Abstract

Introduction: Stent dislodgement is rare but represents one of the serious complications during complex PCI, potentially leading to severe consequences such as myocardial infarction, stroke, or death. Case summary: A 69-year-old male patient with triple-vessel coronary artery disease and heart failure with reduced ejection fraction (30%) underwent percutaneous coronary intervention. During deployment of a stent in the proximal left circumflex artery, the device became dislodged and embolized to the common iliac artery bifurcation. The migrated stent was subsequently retrieved successfully using the twirling wire technique. Discussion: The twisted guidewire technique is one of the simple methods for retrieving a dislodged stent. Although, it carries the risk of intimal injury or distal migration of the fragment, this technique potentially leads to serious complications. In this case, the procedure was successful and the patient remained stable. Conclusion: The operators must be proficient in recognizing and managing both common and rare complications that may occur during PCI. The interventional team should be well prepared and maintain effective teamwork to ensure prompt and appropriate responses to any unexpected events during PCI.

Abstract

Introduction: Wound healing is an important physiological process to maintain skin integrity after trauma. Contact burns are damage or loss of tissue caused by heat sources. The growing interest in natural therapies has prompted the exploration of trigona honey due to its antioxidant, anti-inflammatory, and antimicrobial properties. Another study discusses trigona honey on an incised wound, but only a few discuss burns. This study evaluates the effectiveness of Trigona spp honey as a therapy for burns in Rattus norvegicus. Methods: A quasi-experimental with a non-randomized control group pretest-posttest design method. In this study used 20 subjects, there are Rattus norvegicus of the Sprague Dawley strain weighing 100-250 grams and aged 2-3 months, divided into 5 subjects for 4 groups. This study assesses wound size and use one-way ANOVA as the analysis technique. Result: There is significant wound healing initially in all groups before intervention, average wound area is 3.5 cm. Then treatment was given for 15 days and it was seen that the group with the best wound healing was treatment on group II (Trigona honey 2 g) with an average wound area of 0.42 cm, followed by treatment group I (Trigona honey 1 g) is 0.72 cm, followed by the positive control group (silver sulfadizine 0.5 g) is 0.86 cm and then on negative control group (aquadest) is 1.74 cm. In the one-way ANOVA test, the P value (Between Groups) was 0.000 (<5%). Conclusion: Trigona honey is effective as a therapy for burns in rattus norvegicus.

Abstract

  Introduction: Hypertension is a major global health problem and a leading cause of cardiovascular morbidity and mortality. Hyperuricemia has been increasingly recognized as a potential metabolic factor associated with elevated blood pressure, although its independent role remains unclear, particularly in individuals without obesity, diabetes mellitus, or chronic kidney disease (CKD). Methods: Hypertension is a major global health problem and a leading cause of cardiovascular morbidity and mortality. Hyperuricemia has been increasingly recognized as a potential metabolic factor associated with elevated blood pressure, although its independent role remains unclear, particularly in individuals without obesity, diabetes mellitus, or chronic kidney disease (CKD). Result: Hypertension occurred in 72.5% of participants with hyperuricemia compared with 25.0% without hyperuricemia. Hyperuricemia was significantly associated with hypertension (p < 0.001; contingency coefficient = 0.429). Participants with hyperuricemia had a 2.91-fold higher risk of hypertension (RR = 2.91; 95% CI: 1.64–5.13). This association persisted across age groups, with RR = 2.41 (95% CI: 1.31–4.44) in participants aged <60 years and RR = 3.73 (95% CI: 1.25–11.16) in those aged ≥60 years. Conclusion: Hyperuricemia was associated with an increased risk of hypertension independent of major metabolic comorbidities and age, suggesting its potential role as an additional metabolic risk factor for hypertension.

Abstract

Introduction: Pulmonary function abnormalities are often underrecognized in the community, particularly in their early stages. This pilot study aimed to describe spirometry findings in a community setting and examine their associations with age, sex, body mass index (BMI), and smoking history. Methods: A pilot cross-sectional study was conducted among residents of Dukuh Kupang, Surabaya, during a community-based spirometry program. Fifty-eight participants were recruited consecutively. Variables included age group, sex, BMI category, smoking history, and spirometry results (normal, obstructive, restrictive, or mixed), which were further classified as normal versus abnormal. Data were obtained through structured interviews, anthropometric measurements, and spirometry testing. Associations were analyzed using the chi-square or Fisher–Freeman–Halton test, as appropriate. Result: Abnormal spirometry findings were identified in 32 of 58 participants (55.2%). Obstructive patterns were found in 36.2%, restrictive patterns in 13.8%, and mixed patterns in 5.2%. Abnormal results were significantly associated with age (p < 0.001), sex (p < 0.001), smoking history (p < 0.001), and BMI category (p = 0.009). Conclusion: This pilot community-based study identified a substantial proportion of pulmonary function abnormalities among residents of Dukuh Kupang, Surabaya. The findings support the potential value of spirometry as an early community-level tool for identifying pulmonary function abnormalities and informing targeted respiratory health strategies and follow-up planning.

Abstract

Introduction: Vitamin D deficiency is a global health concern, particularly among pregnant women, with prevalence rates reaching up to 40%. This deficiency poses risks to both maternal and fetal health, leading to complications such as gestational diabetes, preeclampsia, and preterm labor. Content: This study employed a literature review approach, collecting data from Google Scholar, ScienceDirect, and Technology Index (SINTA). Articles were selected based on inclusion criteria: international journals published between 2017 and 2024, focusing on the effectiveness of vitamin D during pregnancy. Exclusion criteria included journals ranked >Q3 (SJR) and >3 (SINTA). The review process involved critical appraisal, skimming, scanning, and thematic analysis. Conclusion: Findings indicate that vitamin D supplementation reduces pregnancy complications. Future research should explore the extent of its effectiveness in optimizing maternal and fetal health outcomes.

Abstract

Introduction: Alcohol-associated cirrhosis is a preventable cause of advanced liver disease, yet it is frequently first recognized after decompensation. This case underscores the diagnostic value of integrating clinical decompensating features with non-invasive fibrosis indices and ultrasonography in a resource-limited setting. Case Summary: A 51-year-old man with a 20-year history of daily alcohol intake presented with progressive abdominal distension, bilateral leg edema, right hypochondrial pain, nausea, jaundice, weight loss, and recent behavioral change. He had previous hematemesis and melena but had declined esophagogastroduodenoscopy. Examination revealed pallor, scleral icterus, ascites, right hypochondrial tenderness, and peripheral edema. Investigations showed anemia, thrombocytopenia, AST-predominant transaminitis, hypoalbuminemia, mild hyponatremia, and non-reactive HBsAg. APRI and FIB-4 were markedly elevated, and ultrasonography demonstrated a shrunken, coarse, irregular liver with ascites, supporting a diagnosis of presumed alcohol-associated decompensated cirrhosis. Behavioral changes without impaired consciousness were clinically suggestive of early hepatic encephalopathy; however, covert hepatic encephalopathy could not be formally confirmed because psychometric testing was not available. Discussion: Ascites, jaundice, cytopenia, hypoalbuminemia, and neuropsychiatric change indicated advanced decompensation, while suspected portal hypertension remained incompletely characterized because endoscopy was unavailable. Management included dietary sodium and fluid restriction, nutritional support, large-volume paracentesis, albumin infusion, empiric antibiotic therapy, acid suppression, ursodeoxycholic acid, diuretic therapy, lactulose, and a non-selective beta-blocker. Conclusion: This case emphasizes early alcohol-use assessment, abstinence-centered care, timely endoscopic risk stratification, and complication-directed management to reduce preventable deterioration in alcohol-associated decompensated cirrhosis.

Abstract

Introduction: The relationship between serum uric acid (SUA) and glycemic status in established diabetes is biologically plausible but inconsistent across studies because insulin resistance, renal urate handling, glycosuria, obesity, kidney function, and glucose-lowering therapy can influence SUA in different directions. This study evaluated the association of SUA with fasting blood glucose (FBG) and glycated hemoglobin (HbA1c) in a hospital-based sample in Semarang, Indonesia. Methods: This retrospective cross-sectional study screened 410 medical records from Sultan Agung Islamic Hospital, Semarang, during 2018–2020. Thirty records with complete SUA, FBG, and HbA1c data were available for analysis. Hyperuricemia was defined as SUA >7.0 mg/dL in men and >6.0 mg/dL in women. The revision reanalyzed the available raw dataset using Shapiro–Wilk testing, Spearman correlation, exploratory partial Spearman correlation adjusted for age, sex, and recorded obesity status, and Fisher’s exact test. Two-sided p<0.05 was considered statistically significant. Results: The mean age was 54.3±6.5 years; 16/30 (53.3%) were women and 18/30 (60.0%) met the sex-specific definition of hyperuricemia. SUA showed a moderate positive monotonic association with FBG (Spearman ρ=0.401, 95% bootstrap CI 0.053 to 0.665, p=0.028); this remained similar after adjustment for age, sex, and obesity (partial ρ=0.396, p=0.041). The association with HbA1c was weak and not statistically significant (ρ=0.270, 95% bootstrap CI -0.126 to 0.624, p=0.149). Conclusion: Higher SUA was associated with higher FBG, but not HbA1c. These exploratory findings should be interpreted cautiously because of the small, highly selected sample and incomplete data on renal function, medication use, diabetes duration, and other confounders.

Abstract

Sindrom nefrotik (SN) merupakan penyakit ginjal kronis yang banyak ditemukan pada anak. Podositopati, yang dapat terjadi melalui mekanisme imunologis maupun non-imunologis seperti stres oksidatif, merupakan hipotesis terkini dalam patofisiologi SN. Sindrom nefrotik berhubungan dengan gangguan koordinasi antara sel endotel dan podosit, terutama yang dimediasi oleh penurunan ketersediaan nitric oxide (NO) akibat disfungsi endothelial nitric oxide synthase (eNOS). Isoform nitric oxide synthase (NOS), khususnya endothelial nitric oxide synthase (eNOS atau NOS III), berperan dalam produksi nitric oxide (NO) yang penting untuk vasodilatasi ginjal, regulasi tekanan darah, dan natriuresis. NO yang dihasilkan oleh eNOS berfungsi sebagai diuretik endogen dengan menghambat reabsorpsi garam di tubulus serta memodulasi umpan balik tubuloglomerular. Disregulasi eNOS berkontribusi terhadap retensi natrium dan hipertensi pada SN, yang ditunjukkan oleh penurunan ekspresi eNOS yang berkaitan dengan proteinuria. Tinjauan literatur ini membahas peran eNOS dalam patogenesis SN, khususnya sindrom nefrotik sensitif steroid (SNSS) yang banyak ditemukan pada populasi pediatrik. Berbagai penelitian menunjukkan adanya polimorfisme eNOS, seperti eNOS4a/b VNTR, yang berkaitan dengan penurunan kadar NO plasma, progresivitas penyakit ginjal, dan kerusakan glomerulus pada kondisi seperti nefropati IgA. Model eksperimental, seperti SN yang diinduksi puromycin aminonucleoside, menunjukkan penurunan ekspresi eNOS dan neuronal nitric oxide synthase (nNOS) yang menyebabkan gangguan ekskresi natrium, sedangkan perbaikan kondisi tersebut meningkatkan natriuresis. Penelitian lebih lanjut mengenai modulasi eNOS diharapkan dapat menjadi target terapeutik potensial dalam penanganan edema dan kekambuhan sindrom nefrotik.

Abstract

Introduction: The urgency to set a new strategy at teaching and learning that improve healthcare quality based on collaborative work among different professionals, interprofessional education becomes an emerging topic, being considered an important approach in health curriculum. The aim of the study is to investigate the research hotspots and theme trends in Interprofessional education in medical student through bibliometric analysis. Methods: Relevant articles on Interprofessional education in medical student published between 2014 and 2024 were retrieved from SCOPUS. The VOSviewer was used to extract and quantitatively analyze the publications data in three dimensions such as network, overlay and density visualization. Result: A total a total of 1702 publications which we limit to those published within the year of 2014-2024 and 512 publications met all of the requirements. Afterwards, co- occurrence analysis was conducted and a network map was also developed using VOSviewer tool. The results showed the trend based on the annual number of publications; most contributing authors; most contributing countries; most published journals; keyword analyses; gaps and potential future study topics. Conclusion: This study demonstrated the publication trends regarding the implementation of interprofessional education in medical student over the past 10 years. The analysis results using VOSviewer showed that there are still some topics about interprofessional education that need to be explored in the future.
