Abstract

Electrolyte disorders may interfere with human bodily function. Potassium is the most dominant intracelullar ion; lack (hypokalemia) or excess (hyperkalemia) potassium might give rise to cardiovascular and neuromuscular symptoms. Management consist of potassium serum correction alongside frequent electrocardiography monitoring. Implications of hypokalemia and hyperkalemia should always be taken considered, especially in elderly populations. Central nervous system involvement is currently unknown.

Abstract

Corticosteroids have been widely used in various medical situations and often used for their anti-inflammatory and immunosuppressive effects. Corticosteroids can be given by many routes such as topical, oral, intramuscular, or intravenous. However, prolonged use of steroids can resultin hypothalamic-pituitary-adrenal axis suppression and other side effects. Dosage and using strategy are needed to get the best benefits while minimizing the risk of side effects

Abstract

Varicella or chickenpox is a highly contagious infectious disease. Transmission of varicella-zoster infections in neonates can occur through transplacental (congenital), or postnatal (acquired) route. Diagnosis of neonatal varicella is based on contact history and clinical presentation. Neonatal varicella had risk for severe disease and high mortality rate.

Abstract

Lambert-Eaton myasthenic syndrome (LEMS) is a neuromuscular junction disorder resulting from a paraneoplastic or autoimmune reaction. More than half LEMS cases are associated with small cell lung carcinoma (SCLC). The main clinical manifestation is muscle weakness. The pathophysiology is due to the generation of the autoantibodies against voltage-gated calcium channels (VGCCs) on presynaptic nerve terminals leading to a decrease in neurotransmitter acethylcholine (ACh). Various treatments such as tumor treatments, immunomodulatory, and symptomatic treatment have been studied, but further research is needed.

Abstract

Background: Exclusive breastfeeding is considered as one of many crucial factors in child development. The research was conducted with the aim of to determine the relationship between exclusive breastfeeding and child development. Method: An observational analytic study with cross-sectional design; the independent variable was exclusive breastfeeding and the dependent variable was child development according to KPSP score. A total of 86 samples were obtained using consecutive sampling method. Data were analyzed using the Chi-square test. Result: A significant positive correlation between exclussive breastfeeding and child development (p=0.035). Conclusion: There is a correlation between exclussive breastfeeding and child development in children at Wangaya Denpasar Regional Hospital.

Abstract

Bakcground: Diarrhea is the leading cause of death among children worldwide, including in Indonesia. However, there is no scientific research on the most common risk factors for diarrhea among children in Indonesia. A systematic literature review was conducted to highlight the most predisposing risk factors of diarrhea based on the epidemiological triangle, in order to improve preventive measures by society to reduce the incidence of childhood diarrhea. Methods: A systematic review of observational studies without limitation of year of publication was performed in 2023 based on PRISMA protocol. Results: A total of 2607 studies were identified from the Pubmed database, only 9 studies were extracted and assessed using the STROBE method. The risk factors were divided into 3 categories: environmental, host and other, to follow the rule of the epidemiologic triangle of disease. The highest odds ratios were found for the factor "child defecation in open places" (OR=10.47) from the environmental category and for the factor "child eating by hand" (OR=5.6) from the host category. Lower maternal education from the host category was also influential (OR=1.52). Conclusion: Improper public disposal of child feces, child behavior of eating with hand and low maternal education are the most significant contributing risk factors of child diarrhea in Indonesia.

Abstract

Dementia causes impaired cognition, behavior, and daily functional activities with physical, mental, and psychosocial consequences for patients, families, and communities. However, dementia diagnosis is still challenging. Classification of dementia subtypes also requires structuraland functional imaging studies. The structural imaging needed to support Alzheimer’s dementia and vascular dementia diagnosis is brain magnetic resonance imaging (MRI), frontotemporal dementia diagnosis can be supported by brain MRI or functional imaging, while Lewy Body dementia diagnosis requires functional imaging and biomarkers findings.

Sepsis is a life-threatening organ dysfunction caused by dysregulation of the immune response to infection. Antithrombin III is a single plasma bound glycoprotein and include a class of serine protease (serpine) inhibitors; its synthesis in the liver decreases during sepsis.

Abstract

Streptococcus Grup A (SGA) atau Streptococcus pyogenes, merupakan bakteri yang sering menyebabkan infeksi saluran napas dan infeksi kulit pada anak. Organisme ini juga dapat menimbulkan infeksi invasif, yaitu sindrom syok toksik streptococcal dan necrotizing fasciitis; serta berbagai komplikasi dan sekuele, di antaranya demam rematik akut, glomerulonefritis akut pasca-Streptococcus, artritis reaktif pasca-Streptococcus dan pediatric autoimmune neuropsychiatric disorders associated with Streptococcus pyogenes (PANDAS). Peningkatan kasus infeksi SGA, baik invasif maupun non-invasif, dengan puncaknya di akhir tahun 2022 lalu, khususnya pada populasi anak, telah menjadi perhatian di beberapa negara. Klinisi sangat perlu mengenali berbagai spektrum klinis infeksi SGA, khususnya pada populasi anak.

Abstract

Citicoline has a neuroprotectant effect, especially in mild-to-moderate stroke. The presence of the ε4 allele in the ApoE gene is associated with a severe acute ischemic stroke (AIS) phenotype, in addition to neurodegenerative risk This study aims to determine the effect of citicoline on cognitive and motoric recovery in severe AIS biomolecularly, such as ApoE4 rs429358 and rs7412 polymorphisms. A 64-year-old woman was diagnosed with AIS with a 6-hour onset and with a NIHSS score of 25 and MMSE score of 21. The patient’s AIS was biomolecularly severe as proven by PCR-RFLP (polymerase chain reaction-restriction fragment length polymorphism) ApoE4 which yielded 482 bp at rs429358 (codon 112) and 249 bp 167 bp at rs7412 (codon 158) on electrophoretic visualization. The patient received 3x 500 mg intravenous citicoline for 4 days followed by another 1000 mg of citicoline intravenous during polyclinic control for 5 consecutive days. On the 9th day, the patient experienced improvement in cognitive function with an MMSE score of 29 and superior (4/5) and inferior (5/5) motoric strength. Citicoline is a neuroprotectant agent that is beneficial for the recovery of cognitive and motoric function in biomolecularly severe SIA (ApoE4, C/C, Arg112/Arg158) because it can save the penumbra area from infarction.
