Abstract

Dengue Fever (DF) or Dengue Hemorrhagic Fever (DHF) is one of the infectious diseases that attracts much global attention, especially Indonesia because it impacts the mortality rate of the people in the world if adequate treatment is not given. Expanded Dengue Syndrome (EDS) is a clinical manifestation involving several organs such as lungs, liver, kidney, heart, and brain-related to dengue infections, with or without plasma leakage. This study aimed to determine the prevalence of expanded dengue syndrome in patients with dengue virus infection at the Dr. Soetomo Hospital in 2017–2018. Based on its purpose, the sampling technique used in this study was the total sampling of patients with DVI who admitted to Dr. Soetomo Hospital Surabaya. Every pediatric and adult patient who met the criteria were included in this study until a certain period in 2017–2018.  After the data collection, only thirty samples of EDS from the 196 sample data were collected. Thirty patients with expanded dengue syndrome had a manifestation of different organs. Such as: neurological, cardiac, respiratory system, gastro-hepatic, and kidneys. The laboratory test results showed that most platelet counts of 51,000 – 100,000 were found in seventeen patients, while the range of hemoglobin and hematocrit was normal. From a total of thirty samples of dengue virus infection patients with EDS, there were only eighteen patients who did a serological test. The most found serological results were (+/-) anti-dengue IgM and (+) anti-dengue IgG (40%).

Abstract

Systemic Lupus Erythematosus (SLE) is an autoimmune disease that has various clinical manifestations. The SLE pathogenesis involves both innate and adaptive immunological components. The system is essentially determined by genetic factors that control certain clinical and serological manifestations. Genetic traits that determine the roles of cytokines, autoantibodies, and complements in SLE vary among ethnicities. The roles of TNF-α, IL-6, anti-C1q, anti-dsDNA, C3, and C4 towards SLE activity need to be evaluated in the Javanese population. This study aimed to determine the correlation of TNF-α, IL-6, anti-C1q antibodies, anti-dsDNA, C3, and C4 with SLE activity. Forty SLE patients were diagnosed based on the American College of Rheumatology (ACR) criteria. Disease activity was measured by the Systemic Lupus Activity Measure (SLAM) index. TNF-α, IL-6, Anti-C1q, and anti-dsDNA levels were measured by ELISA, while MINIMEPH measured C3 and C4. Thirty-nine female and one male patient with SLE were diagnosed according to ACR criteria. The mean of SLAM score, anti-dsDNA, C3, and C4 levels was 20.98±6.7, 224.96±298.6, 68.70±37.08 mg/dL, and 18.75±10.69 mg/dL, respectively. Spearman's correlation test showed a positive correlation between TNF-α (r = 0.971, p<0.001), IL-6 (r=0.835, p<0.001), anti-C1q (r=0.399, p=0.01), and disease activity (SLAM score) by using. The linear regression test for TNF-α, IL-6, anti-C1q, and SLAM showed the strongest association for TNF-α (r=0.891, p<0.000). TNF-α, IL-6, and anti-C1q were correlated to disease activity in SLE patients from the Javanese population.

Abstract

The mortality and morbidity rate of neutropenic fever is quite high. The source and severity of infection must be evaluated quickly in neutropenic patients. C-Reactive Protein (CRP) and Neutrophil Extracellular Traps (NETs) are known as markers for severeinfection. Besides, NETs also play a role in the development and metastasis of cancer but the mechanism is still unclear. C-reactive protein is a marker that is routinely used for infection. However, the correlation between CRP levels and NETs in malignant patients with neutropenia is unknown. Therefore this study was conducted to determine the correlation between CRP levels and NETs in malignant patients with neutropenia. This research was a cross-sectional observational study. The samples were malignant patients with neutropenia who were treated in the Pediatric Ward of  Dr. Mohammad Hoesin Hospital, Palembang and met the inclusion and exclusion criteria. Sampling was done by consecutive sampling. C-reactive protein level and NETs weretested by immunoturbidimetry and ELISA methods. Data were analyzed with the Pearson test (p <0.05). The mean of CRP and NETs level were 170.17±121.52 mg/L and 0.77±0.39 ng/mL, respectively. The correlation test between CRP level and NETs in malignancy patients with neutropenic fever showed r=0.228 and p=0.362. The results showed no significant correlation between CRP level and NETs in malignant patients with neutropenic fever.

Abstract

Wound healing is a complex process that involves the interaction between Extracellular Matrix (ECM) cells and mediated by cytokines and various growth factors, especially platelet-derived growth factor (PDGF) which plays a role in almost every phase of wound healing. Applying snakehead fish and sea cucumber extract containing amino acids and fatty acids in post-operative patients is expected to increase PDGF levels and enable the acceleration of the wound healing process. This study was a double-blind randomized control trial with a pre and post-treatment approach which involved 34 post-operative patients divided into two groups; the control group (K1) and treatment group (K2) and was carried out from April to June 2019 at Dr. Moewardi Hospital in Surakarta. Independent sample T-test was used for data with normal distribution to determine the mean differences between the control and treatment groups, while the Mann-Whitney test was used for data with the abnormal distribution. Also, to determine the mean difference before and after the treatment of one paired group, a paired T-test was used for data with normal distribution, while the Wilcoxon test was used for data with the abnormal distribution. p < 0.05 was significant with 95% Confidence Intervals (CI). The supplementation of snakehead fish and sea cucumber extract significantly increased PDGF levels in the treatment group (K2) compared to the controls group (K1) (p <0.001). Our findings showed that snakehead fish and sea cucumber extract were able to increase PDGF levels in post-operative patients on the fourth day.

Abstract

Managing anemia in Chronic Kidney Disease (CKD) patients with hemodialysis (HD) is a challenge to physicians. The present consensus does not address the proper time of blood sampling in HD patients, but higher ultrafiltration (UF) volume (a process of removing fluid excess during HD) may alter hematologic parameters. The objective of this study was to compare some parameters of the Complete Blood Count (CBC); hemoglobin (Hb), hematocrit (Hct), leukocyte (WBC), and platelet counts (Plt) before and after HD. This method was a cross-sectional study performed in the HD Unit, Dr.Soetomo Hospital, including 51 CKD patients selected consecutively, divided into two groups based on the UF volume (2 L and >2 L). Complete blood count pre- and post-HD were measured using Sysmex XN 1000. The results were 25 males and 26 females in this study, age ranged from 20 to 74-year-old, and 36 patients with UF volume >2 L. Only HD with UF >2 L showed significant increases for Hb (9.35g/dL to 10.00 g/dL), Hct (29.80% to 31.15 %), and Plt (209.00x103/µL to 213.00x103/µL) but WBC did not change significantly. These changes were believed to be caused by ultrafiltration. The conclusion was Hb, Hct, and Plt increased significantly with UF ≥2 L in HD CKD patients.

Abstract

Mortality and morbidity due to stroke rank the highest in Indonesia (15.4%), and most types of stroke are ischemic (87%). Inflammation has a role in the pathophysiology of both ischemic stroke and also inhibits acute symptomatic epileptic seizures (3-6%) in the first 7 days after stroke. Statins have been used for the treatment of dyslipidemia in stroke patients. Some studies showed that statins reduced the inflammatory response after a stroke and prevented the recovery of epileptic seizures. This study aimed to determine the differences in lymphocytes, hs-CRP, Electroencephalogram (EEG) with and without Simvastatin in acute ischemic stroke. This research was an experimental study with a double-blind, randomized control trial design consisting of two groups, a group given Simvastatin 20 mg/day, and a group given a placebo for seven days. The difference in lymphocytes, hs-CRP, EEG, and epileptic seizures between the two groups were then analyzed. The sample was 26 people, consisting of 17 (65.4%) males and 9 (34.6%) females with an average age of 59±5.8 years. Chi-Square and Fisher's test showed a significant difference in hs-CRP (p=0.005) and epileptic seizures (p=0.015), but no significant difference in lymphocytes (p=0.336) and EEG (p=0.42) between groups given Simvastatin 20 mg/day and those given placebo. There was a significant difference in hs-CRP and epileptic seizures, but no significant difference in lymphocyte count and EEG between the two groups with and without Simvastatin administration.

Abstract

Acute leukemia is bone marrow clonal cell malignancy. One of its complications is hypercalcemia. Parathyroid Hormone-Related Protein (PTHrP) activities involve the regulation of Calcium (Ca) metabolism. Vitamin D is a steroid involved in Ca homeostasis and bone mineralization. This study aimed to analyze PTHrP and vitamin D levels with serum calcium ion in acute leukemia. A cross-sectional study was performed in Clinical Pathology Dr. Moewardi General Hospital Surakarta between July and August 2019, consisting of 41 subjects with new acute leukemia who were diagnosed based on bone marrow puncture and or immunophenotyping result. The cut-off value of Ca ion serum and PTHrP level were determined with a Receiver Operating Curve (ROC). The data were analyzed with a 2x2 table, followed by multivariate logistic regression analysis, and p<0.05 was considered significant. Statistical analysis showed the median age of 25 (2-68) years, 23 (56.10%) ALL, and 18 (43.90%) non-ALL patients. The median of Ca ion and PTHrP were 1.08 (0.84-1.21) mmol/l and 307.52 (20.77-1104.26) pg/mL, respectively. The mean level of vitamin D was 26.45±11.40 ng/mL. Bivariate analysis showed that PTHrP levels ≥ 110.09 pg/mL and vitamin D ≥ 20 ng/mL were related to serum Ca ion ≥ 1.07 mmol/l (PR 4.675; 95% CI: 1.211-18.041; p=0.021 and PR 5.143; 95% CI: 1.279-20.677; p=0.017). Multivariate analysis showed that PTHrP ≥ 110.09 pg/mL and vitamin D ≥ 20 ng/mL were associated with serum Ca ion ≥1.07 mmol/l. There was a significant association between PTHrP, vitamin D level, and serum Ca ion in acute leukemia patients.

Abstract

Autism Spectrum Disorder (ASD) is a complex neurodevelopmental disorder, progressive disruption of brain development and synaptic connections, which causes impaired communication and behavioral disorders. However, the etiology is unknown. A damaged perinatal environment causes chronic nerve inflammation in genetically susceptible children of ASD, dysregulation of the immune system, which leads to ASD. The ratio of Neutrophil to Lymphocyte (NLR) as one of the markers of inflammation is frequently associated with inflammation-related psychiatric diseases and is potentially used as a diagnostic instrument. A cross-sectional study was carried out from June to November 2019 by using medical records of Outpatients with growth and development disorders and ASD and involving 69 children. The diagnosis of ASD was confirmed by the fifth edition Diagnostic and Manual of Mental Disorders criteria (DSM-V). A routine hematology test was carried out with a flow cytometry method. Diagnostic tests were performed in 43 children with developmental disorders (mean NLR 1.01±0.07) and 26 ASD children (mean NLR 1.02±0.06), with a cut-off value of 0.68. Sensitivity, specificity, positive predictive value, negative predictive value and accuracy of NLR were 84.62%; 27.9%; 41.51%; 25%; and 50.2%, respectively. Neutrophil to lymphocyte is sensitive to diagnose ASD but its accuracy is low. Immune dysfunction and complex neurodevelopmental disorders that are affected by chronic inflammation play a role in the occurrence of ASD. Other inflammatory markers are needed to improve the accuracy of laboratory tests to diagnose ASD.

Abstract

Syphilis infection, age, gender, sexual behaviour, length of HIV infection and length of ARV treatment are suspected to be associated with amount of cluster of differentiation 4 (CD4+)T-cells (CD4+ T-cell) and CD4+ T-cells: CD8+ T-cells ratio (CD4:CD8) of HIV patients on ARV. However, the evidence remains lacking. The aim of this cross-sectional study on April 24th to June 20th, 2019 was to determine the association of syphilis infection and other risk factors (age, gender, sexual behavior, length of HIV infection and ARV treatment) with the immunity of patients with HIV on ARV based on T CD4+ and CD4:CD8, and got factors those have an influence to T CD4+ and CD4:CD8. Seventy-four subjects with age ≥18 years old with HIV on ARV from Voluntary Counselling Test (VCT) Outpatients of Dr. Moewardi General Hospital in Surakarta (DMGHS) had been examined for syphilis, T CD4+ count and CD4:CD8 in Clinical Pathology Laboratory of DMGHS. Other data had been completed from the anamnesis and VCT data system. All data had been processed with SPSS version 21. Multivariate logistic regression following bivariate analysis of the Chi-Square test was used for categorical variables. Bivariate analysis showed a significant association between age, length of HIV infection and length of ARV treatment to T CD4+ count and significant association between sexual behavior, length of HIV infection, and length of ARV treatment to CD4:CD8 (p<0.05). Multivariate analysis showed that the prevalence of CD4≤500 was higher in male Prevalence Ratio (PR)=3.256; p=0.038) than that of female and subjects aged >42 y.o. compared to those aged >18-42 y.o. (PR=3.451; p=0.047). The PR of CD4:CD8<0.3 in anal sex (PR=3.575; p=0.049) was higher than that of vaginal sex. The PR of CD4≤500 (PR=0.271; p=0.020) and CD4:CD8<0.3 (PR=0.125; p=0.001) in subjects with length of HIV >5 years were lower than those in HIV 0-5 years. Age, gender, and length of HIV potentially affect the probability of T CD4+≤500. Sexual behavior and length of HIV potentially affect the probability of CD4:CD8<0.3. Both CD4 and CD4:CD8 ratio must be tested at baseline and follow-up.
