Abstract

Postpartum psychosis is an acute psychiatric disorder characterized by rapid onset following childbirth, posing significant risks to both maternal and neonatal safety. Its underlying mechanisms remain incompletely understood; however, fluctuations in reproductive hormones particularly estrogen and progesterone are strongly implicated in its pathogenesis. The abrupt decline of these hormones after delivery may disrupt neurotransmitter systems, especially dopaminergic and GABAergic pathways, thereby precipitating neurobiological imbalance in vulnerable individuals. This study employs a literature review method using a narrative review approach, drawing upon databases such as PubMed, ScienceDirect, and Google Scholar, complemented by manual searches. Following the PRISMA framework, a total of 20 articles met the inclusion criteria and were systematically analyzed. The findings indicate that estrogen demonstrates a more consistent association with psychotic symptomatology, whereas progesterone exerts its effects primarily through its metabolite, allopregnanolone, which modulates the GABAergic system. In addition, genetic predisposition, sleep disturbances, and alterations in neuroplasticity contribute to the condition. Postpartum psychosis therefore emerges as a multifactorial disorder resulting from the complex interplay between hormonal fluctuations and underlying neurobiological vulnerability.

Abstract

Patient Safety Incidents (IKP) can impact the quality of hospital services. Both globally and nationally, patient safety incidents remain a significant concern. At Prof. HB Saanin Mental Hospital in Padang, the number of patient safety incidents increased from 2022 to 2023. Patient safety incidents should be zero, or there should be no accidents, based on the Decree of the Minister of Health of the Republic of Indonesia number 129/Menkes/II/2008 concerning Minimum Hospital Service Standards. Objective: Analyze the influence of room layout and design on patient safety incidents at Prof. HB Saanin Mental Hospital, Padang. Method: This study used a mixed-methods cross-sectional study of 25 officers involved in patient safety incidents. Bivariate analysis using the Chi-square test was employed to assess the relationship between room layout and design and patient safety incidents, which was further explored through qualitative interviews. Results: The most common type of patient safety incident at Prof. HB Saanin Mental Hospital is unwanted events, accounting for 64% of total incidents and 52% of incidents with high impact. Inappropriate room design was identified in 52% of cases, and inappropriate furniture layout was identified in 84%. Conclusion: Equipment layout and room design influence the occurrence of patient safety incidents in the Prof. HB Saanin Mental Hospital, Padang. Redesigning rooms is a key factor in reducing patient safety incidents at this hospital.

Abstract

Informed consent is an important aspect of healthcare practice that aims to ensure patients' rights to obtain information and provide approval for medical procedures to be performed. The implementation of informed consent is not only related to medical ethics but also plays an important role in preventing medical disputes between healthcare providers and patients. Lack of understanding and inadequate implementation of informed consent can increase the risk of conflicts and malpractice claims in healthcare services. This study uses a literature review method conducted by collecting data through journal searches in PubMed, Garuda, and Google Scholar using the keywords: informed consent, medical disputes, medical malpractice, patient legal protection, and health law. This study reviewed 12 research articles relevant to the research topic.

Abstract

Breast cancer is one of the most common malignancies among women and remains a major cause of morbidity and mortality worldwide. Because breast cancer may develop without obvious symptoms in its early stages, accurate imaging assessment is essential to support early detection, disease staging, and appropriate treatment planning. Imaging modalities such as mammography, ultrasonography, magnetic resonance imaging (MRI), dynamic contrast-enhanced MRI (DCE-MRI), and artificial intelligence-based imaging technologies play an important role in evaluating tumor size, lesion characteristics, disease extent, multifocality, and treatment response. This study aimed to examine the role of imaging modalities in determining surgical management for breast cancer. This study employed a narrative literature review with PRISMA-guided article selection. Literature was collected through searches in PubMed, Google Scholar, Garuda, and other relevant scientific databases using selected keywords, including breast cancer, imaging modalities, mammography, ultrasonography, magnetic resonance imaging, MRI, DCE-MRI, surgical management, and surgical planning. Based on the screening process, 11 articles consisting of 4 national and 7 international journals were included in the review. The findings indicate that mammography is important for early detection, ultrasonography supports lesion characterization and clinical decision-making, while MRI and DCE-MRI provide more detailed information regarding tumor extent, local staging, multifocal lesions, and response to neoadjuvant therapy. Artificial intelligence-based imaging also shows potential in improving diagnostic accuracy and supporting individualized surgical planning. Therefore, imaging modalities are not only diagnostic tools but also strategic components in determining the type and extent of surgical treatment in breast cancer patients.

Abstract

Food service is one of the hospital nutrition services starting from menu planning to food distribution. Minimum Service Standards for food service include punctuality in providing food and food waste ≤ 20%. Based on the results of a survey conducted at La Temmamala Regional Hospital in 2024, the percentage of patient food waste was <20%. This shows that the SPM of the nutrition installation has reached the target, but there are still efforts to maintain and improve. This study aims to examine the relationship between nutrition installation food service and the level of satisfaction and food waste of inpatients at the UPT La Temmamala Regional Hospital, Soppeng Regency. This study uses a quantitative approach with a cross-sectional study design with a sample of 55 inpatients at La Temmamala Regional Hospital. Sampling by purposive sampling was carried out in June 2025. Data were analyzed using the chi-square test. Hospital food service variables for indicators of waiter attitude, punctuality of food distribution and cleanliness of cutlery were rated as good by 55 respondents (100%) for indicators of food menu variety and patient satisfaction variables were rated as good by 54 respondents (98.2%) and less good by 1 respondent (1.8%). There is a relationship between hospital food service and patient satisfaction with a p value of 0.018 and there is no relationship between hospital food service and patient food waste with a p value of 0.291.

Abstract

The development of digital technology drives the transformation of healthcare services, including thru the implementation of teleconsultation in the National Health Insurance Program (JKN). However, the success of implementing this service heavily depends on the acceptance of doctors as the primary users. This study aims to analyze doctors' acceptance of teleconsultation at First-Level Health Facilities (FKTP) in Palembang City using the Technology Acceptance Model (TAM) approach. This study uses a qualitative method with a case study design. Data were obtained thru in-depth interviews with eight informants consisting of public health center doctors, private clinic doctors, and management personnel. Data analysis was conducted thematically based on the TAM constructs, namely perceived usefulness, perceived ease of use, attitude toward use, and behavioral intention to use. The research results show that teleconsultation is perceived as beneficial in improving service access, initial communication, and patient monitoring, but it has not yet been able to replace face-to-face services due to limitations in clinical aspects. From the perspective of ease of use, the system is considered quite simple, but there are still technical issues such as non-real-time notifications and limited reporting time. Doctors' attitudes toward teleconsultation tend to be positive, but not strong, while the intention to use it is relatively high but more influenced by external policies. In conclusion, the acceptance of teleconsultation is at a fairly good level but not yet optimal. The optimization of implementation requires improvements in system quality, organizational support, as well as clearer policies and incentives so that the utilization of teleconsultation can run effectively and sustainably.

Abstract

This study evaluated the AST/ALT ratio as a non invasive predictor of liver fibrosis by examining its clinical utility, diagnostic relevance, and limitations in contemporary practice. Liver fibrosis is a progressive outcome of chronic liver injury that may develop into cirrhosis, liver failure, and hepatocellular carcinoma if not detected and managed early. Therefore, accessible non invasive markers are needed, particularly in settings with limited diagnostic resources. This study used an analytical literature review approach. Articles published within the last ten years were searched through PubMed, Google Scholar, and ScienceDirect using keywords related to AST/ALT, non invasive liver disease, liver fibrosis, and diagnosis. Relevant studies were selected based on predetermined inclusion and exclusion criteria, then analyzed descriptively and comparatively. The findings indicate that the AST/ALT ratio has potential as a simple, affordable, and widely available marker for the initial assessment of liver fibrosis. It showed a meaningful association with fibrosis severity, especially in patients with chronic hepatitis B and other chronic liver diseases. However, its diagnostic performance varied depending on disease etiology, fibrosis stage, and patient characteristics. Compared with APRI, FIB 4, FibroScan, and elastography, the AST/ALT ratio generally demonstrated only moderate accuracy and was less reliable as a standalone marker. Therefore, it is more appropriate as an initial screening tool or complementary indicator rather than a definitive diagnostic method.
