Abstract

Pregnancy changes that result in looseness and instability of joints in the third trimester of pregnancy often cause lower back pain in mothers. This certainly disturbs the mother's discomfort and causes anxiety. Progressive muscle relaxation is a combination of deep breathing and progressive muscle relaxation techniques by stretching and relaxing the muscles systematically. The purpose of this study was to analyze the effect of progressive muscle relaxation on lower back pain and anxiety in primigravida pregnancies. This study used all experiments with a pretest-posttest controlled group design for primigravida pregnant women with a gestational age of 28-36 weeks at the Misriah Galang Clinic as many as 28 respondents. The sampling technique used was simple random sampling. There were two groups in this study, namely the intervention group with progressive muscle relaxation and the control group. The research instruments used were VAS (Visual Analog Scale), Hamilton Anxiety Rating Scale (HARS) and questionnaires. Data analysis used was the dependent t-test and independent t-test to measure pain intensity and the Wilcoxon test with Mann Whitney to measure anxiety. There was a difference in the intensity of pain and anxiety before and after the progressive muscle relaxation intervention with a value of p = 0.001 and 0.004. The percentage of pain reduction after the intervention was 45.71%. There was a difference in the average pain and anxiety after the intervention between the control and intervention groups with a value of p = 0.000; p = 0.002. Progressive muscle relaxation has been proven effective in reducing the intensity of lower back pain and anxiety. Obstetric practitioners can apply this method to pregnant women who experience lower back pain and anxiety. The progressive muscle relaxation method can be used as material in Complementary Therapy in Pregnancy to develop obstetric science.

Abstract

During the first six months of life, newborns are only given breast milk, which is called exclusive breastfeeding. Breast milk helps improve the baby's immunity, meets the baby's nutritional needs, and reduces mortality and the risk of disease in them. This study aims to determine the relationship between maternal knowledge and attitudes about family lactation management with exclusive breastfeeding in infants aged 0-6 months at RSU Eshum Medan. This study used a quantitative descriptive correlation design using a cross-sectional design. The population of this study was 104 breastfeeding mothers at Eshmun General Hospital, with 64 samples selected using purposive sampling techniques. Data analysis was carried out univariately and bivariately using the Chi Square test. The results showed that the majority of respondents had good knowledge about family lactation management, namely 33 people (51.6%), the majority of respondents had a positive attitude about family lactation management, namely 48 people (75%) and the majority of respondents gave exclusive breastfeeding to newborns, namely 47 people (73.4%). The results of statistical tests showed that there was a significant relationship between maternal knowledge about family lactation management and exclusive breastfeeding in infants aged 0-6 months (p = 0.000) and there was a significant relationship between maternal attitudes about family lactation management and exclusive breastfeeding in infants aged 0-6 months (p = 0.000). Therefore, Eshmun Hospital is expected to improve health education by teaching parents about the benefits of exclusive breastfeeding and effective lactation management practices.

Abstract

Age-related physical decline increases the risk of various diseases, including degenerative conditions, such as osteoarthritis (OA). OA is a chronic, progressive, multifactorial joint disorder characterized by cartilage deterioration, leading to chronic pain, stiffness, and impaired joint mobility. Knee OA significantly reduces patients' quality of life owing to functional limitations and pain. Manual traction therapy has shown potential in alleviating secondary inflammation in OA by lowering serum interleukin-1? levels and reducing pro-inflammatory cytokines and subchondral bone changes. This study aimed to assess the effect of manual traction physiotherapy on the Western Ontario and McMaster Universities Arthritis Index (WOMAC) in patients with knee OA. The study utilized a one-group pretest-post-test design, involving 36 participants selected through consecutive sampling from patients at the Sembiring Deli Tua Hospital Physiotherapy Clinic and Universitas Sumatera Utara's medical laboratory. Participants (aged 48–69 years, 80.6% female) underwent manual traction therapy twice a week for four weeks. The WOMAC questionnaire was used to evaluate changes in pain, stiffness, and physical function pre- and post-intervention. Statistical analysis using the Wilcoxon test revealed significant improvements across all WOMAC components, with pain scores decreasing from 14.81 to 7.58, stiffness scores from 4.0 to 2.5, and physical function scores from 30 to 23 (p < 0.001). These results demonstrate that manual traction therapy effectively reduces pain and stiffness while enhancing joint function in patients with knee OA.
