Abstract

Chronic kidney disease patients undergoing haemodialysis with AV fistula vascular access often experience pain during needle insertion. Recurrent pain can reduce patient comfort and quality of life, necessitating effective non-pharmacological nursing interventions. Hand massage and lavender aromatherapy are non-pharmacological interventions with relaxing and analgesic effects are known to have relaxing and analgesic effects that have the potential to reduce pain intensity. This study aimed to determine the effect of relaxation techniques such as hand massage and lavender aromatherapy on reducing pain during AV fistula needle insertion in chronic kidney disease patients at Bhayangkara Hospital, Bandar Lampung, in 2025. This study used a quasi-experimental design with a pre-test–post-test control group design. The sample size was 50, consisting of 25 in the intervention group and 25 in the control group, The findings are primarily applicable to patients with severe initial pain. The intervention group received hand massage and lavender aromatherapy, while the control group received standard care. Data analysis used within-group differences were analyzed using paired t-test, while between-group differences were analyzed using independent t-test. The average pre-test pain scale in the intervention group was 8.04 ± 0.93 and decreased to 4.28 ± 0.98 in the post-test. In the control group, the average pre-test pain score was 8.00 ± 0.91 and the post-test score was 6.92 ± 0.86. Bivariate analysis showed a significant reduction in pain in the intervention group with a p-value < 0.001 (p < 0.05). Hand massage relaxation techniques and lavender aromatherapy significantly reduced pain during AV fistula insertion in patients with chronic kidney disease. This intervention is recommended for use as a supportive therapy in nursing practice in haemodialysis units.

Abstract

Type II diabetes mellitus is a chronic metabolic disease that may cause hyperglycemia and impaired peripheral tissue integrity. This case study aimed to describe nursing care for one patient with type II diabetes mellitus in the Alamanda Room of RSUD dr. M. Thomsen Nias. A descriptive case study design was applied using the nursing process framework. Data were collected through interviews, observation, physical examination, laboratory documentation, medical record review, nursing assessment format, observation sheet, and SOAP evaluation format. The case involved one 46-year-old male patient with severe hyperglycemia and signs of impaired skin integrity. Assessment showed random blood glucose of 405 mg/dL, fasting blood glucose of 404 mg/dL, HbA1c of 14.8%, leukocytosis, and erythematous peeling skin on the lower extremity. The main nursing diagnoses were unstable blood glucose level, impaired skin/tissue integrity related to circulatory and metabolic changes due to hyperglycemia, and fatigue. Interventions included hyperglycemia management, blood glucose monitoring, diabetic diet education, insulin collaboration, sterile skin care, infection monitoring, activity-rest education, discharge planning, and family involvement. Short-term clinical improvement was observed over three days, but blood glucose remained high at 280 mg/dL; therefore, continued care and self-management education are required.

Abstract

Food hygiene and sanitation compliance among food traders in traditional markets is essential to prevent food contamination and foodborne diseases. Food traders have their own important role in maintaining quality and food safety through the application of good food hygiene and sanitation principles. However, the low level of trader knowledge regarding food hygiene and sanitation remains a factor that influences compliance with the application of food hygiene in traditional market environments. The relationship between trader knowledge and compliance with the application of food hygiene and sanitation in the Baruga Market area is the focus of this study. The approach used is an analytical observational approach with a cross-sectional design. A total of 15 food traders were included using total sampling by integrating the total sampling technique. Data collection was carried out through direct interviews using questionnaires, then the collected data were analyzed using univariate and bivariate analysis, then tested for significance using the association was tested using Fisher’s Exact Test due to the small sample size. The research findings show that the knowledge category is quite dominant among respondents, namely 10 traders (66.67%), the poor category is 4 respondents (26.67%), and the good category is 1 respondent (6.67%). Meanwhile, the level of compliance of traders in the implementation of food hygiene and sanitation is mostly categorized as good with 13 respondents (86.67%) and categorized as poor with only 2 respondents (13.33%). The chi-square test indicates a significant relationship between trader knowledge and compliance with the implementation of food hygiene and sanitation in the Baruga Market Area, as shown by a p-value of 0,022 (<0,05). In this regard, it is necessary to increase education and guidance for traders so that the implementation of food hygiene and sanitation can be carried out optimally to maintain food safety and consumer health

Abstract

Appendicitis is a surgical condition caused by inflammation of the appendix. To prevent further complications, an appendectomy, commonly called a surgical removal of the inflamed appendix, is performed. One of the clinical manifestations that appears after surgery is a pain response. Pain is a complex, unique, and individual sensation. To reduce or control this pain, non-pharmacological therapies can be used, one of which is Benson relaxation. The purpose of this study was to determine the effectiveness of Benson relaxation on reducing pain intensity in post-appendectomy patients. This study used a case study method in patients with acute post-appendectomy pain in the Mawar Kuning Ward of Sidoarjo Regional Hospital for 3 days of treatment. This study was limited to patients with post-appendectomy pain who had agreed to undergo Benson relaxation therapy intervention, and Benson therapy intervention was administered before pharmacological therapy. The instrument used to assess the results of the intervention was the Numerical Rating Scale (NRS). The results of the study showed a decrease in pain intensity after administering relaxation therapy to post-appendectomy patients with NRS before the intervention being 6 and after the Benson therapy intervention being 2. Based on the results of this case study, it can be concluded that the application of Benson relaxation is effective in overcoming post-appendectomy wound pain.

Abstract

Informal riverbank settlements in urban areas generally face limited space, high density, and low environmental quality, which impacts the quality of life of their residents. In this context, communal spaces are an important element that has the potential to improve community welfare. This study aims to explore the role of communal spaces in improving the quality of life of communities in Serangan Village (Winongo River) and Code-Terban Village (Code River), and to identify the principles of their design and management. The method used is a qualitative approach with a case study strategy. Data were collected through literature studies, field observations, in-depth interviews with various community groups, and document analysis. Data were analyzed descriptively through a process of reduction, categorization, and thematic interpretation. The results show that communal spaces play an important role as social infrastructure in improving the quality of life through 5 main dimensions: (1) Social, (2) Health & Psychological, (3) Physical, (4) Economic, and (5) culture and education. Communal spaces strengthen social cohesion and social capital, provide relaxation spaces that support mental health, encourage physical activity and environmental improvement, and support community-based local economies. In addition, communal spaces also function as a forum for cultural preservation and social learning. The discussion shows that the success of communal spaces is determined by the design criteria that are Flexible, Comfortable, Adaptive, and participatory management. Integration of social and economic functions and optimization of unused spaces are important factors in increasing the sustainability of spaces. This study concludes that communal spaces are an effective strategy in community-based development to improve the quality of life of informal settlement communities, and have the potential to be replicated in similar contexts.
