Abstract

Service quality and patient satisfaction are the two most important things in Community Health Center (puskesmas) services. This study aims to see the effect of the quality of health services on outpatient satisfaction at Langsa Timur Public Health Center, Langsa City in 2020. This type of research is a quantitative research which is an analytical survey with a cross sectional study approach. The population in this study were 2,116 people. A sample of 95 people using the Slovin formula. The analysis used is univariate analysis, bivariate analysis with chi-square test and multivariate analysis with linear regression test, measuring instruments used questionnaires and data processing using computerized with a significance level of P-Sign <0.05. The results of this study are based on the results of univariate analysis, as many as 64 people were dissatisfied and 31 people were satisfied. The results of multivariate analysis showed that physical evidence and responsiveness had an influence on patient satisfaction and physical evidence was dominant. The conclusion of this study is that there is an effect of physical evidence and responsiveness on outpatient satisfaction at Langsa Timur Public Health Center, Langsa City.

Abstract

The Mandiri National Health Insurance (JKN) is a national social security system where the premium is paid by participants. In 2018, data on premium arrears for Mandiri participants in Indonesia reached Rp. 5.7 Trillion, in Medan City, the highest premium arrears are in Medan Helvetia Subdistrict, amounting to IDR 4,215,379,613. The results of the initial survey indicate the cause of arrears is due to insufficient income. Green's theory explains that people's behavior in the health sector is formed due to a combination of predisposing, supporting and driving factors. This study aims to determine and analyze the influence of economic factors, knowledge, age, occupation and attitudes towards regulations in arrears of JKN Mandiri premium payments in the working area of Puskesmas Helvetia, Medan City. The research design used a quantitative analytic survey with a cross sectional approach. The study population was all JKN Mandiri participants in the work area of the Helvetia Health Center, Medan City who were in arrears, namely 8260 participants with a sample of 98 people. This research was conducted in the area of Helvetia Health Center, Medan City. Data were analyzed by univariate, bivariate and multivariate with multiple linear regression. The results showed that economic factors, knowledge and work have an influence in arrears of JKN Mandiri premium payments. Multivariate analysis shows that economic factors, knowledge, age, occupation and attitudes towards regulation only contribute to premium arrears of 19.7%. There is an influence of economic factors, knowledge and work in arrears to pay the JKN Mandiri premium, while the age factor, attitudes towards regulation have no effect. It is recommended for Puskesmas to always socialize JKN regulations and together with the local government to find solutions so that the community's economy will be better so that they can pay the JKN Mandiri premium.

Abstract

Exclusive breastfeeding for 6 months is very important for both mother and baby. The number of mothers who exclusively breastfeed their babies is still low (<80%). Medan Sunggal Puskesmas, Medan Sunggal Subdistrict, has a low achievement of exclusive breastfeeding in 2018 at 49.3% and decreased in 2019, namely 45.6%. Many factors influence exclusive breastfeeding, namely predisposing factors, enabling factors and reinforing factors. The purpose of this study was to find the determinants of exclusive breastfeeding in infants aged 0-6 months in Medan Sunggal District. Population of 256 people. Samples using the Slovin formula obtained 156 people. Data collection using a questionnaire. Data were analyzed by univariate, bivariate and multivariate using multiple logistic regression tests at the 95% significance level. The results showed that statistically knowledge (0.016), attitude (0.016), maternal ability (<0.001), breastfeeding facilities (0.001), husband's support (0.004), and parental support (0.010) had a significant effect on exclusive breastfeeding. Trust, ability of health workers, support from friends and the role of cadres have no effect. The dominant factor influencing is the ability of mothers with the value of Exp (B) 30,233, meaning that mothers have good abilities tend to be 30,233 times the mother gives exclusive breastfeeding to babies than the ability is not good. The conclusion is that the determinants of exclusive breastfeeding are knowledge, attitudes, abilities of mothers, breastfeeding facilities, husband's support and parental support. It is recommended that the Medan Sunggal government to increase regular counseling and form a companion group to make exclusive breastfeeding successful through peer groups or groups of mothers who love babies.

Abstract

The quality of service at the Community Health Center (Puskesmas) is an important factor in creating patient satisfaction. The results of measuring patient satisfaction that are objective and accurate can assist the health center in formulating a better form of service. The purpose of this study was to determine and analyze the effect of health service quality on outpatient satisfaction. This type of research is an analytic survey with a cross sectional approach using primary data with questionnaires and secondary data from Community Health Center (Puskesmas) reports. The population in this study were patients who had outpatient visits from January to July 2020 with a total sample of 99 people who were determined using the accidental sampling method. The analysis used univariate, bivariate and multivariate analysis. Chi square test results showed that technical competence (p = 0.001 <0.05), effectiveness (p = 0.002 <0.05), human relations (p = 0.000 <0.05), comfort (p = 0.099> 0, 05) and timeliness (p = 0.001 <0.05). The logistic regression test shows that the variable that most influences patient satisfaction is the relationship between humans with an Exp (B) value of 4.195. The conclusion of this research is that technical competence, effectiveness, human relations and timeliness have a relationship with patient satisfaction, while comfort has no relationship with patient satisfaction. Efforts are needed to improve the quality of outpatient services by establishing a harmonious relationship between health workers and patients in order to achieve services that are able to satisfy patients.

Abstract

One of the jobs that is prone to stress is a nurse. According to PPNI, 50.9% of Indonesian nurses experience work stress. This study was to determine the factors that influence the stress of nurses at the Putri Hijau Level II Hospital, Medan. The research design used was cross sectional with a population of 152 nurses in this study and the sample used a stratified random sampling technique to 60 nurses. Data analysis using binary logistic regression test. Research shows that there is an effect of workload (p = 0.03) and interpersonal relationship (p = 0.03) on nurses 'work stress, while the variables of gender, tenure, career development and role conflict have no effect on nurses' work stress. Workload and interpersonal relationships in the workplace have the potential to cause work stress for nurses. So it is advisable for the management to adjust the physical and mental workload with the capacity and ability of nurses, and strive for a conducive social environment.

Abstract

From the first forty cases of working stress on workers, work stress incidence on nurses is located at the top of the list. The suitable handling and management for nurses are crucially required. Working stress management is a program to help someone overcome stress. The purpose of this research is to find out the effectiveness of stress management and physical activity towards working stress on nurses in TK II Putri Hijau Hospital. This research is using a quasi-experimental design with one group pretest-posttest design. The population in this research consists of 152 people and all of them are being examined using stress questionnaires to earn the stress level. Nurses who have medium stress level are chosen to be the samples of this research, which consist of 42 nurses that are being divided into 2 groups with 21 people in each group. Therefore, 21 people obtain stress management training treatment, and the other 21 people receive physical activity treatment. In this research, the Stress Scale (PSS-10) which is a self-report questionnaire is being used. Moreover, the data is processed using parametric analysis, which is Wilcoxon. Based on the research, it is found that stress management training and physical activity has a significant effect on the reduction of nurses working stress in TK II Putri Hijau Hospital Medan.

Abstract

Hypertension is a cardiovascular disease with a high prevalence and risk of death in developed and developing countries with blood pressure reach ≥140/90 mmHg. Hypertension is the 3rd cause of death after stroke and tuberculosis, reached 6.7% of the population of deaths at all ages in Indonesia. Results of Basic Health Research (Riskesdas) Balitbangkes showed that the national prevalence of hypertension reached 31.7%. The National Formulary (Fornas) is a list of drugs compiled based on the latest scientific evidence by the National Committee for the Compilation of the Fornas. Evaluation of the appropriateness of antihypertensive treatments in outpatients at Batang Beruh Health Centre Sidikalang seen from the National Formulary was the aim of this study. This research is non-experimental, quantitative descriptive was conducted in May - July 2019 at Batang Beruh Health Centre Jl. Pahlawan Sidikalang, Sidikalang District of Dairi, North Sumatra. The population was 231 hypertensive patients using antihypertensive drugs, with sampling using the Slovin formula, amounted to 70 respondents. Based on the research conducted, in the Calcium Antagonist group, the names of the antihypertensive drugs used are Amlodipine and Nifedipine. Meanwhile, in the ACEIs group, the drug used is Captopril. Whereas in the Angiotensin II receptor antagonist group, the drugs used are Micardis, Valsartan and Candesartan. Whereas in the Diuretic group, the antibiotic used is Furosemide.

Abstract

Breathing exercises are designed and performed to achieve more controlled and efficient ventilation. The dilation of blood vessels affects blood pressure, which is to reduce the resistance to blood flow, so that blood pressure tends to be normal. Holding your breath also helps to increase your concentration, and stabilizes your emotions. Thoughts and emotions feel more stable and it is easy to concentrate on holding your breath. In breath-holding meditation practice is very helpful in increasing concentration. By holding your breath when the lungs are filled with air or empty, the process of taking oxygen by the blood stops, resulting in a lack of oxygen in the blood. This condition stimulates the blood to form more Hb cells, so that when you inhale, the amount of oxygen absorbed by the blood increases, as well as when you exhale, the amount of CO2 that is removed is also greater. The holding of breath will also cause a decrease in the amount of oxygen in the body's tissues, which increases the acidity of the body's tissues. The process of moving air in and out of the lungs to facilitate gas exchange with the body's internal environment, primarily by introducing oxygen and removing carbon dioxide. Breathing exercises are designed and performed to achieve more controlled and efficient ventilation.

Abstract

The research objective was to analyze the effect of health service licensing policies on service quality at the Medan City Investment Service and One Stop Integrated Services. This research method is mixed methods, amounting to 27 people. The results of the research on the relationship of health service licensing policies with the satisfaction of the online licensing application for the SiCantik Cloud application, it is known that the licensing policy is good quality service with the relationship between physical evidence and satisfaction of the online licensing applicant for the SiCantik Cloud application, it is known that physical evidence of the applicant is satisfied, the relationship of reliability with the satisfaction of the online licensing applicant of the SiCantik application. Cloud, it is known the reliability of the applicant's clinical licensing officer, the relationship of responsiveness with the satisfaction of the online licensing applicant of the SiCantik Cloud application, it is known the responsiveness of the applicant's clinic licensing officer, the guarantee relationship with the satisfaction of the online licensing applicant for the SiCantik Cloud application, it is known that the clinical licensing guarantee of the applicant is satisfied, the relationship of empathy with the satisfaction of the licensing applicant online application SiCantik Cloud, it is known that the empathy of the applicant's clinical licensing officer is satisfied. Research suggestions are expected to the next researchers to prioritize research related to public services, especially health, because health services have a very important role in people's lives.

Abstract

Work stress is caused by many factors from workplace, family, home, or environment. Excessive work stress will make someone anxious until developing excessive fatigue. Fatigue will decrease performance and increase work error rate that cause fatal workaccident.This study aimed to analyze influence of working stressors against nurses fatigue in patien room in haji hospital Medan. This study was quantitative method with a cross-sectional design, with a sample of 53 respondents. This study used questioners of the NIOSH Generic Job Stress Questionaire and Industrial Fatigue Research Committee which has been translated into Indonesian and has cheeked for validity and reliability. The results of the study regarding the influence work fatigue and work stress (P-value 0,000, r : 1,000), the influence workload and work fatigue (P-value 0,000, r : 1,000), the influence interpersonal conflict and work fatigue (P-value 0,000, r : 1,000), the influence job satisfaction and work fatigue (P-value 0,000, r : -0,588), the influence social support and work fatigue (P-value 0,000, r : -0,892), and based on the logistic regression test workload were variables the most influential, with an odds ratio of 94,5. The conclusion shows that workload is a variable that has strong influence on fatigue. Nurses with heavy workload are 94,5 times more likely to experience fatigue. So that the hospital needs to monitor and evaluate the performances of the nurses so the workload can be fixed and adjusted with the right capacity and ability of the nurses in providing services
