Abstract

Introduction: Indonesian Research of Basic Health 2019 show that 1.5% of Indonesia's population suffers from coronary heart disease (CHD). Coronary heart disease has an impact on all aspects of the patient's life. Good health-seeking behavior is expected to prevent the severity from getting worse, and the risk of complications, up to death. This study aims to analyze the relationship between knowledge, attitudes, perceptions about health-illness and the ease of access to health service providers with health-seeking behavior in CHD patients. Methods: This study used a correlational descriptive design with a cross sectional approach. The population is CHD patients who are registered in the 3 work areas of the Gresik District Health Center. A large sample of 80 patients, using a purposive sampling technique. This study used questionnaires of health-seeking behavior in CHD patients, knowledge, attitude, perception about health-illness, and a questionnaire to measure the the ease of access to health service providers. Data analysis used the Spearman Rho test. Results: Major partial CHD affects patients aged 46-55 years and of good sex. There is a relationship between knowledge and health-seeking behavior in CHD patients (p = 0.001 and r = 0.367), there is a relationship between attitudes and health-seeking behavior in CHD patients (p = 0.000 and r = 0.737) and there is a relationship between perceptions of illness and health-seeking behavior in CHD patients (p = 0.001 and r = 0.361). Conclusion: The higher the knowledge and perceptions about illness and health, the more supportive the patient's attitude will be, and the better the health-seeking behavior in CHD patients. It is hoped that education and assistance can be given to CHD patients so they are more aware of the importance of appropriate treatment behavior.

Abstract

Introduction: The emergency department (ED) is at the forefront of hospital healthcare and is expected to provide timely and effective care. The imbalance of the need for emergency care and the availability of hospitals to provide health services has an impact on the occurrence of overcrowded situations. This study aims to determine the relationship between the implementation of triage and the incidence of overcrowded in the ED of Adi Husada Kapasari Hospital. Methods: This study uses a correlation study design with a cross sectional approach. The population in the study were all nurses in the ED of Adi Husada Kapasari Hospital, Surabaya, Indonesia, and 14 nurses were involved as respondents through a total sampling technique. The independent variable is the implementation of triage, the dependent variable is the incidence of overcrowded. Data collection using a questionnaire sheet to measure the implementation of triage, and the NEDOCS scale (National Emergency Department Overcrowding Scale) to measure the incidence of overcrowded. Data was analyzed using Spearman rank correlation test with α = 0.05. Results: The results showed that more than half respondents (64.3%) practice triage in moderate level, and the overcrowded assessment showed all respondents (100%) in the level 1, which means it was in category not busy. Spearman rank correlation test showed that there was no relationship between the implementation of triage and the incidence of overcrowded in the ED of Adi Husada Kapasari Hospital (p-value 0.898 and r=0.038). Conclusion: For non-large-scale emergency departments like the ED of Adi Husada Kapasari Hospital, the implementation of triage is not significantly related to the overcrowded incidence, although some studies and theories state otherwise, that the accuracy of emergency room nurses in triaging can help reduce patients overcrowding and treat patients immediately. Further research could explore this gap through a larger sample or by involving or comparing more populations of various type of EDs.

Abstract

Introduction: Diabetes is a chronic disease characterized by abnormal blood sugar levels due to pancreatic disorders.  Quality of life and ankle brachial index values are an area of concern for nurses related to non-pharmacological treatments to prevent complications. Progressive Muscle Relaxation (PMR) can improve relaxation and cellular immunity, as well as Neuromuscular Taping (NMT) is a non-pharmacological intervention to prevent lower extremity peripheral blood circulation decline, so both have a potential positive impact on the patient's quality of life. The aim of this study is to analyze the effectiveness of PMR and NMT on ankle brachial index (ABI) and quality of life in Type 2 Diabetes Mellitus (T2DM). Methods: The study was a quasy-experiment with a pre post test control group design. Consecutive sampling was implemented and resulted a sample of 32 T2DM patients with certain criteria, such as patients who were treated in the internal medicine ward, age 40-70 years, suffering from T2DM for more than5 years, not suffering from diabetic ulcers, and still can do some exercise. Data were analyzed using the Wilcoxon Signed Ranks Test with correlation test with alpha 0.05. Results: There was a significant effect of the PMR-NMT combination intervention on ABI (p= 0.034) and quality of life (p= 0.007) in the intervention group. Conversely, the control group showed no difference in ABI (p= 0.142) and quality of life (p= 0.739) before and after the study. Conclusion: PMR and NMT can improve ABI values and Quality of Life in patients with T2DM. This PMR-NMT combination can be an alternative non-pharmacological therapy in patients T2DM to help improve ankle brachial index and quality of life.

Abstract

Introduction: Leukemia is one of the most diagnosed types of cancer and a cause of death worldwide. Patients who experience severe side effects of cancer treatment tend to experience less than optimal self-care management. Leukimia patients in treatment often complain of fatigue, weakness, lack of energy, and nausea which hinder them in self-care management. The purpose of this study was to determine the relationship between motivation and self-care management. Methods: This study used an analytic descriptive research design with a cross sectional approach. Population on this research were members of Chronic Granulocytic Leukemia (it is known as "ELGEKA”), East Java, Indonesia. The sample size was 69 respondents using purposive sampling. The independent variable was motivation and the dependent variable was self-care management. Data was analyzed using Spearman Rho analysis with level of significance 0.05. Results: The results of this study indicated that there was a relationship between motivation and self-care management (p=0.000) and  (r=0,531). Discussion: Motivation has a significant relationship with self-care management in leukemia patients based on Orem's theory. The higher the motivation, the higher self-care management. Good self-care management involves nutrition and fluid management, physical activity, stress management, complementary therapy, information and support seeking, and medication adherence. Conclusion: Nurses have an important role in identifying and increasing patient motivation to carry out self-care management and changing their attitudes to improve health and disease care. The recommendation for future researchers is to develop research with interventions to improve self-care management in leukemia patients.

Abstract

Introduction: The response time in handling prehospital emergencies has not been fully optimal so that people still complain about it. Appropriate response time will reduce mortality and morbidity rates so that patient satisfaction can be met. The aim of this study was to describe the emergency response time and its determinants in the Quick Response Team (it is called "Tim Gerak Cepat”) Surabaya.  Methods: This research was a descriptive research design with a cross sectional approach. The population of the study were all nurses in Tim Gerak Cepat (TGC) at the Integrated Command Post Surabaya totaling 126 nurses and a sample of 96 respondents were taken by purposive sampling. The variables were abilities, skills, education, emergency training, length of service, and motivation of nurses as well as the response time in handling emergencies. Collecting data using a questionnaire and presented in descriptive analytics.  Results: The research results showed that the majority of respondents had high abilities (90.6%) and skills (93.8%). The majority of respondents had achieved a third diploma (D3) in nursing (82.3%) and had undergone basic emergency training (88.5%). Furthermore, more than half of the respondents had worked for more than 3 years (68.7%) and showed moderate motivation (51%). In general, the majority of respondents had a response time of ≤ 8 minutes (79.2%). Conclusion: Nurses at TGC Surabaya actually have a good response time. This is supported by high capacity and skills in emergency situations, as well as educational background, including emergency training. But nurses still need to increase self-motivation. Further research can explore further what are the priority determining factors for emergency response time, especially in Surabaya as the 2nd largest city in Indonesia, in order to assist the government in making the best setting and organization at TGC Surabaya.

Abstract

Introduction: One of the problems often found in patients undergoing hemodialysis is changes in their nutritional status. Patients with hemodialysis who are malnourished have a higher risk of disease and death. This nutritional disorder can be overcome by assessing nutritional status. Apart from using the SGA (Subjective Global Assessment) method, the MIS (Malnutrition Inflammation Score) method can also be used. The aim of this study was to compare differences in nutritional status of patients on hemodialysis using the SGA and MIS instruments.   Methods: This research was a quantitative research with a cross-sectional design. The research sample consisted of 75 patient with hemodialysis at one of military hospital, who were selected using non-probability techniques with a total sampling approach. The statistical test used is the Mann-Whitney test with level of significance 0.05 to compare differences between variables.   Results: Using the SGA tool, it was found that more than half of respondents (66.7%) were in the moderate nutrition category, while only 9.3% were in the malnutrition category. In contrast, with MIS tools, it was found that 34.7% were in the mild malnutrition and not malnourished category, while 65.3% were in the moderate to severe malnutrition category. Furthermore, statistical analysis shows that there is a significant difference (p = 0.001) between the use of SGA and MIS in assessing nutritional status in patients with hemodialysis.   Conclusion: It can be concluded that there is a difference between the use of SGA and MIS in assessing nutritional status in patients with hemodialysis. Further research, could explore more for the advantages and disadvantages of each instrument, including the level of accuracy and sensitivity.
