Abstract

Background : Rheumatoid arthritis is an immunological mediated ,chronic inflammatory multisystemdisease which involves inflammation of peripheral joints along with involvement of haematological ,neurological and cardiovascular abnormality. Studies have shown that the involvement of cervical spineis common commonly affecting C1 and C21 due absence of intervertebral disc and synovial inflammationresulting in alteration in range of motion. Keeping in mind the inflammation and degeneration causesalteration in mobility . So that present study is conducted with the aim to find out proportion of alterationin mobility at the cervical spine range of motion in subjects with rheumatoid arthritis associated with neckpain.Methodology: An observational study was conducted in 33 patients at karad with study duration of 6 months. The inclusion criteria was the patient with rheumatoid arthritis any age and gender willing to participate. The outcome measures were goniometer and inch tape method of assessing the range of motion .Result: The results observational study suggest there is significant decrease in all movement of cervicaljoint with rotational components were hampered most around 26.36% loss of mobility for left rotation and26.12 % loss of mobility for right rotation followed by right lateral flexion , flexion, extension , right rotationand left rotation .Conclusion: the study concluded that rotational movements was more affected in a patients with rheumatoidarthritis associated with neck pain which is attributed to marked involvement of the C1 and C2 vertebraefollowed by other movements at the neck.

Abstract

Background: Ototoxicity is a harmful feature of the cochlea or auditory nerve and sometimes vestibularapparatus. Gentamicin is known to cause irreversible bilateral ototoxicity. Autophagy induction hasbeen proposed as a target for prevention of gentamicin ototoxicity. Aim of the work: to investigate thepossible protective effect of autophagy induction by verapamil compared to rapamycin. Methods: Thisexperiment was conducted on 32 male guinea pigs. At the beginning each animal’s hearing status wasassessed using auditory brainstem response (ABR) audiometry. Then, they were divided into 4 equalgroups: Group 1: control group, Group 2: untreated gentamicin-induced ototoxicity group. Group 3:gentamicin-induced ototoxicity treated concomitantly with rapamycin. Group 4: gentamicin-inducedototoxicity treated concomitantly with verapamil. At the end of the experiment, ABR was repeatedthen the animals were sacrificed, and blood samples were obtained for assaying of reduced glutathioneand malondialdehyde levels. The left cochlea was processed for scanning electron microscope, whilethe right cochlea was processed for histopathology and LC3-II immunohistochemistry. Results:Verapamil revealed superiority compared to rapamycin proved by significant improvement in ABR,histopathological results, in addition to its antioxidant effect. Conclusion: verapamil could be suggestedas a potential therapeutic approach to decrease gentamicin ototoxicity.

Abstract

Background: And aim: The prevalence of DLK has been reported differently in different centers based onits severity. Hence, the aim of present study was evaluate the prevalence, natural course, and final visualacuity in patients with stages III or IV of diffuse lamellar keratitis following laser in situ keratomileusis. Methods: In the present descriptive study, 3941 eyes files from 2125 patients underwent LASIK surgery;were examined. All patients had a complete ophthalmologic examination before LASIK and had suitableconditions for LASIK.Result: Among the studied cases, 39 eyes (0.98%), out of 16 patients (0.75). The mean time intervalbetween LASIK and DLK diagnosis was 3± 2.4 days. A history of allergy was negative in all patients. Themean post-operative spherical refractive error in all eyes was in the range of 1 diopters and the averagecylindrical refractive error in 33 eyes was -0.66±1.2 diopters. The mean of final spherical refraction in34 eyes was 0.5±1.2 diopter and the mean of final cylindrical refraction in 35 eyes was -0.75±1.3 diopter.Severe turbidity at the flap surface in the first week decreased over time in all patients with stage III and IVDLK. So that only in four eyes (10.25%)Conclusion: The prevalence of stages III and IV in DLK is negligible compared to the large number ofLASIK procedures and all patients respond well to topical and systemic corticosteroid therapy.

Abstract

Background: Inflammation is a key factor in COVID-19 progression and is potentially an importantpredictive factor. The ratios of neutrophils to lymphocytes and platelets to lymphocytes may showsignificant inflammatory progression, which may contribute to the development of some majorcomplications and mortality problems as observed in COVID-19. Objective: To examine and investigatethe correlation of neutrophil lymphocytic ratio (NLR) and platelets to lymphocytes ratio (PLR) withincreased mortality risk. Methods: 124 patients admitted to King Abdulaziz hospital with confirmed COVID-19 were enrolled inour study. The blood cell count was used to obtain NLR and PLR. The receiver operating characteristic(ROC) curve was used to determine the sensitivity and specificity of NLR and PLR for severity andmortality of admitted patients with COVID-19. The logistic regression model was used to explore therisk factors associated with mortality and severity. Results: The mortality rate was 23.4%. Among non-survivors’ patients’ lymphocyte significantlydeclined with 96.5%, while neutrophil counts increased. The Empirical optimal cut point of NLRcorrelated with mortality is 4.647, with a sensitivity of 93% and a specificity of 61% (AUC: 0.847, 95%CI 0.774-0.921; P = 0.001). Moreover, the optimal cut point of PLR associated mortality is 17.358,with a sensitivity of 90% and a specificity of 64% (AUC: 0.791, 95% CI 0.703-0.878; P = 0.005).Conclusions: NLR and PLR variables were significantly correlated with severity and mortality. NLRand PLR can be considered independent biomarkers in COVID-19 patients which play a significant roleto predict mortality cases.

Abstract

Acinetobacter baumannii (Ab) is developing resistance to a variety of common antibiotics. and becomemultidrug resistant, extreme drug resistant, and pan drug resistant pathogens, requiring the identificationof new antibiotics as well as the identification of new plant compounds capable of acting against Ab.Recent research has revealed MDR Ab co-infections with COVID-19,, raising alarm bells. Sinceits isolation, Usnic acid has been investigated for a variety of pharmacological activities, includingantioxidant, antitumor, antibacterial, antifungal, antiviral, antiprotozoal, and insecticidal. ManyPlant-derived drugs show promising activity as new antimicrobial agents against multidrug resistant(MDR) strains. There is insufficient data to support the antibacterial activity of (+)-UA against MDRAcinetobacter baumannii..
