Abstract

Optimal health is an essential foundation of human development and is strongly influenced by nutritional status. Exclusive breastfeeding during the first six months of life has been scientifically proven to reduce infant morbidity and mortality, improve nutritional status, and support cognitive development and the immune system of children. However, the coverage of exclusive breastfeeding in Indonesia remains suboptimal due to various social, cultural, economic, and structural factors.  This study employed a qualitative approach with a phenomenological design to explore the experiences of mothers and husbands in practicing exclusive breastfeeding. A total of 13 informants were involved, consisting of breastfeeding mothers, husbands, and healthcare providers. Data were collected through in-depth interviews, focus group discussions (FGD), limited observation, and documentation.  The findings showed that most mothers understood exclusive breastfeeding as providing only breast milk without any additional food or drink for the first six months of life. Husbands generally had a more general understanding of the concept and guidelines of exclusive breastfeeding. Mothers perceived exclusive breastfeeding as an experience that not only benefits infant health but also fosters emotional bonding, satisfaction, and pride in fulfilling their maternal role. In contrast, husbands tended to emphasize the health benefits for the infant and the economic savings for the family. Family and environmental influences also contributed to strengthening the success of exclusive breastfeeding practices.  Husband support was identified as a key factor and included emotional, instrumental, informational, and appreciation-based support. Emotional support was demonstrated through empathy and encouragement, instrumental support through assistance with household tasks and infant care, informational support through efforts to seek appropriate information, and appreciation support through recognition of the mother’s efforts. All forms of support contributed to increasing mothers’ sense of security, confidence, and resilience in practicing exclusive breastfeeding. Overall, husband support plays an important role in the success of exclusive breastfeeding by strengthening mothers emotionally, practically, and socially.

Abstract

Background: Breast milk is the optimal source of nutrition for infants; however, inadequate milk production remains a common problem among postpartum mothers, particularly during the early days after delivery. Objective: This study aimed to analyze the effect of the Breastcare, Oxytocin Massage, and Marmet Technique method on breast milk production in postpartum mothers. Method: This study used a quasi-experimental design with a post-test only control group approach. A total of 20 postpartum mothers were selected using purposive sampling and divided into an intervention group that received the Breastcare, Oxytocin Massage, and Marmet Technique method and a control group that received breastfeeding counseling. Data were collected using a questionnaire measuring indicators of infant adequacy, including urination frequency, defecation patterns, sleep duration, and weight changes, and were analyzed using the Mann–Whitney test. Results: The findings show that breast milk production in the intervention group is significantly higher than in the control group (p = 0.001), with a higher mean rank observed in the intervention group. The method enhances milk production through hormonal stimulation, mechanical stimulation, and psychological relaxation, which support the secretion of prolactin and oxytocin and facilitate milk ejection. Conclusion: The Breastcare, Oxytocin Massage, and Marmet Technique method is an effective non-pharmacological intervention that improves breast milk production and facilitates milk ejection in postpartum mothers, and it can be recommended to support exclusive breastfeeding.

Abstract

Tuberculosis (TB) is an infectious disease that remains a global health problem, with treatment success heavily influenced by patient compliance in taking medication. One effort to improve compliance is through the role of Drug Supervisors (PMOs). This study aims to determine the relationship between the role of PMOs and the success of TB treatment. This study used an analytical design with a cross-sectional approach. The sample size was 43 respondents who were TB patients. Data were collected using observation sheets and analyzed using the chi-square test. The results showed that most respondents with a good PMO role had a treatment success rate of 88.0%, while those with a sufficient PMO role had a treatment success rate of 50.0% and those with a poor PMO role had a treatment success rate of 33.3%. Overall, the treatment success rate reached 69.8%. The statistical test results showed a p-value of 0.007 (p < 0.05), indicating a significant relationship between the role of PMOs and the success of TB treatment. The conclusion of this study is that the role of PMOs is significantly related to the success of TB treatment. Therefore, improving the quality of the PMO role is essential to support patient compliance and improve the success of TB therapy.

Abstract

Cervical cancer is one of the most common types of cancer affecting women worldwide. This study aims to determine the factors associated with women's participation in cervical cancer early detection at Puskesmas Pinarik, Padang Lawas Regency, in 2025. The results showed that the attitude variable had the most significant influence on women's participation in cervical cancer early detection, with a P value = 0.000 and an Exp(B) value = 186.250. Based on these results, it can be concluded that women's attitudes are the most important factor influencing their participation in cervical cancer early detection. Therefore, Puskesmas Pinarik needs to improve health education and promotion about the importance of cervical cancer early detection for women, so that women can have sufficient knowledge and awareness to undergo early detection and reduce morbidity and mortality rates due to cervical cancer. Thus, it is expected that this study can serve as a reference for Puskesmas Pinarik in increasing women's participation in cervical cancer early detection and reducing the negative impact of cervical cancer on women.

Abstract

Poor nutritional status in prospective brides is one of the causes of stunting that needs to be identified early. Therefore, the ELSIMIL application was developed to detect stunting risk factors in prospective brides. This study aims to analyze the utilization of ELSIMIL services to increase knowledge about stunting prevention in prospective brides. This mixed-methods study employed a sequential mixed methods strategy, primarily an explanatory sequential strategy. The study population was 98, and accidental sampling resulted in a sample size of 32. Five informants participated in the study. Data validity was validated through triangulation. Quantitative data collection using questionnaires was analyzed using the chi-square test and logistic regression. Qualitative data collection using in-depth interviews was analyzed and discussed in narrative form. Results showed that of the eight variables, one variable (age) was not significantly related. Through logistic regression testing, the most dominant variable was information, with a p-value of 0.010 and Exp(B) of 28.095. Qualitative results showed that prospective brides and grooms who were introduced to the ELSIMIL app, provided with information on how to use the app, provided educational information within the app, and received ELSIMIL service support increased their knowledge and understanding of stunting prevention. In conclusion, utilizing ELSIMIL services can improve prospective brides and grooms' knowledge of stunting prevention. It is hoped that staff at the Family Planning Extension Center can expand their educational reach through youth social forums to make the delivery process more relaxed and easily understood by young prospective brides and grooms.

Abstract

The construction sector is recognized as one of the industries with a high risk of occupational accidents due to the complexity of work activities, dynamic working environments, and the involvement of multiple stakeholders in project implementation. Data from the Ministry of Manpower of Indonesia show that occupational accident cases have increased in recent years, reaching 298,137 cases in 2022, 370,747 cases in 2023, and 462,241 cases in 2024. The construction sector contributed approximately 0.8% of cases in 2023 and 0.92% in 2024, equivalent to about 4,233 occupational accidents in construction services. This condition highlights the importance of implementing an effective construction safety management system to reduce workplace accidents. This study aims to evaluate the implementation of the Construction Safety Management System (SMKK) in the Kesawan Old Town residential infrastructure development project in Medan City based on the Regulation of the Minister of Public Works and Housing No. 10 of 2021. The study employed descriptive design with a mixed-method approach through field observations, document reviews, and interviews with relevant stakeholders. The research instrument used a monitoring and evaluation checklist with a binary scoring method. The results indicate that the completeness of SMKK documentation reached 97.8%, categorized as highly satisfactory, while field implementation achieved 100%. These findings demonstrate strong consistency between safety planning and its implementation in the project. However, improvements are still needed in worker health awareness programs and hazardous material inspection documentation.

Abstract

Active participation in BPJS is an important indicator of the successful implementation of the National Health Insurance (JKN) program. Although membership coverage is relatively high, inactive participants are still identified. This study aimed to analyze the influence of knowledge level, attitudes, and socialization on BPJS participation in the working area of Bandar Jaya Public Health Center, Lahat Regency.This study employed a quantitative approach with an analytical observational design and a cross-sectional method. The study population consisted of 9,400 household heads, with a sample of 97 respondents selected using simple random sampling. Data were collected through questionnaires and analyzed using the Chi-Square test and multiple logistic regression at a significance level of 0.05.The results showed that knowledge level was significantly associated with BPJS participation (p = 0.002), attitudes were significantly associated with BPJS participation (p = 0.001), and socialization was significantly associated with BPJS participation (p = 0.000). Multivariate analysis revealed that socialization was the most dominant factor influencing active BPJS participation (OR = 5.12; 95% CI = 1.89–13.86; p = 0.001), followed by attitudes (OR = 4.38; p = 0.004) and knowledge level (OR = 3.96; p = 0.011).It can be concluded that knowledge level, attitudes, and socialization significantly influence BPJS participation, with socialization as the most dominant factor. Therefore, strengthening sustainable and community-based socialization strategies is necessary to increase active BPJS participation.

Abstract

Healthcare professionalism is a fundamental foundation in improving service quality and patient safety in hospitals. This study aimed to analyze the influence of skills, responsibility, ethics, and communication on the professionalism of healthcare workers in the Pediatric Inpatient Unit of Lahat Regional General Hospital, Lahat Regency, South Sumatra, in 2025. This research employed an analytic quantitative design with a cross-sectional approach. The population consisted of 82 healthcare workers, with a sample of 68 respondents selected using stratified random sampling technique. Data were collected using structured questionnaires and analyzed through univariate, bivariate (Chi-Square test), and multivariate (multiple logistic regression) analyses.The results showed that the majority of respondents were categorized as professional (70.6%). Bivariate analysis indicated that skills (p = 0.001), responsibility (p = 0.000), ethics (p = 0.000), and communication (p = 0.018) had a significant influence on professionalism. Multivariate analysis revealed that ethics was the most dominant variable affecting professionalism (OR = 7.25; p = 0.004), followed by responsibility (OR = 5.59; p = 0.009) and skills (OR = 4.26; p = 0.021), while communication was not statistically significant after being controlled with other variables (p = 0.078).In conclusion, ethics, responsibility, and skills are the main factors influencing healthcare professionalism. Enhancing professional ethics training, strengthening responsibility, and continuously developing clinical skills are necessary to improve the quality of healthcare services at Lahat Regional General Hospital.

Abstract

The Emergency Department (ED) serves as the frontline of hospital services and requires a rapid response time to ensure patient safety. This study aimed to analyze the service response time in the Emergency Department of Grandmed Hospital Lubuk Pakam in 2025, as well as the factors influencing it. This research employed a quantitative analytic observational method with a cross-sectional approach. The sample consisted of 97 respondents selected using a consecutive sampling technique from the population of ED patients in 2025. Data were obtained from medical records and hospital information systems and analyzed using univariate, bivariate, and multivariate methods. The results showed that triage category (p = 0.001), compliance with response time standards (p = 0.000), number of medical personnel (p = 0.003), availability of treatment rooms (p = 0.002), and patient arrival time (p = 0.004) had a significant effect on service response time. Multivariate analysis indicated that compliance with response time standards was the most dominant factor, with an odds ratio (OR) of 5.21 (95% CI: 2.10–12.95; p = 0.000).In conclusion, service response time is influenced by patient-related factors, human resources, infrastructure, and service management systems. Therefore, it is recommended to improve compliance with response time standards, optimize the number of medical personnel, and enhance ED management to improve service quality and patient safety.

Abstract

Pelayanan Kegawatdaruratan Obstetri Neonatal Emergensi Komprehensif (PONEK) di rumah sakit merupakan ujung tombak dalam upaya penurunan Angka Kematian Ibu (AKI) dan Angka Kematian Bayi (AKB). Sebagai rumah sakit rujukan di Kabupaten Serdang Bedagai, RSUD Sultan Sulaiman wajib menyelenggarakan pelayanan PONEK 24 jam yang didukung oleh fasilitas yang memadai sesuai dengan standar nasional guna menangani kasus kegawatdaruratan secara cepat dan tepat. Penelitian ini bertujuan untuk menganalisis fasilitas pelayanan PONEK di RSUD Sultan Sulaiman Tahun 2025, yang mencakup ketersediaan, kelengkapan, dan kelayakan sarana dan prasarana dalam mendukung pelayanan kegawatdaruratan obstetri dan neonatal. Penelitian ini merupakan penelitian kualitatif dengan pendekatan deskriptif. Data dikumpulkan melalui wawancara mendalam kepada informan utama dan informan triangulasi, observasi lapangan, serta telaah dokumen. Analisis data dilakukan dengan teknik analisis isi untuk mengevaluasi kesesuaian antara kondisi fasilitas di lapangan dengan standar dan regulasi yang berlaku. Hasil penelitian menunjukkan bahwa fasilitas PONEK di RSUD Sultan Sulaiman secara umum telah tersedia dan mendukung pelayanan, namun masih terdapat beberapa keterbatasan. Ketersediaan alat kesehatan, bahan medis, serta dukungan fasilitas penunjang seperti laboratorium dan bank darah 24 jam menjadi faktor krusial dalam menunjang pelayanan. Selain itu, masih diperlukan peningkatan dalam pemenuhan standar fasilitas dan pemeliharaan sarana prasarana agar pelayanan PONEK dapat berjalan secara optimal.
