Abstract

ABSTRACT Background: Male infertility is medical condition attributed by many factors including hormonal imbalance, oxidative stress, lifestyle factors, and dysbiosis of the gut microbiota. The purpose of this review is to discuss alternative microbiota-targeted interventions like probiotics, synbiotics, and phytotherapeutics, as well as to investigate the role of gut dysbiosis in male reproductive dysfunction and assess the therapeutic potential of fecal microbiota transplantation (FMT). Reviews: The review explores the growing role of gut microbiota modulation, specifically through Fecal Microbiota Transplantation (FMT), in enhancing male reproductive health. Preclinical studies show that FMT can improve sperm motility, morphology, and count, restore testosterone levels, and reduce oxidative stress markers in testicular tissues. Furthermore, FMT can modulate the gut-testis axis by influencing immune responses, metabolic profiles, and hormonal regulation. Animal models have demonstrated that microbial restoration may reverse infertility associated with high-fat diet or antibiotic-induced dysbiosis, supporting the potential of FMT as a therapeutic strategy. Summary: The idea of a gut–testis axis, in which intestinal microbiota can affect testicular function, spermatogenesis, and hormonal balance through immune, metabolic, and endocrine pathways, is supported by data from both animal and human studies. Abstract

Background : Erectile dysfunction is the inability to achieve or maintain an erection sufficient for sexual activity. Insomnia is a sleep disorder that commonly occurs in individuals of productive age and may impact physiological functions of the body, including erectile function. Objective : This study aimed to determine the association between insomnia and erectile dysfunction among male employees of Udayana University Hospital. Methods : This analytical study was conducted with a cross-sectional design. The research respondents were obtained through purposive sampling and comprised 42 male employees aged 28–40 years. Data were collected using the Insomnia Severity Index (ISI) questionnaire to assess insomnia, and the International Index of Erectile Function-5 (IIEF-5) to assess erectile function. The collected data were then processed and analysed using SPSS 27 software. Results : Results indicated that 35.8% of respondents experienced insomnia, whereas 45.3% of respondents experienced erectile dysfunction. The chi-square test showed that males with insomnia had a four-fold higher risk of experiencing erectile dysfunction compared to those without insomnia (p=0.038). Conclusion : There is a significant association between insomnia and erectile dysfunction among male employees of Udayana University Hospital. Abstract

Background : Infertility is a global health problem affecting 1 in 6 people worldwide, with one major cause being impaired spermatozoa motility, which is negatively affected by alcohol consumption. Balinese arak, a traditional alcoholic beverage in Bali with a high ethanol content, has the potential to reduce sperm quality. Objective : This study aimed to evaluate the effect of Balinese arak administration on spermatozoa motility in Wistar rats (Rattus norvegicus). Methods : This experimental study used a randomized post-test only control group design conducted at the Integrated Biomedical Laboratory Unit, Faculty of Medicine, Udayana University. A total of 13 male Wistar rats (Rattus norvegicus) were randomly allocated into a control group (P1, n=6) receiving distilled water and a treatment group (P2, n=7) receiving Balinese arak at a dose of 0.5mL/200 g body weight/day for 45 days. Sperm motility (total, progressive, non-progressive, and immotile) was examined using a light microscope (400x magnification) and analyzed using SPSS 26 with an independent t-test. Results : The mean total sperm motility in the treatment group (49.57±5.639%) was lower compared to the control group (69.00±4.865%). Independent t-test demonstrated significant differences in total motility (p=0.026), non-progressive motility (p=0.015), and immotile sperm (p=0.026). However, progressive motility did not differ significantly between the two groups (p=0.354). Conclusion : Administration of Balinese arak resulted in a significant decrease in total and non-progressive motility and a significant increase in immotile sperm in Wistar rats, but no significant effect on progressive motility was found. Abstract

Background: Antibodies against sperm, known as ASA, have been found in infertile men as a potential cause of male infertility. Reviews: Several reasons for ASA production have been discovered, resulting from an abnormal interaction between mature germ cells and the immune system. The WHO manual 5th edition described ASA testing with mixed anti-globulin reaction and immunobead binding test, which is now included in the extended semen tests in the WHO manual 6th edition. The connection between ASA and infertility can be considered intricate. Sperm agglutination may suggest the existence of anti-sperm antibodies (ASA) but is not enough for diagnosing immunological infertility. Nevertheless, ASA can also be found even without sperm agglutination. Summary: ASA testing for immunological infertility is controversial, with limited evidence supporting its use and treatment options, and is not recommended as an initial step but might be considered in specific cases of natural conception attempts.

Abstract

Background: Type 2 diabetes mellitus (T2DM) can cause erectile dysfunction (ED) complications in the long term, the severity of which is considered important to correlate with blood glucose control, as measured by HbA1c levels.Objective: To determine the relationship between HbA1c levels and the severity of erectile dysfunction in T2DM patients at the Andrology Clinic of Dr. Ramelan General Hospital, Surabaya. Methods: Observational analysis using a cross-sectional approach, utilizing secondary medical record data from 25 patients during the period 2019–2024, in which the severity of ED was measured using the Erection Hardness Score (EHS) and analyzed using the Spearman's rank correlation test.Results: A total of 64% of patients were in the diagnostic range for diabetes, and there was a significant relationship between HbA1c levels and the severity of ED, as indicated by a p-value of 0.043 and a correlation coefficient of r=0.407 (moderate positive correlation).Conclusion: There is a relationship between HbA1c levels and the severity of erectile dysfunction, where a moderate positive correlation indicates that higher HbA1c levels are associated with lower severity or improvement of erectile dysfunction. The moderate strength of the correlation suggests that HbA1c is not the only factor influencing the occurrence of erectile dysfunction.

Abstract

Backgrounds: Chronic stress is an important psychological factor associated with impaired male sexual function. However, experimental evidence regarding its effect on the motivational component of sexual behavior remains limited. The Sexual Incentive Motivation test can evaluate female-directed sexual motivation without direct copulatory contact. Aims: This study aimed to evaluate the effect of chronic unpredictable mild stress on sexual incentive motivation in male mice and to observe whether post-stress Tribulus terrestris intervention was associated with behavioral restoration. Methods: This experimental laboratory study used a randomized posttest-only control group design. Twenty-four healthy male BALB/c mice aged 8-10 weeks were randomly allocated into four groups: control, chronic unpredictable mild stress, placebo recovery, and Tribulus terrestris intervention. The chronic stress, placebo recovery, and Tribulus terrestris groups were exposed to chronic unpredictable mild stress for four weeks. The placebo recovery and Tribulus terrestris groups then underwent an additional two-week post-stress period before behavioral assessment. Sexual motivation was evaluated using an EthoVision-based Sexual Incentive Motivation arena for 10 minutes. The observed parameters were total distance moved, female-zone frequency, and female-zone cumulative duration percentage. Results: Chronic stress was associated with reduced locomotor activity and lower female-directed sexual incentive behavior. The placebo recovery group showed a behavioral pattern similar to the stressed group, suggesting limited spontaneous recovery. The Tribulus terrestris group demonstrated higher female-zone exploration and duration, indicating improved female-oriented sexual incentive motivation after stress exposure. Conclusions: Chronic unpredictable mild stress reduced sexual incentive motivation in male mice. Post-stress Tribulus terrestris intervention was associated with improved female-directed exploratory behavior.
