Abstract

Objective : This article objective is to describe cytology diagnosis difficulties of yolk sac tumors of the ovary.Method : Case reports and literature review.Case : The author reports the case of a 24 year old woman who complained of an enlarged stomach. Serum AFP increased to 16,519.7 U/mL. Ultrasound examination revealed solid and irregular mass of ovarian, so the conclusion was suspect ovarian carcinoma. Conclusion of CT scan examination was a solid ovarian tumor. The working diagnosis was suspect ovarian carcinoma. Optimal debulking was performed, accompanied by taking a sample from the peritoneal rinse fluid. Microscopic examination of peritoneal fluid showed the distribution and group of cells with pleomorphic nuclei, partly hyperchromatic, partly vesicular with coarse chromatin and prominent nucleoli. There were also cells with polygonal nuclei, small nuclei, basophilic and vacuole cytoplasm with a mucoid background. These cells formed a solid arrangement. Conclusion from these features was carcinoma metastases to the peritoneal fluid. Microscopic examination from tumor tissue sample showed an ovarian yolk sac tumor appearance.Conclusion : Cytologic examination of peritoneal fluid in cases of ovarian yolk sac tumor is quite difficult to determine the diagnosis. This is due to the microscopic appearance of tumor cells which often looks like a carcinoma and limited literature about this tumors in the peritoneal fluid.Keywords: Yolk sac tumor, ovary,

Abstract

The incidence of labor induction with various indications was to increase. The key to successful labor induction is a ripe cervix. Research shows that the degree of cervical rippening as assessed by the Bishop score is influenced by the level of neutrophils contained in the cervical stroma which can be detected by performing a vaginal swab. Neutrophils will produce collagenase in the form of matrix metalloproteinase - 8 (MMPs - 8) which will degrade cervical collagen fibers, so that the cervix becomes soft and ripe. Misoprostol is the drug most widely used in labor induction today. Purpose: This study was to determine the difference in Bishop score increase between vaginal swab neutrophils ≤ 5 and > 5 in pregnancy ≥ 41 weeks induced by misoprostol.Keywords: Bishop score, neutrophil, vaginal swab, misoprostol

Abstract

The purpose of this study is to know the difference average of maternal serum levels of HIF-1α between early-onset and late-onset severe preeclampsia. This study used a cross sectional comparative study design that conducted in Februari 2020 - Agustus 2020 in the SMF / Obstetrics and Gynecology department of RSUP dr. M. Djamil Padang, RSUD Achmad Mochtar, RSUD Pariaman, RSUD M Zein Painan. We used consecutive sampling method which consists of 60 pregnant women who fulfill the inclusion and exclusion criteria. They were divided into two groups early-onset severe preeclampsia and late-onset severe preeclampsia. HIF-1α tests were done using ELISA method. The average of maternal serum levels of HIF-1α in late-onset severe preeclampsia is found to be the highest when compared to the early-onset severe preeclampsia, 1,37 ± 1,08 ng/ml vs 0,69 ± 0,11 ng/ml. This difference is significant with the Mann-whitney non parametrical statistical test (p <0.05). Abstract

Premature ovarian failure (POF) or premature menopause refers to development of amenorrhoea due to cessation of ovarian function before the age of 40 years. The diagnosis is based on elevated FSH levels in menopausal range (usually above 40 IU/l) detected on at least two occasions a few weeks apart. The term “epilepsy” defines a group of disorders characterized by an enduring predisposition of the brain to produce seizures. The hypothalamic-pituitary-ovarian axis regulation is affected by the abnormal neurophysiology of seizures, and the hypothalamic-pituitary-ovarian associated hormones are affected by medications used to treat seizures in women with epilepsy. Ictal and interictal discharges can disrupt the normal activity of brain structures, including the limbic system, amygdala, hypothalamus, and pituitary gland. Management of epilepsy in women requires not only knowledge of epilepsy, but also recognition of the various roles and priorities women have in their lives (education, career development, child rearing, the role as carer within the extended family), and attention to gender-specific issues and their impact on patients’ wellbeing throughout life.Keywords: premature ovarian failure (POF), epilepsy

Abstract

Breastmilk (ASI) is the best food produced by the breast glands, containing all the nutritional elements needed by infants aged 0-6 months. Apart from nutrients, breast milk also has other non-nutritional components that act as immune factors for babies. Babies who are not exclusively breastfed will be more susceptible to chronic diseases, such as heart disease, hypertension, and diabetes as adults and can suffer from malnutrition and obesity. Dietary intake significantly affects milk production. The mother's diet can affect the composition of her milk through several metabolic pathways, especially the concentration of fatty acids and vitamins that are soluble in fat and water, including vitamins A, C, B6, and B12. Lauric acid contained in breast milk can help your minor fight infection and increase immunity to protect from various diseases. Not only for the baby, but virgin coconut oil is also starting to be commonly used by dieters. Virgin coconut oil contains lauric acid CH3 (CH2) 10COOH 50% and caprylic acid CH3 (CH2) 6COOH 7%. Both of these acids are medium-chain saturated fatty acids that are quickly metabolized and have anti-microbial properties. Abstract

Objective: To report the case of primary fallopian tube carcinoma metastatic to cervix and omentum, but not infiltrate the ovariumMethod: Case Report Case: A 40 years old woman presented with gradual onset pain and swelling of the abdomen. The patient was diagnosed with suspected ovarian carcinoma. After examination, the patient prepared for laparotomy; salpingo-oophorectomy, omentectomy, mass resection from pelvic and rectum  Result: The hystological examination result was high grade serous carcinoma from the fallopian tube metastasis to cavum douglas and omentum. This case is a rare case because the tubal carcinoma was metastasis to cavum douglas and omentum without infiltrate the ovarium.Discussion: Primary carcinoma of the fallopian tube is rare and accounts for about 0.14-1.8% of all gynecological malignancies. Clinically the diagnose of tubal carcinoma very similar to ovarian cancer, the diagnose can not distinguished pre operative. The new staging of FIGO may be reference for diagnostic and treatment of the desease. Thorough staging laparotomy is an important part of early management and systemic treatment. Abstract

Background: Cancer is the second leading cause of death in the world. Cervical cancer is in the 9th position out of 35 types of cancer causing the most deaths in the world. Cervical cancer is the second highest type of cancer after breast cancer in women, which affects more than 1.4 million women worldwide. Every year more than 460,000 cervical cancer incidents occur and about 231,000 women die from the disease.Objectives: This study aims to determine the relationship between husband's support, level of knowledge and motivation, with the behavior of IVA examination in fertile aged women in the city of Padang.Methods: This research is an analytic observational study with  cross-sectional comparative study approach that looks at the relationship between knowledge, motivation, and husband's support with IVA examination behavior in women of childbearing age.Results: The percentage who did not perform IVA examination was higher among respondents with low motivation compared to those with high motivation, namely 54.7% versus 45%. Statistically this difference was not significant (p> 0.05). The number of respondents who did not perform IVA examinations was higher for respondents with less husband's support, namely 32 respondents (42.1%) and more than half of respondents who did IVA examinations, namely 44 respondents (57.9%) who received less support from their husbands. Statistically this difference was significant (p <0.05).Conclusion: There is a relationship between the level of knowledge and the behavior of the IVA examination, there is no relationship between motivation and the behavior of the IVA examination and there is a relationship between the level of knowledge and the behavior of the IVA examination.Keywords: IVA, women of childbearing age, education

Abstract

Background: HIV (human immunodeficiency virus) is a virus that attacks the body’s immune system that can cause immunodeficiency. Immunodeficiency results in increased susceptibility to fungal infections. The other opportunistic infection, condyloma acuminate is prevalent in groups at high risk for HIV acquisition and in HIV-infected individuals. Case: There were ring-shaped reddish patches with scaly and itchy on the right back since 3 weeks ago. Patients often scratch it especially when sweating and hot weather. The itchy reddish patches with scaly were increased in number and size on the right back then spreading over the edge to form ring-like patches. Patient complained ring-shaped reddish patches getting more red, scaly, itchy, and wider. 6 months ago, there was a lump that sometime felt itchy in below part of vulva. Patient in reproductive age and not married yet, but sexually active. Patient had a sexual relation with male who HIV 2 years ago. Patient also suffered HIV and got ARV since 6 month ago. There was no protection (condom) during the sexual intercourse. Dermatologic state in right back with efflorescence is plaque erythema with annular concentric ring forms, scale, and blackish crust. Venereologycal state show vegetation in vulva with size 0,2 - 1,5 x 1 x 0,5 cm with verrucous surface. Acetowhite test is positive result. KOH 10% of skin scrapping show fungal element and fungal culture result is tricophyton rubrum. Patient was diagnosed tinea corporis (TC) and condyloma acuminata (CA) genital griseovulfin 900 mg, cetirizin 10 mg, ketokonazol 2% cream twice a day, tricholoroacetat 90% and showed good improvement.Discussion: Diagnosis tinea corporis and CA genital based on clinical findings and mycological examination. Early diagnosis and proper treatment are important in HIV patients in order to prevent severe infection. After 4 weeks treatment, show improvement on TC however CA lesion no reduced. Abstract

Preeclampsia is a hypertensive disorder in pregnancy that occurs in 5-10% of pregnancies and occurs after 20 weeks of gestation and recovers spontaneously after delivery. Several studies have stated that one of the risks of hypertension in pregnancy is related to magnesium homeostasis. Magnesium plays an important role in forming new tissues (maternal and fetal). Pregnant women need a higher intake of magnesium than non-pregnant women of the same age. Magnesium deficiency during pregnancy not only causes problems for the nutrition of pregnant women and fetuses, but also associated with the occurrence of preeclampsia, preterm labor and muscle cramps during pregnancy. This study aims to determine the relationship between the average increase in blood magnesium levels with the incidence of preeclampsia in hypomagnesemic pregnant women.Keywords: preeclampsia, hypertension, blood magnesium levels

Abstract

Introduction : Wounds are defects of normal anatomical structures and functions. Injuries can occur because of physical, chemical, or biological trauma. Wound healing is a complex process involving many cells, cytokines, growth factors, proteases, and extracellular matrix (ECM) that work together to restore the integrity of the injured tissue. Virgin Coconut Oil is an oil saturated with medium-chain fatty acids (MCFA), one of which is lauric acid. Based on our research, VCO has wound healing properties. Research conducted in vivo on rabbits and rats experienced an increase in collagen, fibroblast proliferation, and neovascularization which influenced accelerating wound healing.Objective : This study aims to determine the difference in the mean REEDA scale in abdominal incision wounds after obstetric / gynecology laparotomy with topical VCO and those without topical VCO.Material and methods : This study is an experimental research design with a randomized clinical trial that was conducted on 36 people divided into 2 groups (treatment and control). Post-obstetric / gynecology laparotomy patients were subjected to topical VCO application on the incision wound and the REEDA scale was calculated at 24, 48, and 72 hours post-surgery. Statistical analysis is using the Mann-Whitney test.Results : Age mean of the respondents in the treatment group was 32.28 ± 10.42 years old, while the untreated group was 30.5 ± 6.54 years old. The mean REEDA scale for abdominal incision wounds applied topical VCO at 24 hours was 3.17 ± 1.04, at 48 hours was 2.11 ± 1.08, and at 72 hours was 1.39 ± 0.85. The mean REEDA scale for abdominal incision wounds that were not treated with topical VCO at 24 hours was 3.83 ± 0.99, at 48 hours was 3.11 ± 1.37 and at 72 hours was 2.72 ± 1.41. The findings of the experimental analyses showed that there was no association with the use of topical VCO in surgical wounds on a 24-hour REEDA scale (p>0.05), there was a relationship between the use of topical VCO in surgical wounds on a 48-hour and 72-hour REEDA scale (p<0.05). In the care group, the mean REEDA scale decreased by 33.4 % from 24 to 48 hours and 34.1 % from 48 to 72 hours.Conclusion : There is a relationship with the use of topical VCO in post-abdominal incision wounds post obstetrics/gynaecology laparotomy with REEDA scale 48 hours and 72 hours post-surgery. The mean REEDA scale decreased by over 24 to 48 hours and 48 to 72 hours in the treatment group (who administered topical VCO in postoperative incision wounds).Keywords: VCO, REEDA scale, Wound healing
