Abstract

Introduction: Noise Induced Hearing Loss is a person's hearing loss either partially or completely which is permanent and can affect one or both ears caused by continuous exposure to noise in the work environment. One of the causes of Noise Induced Hearing Loss (NIHL) is working as a motorboat fisherman. Content: NIHL can arise as a result of the interaction between genetic factors and environmental factors with the role of internal and external risk factors. The diagnosis of NIHL can be established through history taking, physical examination such as otoscopy and tuning fork tests, as well as supporting examinations in the form of pure tone audiometry. Conclusion: NIHL will produce the impression of sensorineural deafness with a permanent nature so that the only treatment that can be given is supportive and preventive management. Keywords: Noisy; noise-induced hearing loss; motorboat fisherman

Abstract

Typhoid fever is an infection caused by the bacterium Salmonella typhi. The prevalence is still high among emerging countries, such as Indonesia. There are limited options for the treatment of typhoid fever in children and more so is complicated by the emergence of antimicrobial resistance. The use of azithromycin has evolved as an important drug in the treatment of typhoid. A male, aged 7 years and 2 months, was admitted to our hospital with remittent fever 4 days prior. The patient also presented with a period of cough since 2 days, and constipation, with full consciousness. The child had basic immunization completed for his age. He had a history of frequent consumption of street food. The diagnosis of typhoid fever was confirmed by serologic testing of positive 8 IgM Salmonella. Initially, the patient was treated with intravenous antibiotics of ceftriaxone for 8 days but shoed no clinical improvement, then the treatment was changed to intravenous azithromycin for 4 days and his condition improved greatly. The patient showed a better response with the use of azithromycin. The management of typhoid fever remains challenging up to this day with variations in treatment response. From this case, we can conclude that azithromycin in some patients may show better improvement in the management of typhoid fever in pediatric patients. Although, ceftriaxone and other third-generation cephalosporins are still highly effective against Salmonella typhi.

Abstract

Background: Indonesia is a country consisting of islands and oceans that are rich in potential natural resources. Abundant marine products make the Indonesian people empower fish catches into long-lasting salted fish processed foods. People living in coastal areas have a high level of consumption of salted seafood products, such as salted fish. Consuming salted fish is one of the factors causing the incidence of nasopharyngeal carcinoma. Contents: Nasopharyngeal carcinoma (NPC) is a type of cancer found in the head and neck. NPC cases can be caused by various factors, one of which is caused by environmental factors. Environmental factors that can cause the incidence of NPC are environments that have a population of both passive and active smokers, formaldehyde exposure, and consumption of salted fish. Clinical manifestations that can arise in patients with NPC, such as epistaxis, nasal congestion, ringing in the ears, headaches, facial pain, and lumps can be seen on the top of the neck of the patient. Conclusion: NPC is the fourth most common malignant tumor in Indonesia after cervical cancer, breast cancer, and skin cancer. NPC cases are most commonly found in men compared to women in the elderly age group, in the range of 45-55 years. Several treatments such as radiotherapy, chemotherapy, and surgical intervention can be done to treat NPC cases.

Abstract

Introduction: Skeletal muscle produces moderate quantities of oxidant species, such as reactive oxygen species (ROS) and reactive nitrogen species (RNS), due to its contractile action, high oxygen consumption, and metabolic rate (RNS). Under normal physiological circumstances, the generation and removal of ROS/RNS are in dynamic equilibrium. Content: The body reaches a condition of oxidative stress, however, when the oxidation products surpass the antioxidant defense capability. Increased oxidative stress has significant ramifications for the molecular, structural, and functional integrity of muscle. The release of reactive oxygen species (ROS) under pathological circumstances leads to cellular dysfunction and the progression of muscle disorders. Conclusion: The antioxidants can put ROS in optimal concentrations to perform physiological signals in muscle. At appropriate concentrations, ROS and RNS can regulate intracellular signal transduction. Thus, moderate quantities of radicals are advantageous to muscle, but high doses of ROS are harmful. The aim of this review is to know about the role of ROS in muscle.

Abstract

Introduction: Malaria is an infectious illness caused by various Plasmodium parasites transmitted by Anopheles mosquitoes. Currently, malaria is still a health problem with a significant incidence globally and has high morbidity and mortality. This makes researchers continue to seek solutions for malaria eradication. One of the things that is known to be protective against severe malaria infection is the sickle cell trait or HbAS. Content: Sickle cell trait is a condition in which an individual is a carrier or has a single gene that causes sickle cell disease. The way sickle cell trait protects against malaria has been suggested for a long time. Along with the development of science and technology, various mechanisms of how the sickle cell trait protects against severe malaria are becoming known. Several mechanisms include sickling, miRNA, decreased cytoadherence, and immunological mechanisms. Conclusion: Malaria is most often caused by Plasmodium falciparum. Indonesia is still an endemic country with the most frequent cases of malaria in children. One thing that is known to be protective against severe malaria, both symptomatic and asymptomatic malaria, is the sickle cell trait with various underlying mechanisms.

Abstract

Introduction: Diabetes Mellitus is a chronic disease that will be suffered for a lifetime so the progressive disease will continue and can cause complications. Diabetes mellitus usually progresses slowly with symptoms that are mild to severe and can even cause death due to acute or chronic complications. Content: Hyperglycemic crisis is one of the acute complications that can occur in Diabetes mellitus, both type 1 and type 2. This situation is a serious complication that may occur even in well-controlled diabetes mellitus. Conclusion: Diabetic ketoacidosis and hyperglycemic Hyperosmolar State are acute complications of Diabetes mellitus. Both of these complications can be triggered due to inadequate insulin therapy and the presence of infection. Diabetic ketoacidosis is a condition characterized by metabolic acidosis due to excessive formation of ketones, while a hyperglycemic hyperosmolar state is characterized by severe hyperosmolality with serum glucose levels which are usually higher than diabetic ketoacidosis.

Abstract

Biliary atresia (BA) is a condition of biliary obstruction. Kasai portoenterostomy (KPE) is a treatment option. The aim to determine the success rate of kasai procedure and prognosis in BA patients with other underlying diseases. This is a descriptive case study research. We report a boy, 2 months and 11 days with jaundice all over his body since one week old; putty stool, and dark urine. Old man's face, icteric, rib xylophone, subcostal retraction, rales, continuous heart murmur, and wasting. Total and direct bilirubin, SGOT, SGPT, and gamma GT are increased. A Kasai surgical procedure was carried out, but did not significantly improve his condition because the operation was performed at the age of 4 months and 8 days with liver cirrhosis, sepsis, pneumonia, and malnutrition. Cytomegalovirus (CMV) infection and acyanotic congenital heart disease also worsened his preoperative condition. Kasai portoenterostomy is a method of managing biliary atresia, but our patient was diagnosed with biliary atresia accompanied by CMV infection, congenital heart disease, or pneumonia, so the procedures not enough for this condition, and mortality rate is higher than biliary atresia alone. Age at surgery time also influences the prognosis.
