Clinical and Research Journal in Internal Medicine is the official open access journal of Internal Medicine Research Center, Department of Internal Medicine, Faculty of Medicine, Universitas Brawijaya, Malang, Indonesia. It publishes articles two times per year. It is a peer reviewed publication of Indonesian Internal Medicine journals and accepting articles for publication from around the world. CRJIM only publishes articles in the English version. The objective of this journal is to publish the selected clinical and basic research relevant to Internal Medicine. It covers the following topics, nephrology, endocrinology, metabolic, and diabetes, rheumatology, geriatrics, tropic infection, hematology oncology. CRJIM publishes original researches, reviews, brief reports, editorial, case series, case reports, and commentary. Additionally, it also considers publishing animal studies relevant to Internal Medicine topic. It is an international journal dedicated to providing new information that could give new insight for alternative solutions, diagnosis, therapy and prevention for researchers and practitioners in Internal Medicine.

Abstract

Background: Liver abscesses continue to present a significant diagnostic and therapeutic challenge to the medical community. Early clinical distinction between an amoebic and a pyogenic liver abscess is challenging due to their overlapping clinical presentations. Aim: The present study was undertaken to evaluate the clinical characteristics, lab parameters as well as management in patients suspected of liver abscess. Methods: A retrospective study was carried out in 60 consecutive patients presenting with liver abscess, admitted at tertiary care hospital. Lab investigations were carried out. All patients underwent ultrasound-guided aspiration of a liver abscess either by percutaneous needle or by a pigtail catheter.  Patients were monitored during the period of hospital stay to note their outcome. The data variables collected were compiled by using an excel spreadsheet and later analysed using Medcalc (version 20.118). Results: The commonly observed clinical presentation was abdominal pain (95%) followed by fever (57%) and jaundice (7%). On examination, 60% of the liver abscess cases had right hypochondrial tenderness while Hepatomegaly was observed in about 17% of the subjects. Leucocytosis was observed in 67% of the cases in view of underlying infectious etiology. The size of the majority (60%) of the liver abscess were in the range of 5.1 to 9.9 cms and right lobe was the most affected region (75.2%). Conclusion: Early recognition of symptoms, institution of antibiotic and amoebicidal therapy and drainage of abscess via appropriate modality improves the prognosis of liver abscess.

Abstract

Cryptococcosis causes invasive fungal infections and the two common species are cryptococcus neoformans and cryptococcus gattii, which are responsible for the majority of the clinical manifestations. We are presenting a case of a 35-year-old individual who was admitted with the complaints of fever associated with acute onset altered sensorium. His CSF PCR testing showed cryptococcus species, and so diagnosed as cryptococcal meningoencephalitis and then eventually found to have HIV/AIDS. CSF PCR testing can be done rapidly for variety of common bacterial and fungal infections, and its results are available within one hour. The early diagnosis and treatment initiation is very vital in immunodeficiency states such as HIV, as it can have positive prognostic effect on natural course of disease process. The patient did receive dual anti-fungal drugs, along with Antiretroviral therapy (ARV) and showed drastic clinical improvement.

Abstract

Background: Dengue is an entity caused by mosquito bites in humans. It can change into a life-threatening entity within a very short time. Not every dengue case requires a blood transfusion. While plasma leakage and a drop in platelet count are possible side effects of severe dengue, not all patients suffer from them. The main goals of treatment are to control symptoms such as pain, fever, and dehydration by giving patients enough fluids and medicine. Aim: To identify the various clinical features associated with dengue along with the manifestations not warranting blood transfusion. Methods: This observational study includes a total of 300 laboratory-verified dengue patients who were admitted over two months. Patients with other causes of fever were excluded. Results: The majority of patients (80%) were discovered to be between the ages of 10 and 40, indicating that people in this age bracket are often more afflicted. Males were predominantly affected in comparison to females. Thrombocytopenia is the most prevalent anomaly, present in all the patients. Of these, 3.0 (1.0%) had platelet counts < 10,000/cmm, while 198 (66%) had platelet levels < 10,000–20,000/cmm. Conclusion:  We conclude that the main factors influencing the prognosis are intra-abdominal visceral oedema as gall bladder wall edema, leucopenia (TLC count below 2000 cells/cmm), deranged liver functions, capillary leakage leading to peritoneal/pleural effusions as well as shock, and platelets. Platelets do not determine the severity and fatality of dengue infection.

Abstract

There is a lot of research to support that cancer stem cell attract normal stem cell. The tendency of tumor cells to attract stem cells then becomes the basis for the use of stem cells in breast cancer theraphy. When neural stem cell applied as a carrier, it will make the amount, timing, and location of viral oncolytic or drug release can be precisely controlled. Finally, this situation will increase the effectiveness of breast cancer therapy and reduce side effects. This is a brief review about potency of stem cell as breast cancer treatment. This article aim to increased our knowledge about the potency of stem cell as breast cancer treatment. The tendency of tumor cells to attract stem cells then becomes the basis for the use of stem cells as enzyme/prodrug theraphy, secreted agent, viral oncolytic carrier or nanoparticle (drug) carrier which tends to be more localized around the tumor. When neural stem cell applied as a carrier, it will make the location, amount and timing of drug release or viral oncolytic can be precisely controlled.

Abstract

Background: Fatigue is one of the common symptoms in patients with chronic medical conditions like kidney failure that require hemodialysis. It is also associated with bad quality of life. Therefore, it is crucial to evaluate it using tools like Fatigue Severity Scale (FSS). Aim: The purpose of this study was to evaluate the reliability and validity of the translated FSS. Methods: FSS was performed on 41 chronic kidney patients undergoing dialysis. The FSS used in the study was translated from English to Bahasa Indonesia by the team. The questionnaire was assessed for its reliability and validity using Cronbach’s Alpha (CA) and Pearson Validity Test, respectively. Reliability for each item was also evaluated using the corrected item-total correlation. The scale is reliable if its CA value >0.6 and valid if the r-value is higher than the expected r on Pearson's table. Results: The scale was perceived as reliable based on the CA values from both respondent groups: 0.968 for HD and 0.952 for the CAPD group. All nine items were also deemed reliable based on the corrected item-total correlation score. Based on both r and Sig values, the translated FSS was valid for all groups: r>0.433 for HD, r>0.444 for CAPD, and Sig value <0.05. Conclusion: The translated FSS is valid and reliable, hinting at its ability to be utilized in assessing fatigue in chronic kidney patients on dialysis

Abstract

Oncolytic viruses (OVs) represent a category of viruses that selectively replicate within tumor cells, destroying cancer cells through a dual mechanism involving viral oncolysis and the stimulation of the host's anti-tumor immune response. This selectivity ensures the virus targets and eliminates cancer cells while sparing healthy cells. Oncolytic virus immunotherapy refers to treatments utilizing either native or genetically modified OVs designed to replicate within tumor cells selectively. Mesenchymal Stem Cells (MSCs) have demonstrated potential in facilitating the safe transportation and targeted release of oncolytic viruses at the tumor site due to their inherent tumor-specific homing capability. Utilizing MSCs as carriers for oncolytic viruses can enhance the delivery of these therapeutic agents to the tumor site, eliminating the need for direct tumor injection and reducing the associated side effects.

Abstract

Snake bites pose a significant public health issue in Indonesia, leading to high morbidity and mortality rates. Proper identification of snake species and timely management are crucial. A 70-year-old woman was admitted to the emergency department 1.5 hours after being bitten by a snake. Initial management included applying a tourniquet and symptomatic treatment. However, local envenomation signs, such as edema and bullae, developed within 24 hours. Due to financial constraints, the patient could not receive continuous care, leading to complications, including necrosis and signs of secondary infection during follow-up. This case underscores the importance of early intervention and proper wound management in snake bites, particularly when the species is unidentified. Close monitoring is essential, as symptoms may evolve over time. Educating the patients and public regarding signs of necrosis and proper initial treatment is necessary. Effective management of snake bites requires a comprehensive approach, including prompt medical attention and ongoing education to reduce complications and improve patient outcomes. Abstract

Abstract: The global burden of disease is dominated by non-communicable diseases, but for African and developing countries, infectious diseases and febrile syndromes are the most frequent. At the community level, the actual disease burden remains understudied. Aims: Our study aimed to determine the profile and trends of morbidity diagnosed at the community level and to determine the most frequent reasons for consultation in routine consultation. Methods: we conducted a cross-sectional and retrospective study on data from the CSCom consultation register in Nyamina. The data was collected using epi info 7 software and analyzed with R version 4 software. Results: at the end of our study, we recorded 8778 patients in routine consultation for 55.2% of women. Malaria was the most prevalent diagnosis with 68% followed by ARI 8%. While 75% of malaria cases occurred before the age of 20, non-communicable diseases such as hypertension were more common among the elderly. Conclusion: the morbidity profile was globally more dominated by communicable diseases, in particular malaria and ARI.

Abstract

Background: Conventional discharge reports often suffer from inconsistencies, incomplete information, and lack of clarity, impeding effective communication and continuity of care. ChatGPT, a powerful language model based on deep learning techniques, can enhance the quality and efficiency of discharge reports. Aim: This article explores the possibility of using ChatGPT in geriatric medicine specifically, where complex medical conditions and multidimensional care plans are common. ChatGPT can assist in capturing detailed patient histories, comprehensively documenting geriatric syndromes, and incorporating personalized recommendations for geriatric care management. Methods: Using GPT-3, we employed the chatbot to generate a discharge report based on comprehensive patient data. The AI-generated report was compared with the actual discharge report, and the AI's approach was validated by requesting supporting sources. This evaluation aimed to assess the reliability and credibility of the AI-generated discharge report. Discussions: Despite its potential advantages, the integration of ChatGPT into healthcare workflows necessitates addressing various challenges. These include ensuring patient data privacy, validating the accuracy and reliability of generated information, and establishing guidelines for responsible use. Conclusion: ChatGPT holds promise as a transformative technology for improving the quality of discharge reports in geriatric medicine. By leveraging its language processing capabilities, ChatGPT can enhance communication, patient understanding, and care coordination, ultimately leading to better patient outcomes and healthcare efficiency. Future research and collaborative efforts are necessary to fully explore and refine the application of ChatGPT in this context and to ensure its seamless integration into clinical practice.
