Abstract

Respiratory Distress of Newborn (RDN) is a common cause of newborns respiratory distress. The RDN appears within a few hours after birth, and mostly often appears soon after birth. RDN usually has symptoms of tachypnea with a respiratory rate of more than 60 breaths per minute accompanied by grunting, chest retractions, nostril breathing, and cyanosis. Asphyxia is one of the causes of RDN. In addition, asphyxia can cause an increase in bilirubin levels in newborns leading to hyperbilirubinemia. This study was to observe the patient's breathing pattern and bilirubin levels among infants with RDN. A descriptive study, specifically case study design was utilized. The subject of this case study was baby H who diagnosed with RDN with CPAP installed on the 4th day. Results showed that monitoring vital signs for 3 days can improve breathing patterns and prevent respiratory failure. Meanwhile, phototherapy along with bed transfer has been shown to reduce bilirubin levels from bilirubin levels before the intervention, namely 20.65 mg/dl to 7.57 mg/dl after the intervention. Respiratory monitoring can improve respiratory rate. In addition to phototherapy, infants can also be given bed rest while monitoring vital signs to treat hyperbilirubinemia in RDN infants.

Abstract

Patients with Chronic Obstructive Pulmonary Disease (COPD) can have low self-efficacy due to symptoms of shortness of breath and cough. Proper use of an inhaler is able to overcome these symptoms. The majority of COPD patients are still using inhalers incorrectly. Inhaler use training can improve the technique of inhaler use and is expected to improve self-efficacy in COPD patients. The purpose of this study was to determine the effect of inhaler training on self-efficacy in COPD patients. The research design used a quasi-experiment with a nonequivalent control group pretest-postest. Thirty-six patients with COPD were selected using consecutive sampling and were divided into groups that followed the inhaler training and those who did not follow the inhaler training. The results showed that there was a significant improvement in self-efficacy in COPD patients by 9.33 score after the inhaler training (p=0.001; p<0.05) and there was a significant difference in self-efficacy change of 6 scores compared to COPD patients who did not follow the inhaler training (p=0.000; p<0.05). In conclusion, there was an influence of inhaler training on self-efficacy in COPD patients so inhaler training is recommended to that nurses to be given to COPD patients.

Abstract

Diabetes mellitus is known as "the silent killer" because it can affect all body organs and cause complications, including complications in the feet. These complications can increase the rate of amputations and are life-threatening. Prevention can be done by doing foot exercises. In principle, leg exercises are done by moving all the joints of the legs to improve blood circulation. Blood circulation in the leg area can be measured through the Ankle Brachial Index examination. This study was to identify the effectiveness of diabetic foot exercise. The article search begins with conducting a PICO analysis, then continues with an electronic search for articles using Google Scholar, the year the article was published from 2016 to 2020. The keywords used are "Diabetes Mellitus" and "Leg exercise.” Screening is carried out through inclusion and exclusion criteria to determine the articles to be reviewed. So that articles were obtained that matched the inclusion criteria for review. Results showed nursing interventions for foot exercise on the ankle-brachial index in patients with type II Diabetes Mellitus can be categorized as safe and effective interventions that are given five times a week or for 30 minutes for 5 days or 6x2 a week to reduce the risk of injury to the ankle. foot. Performing effective foot exercises can increase the value of the Ankle Brachial Index to normal. 30-minute foot exercise for 5 days or 6x2 a week should be promoted and implemented as a routine to reduce the risk of injury in patients with diabetes mellitus. In conclusion, foot exercise in type II DM patients can increase the Ankle Brachial Index value which tends to be low to normal.
