Abstract

Background & Objective: Chronic Kidney Disease (CKD) is a progressive and irreversible decline in kidney function that causes the body to be unable to maintain metabolism and fluid and electrolyte balance, which can trigger hypervolemia. This study aims to investigate the application of fluid balance monitoring in Chronic Kidney Disease (CKD) patients with hypervolemia. Method: A descriptive method was used, incorporating case study results.  Result: Fluid balance monitoring was conducted over three days in CKD patients with hypervolemia. The results indicated that the hypervolemia issue remained unresolved, as evidenced by persistent edema in the lower extremities, unchanged urine output, and fluid balance monitoring results showing intake of 1,076 cc, output of 592 cc, and a fluid balance of +484 cc. Conclusion: Fluid balance monitoring is important in managing hypervolemia in patients with Chronic Kidney Disease (CKD), but it requires time and further approaches to achieve nursing care objectives.

Abstract

Background & Objective: Diabetic ulcers are a condition that occurs in patients with diabetes mellitus due to nerve abnormalities and peripheral artery disorders, leading to infection in wounds and tissue damage in the skin of the feet (Roza et al., 2015). The objective of this scientific study is to apply wound care and evaluate the effectiveness of wound care using 0.9% NaCl in patients with Type II diabetic ulcers. Method: The method used in this scientific study is a case study. The subject of this study is one patient with Type II Diabetic Ulcer who is undergoing treatment in the Wijaya Kusuma Bawah ward of Kardinah General Hospital in Tegal City. Data collection was conducted using interview techniques, observation, and documentation. Result: Nursing care was provided for three consecutive days. During the assessment, it was found that the patient experienced pain in the left foot wound, which had been present since September but had not healed and had worsened. The wound was dirty, smelly, and oozing pus. The intervention provided was wound care using EBN with 0.9% NaCl, implemented over three consecutive days. By the third day, the evaluation showed that skin integrity had been restored. Conclusion: The author concludes that wound care using 0.9% NaCl is effective, as there was an improvement in the wound condition.

Abstract

Background & Objective: Breast milk is an important component that needs to be considered, especially in terms of production and flow, as it has various benefits for the growth and development of infants. Mothers who have undergone a cesarean section often experience problems with breastfeeding due to inadequate breast milk production. Breastfeeding challenges in postpartum cesarean section mothers are caused by a decrease in oxytocin and prolactin hormones, which play a role in the smooth production of breast milk. To facilitate breast milk production, non-pharmacological efforts are needed, such as Breast Care. Breast Care is an effort to maintain the condition of a breastfeeding mother's breasts, both during pregnancy and after childbirth (postpartum period). Method: A descriptive method was used with the application of case study results. Result: The application of Breast Care conducted on January 2–4, 2025, to a post-cesarean section patient with breastfeeding issues was ineffective in the Mawar Room of Kardinah General Hospital in Tegal City. On the third day of the study, breast milk flow improved from a scale of 5 to a scale of 2, showing a significant increase. Conclusion: It can be concluded that the Breast Care technique is effective in increasing breast milk production in post-cesarean section patients in the Mawar Ward of Kardinah General Hospital in Tegal City.

Abstract

Background & Objective: A preterm infant is defined as a baby born before 37 weeks of gestation (Kemenkes, RI, 2024). The World Health Organization (WHO), 2023 , categorizes preterm births based on gestational age: ≤28 weeks is extremely preterm, ≤32 weeks is very preterm, and 37 weeks is moderately preterm. According to (Kemenkes, RI, 2024), the WHO estimates that 13.4 million babies were born prematurely in 2020. Indonesia ranks fifth highest in the world for premature births, with approximately 657,700 cases. One of the issues faced by premature infants is metabolic disorders, where premature infants are at risk of experiencing a decrease in body temperature due to low body fat reserves and incomplete brain maturation in regulating the infant's body temperature (Rufaindah et al., 2022). The objective is to analyze the effect of nesting therapy on changes in thermoregulation in premature infants in the NICU of Kardinah General Hospital, Tegal City. Method: descriptive study using case study results with a nursing care process approach. Instruments: pediatric nursing care format, digital thermometer. Result: The application of nesting therapy for three days on premature infants with hypothermia resulted in changes in body temperature within the normal range for premature infants. Conclusion: Nesting therapy is effective in increasing body temperature in premature infants, as their minimal movement contributes to energy conservation. This study is expected to enhance the application of developmental care, particularly in premature infants.

Abstract

Background & Objective: Dengue Hemorrhagic Fever (DHF), medically known as Dengue Hemorrhagic Fever/DHF, is an infectious disease caused by the dengue virus and transmitted through the bite of the Aedes aegypti mosquito. It is characterized by sudden fever lasting 2 to 7 days without a clear cause, weakness/fatigue, restlessness, abdominal pain, accompanied by skin bleeding manifestations such as petechiae, ecchymoses, or rashes (petechiae). A warm compress is a procedure using a cloth, washcloth, or towel soaked in warm water, then applied to specific body parts to provide comfort and lower body temperature. The purpose of this study is to describe cases of Dengue Hemorrhagic Fever in detail and depth. Method: This study was conducted using a qualitative descriptive method with a case study approach. The study was conducted at Kardinah General Hospital in Tegal City on patients with Dengue Hemorrhagic Fever/DHF on December 10, 2024. Result: The application of warm compresses as a non-pharmacological therapy was effective in reducing the body temperature of patients. Warm compresses were proven to have a positive effect in managing fever caused by DHF, resulting in a decrease in patients' fever after treatment was administered. Conclusion: It is recommended that research in the field of nursing, particularly pediatric nursing for patients with DHF, be further enhanced to improve the quality of care and provide more specific and targeted nursing interventions.

Abstract

Background & Objective: Lymphadenopathy is the enlargement of lymph nodes or a condition in which lymph nodes become enlarged to a size greater than 1 cm and the epicondylar lymph nodes are palpable with a size greater than 5 mm, which is an abnormal condition. Method: This study employed a qualitative approach using a case study design with a nursing perspective. Result: The case study results indicated that warm compresses were effective in reducing acute pain in An. N.S.A. with lymphadenopathy. Conclusion: After receiving nursing care for 4 days, the evaluation results for the initial diagnosis were supported by the client's subjective data stating that the pain was no longer felt, reinforced by objective data showing a body temperature of 36.5°C, a pulse rate of 95 beats per minute, a respiration rate of 20 breaths per minute, and no further pain reported on the pain scale. The assessment results indicated that the issue was resolved, and the intervention plan was discontinued as the intervention had been implemented for 4 days. In the second diagnostic evaluation, the subjective findings indicated that the client's family reported that the child had finished their meal portion. This was supported by objective data showing that the patient did not appear pale and had finished their meal portion. The assessment concluded that the issue had been resolved, and the intervention plan was discontinued as the necessary actions had been implemented over the 4-day period.

Abstract

Background & Objective: Stroke is damage to the brain that occurs due to a sudden decrease or interruption in the flow of oxygen in the blood. The objective of this study is to provide appropriate nursing care for patients with non-hemorrhagic stroke in a detailed and comprehensive manner, with a focus on the application of physical exercise for patients with non-hemorrhagic stroke and physical mobility impairments at RSD Gunung Jati Hospital in Cirebon City. Method: This study adopts a descriptive design using a case study approach. The steps include data collection, diagnosis, planning, intervention, and evaluation. The study was conducted at RSD Gunung Jati Hospital in Cirebon City in February 2025, with data collected through interviews, observations, physical examinations, and medical records. Result: After undergoing Range of Motion (ROM) training, patients showed improvements in flexibility, muscle strength, and movement coordination. Conclusion: These findings indicate that an approach using joint strengthening exercise techniques contributes to reducing physical mobility issues and enhancing patients' independence in performing daily activities.

Abstract

Background & Objective: One of the symptoms of schizophrenia is auditory hallucinations. Hallucinations are false perceptions that occur as a result of maladaptive neurobiological responses. Hallucinations can be treated using two techniques: pharmacological and non-pharmacological. Pharmacological techniques involve collaborating with a doctor to administer medication such as Haloperidol, Serenace, Lodomer, and Chlorpromazine. Non-pharmacological techniques in this study include video therapy. The objective is to determine the effectiveness of video therapy in treating sensory perception hallucinations at Magelang Mental Hospital. Method: This study uses a qualitative approach with a case study design. Result: The case study results showed that sensory perception disorders were resolved. Therefore, it can be concluded that nursing care, which includes assessment, diagnosis, intervention, implementation, and evaluation, combined with the application of video therapy, effectively reduces the level of hallucinations, frequency of hallucinations, and duration of hallucinations in schizophrenia patients. Conclusion: After implementing video therapy for 3 days, it was found that video therapy can be applied to schizophrenia patients with auditory hallucinations.

Abstract

Background & Objective: The increase in non-communicable diseases (NCDs) poses a challenge to the healthcare sector in maintaining quality of life. One of the most prevalent NCDs today is hypertension. Hypertension is classified as a silent killer and is spreading worldwide. Hypertension can be managed through pharmacological and non-pharmacological therapies. Non-pharmacological therapy can be administered through complementary therapy, such as herbal medications, and one herbal treatment believed to lower hypertension is the use of boiled bay leaf water. Method: The research method used a literature review of studies published between 2020 and 2024. The literature search employed a data strategy using the keywords “hypertension” and “bay leaves,” utilizing the Google Scholar and PubMed databases. The criteria included articles discussing the relationship between hypertension and bay leaves, with full-text availability. Result: The results from 10 articles indicate that consuming a decoction of bay leaves is effective in lowering blood pressure in hypertensive patients. Conclusion: It is recommended that hypertensive patients consume a decoction of bay leaves regularly to achieve effective results, but they should also maintain a balanced diet and engage in sufficient physical activity.

Abstract

Background & Objective: Breastfeeding is very important for a mother for the development and growth of her child. Breastfeeding problems often occur in mothers due to low milk production in the first few days postpartum, which becomes an obstacle in providing exclusive breastfeeding to their babies. This case study aims to determine the application of oxytocin massage in increasing milk production in postpartum mothers. Method: The research design used in this study is descriptive with a case study method. The subject of the case study is Mrs. N, a postpartum mother on her first day in the Mawar Ward of Kardinah General Hospital in Tegal City. Result: The results of the case study conducted over three days showed an increase in breast milk production in Mrs. N, as evidenced by: the client stating that her breast milk flowed smoothly after the oxytocin massage. Conclusion: The application of oxytocin massage as a nursing intervention for Mrs. N, who was experiencing breastfeeding issues, was proven to be effective in increasing breast milk production. This intervention was conducted over three days and showed positive results on the third day, indicating that oxytocin massage could serve as an effective alternative intervention to address insufficient breast milk supply.
