Abstract

Phlebitis is a common complication of peripheral intravenous catheter (PIVC) use that often leads to venous inflammation, pain, and significant disruption of therapy among hospitalized patients. The incidence of phlebitis in patients with PIVCs remains high, frequently exceeding the recommended threshold of ≤5%, and is associated with adverse clinical outcomes. This study aimed to analyze predictors of peripheral intravenous phlebitis among patients with intravenous fluid devices (IVFDs) at Kotamobagu Regional General Hospital. A quantitative study with a prospective cohort design was conducted in the inpatient wards of Kotamobagu Regional General Hospital from October to December 2025, involving 328 adult patients (≥18 years) with peripheral IVFDs. Pediatric patients, users of central venous catheters or PICC lines, and uncooperative patients were excluded. The results showed that 29.9% (98 out of 328 patients) had phlebitis Infusion insertion site (OR=4.32; p<0.001) was the most significant factor associated with peripheral intravenous phlebitis, followed by aseptic technique (OR=3.08; p<0.001), type of intravenous fluid (OR=2.14; p=0.003), age (OR=1.21; p=0.041), and sex (OR=1.67; p=0.018). In conclusion, prevention of phlebitis should prioritize appropriate selection of IV insertion sites, strict adherence to aseptic techniques, and careful consideration of the type of intravenous fluids, particularly in patients at higher risk based on age and sex. Non-recommended (distal) insertion sites increase the risk of phlebitis by 4.32 times compared to proximal sites

Abstract

The rising prevalence of diabetes mellitus highlights the need for food-based strategies that reduce postprandial glycaemic excursions. Snacks are a critical dietary target due to their frequent consumption, yet most commercial options are rich in rapidly digestible carbohydrates. Fish protein hydrolysates (FPHs) offer peptide-mediated bioactivity beyond simple macronutrient substitution, but their application in antidiabetic snack development remains limited. This review evaluates the potential of FPHs as functional ingredients in antidiabetic snack formulations. A narrative synthesis of literature from major scientific databases was performed, focusing on antidiabetic mechanisms of FPH-derived peptides and their incorporation into finished snack products. Evidence suggests that FPHs may support glycaemic regulation through inhibition of α-amylase, α-glucosidase, and DPP-IV, modulation of incretin pathways including GLP-1, and antioxidant activity, mainly demonstrated in in vitro and in vivo models. Product studies indicate that FPH incorporation at optimal levels of approximately 2–15% improves protein density and digestibility in snack matrices such as cookies, biscuits, bars, noodles, and bread. However, sensory acceptance is formulation-dependent, and bitterness or fishy off-flavours may require mitigation strategies such as enzymatic debittering or fermentation. Despite promising functional properties, human evidence confirming glycaemic benefits of FPH-enriched snacks remains limited. Standardized characterization and well-controlled clinical trials using finished products are essential to translate mechanistic potential into validated dietary applications.

Abstract

Yogurt is a fermented dairy product whose quality is largely governed by the metabolic activity of lactic acid bacteria (LAB). In recent years, the development of plant-based yogurt has gained attention as a strategy to diversify fermented products, particularly legumes. Jack bean and kidney bean are legumes rich in complex carbohydrates and proteins that may support LAB growth during fermentation. This study aimed to evaluate the microbiological and physicochemical properties of legume-based yogurt drinks from jack bean and kidney bean extracts. Yogurt drinks were fermented by Streptococcus thermophilus and Lactobacillus bulgaricus as starter cultures. The study employed a Completely Randomized Design with formulas which was divided of different ratio of jack bean extract and kidney bean extract (P1: 0%; P2: 80%:0%; P3: 0:80%; F1: 40%:40%; F2: 60%:20%; F3: 20%:60%). All formulations were evaluated for LAB viability, and the highest LAB formula count was analyzed for pH, lactic acid content, viscosity, solids-non-fat (SNF), and reducing sugar levels. The results showed that Formula 2 containing 60% jack bean extract and 20% kidney bean extract met all evaluated parameters of SNI 2981:2009 which exhibited the highest LAB viability with 10.70±0.09 log CFU/mL (p<0.05), exceeding the minimum requirement (≥7 log CFU/mL). This formulation showed a pH of 4.23±0.00, lactic acid content of 1.954±0.03%, viscosity of 5.139±0.05 cP, SNF content of 9.410±0.09%, and relatively low reducing sugar content (2.017±0.06%). These results suggest the combination of jack bean-kidney bean shows strong potential as raw materials for legume-based yogurt drinks with favorable microbiological viability and physicochemical properties.

Abstract

The nutritional status of children aged 0 – 59 months remains a major public health concern in Indonesia, particularly in rural areas. Although local foods are often rich in essential nutrients, their utilization is frequently constrained by limited maternal food literacy. This study aimed to examine the association of maternal local food literacy and food regulation on the nutritional status of children under five in Keude Amplah, Nisam District. This study employed a quantitative analytical design with a cross-sectional approach. A total of 87 mothers with children aged 0–59 months were selected using proportional random sampling across 12 villages to ensure geographical representation. Data were collected using structured questionnaires assessing maternal local food literacy and food regulation, along with anthropometric measurements of children’s nutritional status based on weight-for-age and height-for-age indicators. Data analysis was conducted using the Chi-square test and multiple linear regression. The Chi-square analysis revealed a significant association between maternal local food literacy and the nutritional status of children under five (p < 0.001), as well as between food regulation and nutritional status (p < 0.001). Descriptive results showed that 12.6% of the children were stunted and 17.2% were undernourished. Multiple linear regression analysis indicated that maternal local food literacy (B = 0.482; p < 0.001) and food regulation (B = 0.635; p < 0.001) were significant predictors of child nutritional status, explaining 72.2% of the variance (R² = 0.722). The study concludes that maternal food literacy and regulatory support are critical determinants of child health; therefore, local food literacy should be integrated into community health programs, such as posyandu (a community-based health posts) to optimize child nutritional outcomes

Abstract

Nutritional problems among school-age children in Indonesia remain a concern due to low consumption of balanced diets. Basic Health Research 2018 reported that only 5.3% of children meet the recommended fruit and vegetable intake. This highlights the need for more effective nutrition education strategies, with multimedia-based approaches offering a more engaging and understandable way to promote healthy eating behaviors. This study aimed to assess the effectiveness of multimedia-based balanced nutrition education in improving the eating behavior of school-age children. A quasi-experimental study with a one-group pre-test–post-test design was conducted among 75 students in grades IV and V at SDN Seuriget, Langsa City, selected using total sampling. The intervention was implemented over five weeks through a gradual educational approach combining leaflets and animated videos on balanced nutrition. Data were collected at four time points: before the intervention (pre-test), after leaflet education (post-test 1), after animated video education (post-test 2), and one week after the intervention (follow-up). Eating behavior was assessed using the IDDS and dietary intake measurements. The results showed significant improvements in several variables. The median IDDS increased from 6 to 11 (p < 0.001). Median protein intake increased from 49.9 g to 57.4 g (p < 0.001). Median fat intake decreased significantly from 72.7 g to 54.8 g (p < 0.001), while carbohydrate intake showed significant changes (p < 0.001). However, energy intake did not show a statistically significant change (p = 0.667). In conclusion, multimedia based nutrition education improved dietary diversity and protein intake. These result suggest that external factors such as family dietary habits and household food availability may affect children’s overall nutrient intake.

Abstract

The preparedness of healthcare workers to respond to disasters in coastal areas is a crucial aspect that must be optimally developed. This preparedness is influenced by various factors, such as government policy, perceptions and resilience. This study aims to analyse the direct and indirect effects of government policy, perceptions and resilience on the preparedness of healthcare workers to respond to earthquakes and tsunamis in the coastal areas of Sukabumi Regency. This study employs a quantitative design with a cross-sectional approach involving 512 respondents, selected using total sampling. Data were collected via a structured questionnaire measuring government policies, perceptions, resilience, and disaster preparedness. All instruments underwent validity and reliability testing and were found to be valid and reliable. Data analysis was conducted using the SEM-PLS method to test the direct and indirect relationships between variables. The results of the study indicate that there is a significant direct effect of government policy on disaster preparedness (O=0.348, t=7.265, p=0.000), perceptions of disaster preparedness (O=0.296, t=7.785, p=0.000), and resilience to disaster preparedness (O=0.227, t=5.870, p=0.000). There is an indirect effect of government policy on disaster preparedness via perception (O=0.176, t=7.206, p=0.000), government policy on disaster preparedness via perception and resilience (O=0.116, t=2.120, p=0.036), and government policy on disaster preparedness through resilience (O=0.118, t=2.113, p=0.040). Based on the modelling results, government policy, perception, and resilience were identified as important determinants of healthcare workers’ disaster preparedness in the coastal areas of Sukabumi Regency. These findings highlight the need to strengthen disaster training, improve understanding of disaster policies, and enhance the psychological resilience of healthcare workers to support preparedness.

Abstract

Coastal communities living in mangrove ecosystems are exposed to various environmental and health risks that may influence their perceived vulnerability to illness. Understanding the determinants of perceived vulnerability is important for improving community health resilience. Objective: This study aimed to identify the determinants of perceived vulnerability to illness among communities living in mangrove ecosystems. An observational cross-sectional study was conducted involving 67 residents of the Baros Mangrove Ecosystem, Bantul, Yogyakarta, who were recruited consecutively. Data were collected using a structured questionnaire measuring perceived environmental health, mangrove knowledge, community participation in mangrove management, and formal education, with gender treated as a covariate. Descriptive statistics, bivariate analysis, and multiple linear regression were used to analyze the data. Perceived vulnerability to illness was significantly and negatively associated with perceived environmental health (β = −1.112, p = 0.001), mangrove knowledge (β = −0.106, p = 0.032), and formal education (β = −0.106, p = 0.004). Community participation showed a positive but non-significant association (β = 0.181, p = 0.085), while gender was not significantly associated with perceived vulnerability. The model explained 92.8% of the variance in perceived vulnerability to illness (adjusted R² = 0.928). Perceived environmental health, mangrove-related knowledge, and formal education were significantly associated with perceived vulnerability to illness among coastal communities in mangrove ecosystems. These findings highlight the potential importance of health and environmental literacy in shaping risk perceptions within coastal populations. Further studies with longitudinal designs are needed to clarify the causal relationships between these factors.

Abstract

Spirulina (Arthrospira sp.) is widely recognized as a nutrient-dense microalgal biomass with potential functional properties. This study aimed to characterize the nutritional composition, bioactive pigment profile, and antioxidant activity of locally produced spirulina powder from Central Java (Indonesia), with particular emphasis on its radical scavenging capacity evaluated using the DPPH assay. Spirulina powder was analyzed for proximate composition (moisture, crude protein, crude fat, carbohydrate, and ash), as well as major bioactive pigments including phycobiliproteins, chlorophyll, and β-carotene. Antioxidant activity was determined using the DPPH radical scavenging assay. The spirulina powder contained 56% protein. The bioactive pigment profile was dominated by phycobiliproteins, particularly C-phycocyanin, alongside high levels of chlorophyll and β-carotene. Antioxidant evaluation showed a concentration-dependent DPPH radical scavenging activity, with an IC₅₀ value of approximately 396 ppm, indicating moderate direct radical scavenging capacity within the DPPH system. These findings suggest that local spirulina from Central Java possesses limited instantaneous free radical scavenging activity when assessed using a single-mechanism chemical assay. However, its complex nutritional matrix may contribute to antioxidant mechanisms beyond direct radical neutralization, although this was not specifically evaluated in the present study.

Abstract

Patients with chronic kidney disease undergoing hemodialysis experience significant changes in quality of life, including physical, psychological, spiritual, and social aspects. The therapy leads to physical limitations, loss of autonomy, and reduced self-efficacy, which may impair patients’ ability to manage their condition and increase the risk of stress, depression, and complications. This study aimed to explore patients’ experiences and identify self-efficacy-related factors affecting quality of life among hemodialysis patients at Prof. Dr. W. Z. Johanes Kupang Hospital. A qualitative phenomenological approach was employed using in-depth interviews with 10 informants. Data were collected using unstructured interview guides, supported by audio recordings and field notes, and analyzed using the Stevick-Colaizzi-Keen method. Findings indicated that, physically, most patients reported bodily lightness, improved sleep quality, and increased ease in daily activities following routine hemodialysis; however, some continued to experience dizziness, pain, and unstable physical conditions. These physical changes were associated with patients’ self-efficacy, where improved conditions increased confidence in managing daily activities and treatment, while persistent symptoms reduced confidence in controlling their health condition. Psychologically, patients initially experienced anxiety, which decreased with adaptation and acceptance. Spiritually, patients showed increased religious devotion as a coping mechanism, while socially, patients maintained interactions with family and their environment, contributing to sustained support. These findings indicate that physical condition, psychological adaptation, spiritual coping, and social support are key factors influencing patients’ self-efficacy. Although hemodialysis necessitates complex changes in quality of life, the integration of emotional, spiritual, and social support helps patients in the process of acceptance and adaptation.

Abstract

Implementing electronic prescribing systems (EPS) in hospitals is a complex process that extends beyond technological integration. It necessitates a comprehensive approach that considers the human factors involved, particularly the acceptance and willingness of healthcare professionals to adopt and utilize these systems effectively. This study aimed to evaluate the perceptions of doctors and pharmacists regarding the implementation of EPS at UNS Hospital using the Technology Acceptance Model (TAM). A validated questionnaire was administered to 50 respondents in UNS Hospital, assessing perceived ease of use, benefits, attitude towards usage, intention to use, and external factors. Most respondents (80%) agreed or strongly agreed that the system was easy to use. Furthermore, 90% acknowledged the benefits of EPS, including time savings, increased efficiency, accuracy, safety, and improved interprofessional communication. Attitudes towards usage were highly positive, with 92% of respondents expressing enthusiasm and comfort in transitioning to the electronic system. Intention to use was also high, with 86% committed to long-term adoption. However, external factors showed more varied results, with 66% agreeing or strongly agreeing, while 26% were doubtful and 8% disagreed, suggesting potential shortcomings in training, infrastructure, or technical assistance. These findings highlight the importance of addressing external factors to ensure the sustainability of EPS implementation Overall, the study indicates that the UNS Hospital EPS has the potential for widespread adoption and integration into healthcare services, given the positive perceptions of its ease of use, benefits, and user acceptance
