Abstract

Introduction: Pseudomonas aeruginosa may cause infections in various organs. Due to increase resistance, single antibiotic option for treatment of those infections is more limited.Objective: to elcucidate in vitro antibacterial effect of fosfomycin and sulbactam-cefoperazone alone and its combination against clinical isolates of Pseudomonas aeruginosa.Methods: Minimal Inhibitory concentration of fosfomycin and cefoperazone-sulbactam as a single drug and their combination was determined in liquid media against 30 clinical isolates of the Pseudomonas aeruginosa. Effect of antibiotic combination was calculated by Fraction Inhibitory Index. Time-kill curve was plotted according to the number of viable bacteria after antibiotic exposure.Results: The proportion of susceptible isolate to sulbactam-cefoperazon alone was 66.7% and increased to 90 % to combined antibiotics. Proportion of susceptible bacteria to fosfomycin alone was 10 % and increase to 20 % to combined antibiotics. Synergistic, indifferent and antagonistic effects were found in 60%,36.7% and 3.3% strains respectively. Rapid bacterial death occurred within the first 4 hours of exposure.Conclusion : Combination of sulbactam-cepoferazone and fosfomycin have sinergism in majority of tested strain of Ps aeruginosa and only one strain show antagonistic effect.

Abstract

Introduction: Preterm labor is one of the biggest direct cause of neonatal death and determined by various risk factors. Objective: To determine the effect of mother’s age, parity, preterm labor history, multiple pregnancy and pregnancy complications on the incidence of preterm labor. Method: Retrospective case-control method was conducted. Data were obtained from medical records of mothers who did labor in RSUD Dr. M. Soewandhi in 2017. Result: The analysis showed significant relationship between mother’s age (p=0,004), preterm labor history (p=0,006) and pregnancy complications (p less than 0,001) with the incidence of preterm labor. Based on multivariate analysis, mothers aged less than sign 20 and greater than sign dari 35 years old and those who had pregnancy complications were at risk of 3,510 (95% CI 1,481-8,230) and 5,203 (95% CI 2,248-12,040) times of having preterm labor.Conclusion: Maternal age and pregnancy complications were two contributing factors that increase the risk of preterm labor. These factors should be given special attention while assessing pregnancy to prevent preterm labor.

Abstract

Introduction: Autonomic dysfunction is one of the non-motor symptoms of parkinsonism but has not been of clinical concern. The purpose of this study was to assess the relationship of autonomic dysfunction with the severity of parkinsonism patients.Method: A total of 121 outpatient subjects in the Neurology Clinic of Prof. Dr. R. D. Kandou Hospital Manado, sample selection is done consecutively in analytic cross-sectional studies. The patient characteristics, symptoms and examination of autonomic dysfunction are recorded. Data were analyzed using chi square, Mann Whitney U, Kruskal-Wallis and logistic regression. Result: Almost all variables measuring autonomic function tended to show a higher degree of severity of parkinsonism (p < 0.001). Each increase in the SCOPA-AUT INA score tends to increase the odds of the Hoehn and Yahr stage (OR > 1) also in the greater orthostatic blood pressure difference test, older age, longer illness, longer treatment period, and use of a combination of antiparkinson drugs.Conclusion: Autonomic dysfunction is significantly related to the severity of parkinsonism.

Abstract

Introduction: This study aims to assess the association of severe occupational stress among managerial staff with the tendency of mental emotional disorder. Methods: The study was conducted from March to May 2016 at a Foreign Cooperation in West Java with comparative cross-sectional method. Occupational stress was measured with Stress Diagnostic Survey (SDS), whilst the tendency of mental emotional disorder was assessed with Symptom Check List 90 (SCL 90). A hundred and five respondents were asked to fill out the SDS questionnaire. Furthermore, 30 respondents were randomly selected from each group of mild-moderate and severe occupational stress. Results: The prevalence of severe occupational stress on managerial staff is 35.2%. There was a significant association between severe occupational stress with the tendency of mental emotional disorder (OR 47; 95% CI 7.37-300.17; p less than sign 0.001). Components of occupational stressors which statistically significant with mental emotional disorder are the qualitative workload (OR 10.67; 95% CI 1.03-109.94; p 0.047) and career development (OR 10.83; 95% CI 1.03 -114.15; p 0.047). Education is an individual factor that statistically significant against the tendency of mental emotional disorder (OR 0.17; 95% CI 0.03 to 0.83; p 0.029). Conclusion: There was no significant relationship between work factors with the tendency of mental emotional disorder.

Abstract

Schizophrenia is a severe mental disorder, characterized by deep thinking disturbances, affecting language, perception, and sense of self. This often includes psychotic experiences, such as hearing sounds or delusions. The most common age of onset for schizophrenia in women is from ages 25-35 (reproductive age). We report a case of a 27-year-old woman with pregnancy and schizophrenia paranoid. Reproductive age women with schizophrenia need comprehensive management during their reproductive years, including contraception, prenatal care, antenatal care, postnatal care, as well as safe and effective parenting.

Abstract

Fatigue is defined as a physiological state of reduced mental or physical performance capability resulting from sleep loss or extended wakefulness, circadian phase, or workload (mental and/or physical activity) that can impair a crew member’s alertness and ability to safely operate an aircraft or perform safety-related duties. An aircraft incident or accident may arise as a result of an error at an individual level (aviator or other crew), where fatigue is one of the main contributing factors.The International Civil Aviation Organization (ICAO) has recommended Fatigue Risk Management System (FRMS) to its member States, including Indonesia. This FRMS is a monitoring system and risk factors management related to fatigue-based burnout, which is carried out continuously. In Indonesia, there are currently few airlines that implement FRMS. It takes practical steps to manage fatigue risk factors in the aviator, especially concerning physical activity, activity in leisure or resting time and time management Sleep.A practical guideline of Fatigue Management for aviators with physical activity, physical exercise and sleeping arrangement is targeted to become a reference to the Fatigue management for pilots, airline management and can be a recommendation for policy stakeholders to become part of the flight safety procedures in Indonesia.

Abstract

Di penghujung tahun 2019, laporan pemerintah China membuat publik ibuat penasaran dan tim ahli sibuk meneliti. Tepat tanggal 31 Desember 2019, China melaporkan kejadian luar biasa, kasus pneumonia misterius yang belum diketahui penyebabnya, tepatnya di Kota Wuhan, Provinsi Hubei.1 Berdasarkan penelitian terhadap 41 pasien pertama kasus diduga terkait atau terpapar dengan satu pasar hewan di Wuhan, China.

Abstract

Introduction: World Health Organization (WHO) estimated the number of DM patients in Indonesia in year 2030 could reach up to 21,3 million people. This study was conducted to understand the correlation between HbA1c levels and albuminuria in DMT2 patients.Methods: Data included age, sex, levels of HbA1c, blood pressure, proteinuria, smoking habit, usage of ACE-Inhibitor and/or Angiotensin Receptor Blocker, and duration of treatment that was collected from the medical records of patients DMT2 in Atma Jaya Hospital according to certain inclusion and exclusion criteria using the cross-sectional study with the Spearman correlation method with 95% confidence interval (á = 0.05).Result: There’s a correlation between HbA1c levels and albuminuria (p less than 0,0001) with a quite strong correlation (r=0,439). There is also a relation between age (p less than 0.05), while blood pressure, smoking, and ACE-Inhobitor or Angiotensin Receptor Blocker consumption are not related significantly. Conclusion : This study showed a correlation between HbA1c levels and albuminuria in Type 2 DM patient in Atma Jaya Hospital.

Abstract

Aim: Acute myocardial infarction (AMI) is the number one cause of death in 2000. Usually, coffee restriction was advised to patients with cardiovascular diseases. Recent studies indicate coffee might have a cardioprotective effect.Method: Systematic search was done on Pubmed and Cochrane. Title and abstract screening were initiated on 12 articles, followed by selection with inclusion and exclusion criteria, and selected one meta-analysis study to be critically appraised. Result: The meta-analysis study appraised resulted with a good score. Reduction in mortality rate was found, and an inverse relationship was concluded between heavy coffee consumption versus no consumption and mortality in acute myocardial infarction patients. RR = 0.54 95% CI [0.45 – 0.65].Conclusion: Coffee consumption and mortality in myocardial infarction patients have an inverse relationship, with the highest risk reduction found in heavy coffee drinkers.
