Abstract

The environment in labor can have a great effect on the fears and anxieties experienced by the mother. Anxietycan inhibit cervical dilatation, so that the perception of pain increases. Cortisol is secreted in response totension and anxiety, it affects the progress of cervical dilatation. The increasing concentration of cortisolduring pregnancy and continue to rise until labor. Anxiety is usually associated with increased pain duringlabor and modification of labor pain can be done through psychological and physiological mechanisms. Oneway to modify psychological labor pain is by setting the delivery environment. The purpose of this studywas to analyze the effect of the setting of the labor environment on the progress of labor. The results showed the average delivery progress (cm/hour) in the treatment group 1.38 (0.43) and thecontrol group 1.23 (0.52), obtained p = 0.058, this indicates there was no difference in labor progress in bothstudy groups. Based on the statistical test results of the effect of the setting of the delivery environment onthe progress of labor, the p value = 0.015 shows that there is an influence of the delivery environment settingon the progress of labor and the RR value of 3.67 (IK = 1.14-11.84), which means that the mother givesbirth in a Independent Midwifery Practice (IMP) who did not manage the delivery environment had a risk ofhaving a cervical opening < 1 cm/hour by 3.67 times compared to mothers giving birth at the IMP who didthe setting of the delivery environment. The conclusion of this research is that there is a positive influence of the delivery environment to the progressof labor.

Abstract

Introduction: Children are the main value of any society, which plays a huge role in its development andprosperity. Meanwhile, they are the most vulnerable sector that have to be protected from negative physicaland mental factors as well as diseases. A wide range of factors can affect the health of children, especially intheir first year of life. Deep knowledge on those triggers may help to reduce the risk of developing differentpathological conditions among them.Materials and Method: An integrated analysis and risk assessment of different pathologies in 767 outpatientchildren under 1 year were cunducted. A survey of the parents of those children was also conducted. Theobjectivity of the information obtained is guaranteed by the directed application of the method and techniquesused in public health and healthcare, namely, medical-statistical analysis, sociological, psychological andexpert analysis. Results: Many childrenhad a low risk of developing chronic diseases, including those with a hereditarypredisposition and had a IIB health group, these are children with background of conditions and risk factorsthat can cause the formation of a chronic pathology. The third group of healthy children was representedby children with chronic pathology in the stage of compensation, the number of such babies was small(5.9±0.85%), as well as healthy babies with I (6.0±0.86%) and II A (4.2±0.72%) by health groups.Furthermore, more than half of the children of the first year of life had a normal functional state of the body. However, in 43% of cases, the functional state was impaired, which is manifested by the presence of variouspathological conditions. Conclusion: Comprehensive integrated approach and using evidence-based medicine principles for studyingthe medical conditions in children under 1 year-old can be valuable and help us to obtain reliable data on thehealth conditions and affecting risk factors.

Abstract

Walkability Indexis the result of measuring the comfort level of pedestrian paths which will be a benchmarkfor developing pedestrian paths that are feasible for everyone. Jpedestrian paths (sidewalks) can be saidto be feasible if they can meet pedestrian needs based on several criteria such as accessibility, circulation, safety, security, beauty, cleanliness, noise, shade, and connectivity. The purpose of this study is to obtaina pedestrian comfort index based on pedestrian needs in the Losari Beach Area and Fort Rotterdam. Toproduce this comfort index, it is necessary to identify the factors and measure the level of comfort based onpedestrian needs, and to determine the priority of pedestrian needs..The pedestrian path available in this areais known as “Bundaratta”, located in the recreation/natural tourism area (Losari Beach) and cultural tourism(Fort Rotterdam) people of Makassar City. With the function of the land as a recreation area, a comfortablepedestrian path is urgently needed by local and foreign tourists to enjoy and reach the existing facilitiesaround the area. This research is a quantitative descriptive study, using a Likert scale to obtain a level ofcomfort and the Analytical Hierarchy Process (AHP) to obtain priority for the comfort factor. The resultsshowed that the pedestrian comfort index along Losari Beach to Fort Rotterdam is 60.20 and it is in thecomfortable category, but it still needs some repairs and improvements in several location segments so thatthis pedestrian lane falls into the category that is truly comfortable and even very comfortable. for its users.

Abstract

Transient ischemic attacks (TIA), as precursors of cerebral strokes, occupy an important place among allforms of cerebrovascular insufficiency. Regarding epidemiology of transient ischemic attacks (TIA), thereare no accurate data in most countries. In the United States, up to 5 million adult citizens per year suffer fromthese episodes, many TIA remain undiagnosed. These episodes of a sudden and short-term neurological deficithave long been considered as benign and harmless. Most general practitioners and patients misunderstandor insufficiently understand TIA’s nature and significance, perhaps, this is the reason of insufficient medicalinterest, lack of statistical data on this nosological unit. Transient ischemic attacks (TIA) are clinicallydefined as rapidly developing focal and sometimes diffuse (cerebral) dysfunctions of the brain that arecaused by local ischemia and last no longer than a day1. Over the past two decades, many views on TIA havechanged significantly, approaches to the diagnosis and treatment of patients have become much more intenseand aggressive. Modern knowledge about TIA is of paramount importance both for proper organization ofpatient care and educational programs among the population; their importance can hardly be overestimated.

Abstract

Purpose: Improving the results of treatment of stenotic developmental abnormalities of the ureterovesicaljunction (SDA UVJ) in children.Materials and Method: Treatment analyzed included 88 pediatric patients (125 ureters) with SDA UVJ,aged 3 months - 15 years. Results: Endoscopic stenting has been performed using antireflux stents, with subsequent bladder drainageusing a urinary catheter. We have also developed an inverted V-shaped ureterocele incision. The UVJ controlcalibration has been performed using calibration catheters. If the UVJ diameter did not correspond to theage norm, the second stage of treatment was carried out by dilating with conical bouges or by UVJ balloondilating. Transient complications developed in the immediate postoperative period have been successfullyeliminated using conservative measures, in all cases. Conclusions: The study results imply the effectiveness of endoscopic method in correcting SDA UVJ inchildren.

Abstract

Introduction: The article provides an epidemiological description for patients with chronic recurrentaphthous stomatitis (RAS) and the frequency of their reversal in the general structure of diseases of the oralmucosa from 2014 to 2018. The structure of clinical forms of RAS in the Republic of Uzbekistan (ICD-10) was taken. The study of the dynamics of frequency and debut of various clinical forms of RAS, thegender characteristics of RAS has led to the need for the introduction of modern standards for examination, treatment, and prevention of the disease. The goal of the research is to study the frequency of occurrenceof RAS, as well as the real clinical practice of managing patients in the Republic of Uzbekistan and itscompliance with existing recommendations.Materials and Method: Patients’ medical records and records of the patients’ consultation journal for fiveyears of work (from 2014 to 2018) were studied. Results: Taking into account the gender and age characteristics of patients with RAS will determine the realclinical picture and prevalence of this disease, increase the ability of monitoring, medical examination andproviding timely specialized care, allowing to prevent the progression and maintain the working capacity ofpatients with this pathology of OCR.Conclusion: The frequency of RAS at the general reception for diseases of OCR is 6.22 ± 0.42%; in thegeneral structure of the disease, the fibrinous form prevails 3.46%; the incidence of necrotic periadenitis is3.15%; herpetiform aphthous stomatitis is recorded in 2.25% of patients and 1.25% - a symptom in Behcet’sdisease. The debut of RAS falls on the age period of 18-34 years (62.50%), which indicates a negativetrend of “rejuvenation” of this pathology. For effective treatment and rehabilitation of patients sufferingfrom RAS, it is necessary to conduct a comprehensive examination attracting internists, an appointmentof means of immunomodulating therapy and means effectively correcting the most important homeostaticmechanisms in humans.
