Abstract

Recurrent abdominal pain is the most common symptom experienced by children around the world and causes high rates of children absent from school. Gastroduodenitis is a disease with abdominal pain symptoms. The initial clinical symptom was dyspepsia. In most pediatric cases the etiology is unknown (idiopathic). This study presented a case of gastroduodenitis in a 10-year-old boy hospitalized with the chief complaints of heartburn, accompanied by vomiting. The patient has a history of the same complaint in the last 3 months ago, causing the patient to be admitted to the hospital repeatedly. Pain is not relieved by defecation or flatus. On physical examination, tenderness was found in the epigastric and umbilical regions. Chronic gastroduodenitis was confirmed after endoscopic examination and tissue biopsy. Therapy including proton pump inhibitors and lifestyle modifications can improve the child's condition. In the case of gastroduodenitis, it is important to prevent complications, so proper examination and prompt treatment are needed for the sufferer.

Abstract

Turner syndrome (TS) is a rare genetic disease that occurs only in girls and is the result of the complete or partial absence of the X chromosome. TS has often delayed diagnosis in late childhood or adolescent age and is rarely identified during the neonatal period. The clinical features are primary amenorrhea, short stature, infertility, and characteristic dysmorphic features. Late diagnosis is the main problem because early detection and appropriate management can improve the final height, sexual health and psychological development of patients. We report a case of turner syndrome in a 16-years and 4-months old female adolescent. The patient had specific clinical features of turner syndrome such as amenorrhea, absence of secondary sex growth and posture short stature since 14 years old. This case was confirmed from anamnesis, physical examination and chromosomal analysis, which demonstrated a gene karyotype of 45, X monosomy.

Abstract

Tuberculosis is still a threat to public health in Indonesia. Based on WHO, TBC Report 2020, TBC Case in Indonesia in 2019 is estimated at 845,000 cases with the incidence of 312 per 100,000 population, so Indonesia be the second largest TBC cases of the world after India. The family's input approach provides holistic treatment by improving family functions. The Primary data was obtained through anamnesis (Autoanamnesis), physical examination and home visits to assess the home condition and relation with the family. The secondary data were obtained from the patient's medical record in Hospital. Assessment is based on the holistic diagnostic. The result is presented in the case report. The patient who has a functional degree 1 with tuberculosis has an internal risk factor that does not pay attention to the initial complaints so the complaints increase weight, patient awareness efforts in avoiding risk factors and lack of knowledge of the disease. As for External risk factors are the lack of Family knowledge about the disease and home environment which is so close to the farm. Then do educate the patient and family about drugs that should he consume and eat right, side effect of the drugs and changes in his lifestyle. After the evaluation is compliance in taking medicine and do healthy lifestyle. The complex clinical activities take a long time and cooperation between health family workers. Where officers not only resolve the patient's clinical issues but also seek and provide solutions to problems - the problems in an environment affecting the health of patients and families.

Abstract

Background: Chronic low back pain (CLBP) is one of the most prevalent and expensive musculoskeletal issues in contemporary culture. It is defined as low back pain that lasts for more than three months or longer than the anticipated recovery period.[1] 70 to 80 percent of adults have dealt with CLBP at some point in their lives. This study sought to assess the McKenzie method's contribution to the treatment of low back pain and determine whether it could be compared to other forms of therapy. Content: A narrative review of the literature was performed using the following search engines: PubMed, Cochrane, and Scopus. Inclusion criteria were articles published in the last 10 years, randomized clinical trials, the mean age of patients between 18 and 70 years and full English text. Exclusion criteria were observational studies, case reports and articles without abstract or full text. The outcomes that were used to review included chronic pain, disability, Quality of Life (QoL) and psychological aspects. Conclusion: Even though it has helped treat patients with chronic low back pain, the McKenzie method needs more research to be proven effective in particular patient populations. Abstract

Introduction: Infectious keratitis is one of the main causes of visual impairment. The incidence of acanthamoeba keratitis is relatively rare, while the prevalence is around 1 to 9 cases per 100.000. One of the factors supporting the occurrence of acanthamoeba keratitis is the increasing use of contact lenses around the world. Content: When there is microtrauma to the cornea, it can cause the expression of the mannose on the surface. With the emergence of mannose in the form of glycoproteins, it will be a requirement for the attachment of Acanthamoeba spp. Conclusion: Keratitis Acanthamoeba is an infectious disease of the cornea due to the use of contact lenses that are too long exposed to water which causes the entry of Acanthamoeba. This disease has a good prognosis with immediate drug administration within the first 3 weeks after the onset of symptoms.

Abstract

Introduction: Caisson disease is one of the most common emergencies in divers with very common signs and symptoms that clinicians must be careful to identify based on a complete history and physical examination. The disease also requires immediate and definitive management, usually hyperbaric therapy for a better prognosis. Content: This article discussed the types, pathophysiology, and management of caisson disease caused by the release of nitrogen gas bubbles into the blood or tissues during or after a pressure drop in the environment. This is a disease that can be prevented by paying attention to existing risk factors. Summary: Caisson disease or decompression sickness is a collection of symptoms that are often encountered in archipelagic countries and can affect anyone diving for seafood or recreational activities. The symptoms are common, but by knowing the pathophysiology, clinicians can identify the disease and immediately perform therapy, thereby preventing complications such as arterial a gas embolism (AGE) so the mortality and morbidity due to caisson disease can be reduced.

Abstract

McCune-Albright syndrome (MAS) is a rare genetic disordered originally recognized by the triad of polyostotic fibrous dysplasia, precocious puberty, and cafe-au-late spots. We report a case of 2 years-11-month-old girl who came with complaints of menstruation 5 days ago. Breasts appear enlarged. On physical examination, it appears that the café au lait region of the abdomen and left femur resembles the "coast of Maine". There was an increase in the levels of FT4, estradiol, and alkaline phosphatase, and low TSHs, LH, and FSH levels. The radiological examination of the bones of the left hand corresponded to the bone age of 6 years. Bone survey photo showing multiple lytic and blastic lesions in the proximal 1/3 of the left femur. We are monitoring this patient, and planning to administer Tamoxifen and Zoledronic acid. Tamoxifen (an estrogen agonist/antagonist) or Letrozole (an aromatase inhibitor) have been used for the management of precocious puberty and rapid bone maturation. Despite differing in presentation, all patients with precocious puberty were successfully treated with Tamoxifen and/or Letrozole, to emphasize the significant progress of bone age. A systematic approach to diagnosis and management is essential to optimize outcomes for patients with MAS, especially with fibrous dysplasia. No medical therapy is able to change the course of the disease in fibrous dysplasia. However, screening and treatment for endocrinopathy can reduce some bone morbidities.

Abstract

Background: Due to geographic conditions, Indonesia has gotten exposed to sun rays consistenly for the whole year. Besides, Many parts of the Indonesian coastline are open areas while at the same time there's not much place to take shelter caused exposure to sun rays become more frequent. This condition puts the people around coasts at high risk of Age-related macular degeneration (AMD). Content: AMD is a chronic eye disease that causes central vision impairment due to degeneration of retinal photoreceptor. 8 millions of people are enduring blindness due to retina complications not excluding AMD. Causes for AMD are ranging from sun exposure, smoking habit, diet, hypertency, and genetic factors. Blue light and ultraviolet light that is contained within sun rays are the main cause of harm to tissues in the retina and reduces vision function. In preventing AMD progression, photodynamic laser therapy, photocoagulation, and anti-vascular endothelial growth factor (anti-VEGF) therapy is highly recommended. Conclusion: Best action that can be done currently is preventing it by holding counseling sessions and increasing public awareness through education and therapies like laser therapy and anti-VEGF won't cure AMD but will slow down the progression, especially wet type AMD.

Abstract

Background: Glioblastoma (GBM) is a malignant brain tumor that can occur in children and is very aggressive which means it can grow and spread quickly. Approximately 16% of all primary brain and central nervous system neoplasms are glioblastomas. This makes GBM a major public health problem. The incidence of glioblastoma in children varies.glioblastoma Low-grade has a higher incidence in children aged 0 to 14 years, which is 1.8/100,000 while high-grade glioblastoma is 0.5/100,000 lower than low-grade glioblastoma. where the incidence rate of glial and neuroepithelial tumors is higher in children aged 0-14 years. Content: Various factors such as genetic and environmental factors have been studied in terms of the etiology of glioblastoma. GBM develops more frequently in children with specific genetic syndromes, such as neurofibromatosis 1 (NF1), Li-Fraumeni syndrome, hereditary nonpolyposis colon cancer, and tuberous sclerosis. However, most cases of GBM have no known cause. Symptoms of glioblastoma are non-specific. The duration of symptoms is usually short and lasts for several months Summary: In children, central nervous system tumors are the most common, with 3-15% being glioblastomas. Glioblastoma usually presents with non-specific symptoms and lasts a short duration of several months. In addition, the prognosis is also poor with high morbidity and mortality so that to improve the patient's quality of life, effective therapy is needed. Therapy can be in the form of surgery, radiotherapy, or chemotherapy.
