Abstract

Hydropneumothorax is a medical emergency, a condition in which the presence of air and fluid in the pleural cavity results in the collapse of lung tissue. The prevalence of hydropneumothorax has not been recorded, but ranges from 2.4-17.8 per 100,000 population each year. This case study aims to describe the nursing care of spontaneous ventilation disorders in Mr. D with post-thoracostomy decorticization of indications of hydropneumothorax dextra in the Intensive Care Unit (ICU) Room of Prof. Dr. Margono Soekarjo Purwokerto Hospital. The research method is a descriptive case study. Data collection techniques, including: interviews, physical examinations, observations, documentation studies. Patients with a nursing diagnosis of spontaneous ventilation disorders are given a semi-fowler position. It was concluded that the case of hydropneumothorax dextra, post thoracostomy was a problem with airway obstruction. This leads to weakening of the respiratory muscles and impaired spontaneous ventilation. The action given, namely the provision of a semi-fowler position, effectively helps reduce shortness of breath, increases respiratory rate. After nursing care was carried out, spontaneous ventilation disorders in Mr. D with post-thoracostomy decortication of indications of hydropneumothorax dextra in the Intensive Care Unit (ICU) Room of Prof. Dr. Margono Soekarjo Hospital Purwokerto have not been resolved.

Abstract

Background: Obesity is a major contributor to degenerative diseases and continues to show high prevalence. Identifying key risk factors is essential for targeted intervention. Objective: This study aimed to analyze the dominant risk factors for obesity at the Johar Baru Community Health Center, Central Jakarta. Methods: An observational analytic study with a cross-sectional design was conducted using secondary data. From a population of 102,647 individuals, 72,680 were included through total sampling. Data were analyzed using Chi-square tests and multiple logistic regression (enter method) with a significance level of 0.05. Results: Bivariate analysis showed that gender, lack of physical activity, high sugar and fat consumption, and low fruit and vegetable intake were significantly associated with obesity (p </ 0.05). Multivariate analysis identified gender as the strongest risk factor (OR = 12.912; p = 0.000). Conclusion: Gender, physical inactivity, excessive sugar and fat intake, and inadequate fruit and vegetable consumption were significantly linked to obesity. Gender emerged as the most dominant factor. These findings support the development of targeted interventions focusing on promoting balanced diets, regular physical activity, and awareness of dietary fat types to address obesity at the community level.

Abstract

Background: Management of puskesmas is critical to providing quality health services through effective planning, organising, implementing, and controlling. Improving the quality of health services builds patient trust. The Healthy Indonesia Programme aims to improve services at first level health facilities, including puskesmas, but many still do not meet quality standards. This study evaluates the management cycle of puskesmas at the South Sorong District health office. Objectives: This study aims to analyse the evaluation of the implementation of the puskesmas management cycle at the South Sorong District Health Office. Methods: A qualitative approach was used with informants from four puskesmas: Teminabuan, Konda, Kokoda, and Kais Darat, representing 16 puskesmas in the region. Results: Some puskesmas follow the planning stages involving mini-workshops and Introspection Survey data, but not consistently. There were challenges maintaining the monthly mini-workshop schedule and using the monthly work programme plan results. Monitoring processes were not always optimal, with most responsibility falling on the head of the puskesmas and administrative staff, while the role of the programme focal point was not fully maximised. Conclusion: puskesmas should improve the consistency, quality, and effectiveness of management through increased managerial competence, efficient budget management, and optimisation of planning, implementation, and monitoring processes.

Abstract

Background: Quality health services are a basic need and expectation for all individuals seeking care. Health facilities are expected to meet patient expectations through service delivery. Objective: This study aimed to evaluate health service quality based on accreditation implementation across five dimensions responsiveness, assurance, tangibility, empathy, and reliability and their effect on patient satisfaction. Methods: A quantitative study with a cross-sectional design was conducted at the Teminabuan Health Center, South Sorong Regency. A total of 92 patients participated, and data were collected using a structured questionnaire assessing satisfaction across the five service quality dimensions. Results: Partial analysis showed that assurance and tangibility significantly influenced patient satisfaction (p < 0.05), while responsiveness, empathy, and reliability did not (p > 0.05). The F-test indicated a significant simultaneous effect of all five dimensions on satisfaction (p = 0.000 < 0.05). Conclusion: The five dimensions of service quality collectively impact patient satisfaction at Teminabuan Health Center. However, assurance and tangibility were the only dimensions with a significant partial effect. These findings highlight the need to strengthen specific service aspects to enhance overall patient satisfaction.
