Abstract

Fertility and infertility are major issues in global reproductive health influenced by various biological, hormonal, environmental, and medical factors. Infertility affects approximately 8–12% of couples worldwide, and several studies indicate that the prevalence continues to increase due to lifestyle changes and exposure to risk factors. Ovarian factors, tubal disorders, spermatogenesis, reproductive age, and metabolic conditions such as Polycystic Ovary Syndrome (PCOS) play important roles in determining fertility outcomes.  To comprehensively analyze scientific evidence regarding the determinants of fertility and infertility among couples of reproductive age, as well as to describe clinical implications and evidence-based recommendations. This study employed a scoping review design conducted through searches of scientific articles in PubMed Central, BioMed Central, and Google Scholar Open Access. Eligible articles were full-text publications in English published between 2016 and 2025. Most studies indicated that female infertility is associated with ovulatory disorders, tubal damage, diminished ovarian reserve, and endometriosis, whereas male infertility is commonly influenced by sperm quality, varicocele, reproductive tract infections, and genetic factors. In addition, age, obesity, smoking, oxidative stress, and exposure to toxic environmental factors were also associated with reduced chances of pregnancy. Assisted Reproductive Technologies (ART), such as In Vitro Fertilization (IVF) and Intracytoplasmic Sperm Injection (ICSI), have been shown to improve fertility outcomes; however, their success rates are highly dependent on the age and baseline conditions of the couples. Infertility is caused by multifactorial interactions involving biological, hormonal, and environmental aspects. Early detection of reproductive disorders, education regarding healthy lifestyles, and access to fertility services are essential to improve the chances of achieving pregnancy.

Abstract

The National Health Insurance (Jaminan Kesehatan Nasional or JKN) era has changed hospital financing patterns, particularly through the claim mechanism submitted to BPJS Kesehatan. Under this system, hospitals are highly dependent on the smooth process of claim submission, verification, and payment. However, several problems are still frequently found in practice, such as delayed claim payments, pending claims, claim disputes, and weak receivables and cash management. These conditions may disrupt the stability of hospital operating cash flow, especially in financing health services, medicines, medical supplies, service fees, and daily operational needs. This article aims to analyze the role of receivables management and cash management in supporting the smooth flow of hospital operating cash in the JKN era. The method used is a narrative literature review by examining relevant journal articles, regulations, official reports, and scientific publications. The reviewed literature was published between 2015 and 2025 and obtained from Google Scholar, Garuda Kemdikbud, Neliti, DOAJ, and government regulatory portals. The search keywords included hospital receivables management, BPJS receivables, pending BPJS claims, hospital cash management, hospital operating cash flow, JKN, INA-CBG, and related terms. The findings show that receivables and cash management play an important role in maintaining hospital cash flow stability. BPJS receivables that are not collected on time may cause liquidity constraints. Therefore, hospitals need to strengthen receivables aging monitoring, pending claim settlement, claim audits, staff competency, system digitalization, and periodic cash flow projections.

Abstract

Construction is a high-risk sector because workers are exposed to physical, mechanical, electrical, ergonomic, and dynamic workplace hazards. This study aimed to analyze the association between worker characteristics (age, work experience, education level, and work division), occupational safety and health (OSH) knowledge, personal protective equipment (PPE) use, and occupational accidents among high-rise building construction workers at PT. Razar Bumi Perkasa, South Lampung Regency. The study was motivated by the high occupational accident rate in Indonesia in 2024 and the absence of division-specific accident risk data in this company. The study was conducted in April 2026 at the high-rise building construction site of PT. Razar Bumi Perkasa in South Lampung Regency. This study utilized a quantitative-analytical research design with a cross-sectional method. The population consisted of 179 workers, and 50 respondents were selected using simple random sampling based on Slovin's formula (e=10%). Data were collected using a structured questionnaire that had been tested for validity and reliability. The OSH knowledge variable was measured with 32 items (cut-off point: median = 122), PPE use with 16 items (cut-off point: median = 65), and occupational accident risk with 19 items (cut-off point: median = 39). Data were analyzed using univariate and bivariate analyses with the Chi-Square test (α = 0.05). Most respondents were categorized as having a high risk of occupational accidents (52.0%). Work division was significantly associated with occupational accidents (p=0.019), with the highest high-risk proportions observed in the MEP (80.0%) and civil (61.3%) divisions. Age (p=0.534), education (p=0.384), work experience (p=0.623), OSH knowledge (p=0.089), and PPE use (p=0.963) were not significantly associated with occupational accidents. Occupational accident risk was more strongly related to job characteristics and division-specific hazard exposure than individual factors. Accident prevention should prioritize division-based risk control, field supervision, housekeeping improvement, and OSH culture strengthening
