Abstract

Hypertension is a chronic non-communicable disease that has threatened the health of Indonesian people. Hypertension complications have resulted in various types of disability and death. One of the factors causing complications of hypertension is patients’ non-compliance in taking anti-hypertension drugs. The purpose of this study was to determine the effect of humanistic nursing practice on compliance in taking medication for hypertensive patients in the working area of Public Health Center in Sikumana, Kupang City, East Nusa Tenggara Province, Indonesia. This experimental research method was conducted through a quasi-experimental design. The number of samples were 20 hypertensive patients who were taken by purposive sampling technique. The study sample was divided into two groups, namely the treatment group consisting of 10 patients and the control group consisting of 10 patients with inclusion criteria. The instruments used in the study were nurses worked as a therapist and a checklist to control the medication compliance. Therapy was given using the humanistic nursing practice therapy. The results of the study in the treatment group showed that 40% of the patients never took medication while 60% of the patients took medication occasionally. The reason for not taking medicine was because there were no complaints by 50%, herbal medicine consumption by 30% and running out medicine by 20%. The results of Manwhitney Test analysis of pre and post treatment obtained α = 0.000, z = -4.11. It can be concluded that there was a significant effect of humanistic nursing practice on the compliance in taking medication for hypertensive patients at the Public Health Center in Sikumana. Patients who were given humanistic nursing practice had 4.11 times greater compliance than patients who did not receive humanistic nursing practice services.

Abstract

The prevalence of LBW in Indonesia is 6.2% of 47,011 births, while in Bali Province LBW is 5.6% of 786 births (Riskesdas, 2018). Data shows that more than 20 million babies worldwide are estimated to be born with LBW. Hypothermia is the most common problem found in babies with LBW, so nursing actions are needed that can overcome the problem of hypothermia in LBW. Kangaroo Mother Care (KMC) is one of the nursing actions that can be attempted to overcome this hypothermic problem. The purpose of this study was to determine the effect of the Kangaroo Mother Care (KMC) method on temperature stability in Low Birth Weight (LBW) infants in the Perinatology Room of the Gianyar Regional General Hospital. This type of research is pre experimental design with one-group pre-post test design. Sampling using quota sampling technique with a sample size of 17 babies. Samples were selected based on inclusion and exclusion criteria, where one of the inclusion criteria was LBW who did not have congenital abnormalities, babies who had reflexes and had good coordination of suction and swallowing. After the sample is selected, then a pretest is carried out by measuring the baby's body temperature, then the mother of the baby is taught about KMC for 15 minutes and then asked to repeat independently for 60 minutes, and after that do a posttest, namely measuring the temperature after giving the KMC method. Analysis of the effect of Kangaroo Mother Care (KMC) on the temperature stability of infants using the statistical test paired t-test results obtained p value = 0.000 so that p value <0.05. This shows that there is an effect of the Kangaroo Mother Care (KMC) method on temperature stability in LBW babies in the Perinatology Room of the Gianyar Regional General Hospital.

Abstract

Maternal death is the worst performing of the Millennium Development Goals (MDGs) and Indonesia did not achieve its MDG #5 target in 2015. Therefore, the Indonesian government needs to review its strategies to decrease maternal deaths. One cause of maternal death is a lack of infrastructure, which can delay reaching emergency obstetric care (called Poned). This qualitative study investigated problems with priority planning in the Kupang district in order to improve access to emergency obstetric care (EMOC). A number of observations were conducted in the musrenbang (the district planning process). Interviews were carried out to gain an insight into the planning process from village to district level. A model for travel time to EMOC was created to support this study. In this study, six problems were identified in the district planning (musrenbang) regarding improving access to EMOCs. Those were (1) no planning proposal about improving access to EMOC, (2) budget constraints, (3) decision on the priority planning not being based on the level of urgency, (4) undue political influence, (5) lack of evidence and (6) incorrect measure of accessibility to obstetric care. Hence, to improve access to EMOC, scenario modelling with combined cost-benefit analyses (CBA) as an evidence-based planning should be applied in the musrenbang. It will help the health planners in Eastern Indonesia to gain a deep understanding about the problems in district planning process for access improvement to EMOC. Scenario modelling with CBA could provide the evidence during musrenbang.
