors behind the return of medical record files based on the 5M framework (Man, Machine, Method, Material, and Money). According to Sugiyono (2018), qualitative research methods are used to examine natural object conditions, where the researcher serves as the key instrument, data collection techniques are carried out through triangulation (combined), data analysis is inductive, and the results emphasize meaning rather than generalization. The results of this study indicate that the Integrated Referral Information System (SISRUTE) has been implemented. SISRUTE is an information technology and internet-based health referral information system application designed to integrate and simplify the patient referral process between healthcare facilities, from primary levels (e.g., community health centers/clinics) to vertical hospitals or higher healthcare facilities online. The objective is to ensure that referrals are carried out quickly, accurately, effectively, and efficiently according to the patient's medical needs and the competence of the destination facility.

Abstract

There has been an increase in the number of floods and victims affected. This incident has caused health problems for the community in Banjarmasin. Emergency response in the field is still hampered by the information system. This study is limited to the city of Banjarmasin, but it can describe South Kalimantan Province because the mechanisms are the same. Referring to the concepts of convergence and divergence, this study was conducted using interpretive phenomenology. It was analyzed using the I.P.A. principle, assisted by NVIVO software to facilitate visualization. The results of the study show that, in terms of structural convergence, the Banjarmasin health cluster implements a top-down mechanism that is still in use. In terms of operational convergence, although the Health Office has an SKDR information system, the process still relies heavily on manual systems. Operations only rely on WhatsApp groups, and the reporting of disaster victim and patient data is still manual. This causes divergence in the field, which is prone to data and personnel errors. It is hoped that in the future there will be an integrated one-stop information system that can be utilized by health clusters both specifically and generally in Indonesia.

Abstract

Banjarmasin is a flood-prone area with recurring incidents. Therefore, hospitals are urgently needed as one of the health facilities that are ready to respond when disasters strike. In managing this risk, SMK3RS is important for protecting staff and patients during disasters. The preparedness of hospital personnel is assessed based on three levels of competence, namely awareness level, mid-level, and advanced level. This qualitative case study research determined the research subjects using purposive sampling so that the quality of information was appropriate to the research context. Data analysis used coding techniques with the help of NVIVO software. The results of the study showed that Dr. R. Soeharsono Level III Hospital was at the advanced level in terms of K3RS preparedness for disaster management, with complete and adequate equipment, structured work procedures, and responsible leadership. There are still obstacles at the lower levels, where awareness-level personnel are still negligent. The mid-level is still hampered by ineffective education. This study recommends re-improvement by ensuring that there are written procedures during disasters and periodic education.

Abstract

Traditional herbal medicines are widely used due to their perceived safety and natural origin; however, illegal addition of pharmaceutical substances such as paracetamol and sodium diclofenac poses adverse health effects. The objective of this study was to determine whether paracetamol and sodium diclofenac were present in jamu pegal linu samples via Thin Layer Chromatography (TLC) analysis. The study obtained all samples from local herbal stores in Pajarakan, Probolinggo. TLC analysis on silica gel GF254 plates was performed with compound-specific solvent systems: a 4:1 (v/v) ethyl acetate-chloroform mixture for paracetamol, and a 3:2 (v/v) ethyl acetate-n-hexane mixture for sodium diclofenac. Results revealed paracetamol was detected in three out of four samples with a retardation factor (Rf) of 0.61, while sodium diclofenac was found in three samples with Rf values around 0.90. One sample showed no presence of both compounds. These findings indicate illegal adulteration of herbal medicine, which may cause liver toxicity, kidney impairment, and cardiovascular risks. Therefore, strict supervision and consumer education are essential to prevent adverse health impacts.
