Abstract

Background: Diabetic foot ulcer is a problem in patients with diabetes mellitus (DM). One adjuvant therapy that can enhance wound healing is Low-Level Laser Therapy (LLLT), but there is no established guideline regarding the dosage. In Indonesia, there has been no study comparing the energy density of LLLT on diabetic ulcer healingMethods: This is an experimental study on 28 subjects with a randomized diabetic foot ulcer. Group A received standard treatment of ulcer and LLLT 5J/cm2. Group B received standard treatment of ulcer and LLLT 10J/cm2.Results: The difference in wound size between group A and group B were 4.15 mm2 and 7.5 mm2 (p=0.178). The healing rate of group A and group B were 4.15 (-10-34.5) mm2/4 weeks and 7.5 (-2.8-34) mm2/4weeks (p=0.168).Conclusions: There was no statistically significant difference between the group receiving LLLT 5J/cm2 or 10 J/cm2 in diabetic foot ulcer healing.

Abstract

Background: Thalassemia is a hereditary hemolytic anemia disorder. Periodic transfusion for thalassemia patients may lead iron deposit in pituitary gland and induce hypogonadotropic hypogonadism. MRI examination has been started to be used for measurement of iron level in pituitary gland. Method: This study uses cross sectional method. MRI T2 and T2* relaxometry value and SIRT2* of pituitary gland was correlated with FSH and LH level in patients with major thalassemia. This study involves 28 subjects and conducted from December 2016 to March 2017. Result: There is correlation between relaxometry values of coronal slice T2 pituitary with FSH and LH level. There is also a correlation between pituitary SIRT2* value with LH level. Conclusion: Relaxometry value of coronal slice T2 pituitary and pituitary SIRT2* value may be use as reference for iron deposit on pituitary gland as well as to monitor chelating therapy in major thalassemia â patients.

Abstract

Objective: Mechanical complications of peritoneal dialysis (PD) may occur because of surgical complications when inserting a Tenckhoff catheter or non-surgical complications during chronic care of a PD catheter. We aim to highlight the latter by presenting a case report of twisted external catheter. Method: We report an 11-year-old Indonesian girl with end-stage renal disease on continuous ambulatory PD at Cipto Mangunkusumo Hospital who was hospitalized due to repeat peritonitis.Result: Upon examination, the external catheter seemed twisted without inflow, outflow, or ultrafiltration problems. Her exit score was 4 with gaping. Both abdominal X-ray and ultrasound showed that the catheter and its cuffs were properly placed. The patient frequently pulled and manipulated her PD catheter. Additionally, the catheter-site care procedure was frequently done by untrained caregivers and catheter fixation was also not performed. A retraining program for all involved caregivers was carried out. Peritonitis resolved after 14-day-treatment using intraperitoneal gentamicin. We determined that the twisted catheter and repeat peritonitis were due to a combination of mechanical trauma, poor chronic catheter-site care, and suboptimal PD catheter training. Conclusion: Maintaining compliance for chronic PD catheter exit-site care by well-trained caregivers and by patients themselves, as well as the external catheter fixation are important.

Abstract

Introduction: Society of Critical Care Medicine (SCCM) and European Society of Intensive Care Medicine (ESICM) defined sepsis as life-threatening organ dysfunction caused by dysregulated host response to infection. Sequential Organ Failure Assessment (SOFA) was chosen and validated as a scoring system to predict organ dysfunction in adult patients with infection because SOFA score showed an equal or even better result than another scoring system in predicting hospital mortality. To adapt sepsis-3 definition in pediatric patients, SOFA was modified according to age into pediatric sequential organ failure assessment (pSOFA) score. The result showed that pSOFA score on admission day, day 2, 4, 7 and 14 in Pediatric Intensive Care Unit (PICU) was better in predicting prognosis for pediatric oncology patients on mechanical ventilator, pediatric patients with sepsis and better in predicting 30-days mortality than another scoring system. However, pSOFA failed to predict the length of stay pediatric patients in PICU.

Abstract

In the last decades, concerns have been raised about the shift in the dietary patterns towards a high energy dense diet, mainly higher red meat consumption. The amount of meat consumption per person has almost doubled between 1961 and 2013 from 23 kilograms to 43 kilograms per year worldwide.

Abstract

Background: Breastmilk is the best natural food for babies. Human milk consists of nutrients, such as proteins, lipid, carbohydrate, minerals, vitamins, and trace elements that are important for babies to ensure optimum growth and development. There is still low percentage of breastfeeding coverage in Indonesia. This study was designed to determine the factors that associated with the success of exclusive breastfeeding of children at Wangaya General Hospital. Methods: This study uses case-control method. Data was collected from medical record and interviews in Wangaya Regional General Hospital between April to May 2019. This study uses consecutive sampling. Data analysis uses chi square for bivariate and logistic regression for multivariate. Results: Factors associated with the success of exclusive breastfeeding were multipara (p=1,000; OR=1.214), labor method (p=1,000; OR=1.214), information availability (p less than 0,001; OR=46.000), occupation (p=0,148; OR=2.778), and educational level (p=0,240; OR=2.364). From multivariate analysis, information availability (p=0,001) was significant. Conclusion: Factor associated with successful breastfeeding in children at Wangaya General Hospital was information availability about breastfeeding to the mother.

Abstract

Aim: Determine the effects of continual physical exercise on lymphedema and limitations of Range of Motion (ROM) of shoulder joint events in post-Modified Radical Mastectomy (MRM) patients. Methods: The Pretest-Posttest Control Group Design study with inclusion criteria are women aged d”60 years and BMI d”30 kg/m2 with a diagnosis of breast cancer post MRM. The control group received physical exercise during hospitalization and the treatment group received continual physical training until 16 weeks. This study using chi-square and Kolmogorov-Smirnov test for statistical analyzing.Results: Study was carried out on 24 subjects. There were no lymphedema and limitations of shoulder ROM in the group who receive continual physical exercise, but it is not statistically significant difference between groups (p greater than 0,05).Conclusion: This study shows that continual physical exercise could prevent the occurrence of lymphedema and limited ROM of shoulder joint in post MRM patients.

Abstract

Introduction: Cough is one of the main human defense mechanisms against respiratory tract infection and aspiration. Until now, there has not been any Peak Cough Flow (PCF) standard value in Indonesia. The preexisting standard PCF values were measured in Brazil (2012) and New Zealand (Caucasian). There were anthropometric differences based on race, which affect PCF. The aim of this study is to determine the PCF of healthy Indonesian adults and affecting factors.Methods: This study used crossectional in design. A number of 30 healthy adult subjects were recruited and analyzed. Chi-Square and multivariate analyses. Screening subjects was measured using spirometry test and cough ability using peak flow meter.Results: The mean PCF was 477.17 (L/min). There was significant correlation between gender (p=0.000), age (p=0.012;r=-0.430), and height (p=0.000;r=0.741) to PCF. Height was the most contributing variable (p=0.003;IK95% 2.37-10.77).Conclusion: The mean PCF in healthy adults was 477.17(L/min). Height was the most contributing factor, followed by age and gender.

Abstract

Introduction: Preeclampsia-eclampsia is the second highest risk factor of maternal mortality after hemorrhage. This research aimed to analyze preeclampsia-eclampsia as a risk factor of maternal mortality in The General Hospital of Ulin Banjarmasin. Methods: This research used observational analytic method with case control approach. Data were taken from registration book in VK Bersalin (Delivery Room) of The General Hospital of Ulin Banjarmasin and patient’s medical record. The sample of this research was 30 samples for each case and control group which used simple random sampling. Results: In maternal mortality group, 50% is caused by preeclampsia-eclampsia and 50% is caused by non preeclampsia-eclampsia. In non maternal mortality group, 10% is caused by peeclampsia-eclampsia and 90% is caused by non preeclampsia-eclampsia. Chi-square test shows a significant result ('=0,001) and Odds Ratio (OR=9,000). Conclusions: Preeclampsia-eclampsia has 9 times higher risk of maternal mortality than non preeclampsia-eclampsia.

Abstract

Background: Hunter’s syndrome is characterized by congenital metabolic defect due to lysosomal enzyme deficiency. In this syndrome, glycosaminoglycans, a type of mucopolysaccharide cannot be metabolized, and therefore its deposit accumulates in tissue. This substrate deposits progressively over time and eventually interfere with organ function and causes symptoms. Multiple surgeries would be needed to counter its effect throughout patients’ life. Anesthesiologists are expected to prepare the patient for anesthesia, manage intraoperative, and provide postoperative care. The substrate accumulation on airway presents a special challenge for airway management. We describe a case of difficult airway management in a patient with Hunter’s Syndrome who was under enzyme therapy. The main airway challenge are short neck and limited neck extention. Glycosaminoglycans in joints, spine, and heart also require anesthetist’s attention. Drug metabolism in Hunter Syndrome may be different from normal. As this is a rare disease, patients with Hunter’s syndrome is recommended to undergo anesthesia with experienced anesthesiologist in health care facilities with experience in provide care for Hunter’s syndrome.
