Abstract

Globally, the prevalence of iron deficiency remains a significant concern among pregnant women, with an estimated 43.9% affected. The estimated prevalence of iron deficiency in pregnant women is 49.4% in Asia, 59.1% in Africa, 28.1% in the Americas, and 26.1% in Europe. The prevalence of substance deficiency in pregnant women in Indonesia remains relatively high, and deficiency plays a role in contributing to the high maternal and infant mortality rates observed in the country (Paridah, 2022). According to the Health Office profile (2020), the TTD coverage in pregnant women in North Sumatra Province in 2019 was 92.49%, representing a decline from the 2018 figure of 94.30%. The provincial coverage remains below the performance indicator and target for nutrition improvement activities in 2019, which is 95%. Furthermore, there was a reduction in the provision of TTD in four districts/cities. In Kisaran City, for instance, the coverage rate decreased from 93.30% in 2018 to 92.93% in 2019. This study employs a qualitative approach with a phenomenological design, with the aim of elucidating the phenomenon of implementing the Fe tablet program at the Nasywa Clinic in Kisaran in 2022. The study at Nasywa Clinic in Kisaran involved 50 pregnant women. It evaluated the Fe tablet supplementation program in 2022, revealing that health workers' collaboration in procuring and planning Fe tablets, along with midwives' distribution efforts, were crucial. However, the program's coverage did not meet targets, and pregnant women's compliance in taking Fe tablets was less than optimal.

Abstract

The early initiation of breastfeeding is one of the factors affecting uterine involution. This is due to the stimulation and release of hormones, including oxytocin, which functions to stimulate the contraction of breast smooth muscles and also causes the contraction and retraction of uterine muscles. This will result in the compression of the blood vessels, thereby reducing the blood supply to the uterus. The objective of this study is to ascertain the relationship between early breastfeeding initiation and uterine involution in postpartum mothers. The study was conducted at Ninda Clinic on 26 postpartum mothers, of whom 13 had undergone IMD and 13 had not undergone IMD. The data were analyzed using the chi-square test. The results of this study demonstrated that 12 respondents (46.2%) exhibited normal TFU, while 14 respondents (53.8%) demonstrated abnormal TFU. It can be concluded that there is a relationship between early breastfeeding initiation (IMD) and involution in postpartum mothers, with a value of p=0.018 (0.018 <0.05). It is recommended that health workers, particularly midwives, provide health education about IMD in the postpartum period.

Abstract

Neuropathic pain in DM sufferers can be reduced by providing non-pharmacological therapy by providing relaxation or by providing education such as providing music therapy, relaxation and guided imagery. Physical exercise with relaxation that can be applied to DM patients includes Progressive Muscle Relaxation. The movements in PMR exercises aim to tighten and relax the muscles in one part of the body at a time to provide a feeling of physical relaxation. Relaxation can calm the nervous system so that the sufferer's body relaxes. This study aims to determine the effect of PMR in reducing neuropathic pain in patients with type 2 diabetes mellitus. The research design used the Quasy Experiment One Group Pretest – Posttest Control Group Design. The population in this study were all type 2 DM patients who experienced neuropathic pain. The sampling technique used was purposive sampling. Progressive Muscle Relaxation was given once a day for 7 days, and the pain scale was measured using the Visual Analog Scale (VAS) before and after administering PMR. The results of the study showed that the average pain level in the pre-test control group was 6 and post-test 5, while in the intervention group the average pre-test was 6 and post-test 4.81. The results of the Wilcoxon test in the control group showed a p value of 0.187 so there was no change in pain levels in the control group, while the intervention group showed a p value of 0.0001. The conclusion of this study is that there is a change in pain levels before and after PMR, so PMR can reduce neuropathic pain in patients with type 2 diabetes mellitus.

Abstract

Hypospadias is a congenital disorder in boys due to low fetal androgen hormones. Hypospadias is one of the congenital disorders with the highest prevalence. Several studies have revealed the association of hypospadias with cardiovascular disease. There are still very few studies that summarize cardiovascular manifestations in pediatric hypospadias. Therefore, this study was designed to describe the cardiovascular risk in pediatric patients with hypospadias. Method : Journal searches were carried out through several research databases including Google Scholar, PubMed, and Sciencedirect . The keywords used were "hypospadias", "cardiovascular", and "pediatric". The results of the literature search were 1,543 journals with 20 journals meeting the criteria. Results and Discussion : Hypospadias is one of the second most common congenital abnormalities in men after testicular undescensus . Children born with hypospadias are proven to experience changes in vascular structure, namely increased carotid intima media thickness (CMIT) and are associated with hypertension. Hypospadias patients have impaired endothelial function, decreased vasorelaxation, and increased vasoreactivity. The underlying mechanism is a Rho kinase and ROS dependent process. Conclusion: Children with hypospadias have a higher risk of cardiovascular disease.
