Abstract

The purpose of this study was to determine the effect of providing education on increasing knowledge, attitudes, and behavior of medical officers in solid medical waste management at the Regional General Hospital dr. Zainoel Abidin Banda Aceh, Indonesia. The research design used was a quasi experiment with the Wilcoxon comparison test. The research sample was the medical officer of dr. Zainoel Abidin Banda Aceh as many as 94 people who were divided into treatment and control groups. Knowledge, attitude, and the behavior of medical personnel was measured by interview using a questionnaire. The results showed that there was an effect of educational interventions on increasing the knowledge, attitudes, and behavior of medical personnel in the management of medical solid waste in the treatment and control groups (p <0.05). The results of the Wilcoxon test between the treatment groups on knowledge, attitudes, and behavior before and after the educational intervention showed a significant difference (p = 0.000), whereas in the control group between before and after the educational intervention did not show a difference with the p-value of knowledge (p = 0.102); attitude and behavior (p = 0.157). In the control group, the level of knowledge, attitudes and behavior tends to remain at a sufficient level. The conclusion of this study shows that educational interventions can provide an increase in the knowledge, attitudes and behavior of medical personnel in implementing solid medical waste management at dr. Zainoel Abidin Banda Aceh.

Abstract

The aim of this study was to investigate industrial logging in Businga Territory. The survey was carried out by means of a questionnaire addressed to 235 people including 150 men (64%) and 85 women (36%) between 31 and 100 years of age, on the basis of free and informed consent. It emerged from this study that 110 subjects out of the 235 (46.8%) were from SICOTRA company and its surroundings and 125 respondents (53.2%) belonged to from KKF company and its surroundings. Of the 235 people surveyed, only 104 (or 44.3%) were workers from two companies, the rest were heads of groups, capitas, local notables and those who had not worked in either company. 44.6% of the respondents had a primary level followed by 31.9% (or 75 people) with a secondary level. There are 55 illiterate people (23.4%). No university executives were met in the area during the survey. The logging by these two companies (KKF: from 1949 to 1998 (49 years of exploitation) and SICOTRA: from 1980 to 1998 (18 years)) has caused enormous destruction of the ecosystem and the population is today confronted with multiple difficulties and problems of supply of Non Timber Forest Products (caterpillars, snails, mushrooms, Cola acuminata, etc.) and the erosion of biodiversity. Of the 17 forest species exploited in this Sector, 03 have totally disappeared: Diospyros crassiflora Hiern, Nauclea diderrichii (De Wild.) Merr. and Prioria balsaminfera (Vermoesen) Breteler. The majority contacted had no notion of protecting the forest (67.2% vs. 32.8%); the population had no notion of drawing up specifications (95 vs. 5%). The results revealed that the indigenous population did not benefit from their forest, nor did they benefit from basic social infrastructures of community interest. In order to remedy this situation, a good regulation of the forestry exploitation code, the popularization of the new forestry code and the elaboration of specifications in collaboration with local communities are necessary to correct this situation in the future.

Abstract

Breast cancer is a type of cancer with high incidence and mortality especially in developing countries. Vitamin D regulates the expression a number of genes involved in the development of cancer cells. The aim of this study is to analyze the relationship between 25-hydroxyvitamin D (25 (OH) D) plasma level with breast cancer stage based on menopausal status. This is an observational research method with cross sectional design. Research subjects were 53 newly diagnosed breast cancer patients and had not received chemotherapy. Menopausal status and stage data were obtained from interviews and medical record data. Levels of 25-hydroxyvitamin D plasma were measured (ELISA) method. The results obtained Stage II, III and IV each have an average level of vitamin D of 28,56 ng/ml (95% CI; 23,61 – 33,52 ng/ml), 28,18 ng/ml (95% CI: 24,49 – 31,87 ng/ml) and 27,86 ng/ml (95% CI: 22,68 – 33,04 ng/ml).The average plasma concentration of 25 (OH) D in pre-menopausal patients is 28,54 ng/ml and average plasma 25 (OH) D levels in post-menopausal patients is 27,79 ng/ml. There was no significant relationship between plasma levels of 25 (OH) D and breast cancer stage in both pre-menopausal and post-menopausal patients.

Abstract

Asphalt pavement susceptibility to moisture damage is considered as a major issue in the durability and service life of the roadway. Quick and nondestructive testing of asphalt concrete pavement are the major concern for predicting its suitability for evaluation. In the present investigation, nondestructive test has been implemented to detect the moisture damage issue of asphalt concrete mixture. Asphalt concrete specimens were prepared using Marshall method. Aggregates gradation of wearing, binder and base course was implemented for the preparation of the specimens. Specimens were tested for ultrasonic pulse velocity before and after practicing the moisture damage procedure. The variations of seismic modulus among various gradation before and after the moisture damage were considered as a criterion for moisture damage and related to tensile strength ratio TSR. It was observed that the pulse velocity decline by a range of (11 to 16) for asphalt concrete after moisture damage. It was concluded that the Seismic modulus as calculated from the ultrasonic pulse velocity test was effective in distinguishing the impact of moisture damage. The seismic modulus at optimum asphalt content decline by (34.7, 46.7, and 52.6) % after moisture damage for wearing, binder, and base course mixtures respectively. The ultrasonic pulse velocity test is recommended for assessing the susceptibility of asphalt concrete mixture to moisture damage.

Abstract

The present research has been carried out in three different groupings, namely Mudikwiti, Tanganga and Nkata in Kwilu province, in the Democratic Republic of the Congo. 165 coalmen from eighteen villages have been considered as the sample used in this work. They are selected from all social strata, and their ages vary from 18 to 30 years old and from 44 to 56 years old. They have made 21,899 ovens, among which there were 12, 287 ovens of small dimensions, that is, 1 m x 6 m x 3 m, and 9,612 ovens of big dimensions: 6 m x 20 m x 3 m. They have cut 196,694 trees of different species to fill in the two kinds of ovens, say 99,984 trees for filling in small ovens and 96,710 trees to fill in big ovens. On the whole, they have produced 852,708 sacks of charcoal per year. The Nkata grouping, with its 11 villages, has produced 129,600 sacks of 45 kilograms, divided as follows: 14,400 sacks from the small dimension ovens and 115, 200 sacks from the big dimension ovens.

Abstract

The 2019 coronavirus pandemic (covid-19), which spread rapidly around the world, has disrupted all organizations and led to a decline in their productivity. Their reorganization was therefore essential. The purpose of this study was to analyze the resilience strategies adopted by the National Blood Transfusion Service (NBTS) to reverse the trend of population participation. Thus, based on the qualitative approach centered on free interviews conducted with 30 people including 27 managers of Blood Transfusion Establishments (Blood Transfusion Center (BTC), Blood Transfusion Antenna (BTA) and Sampling Site (SS)), 2 heads of department (Communication and Coordination of services), the chairman of the scientific advisory council and a direct observation, of the theory of resilience and the systemic method, the analysis of the data indicates that the resilience strategies adopted by the NBTS are four: the dynamic communication on blood donation, rational blood using, the reorganization of blood collections in fixed site (Sundays and extension of the collection time) and the reorganization of mobile collections (fragmentation of mobile collection activities on site).

Abstract

The world is facing an unprecedented health crisis caused by the COVID-19 pandemic. The DR Congo with its 90 million inhabitants and more than 500,000 refugees from neighboring countries recorded the first confirmed case in Kinshasa on March 10, 2020. Currently, the country is counting 16,039 cases of infection, 566 cases of death and 13,536 cases of recovery according to the Technical Secretariat Response Team COVID-DRC, (December 23, 2020). After reporting the first case, Congolese government has taken protective health measures whereby it is in a worrying economic situation that was immediately noticeable by the increase in the rate of the foreign currency (the dollar) compared to the local currency. From an extroverted economy, the Covid-19 pandemic has put it in a situation of stagnation, which could lead country into an economic recession if this scourge persists.

Abstract

Foville Syndrome is a rare clinical feature of stroke or brain hemorrhage. Few cases have been reported worldwide particularly in Asian, Indonesia, Aceh. Patient 1: We report the case of a female 64 years old patient, was admitted in the Neurology Department of Dr. Zainoel Abidin General Hospital, Banda Aceh, Indonesia, for an abrupt onset of right body side weakness two day before admission. That weakness was preceded by acute headaches and rotatory vertigo with vomiting. with hypertension history since 5 years. Physical examination found paralysis of right oculomotor movements with intact response of pupils to light, right trochlear nerve palsy, left ipsilateral facial nerve palsy, and right hemiparesis which constitute of Foville syndrome. The brain CT scan shown in the right pontine hypodensity area is pontine infarct. Etiological investigations done to chronic hypertension. Patient received antiplatelet therapy and control of vascular risk factors for secondary prevention (hypertension theraphy), and physiotherapy as treatment with clinical improvement. And she has been discharge 8 days later. Patient 2: 45-year-old right-handed female, was admitted in the Neurology Department of Dr. Zainoel Abidin General Hospital, Banda Aceh, Indonesia, for an abrupt onset of right body side weakness two day before admission. That weakness was preceded by acute headaches and rotatory vertigo with vomiting. with hypertension history since 5 years. Physical examination found paralysis of right oculomotor movements with intact response of pupils to light, right trochlear nerve palsy, left ipsilateral facial nerve palsy, and right hemiparesis which constitute of Foville syndrome. The brain CT scan shown spontaneous hemorrhage in the left pontine’s. Etiological investigations done to chronic hypertension. Patient received therapy control of vascular risk factors for secondary prevention (hypertension theraphy), and physiotherapy as treatment with clinical improvement. And she has been discharge 10 days later. The diagnosis and management of brainstem stroke bring a considerable burden to the healthcare system, the patient, the family members, and the society at large. The slow increase in the global burden of stroke has been steadily increasing.

Abstract

The Indonesian government has developed a Health Insurance program National Social Security Administering Bodies (BPJS) Health. One of the ways to pay for health services for hospitals in Era JKN is the Indonesian Case Base Groups (INA-CBG's) payment system, namely the amount of claim payment by BPJS Health to the hospital for the package services based on diagnosis of disease and procedures. This study aims to analyze hospital real rates and INA CBG rates on the action of sectio caesarea and to find out the efforts made hospitals to cover INA-CBG's shortfall in claims costs. Types of research Qualitative descriptive conducted by in-depth interviews. Research sites at the hospital. Imelda Indonesian Worker having his address at Jalan Bilal No. 24 Kelurahan Pulo Brayan Darat I, East Medan District. The research was carried out since months January 2019 until finished. The population in this study is claim data delivery of inpatients who performed sectio casearea at month March and April as many as 143 cases The samples in this study were all population of 143 cases. The results showed that the difference in real rates hospital and INA-CBG rates on Sectio Caesaria at RSU. Imelda Worker Indonesia Medan in 2019 amounted to 73% of the real hospital rates exceeding the tariff INA-CBG's with a total difference in costs reaching IDR 199,661,028. and 39% real hospital fee less than INA-CBG's package rate. Efforts are conducted by the RSU. Imelda Indonesian workers to cover the shortage of costs INA-CBG's claims include using students who come from schools Imelda's own foundation, set the Sectio Caesare service package on general patients and providing supporting facilities. Suggested to RSU. Imelda Indonesian workers evaluate the financial risks received from services Sectio Caesare and evaluating the distribution of INA-CBG's claim costs for services Sectio Caesare is up to standard.

Abstract

In the midst of the Covid-19 pandemic, there is one social phenomenon that has the potential to aggravate the situation, namely stigma in patients suspected of Covid-19 or rapid reactive results. The lab results have not actually confirmed that the patient is positive for Covid-19 but the social environment or negative associations of a person or group of people who experience symptoms or bear certain diseases result in stigma against a person. This study aims to find out how the experience and stigma in patients suspected of Covid - 19. This study uses qualitative approach method of phenomenological studies and in-depth interviews with telenursing. The number of participants in this study was 10 participants. This study obtained 5 themes, namely physiological experience, psychological experience, biological experience, sociological experience and spiritual experience. The high positive number of Covid-19 in Indonesia makes the emergence of negative stigma of society to patients exposed to Covid-19, not even just Patients who are positive of Covid-19 who get negative stigma, therefore for diagnostic enforcement it takes examination by Molecular Test: Real Time PCR (Polymerase Chain Reaction) method. The sample types used are bronchoalveolar lavage, nasopharyngeal or oropharyngeal swabs, sputum, tracheal aspirate, nasopharyngeal aspirate or nasal wash, and biopsy tissue including from the lungs. Real Time PCR check method is the golden standard for the enforcement of diagnostic Covid - 19.
