Abstract

Background. One indicator of a country's health status is the Maternal Mortality Rate (MMR). Every pregnant woman has potential dangers during pregnancy, childbirth, and puerperium. Until now, in all parts of the world, risky pregnancies, especially high-risk pregnancies, are still a problem in health services, especially in developing countries. In Indonesia, MMR decreased during the period 1991-2015 from 390 to 305 per 100,000 live births. However, this is still far from the MDGs target goal, which is 102 per 100,000 live births. Many women are still pregnant or giving birth at a risky age. This condition leads to many complications (especially high-risk pregnancies) such as miscarriage, premature birth, genetic disorders, and even death. This may occur due to the lack of knowledge and education of general practitioners who serve pregnant women, especially in peripheral areas. Objective. This study aims to determine the knowledge level of Medical students of Universitas Sumatera Utara about risky pregnancy. Methods. This research was conducted in a quantitative descriptive study with a cross-sectional design. This research was conducted at the Faculty of Medicine, Universitas Sumatera Utara. The samples of this study were students of the Faculty of Medicine, Universitas Sumatera Utara, which were taken by proportionate stratified. Primary data was collected using a questionnaire containing 21 true and false questions. Result. It was found that the knowledge of students from the Faculty of Medicine, Universitas Sumatera Utara class 2018 and 2019 regarding risky pregnancies was good (85.7%). Conclusion. Students of the Faculty of Medicine, Universitas Sumatera Utara, class of 2019 have better knowledge than students of class 2018.

Abstract

Abstract. Stress is a non-specific response of the body to any form of changes. Stress is generally considered synonymous with distress. Long study duration and high academic achievement targets have the potential to cause stress in medical students. Prolonged stress can cause sleep disturbances in a person. Sleep disorders are a group of conditions that can disrupt sleep patterns. Objective. The purpose of this study was to determine the effect of stress on sleep disorders in undergraduate students at the Faculty of Medicine, Universitas Sumatera Utara. Methods. This research is an analytic study with a cross-sectional design. Respondents are undergraduate students at the Faculty of Medicine, University of North Sumatra, class of 2018-2021. Data was obtained through a questionnaire instrument. Results. Of the 103 respondents, 58 (56.3%) reported experiencing stressful events with sleep disturbances. Conclusion. There was a significant relationship (P = 0.047) between stress and sleep disorders in students at the Faculty of Medicine, University of North Sumatra. In majority, the type of their sleep disorder are nightmares.

Abstract

Introduction. Probiotics are widely used in health research because it is considered to be beneficial for human health. Lactobacillus casei strain Shirota is one of the lactic acid bacteria which mostly found in commercial drink. This bactery can improve gut’s immunity towards pathogen by producing bacteriocin which is able to lower the pH in the gut. However it was found inappropriate probiotic product storaging, inappropriate way of consuming probiotic drink and the mixing of probiotic drink with other drinks randomly could disrupt the viability of probiotic bacteria which would reduce the benefits of the probiotic drink. Aim. The aim of this study is to observe the knowledge, attitude and behaviour level from students of faculty of medicine Universitas Sumatera Utara towards Lactobacillus casei strain Shirota in probiotic drink. Method. This is a descriptive research with cross sectional approach. The research is collecting primary data using questionnaire about knowledge, attitude and behaviour level towards Lactobacillus casei strain Shirota in probiotic drinks. Result. From 152 respondents who met all the criteria, there are 48,7% respondents have good knowledge level, 45,4% sufficient and 5,9% deficient. Moreover, there are 22,4% respondents have good attitude level, 47,4% sufficient and 30,3% deficient. Also, there are 77% respondents have good behaviour level, 21% sufficient and 2,0% deficient. Conclusion. Majority of respondents have good knowledge level, sufficient attitude level and good behaviour level towards Lactobacillus casei strain Shirota.

Abstract

Background: Tinea pedis occurs in 30-70% of the world's population. In Indonesia, some data are known, such as statistical data on the prevalence of tinea pedis in several teaching hospitals, such as RS. Dr. Soetomo, RSCM, RS. Dr. Hasan Sadikin, RS. Dr. Sardjito obtained a relative yield of 16%. Therefore, public knowledge about tinea pedis is something that is very relatively owned by the community to respond to, control, and prevents the occurrence of tinea pedis. Objective: The purpose of this study was to compare the level of parental knowledge of Interdigital Tinea Pedis (Athlete’s foot) in parents with and without Interdigital Tinea pedis in Namu Trasi. Method: This research was conducted using an analytical method of cross-sectional design with a collection technique using a questionnaire to be filled out by the respondent and then analyzed using a statistical program application. Results: The level of knowledge of parents with and without Interdigital Tinea pedis in Namu Trasi Village was 45.8% with good knowledge, 34.7% with sufficient knowledge, and 19.4% with poor knowledge. Conclusion: There is a significant difference between the level of parental knowledge of Interdigital Tinea pedis (Athlete’s foot) in patients and non-patients with Interdigital Tinea pedis in Namu Trasi

Abstract

Background. Low back pain is defined as feeling of pain and discomfort in the area between the costal margin and inferior gluteal fold, with or without radiating pain. In Indonesia, LBP was fifth ranks in term of disability-adjusted life years (DALYs). Aim. This study aims to determine risk factors for LBP in employees of drinking water store in Medan Selayang sub-District, Medan. Method. This research is an observational analytic study with cross-sectional design. This study used consecutive sampling method. Minimum sample size in this study was 97 samples. Subjects in this study were employees of drinking water store in Medan Selayang sub-District. Data used in this study was primary data from samples through filling out questionnaires. Analysis used in this study was univariate analysis, bivariate with chi-square, and multivariate with logistic regression. Result. From 100 samples, 39 respondents (39%) experienced LBP. Based on bivariate analisis, p value ≤ 0.05 was found for age, body mass index, and duration of work. P value ≥ 0.05 was obtained for years of work and lifting frequency. Based on multivariate analysis with regression logistic, p value ≤ 0.05 was obtained for body mass index (p = 0.089) and work duration (p = 0.011) which indicated that work duration was the most dominant risk factor. Conclusion. There’s a relationship between age, body mass index, and work duration with the work duration being the most dominant risk factor for the incidence of low back pain in water gallon worker. .

Abstract

Abstract. Glaucoma is a multifactorial disease and the second leading cause of irreversible blindness worldwide, and incidence of open-angle glaucoma more frequent, including in Indonesia. The etiology is still unclear, but the risk factors are the immune system and increased systemic microvascular abnormalities affected by platelets. Objective. The purpose of this study was to determine the relationship between platelet parameters and visual field defect in POAG. Methods. This research is an analytic study with a case control design. Respondents are 30 POAG patients and 30 normal subjects as a control in Universitas Sumatera Utara Hospital and Satellite Hospital from August to December 2021. Venous blood samples were taken as much as 3 cc to assess platelet parameters, then analyzed in the clinical laboratory. Each group was examined with Humphrey's Perimetry which was further classified based on Hodapp Anderson Parrish criteria. Results. Most subjects in the POAG group are male, 53.3%. In POAG group the mean values of platelet count 267.28 103/μL, PDW 11.41 fL, PCT 0.25%, and MPV 9.49 fL. Conclusion. There was an increase in PDW values in advanced POAG patients, but no significant differences were found in visual field defect between the two groups.

Abstract

Abstract. Fetuin-A is a plasma glycoprotein expressed in the liver. Fatty liver accumulation because changing fetuin-A expression leads to nonalcoholic fatty liver disease (NAFLD). The benefits of serum fetuin-A as a predictor of fibrosis biomarkers in NAFLD patients remain unclear. Non-invasive scoring system predicting fibrosis among patients with NAFLD is the NAFLD fibrosis score (NFS). Objective. This study aims to find the correlation between circulating fetuin-A levels and NFS in NAFLD patients. Methods. Eighty subjects (male, 52; female, 26) followed this cross-sectional study. NAFLD is diagnosed with an abdominal ultrasound. The serum levels of fetuin-A measured by ELISA. NFS is used to identify the probability of fibrosis in subjects. The correlation between Fetuin-A levels and NFS was determined using Pearson correlation tests. A p-value < 0.05 was significant. Results. There was significant difference in fetuin-A levels (p < 0.001) between NAFLD degree I (421.71 pg/mL), NAFLD degree II (494.88 pg/mL), and NAFLD degree III (692 pg/mL). The NFS low probability score in 46 subjects and NFS indeterminate score in 34 subjects. Serum concentrations of fetuin-A correlate significantly with NFS (r = 0.478, p < 0.001). Conclusion. There is a positive correlation between serum levels of fetuin-A and NFS.

Abstract

Introduction: Diabetic retinopathy is an ocular complication of diabetes mellitus (DM), and is one of the leading causes of visual impairment and blindness worldwide.. More than one third of people with diabetes have signs of diabetic retinopathy. Methods: This was an analytic observational study with cross-sectional design. The sample was type 2 diabetes mellitus patients with diabetic retinopathy who visited the eye clinic at the University of Sumatera Utara General Hospital from September 2021-December 2021. The sample were then analyzed with the Mann-Whitney and Kruskal Wallis test to find the relationship between diabetic retinopathy degree with visual acuity and retinal nerve fiber layer thickness in type 2 diabetes mellitus patients. Results: The sample of this study were 20 subjects with diabetic retinopathy and 20 subjects without diabetic retinopathy as the control group. The mean visual acuity in subjects with mild diabetic retinopathy was 0.65+0.48. Average visual acuity in subjects with the degree of moderate diabetic retinopathy was 0.83+0.46. The mean visual acuity in subjects with a proliferative degrees diabetic retinopathy was 0.77+0.64. Mann Whitney test revealed no statistically significant relationship between the degree of diabetic retinopathy and visual acuity (p=0.734). Using the Kruskal Wallis test revealed that there was no significant relationship between the degree of retinopathy diabetic with Avg RNFL (p=0.495), superior RNFL (p=0.385), inferior RNFL (p=0.111), temporal RNFL (p=0.064), nasal RNFL (p=0.535). Discussion: Controlling blood glucose levels becomes more important than the duration of diabetes in preventing the development of retinopathy. Most patients are advised to have an HbA1c of 7% or lower, and for certain patients it is recommended to be lower than6.5%. Diabetic retinopathy slowly damages the retinal blood vessels or the optic nerve layer, leading to leakage, thus resulting in accumulation of fluid containing lipid and blood in the retina which will gradually lead to visual impairment, and even blindness. Conclusion: There is no significant relationship between diabetic retinopathy degree with visual acuity and retinal nerve fiber layer thickness in type 2 diabetes mellitus patients.

Abstract

Stroke is one of the top three cause of death and disability globally. Approximately, only 10% to 15% of first-ever stroke are intracerebral hemorrhages (ICHs), but the rates of disability and death are significantly higher. Hydrocephalus may occur in more than 50% of patients with intraventricular hemorrhage (IVH), which is secondary to ICH. Intraventricular hemorrhage (IVH), an accumulation of blood to ventricles that may be cause by extension of ICH, occurs in up to 50% of patients with primary ICH. Hydrocephalus itself may serves as a predictor of poor outcome after ICH.1,2,3 A 71 years old male came to the hospital with the complaint of loss of consciousness, difficulty in communicating, and weakness of extremities on the left side of the body since 12 hours ago. The complaint was preceded by headache that did not alleviate with medication since 1 day before admission and there was history of slurred speech. A head CT scan without contrast was done and the result showed an intracerebral hemorrhage and a suggestive obstructive hydrocephalus. The patient was diagnosed noncommunicating hydrocephalus due to spontaneous intracerebral haemorrhage (ICH) at the right thalamus with intraventricular haemorrhage due to suspect hypertension with differential diagnosis of cerebral amyloid angiopathy with emergency hypertension. The patient was advised to underwent an emergency VP shunt placement. Implantation of a ventriculoperitoneal (VP) shunt is the most widely used treatment of hydrocephalus. Considered to be a major privonce of neurosurgery, it accounts for 70.000 hospital admission in the US. Even though, VP shunting of CSF reduces the morbidity and mortality of post-hemorrhagic. hydrocephalus, it is associated with potential complications requiring multiple surgical procedures, as well as shunt revision due to its failure during the patient’s lifetime.4

Abstract

Patients with atrial fibrillation (AF) have a greater probability of a stroke event than patients without AF. As a result of developments in cardiac monitoring, the diagnosis of AF during an ischemic stroke or transient ischemic attack has improved these years. More cases of AF detected after stroke (AFDAS) are reported, which has implications for future risk of recurrent stroke and prevention. This article provides the current review of AFDAS's monitoring and brief management.
