Abstract

Primary health care is the foundation of the health system with a focus on promotion and prevention. To strengthen this, the Indonesian Ministry of Health launched Integration Primary Care (ILP) policy in 2023, although its initial implementation faced various obstacles. This study aims to assess the input (human resources, infrastructure, and funding) and process (planning, organizing, implementing, and evaluating policies) dimensions of ILP implementation in Banjarbaru City Health Centers. The study used a descriptive qualitative approach with 20 informants consisting of health center heads and program managers in 10 health centers through purposive sampling. Data were collected through focus group discussions. Validity was tested using source and technique triangulation. This study uses an evaluative approach based on Azwar's (1996) health systems theory, which allows for a comprehensive analysis of the input and process components in ILP implementation. The results showed that ILP was still constrained by limited human resources, especially the limited core implementing staff and ILP supporting infrastructure does not fully meet standards. However, adaptive strategies such as cluster role sharing, cross-sector collaboration, and community involvement supported the policy's sustainability. In terms of process, planning, organization, implementation, and evaluation were carried out, but were still influenced by old mechanisms and minimal budgetary and organizational support. This study provides an understanding of ILP implementation in Primary Health Centers by highlighting systemic barriers and local strategies. This study concludes that there is a need for preparedness for the procurement and distribution of strategic medical devices, particularly screening tools and basic services, as well as strengthening the guidance, monitoring, and evaluation of ILP implementation periodically through technical assistance

Abstract

Stunting remains a critical public health challenge in Indonesia. Posyandu cadres serve as frontline community health workers whose performance is vital for accelerating stunting reduction.  This study aimed to evaluate the influence of age, educational level, and length of service on the performance of Posyandu cadres in Barito Kuala Regency.  A quantitative cross-sectional study was conducted involving 377 Posyandu cadres selected through multistage random sampling. Data were collected using a structured Individual Work Performance Questionnaire (IWPQ). Data analysis included univariate, bivariate, and multivariate binary logistic regression to determine the predictors of cadre performance.  The findings revealed that age and educational level significantly impacted cadre performance. Specifically, age was inversely associated with performance (p = 0.020; OR = 0.754), indicating that younger cadres outperformed their older counterparts. Furthermore, a higher educational level was positively correlated with high performance (p = 0.011; OR = 1.394). Conversely, length of service did not significantly influence performance outcomes (p = 0.387; OR = 1.290), suggesting that longer work experience does not necessarily equate to improved field efficacy.  The performance of Posyandu cadres in community-based stunting prevention is driven by functional capacities linked to younger age and higher educational backgrounds, rather than accumulated years of service. Policymakers should prioritize educational qualifications and maintain an optimal age composition when designing capacity-building and recruitment strategies to optimize community health interventions.

Abstract

Childbirth is a physiological process involving significant physical, emotional, and hormonal changes. Severe, uncontrolled labor pain can lead to an unpleasant experience and complications, including prolonged labor fetal distress and postpartum hemorrhage. A holistic approach incorporating non-pharmacological methods, such as birth ball exercises and acupressure at LI4 (Large Intestine 4) and SP6 (Spleen 6) points, may alleviate pain by modulating endorphin and cortisol levels, thereby optimizing labor. This quasi-experimental study with a pretest-posttest control group design aimed to compare the effectiveness of acupressure and birth ball exercises on pain intensity and the duration of the first stage of labor. Conducted at the independent midwifery practice Iin Sepnita between March and August 2025, it recruited 48 participants via purposive sampling (24 per group) based on inclusion criteria. Data were collected using a Visual Analogue Scale (VAS), an active phase observation sheet, and a partograph. Analysis employed descriptive statistics and the Mann-Whitney U test. Results demonstrated that birth ball exercise was more effective than acupressure in reducing pain intensity and shortening the first stage of labor. This finding is supported by significantly lower mean ranks in the birth ball group for both posttes pain (17.96 vs. 31.04; p = 0.001) and labor duration (18.50 vs. 30.50; p = 0.003). Notably, the mean duration of labor in the birth ball group was 1.10 hours shorter than in the acupressure group.

Abstract

Breastfeeding self-efficacy plays a vital role in determining breastfeeding success, yet many mothers face challenges in accessing timely support after childbirth. Nurses hold a key position in providing breastfeeding counseling, and digital technology enables them to deliver care more effectively to a wider population. This scoping review aims to explore and describe the current evidence on nurse-led telecounseling, and digital interventions designed to improve breastfeeding self-efficacy. The review followed the Joanna Briggs Institute (JBI) methodology and PRISMA-ScR reporting guidelines. Articles were identified from PubMed, Scopus, ScienceDirect, SAGE Journals, and Google Scholar, focusing on English-language studies published between 2015 and 2025. Eligible studies included nurse-led digital counseling programs or telecounseling interventions targeting breastfeeding mothers with infants aged 0–6 months. A total of 515 articles were screened, 12 met the inclusion criteria. Most studies used quasi-experimental and randomized controlled trial designs. The results indicated that nurse-led interventions delivered through phone calls, messaging applications, and mobile health platforms significantly improved breastfeeding self-efficacy. Proactive and continuous interventions were found to be more effective than reactive approaches, particularly when the support was interactive and structured. Overall, nurse-led digital and remote counseling interventions effectively enhanced maternal confidence in breastfeeding by providing education, emotional support, and therapeutic communication. The integration of digital health technology in nursing practice can expand access to consistent, high-quality breastfeeding support services, especially for mothers living in areas with limited healthcare access.

Abstract

Maize (Zea mays L.) is one of the major sources of dietary energy and protein for humans; however, its protein quality is limited due to deficiencies in essential amino acids, particularly lysine and tryptophan. Traditional lime-based processing (nixtamalization) has long been applied in Mexico for tortilla production and shares similarities with local maize-processing practices on Buton Island, Indonesia, which also utilize lime solutions. This study applied a modified nixtamalization approach, in which maize kernels were soaked in a lime solution under non-thermal conditions, followed by water-based heating. The objective of this study was to evaluate the effects of the treatment on the amino acid profile and amino acid scores of maize protein, with water soaking used as the control. Amino acid profiles were analyzed using high-performance liquid chromatography (HPLC), and amino acid scores were calculated based on FAO/WHO reference patterns for children aged ≥3 years and adults. The results showed that the relative ratios of lysine and tryptophan were higher than those of the control (1.39 and 1.09, respectively), whereas arginine exhibited a markedly lower ratio (0.19). This non-thermal alkaline treatment reflects changes in the amino acid profile of maize protein, although it was accompanied by a reduction in total protein content. However, the implications of these changes in the amino acid composition for the biological quality of protein still require further verification through digestibility-based measurements. This lime-based approach inspired by local practices may represent an alternative processing strategy for modulating the amino acid profile of maize, with indications of alterations in essential amino acid composition and increased amino acid scores

Abstract

Psychological well-being plays a crucial role in the recovery and quality of life of women diagnosed with breast cancer. Understanding the factors influencing well-being is essential for developing effective psychosocial interventions. This study aims to evaluate the association of several factors with the psychological well-being of women with cancer. A quantitative cross-sectional design was employed at Jenderal Ahmad Yani Regional Public Hospital, Metro, in May 2024. A total of 73 respondents were selected through convenience sampling from a population of 273 patients. Data were collected using the Multidimensional Health Locus of Control Scale (Cronbach’s α = 0.791) and Ryff’s Psychological Well-Being Scale (Cronbach’s α = 0.805). Data were analyzed using Chi-square tests and logistic regression with a significance level of p < 0.05. Chi-square analysis showed that cancer stage (p = 0.003), mastectomy status (p = 0.006), and health locus of control (p < 0.001) were significantly associated with psychological well-being. Logistic regression identified mastectomy and health locus of control as significant predictors. Women who underwent mastectomy were more likely to experience lower psychological well-being (OR = 4.34; p = 0.050), while those with a high health locus of control were more likely to report better well-being (OR = 0.021; p < 0.001). Psychological well-being in women with breast cancer is strongly influenced by health locus of control and mastectomy experience. Enhancing patients’ internal control beliefs and providing targeted psychosocial support, particularly for post-mastectomy patients, may improve psychological adaptation and overall quality of life.

Abstract

Sinom is a traditional beverage composed of turmeric (Curcuma longa) and young tamarind leaves (Tamarindus indica L.), recognized for its phenolic compounds. To address the limited shelf-life of liquid, ready-to-drink Sinom mixed with lime, this study aimed to develop a more practical, dip-type (tea-bag) formulation. A D-Optimal Mixture Design was applied to optimize the ratio of three dry ingredients: young tamarind leaf powder, lime powder, and turmeric powder. The optimization determined an ideal mixture ratio of 48.393 : 18.607 : 33.000 (% w/w), respectively, achieving a desirability value of 0.772. Physicochemical evaluation of the optimized prototype demonstrated a relatively low moisture content (9.572 ± 0.33%), which theoretically supports storability, and an acidic profile (pH 3.33 ± 0.0186). Colorimetry yielded L*, a*, and b* values of 31.9 ± 0.20, 0.9 ± 0.14, and 15.3 ± 0.13. The antioxidant capacity was moderate, indicated by a DPPH IC₅₀ value of 238.067 ppm and a total polyphenol content of 14.392 ± 1.12 mg GAE/g. Furthermore, hedonic sensory evaluation revealed a moderate overall consumer acceptance (scored 3.292), with the beverage's color receiving the highest preference rating. In conclusion, the D-Optimal Mixture Design successfully produced a ready-to-brew herbal tea candidate with acceptable physicochemical parameters and moderate functional properties. While this dry formulation presents a practical alternative to liquid preparations, further comprehensive investigations—specifically regarding shelf-life profiling, microbiological safety, and cost evaluations—are essential before establishing definitive claims about its long-term stability or market readiness.

Abstract

Elementary school children are accustomed to consuming snacks available in the school environment, sold by street vendors. The snacks consumed by school children can determine the health condition of school children due to the risk of chemical and microbiological contamination contained in these snacks. This study aimed to analyse the quality of elementary school snacks based on food safety aspects in Bekasi City.  The study design was experimental study. The samples used were snacks available at four elementary schools in East Bekasi. The sample analysis included examination of Escherichia coli, Salmonella sp, borax, formalin, rhodamine B, metanil yellow, saccharin, and cyclamate with repeated examinations for each sample.  The number of samples was 33 snacks. The examination results reported E. coli content in AJ iced tea (3.1 X 102; 1.4 X102) and pudding (1.4X102). All samples were declared as safe from the substances, including borax, formalin, rhodamine B, and metanil yellow. Meanwhile, saccharin content beyond the safe limit was found in es gayung mangga (mean 157.46 mg/kg), es gayung coklat (mean 118.11 mg/kg), and es teh MH (mean 131.44 mg/kg). Cyclamate content beyond the limit was found in es doger (mean 844.01 mg/kg), es lilin (mean 485.55mg/kg),  es gayung coklat (mean 942.28 mg/kg), es teh MH (mean 498.05 mg/kg). From this study, it was concluded that not all elementary school snacks have been declared to meet the safe criteria according to SNI and BPOM Regulations. It is suggested to have a routine evaluation and education by the school or health officers regarding snacks available sold by street vendors in the school environment.

Abstract

Kaempferol, a widely distributed natural flavonoid, is increasingly studied for its antidiabetic properties. However, a comprehensive bibliometric analysis mapping the global research landscape of this field is currently lacking. This study aims to map research trends, collaborations, and the thematic evolution of kaempferol in diabetes-related studies from 2000 to 2025, while identifying emerging hotspots such as nutrigenomics and metabolomics. A total of 700 scientific documents were retrieved from the Scopus database. Bibliometric indicators and science mapping techniques—utilizing VOSviewer and Biblioshiny (RStudio)—were employed to analyze keyword co-occurrence networks, thematic evolution, and research dynamics. The field demonstrated a 19.5% average annual growth rate, encompassing 265 journals and 4,089 authors, reflecting a highly collaborative global environment. Thematic evolution revealed a distinct shift from early foundational studies on antioxidant activity and enzyme inhibition toward current research emphasizing molecular docking, gene modulation, and metabolic profiling. Recently, nutrigenomics and metabolomics have emerged as prominent focus areas. This bibliometric analysis provides a quantitative overview of the research landscape surrounding kaempferol and diabetes. The findings highlight an ongoing transition toward integrative, omics-based strategies, offering a structured foundation to guide future mechanistic and translational investigations.

Abstract

Accurate detection of Helicobacter pylori is essential for the effective management of chronic gastritis. While Modified Giemsa (MG) staining is a common diagnostic tool, its sensitivity can be limited in cases with low bacterial density or atypical morphology, often leading to under-detection. This study aimed to evaluate the comparative diagnostic performance and bacterial density grading of monoclonal antibody immunohistochemistry (IHC) versus MG staining to establish a more precise diagnostic algorithm. This analytical observational study utilized a retrospective cross-sectional design, analyzing 100 gastric biopsy specimens from patients with chronic gastritis. Detection rates and bacterial density, graded according to the Updated Sydney System, were compared between MG and IHC using monoclonal anti-H. pylori antibodies. Monoclonal antibody IHC demonstrated a significantly higher detection rate (72%) compared to MG (65%; p = 0.016). Discordance analysis identified seven cases that were positive by IHC but missed by MG, typically representing low-density infections. Although overall inter-method agreement was "almost perfect" (Cohen’s kappa = 0.839), IHC staining resulted in a significant ordinal shift toward higher bacterial density grades compared to MG (p = 0.016), providing more accurate staging of the infection. Monoclonal antibody IHC is superior to Modified Giemsa for the detection of H. pylori, particularly in identifying cases with low bacterial density or compromised morphology. While MG remains a cost-effective primary screening tool, a stepwise approach is recommended: IHC should be prioritized as a mandatory confirmatory test when histological or clinical suspicion remains high despite a negative MG result.
