Abstract

According to the UNICEF report, there were 54 neonatal infant deaths per 1,000 live births worldwide in 2020. The highest neonatal mortality rate in 2020 was found in the Sub-Saharan Africa region, namely 27 deaths per 1,000 live births. In Indonesia, only 1 out of 2 babies under 6 months is exclusively breastfed, and only a little more than 5% of children are still getting breast milk at 23 months of age. Nationally, the coverage of babies receiving exclusive breastfeeding in 2019 is 67.74%. The inhibiting factor in breastfeeding is the production of breast milk which is less and slow to come out which can cause the mother not to give her baby enough. A method that is rarely used to speed up milk release and stimulate milk productivity is a combination of acupressure therapy and oxytocin massage. Knowing the effectiveness of the combination of acupressure therapy with oxytocin massage on the length of time to express breast milk in postpartum mothers. Quasi experimental with posttest only control group design. The sample in this study were all postpartum mothers at BPM. Indah Susilowati in August-October 2022 as many as 30 people, the sampling technique is total sampling. The average time for breastfeeding in the intervention group was 2.16 hours and in the control group 2.79 hours. Abstract

Difficulty eating in children is still a complaint from parents. Toddlers often experience a decrease in appetite which results in reduced nutritional intake resulting in decreased body weight. This can result in decreased immune system, impaired brain function which can cause disability and death. According to the World Health Organization, in 2013 there were 99 million children under five in the world suffering from malnutrition and 67% were in Asia. There are 17 thousand deaths every day due to nutritional problems and infectious diseases. In Indonesia, especially the Javanese, believe that herbal medicine has the property of increasing appetite in children. Research Objectives To determine the effect of herbal medicine in increasing appetite in toddlers 12-36 months. Pre-experimental designs with one group pretest-posttest. The sampling technique was total sampling, namely 10 toddlers or all toddlers aged 12-36 months in the Panggarangan Health Center area with decreased appetite problems in November 2022. Appetite assessment results before being given herbal medicine to 10 respondents with a minimum score of 2 and a maximum of 5, and an average of 3.70 (SD 0.949). While the appetite after being given herbal medicine with a minimum value of 7 and a maximum of 10, and an average of 8.70 (SD 1.059). So, herbal medicine has the effect of increasing appetite in toddlers aged 12-36 months in the Penggarangan Health Center (p-value <0.000). Herbal medicine has an effect on increasing appetite in toddlers 12-36 months. Abstract

Childbirth often results in birth canal injuries. Perineal injuries are found in about 70% of mothers who give birth due to inability of the muscles and soft tissue of the pelvis during the baby's birth. The process of healing perineal wounds has a healing time that varies, namely 6-7 days, and is influenced by several things such as the characteristics of the mother giving birth, nutritional status, treatment and condition of the wound. One of the effects of delayed perineal wound healing is the occurrence of infection, prevention of infection can be done with pharmacological and non-pharmacological treatment. To find out how the effectiveness of betel leaf decoction and consumption of snakehead fish on the healing of perineal wounds in postpartum mothers. The research design used was a literature review (journal review). Journals used as data include systematic search studies of databases and interventions for respondents. The results of a journal review of 10 data used as references state that consumption of snakehead fish is proven to be used as a non-pharmacological treatment for postpartum wound healing. Similarly, boiled betel leaf water which is used as a treatment for birth canal injuries has been shown to accelerate the healing of perineal wounds.

Abstract

Background: Medical students are exposed to high academic demands, which often lead to poor sleep quality and may impact cognitive function and academic achievement. This study aimed to analyze the relationship between sleep quality and grade point average (GPA) among medical students at Prima Indonesia University, class of 2023. Methods: A quantitative analytic approach with a cross-sectional design was used. The population included all active students from the 2023 cohort, with 162 respondents selected by simple random sampling. Sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI), and GPA data were obtained from official academic records. Data analysis involved univariate and bivariate statistics, with normality tested using Kolmogorov–Smirnov and correlation assessed by Spearman's rank test. Results: The majority of students (63.6%) had poor sleep quality (PSQI > 5), and the average GPA was 3.29. Spearman analysis showed a moderate negative correlation (rₛ = –0.505, p < 0.05) between sleep quality and GPA, indicating that poorer sleep quality was associated with lower academic achievement. Conclusion: Poor sleep quality significantly affects academic performance in medical students, highlighting the need for sleep hygiene education and time management interventions.

Abstract

The present case report describes the clinical features, diagnostic findings, and management of a 61-year-old female patient diagnosed with Chronic Suppurative Otitis Media (CSOM) of the right ear, benign type. CSOM is a persistent inflammation of the middle ear characterized by continuous or recurrent ear discharge through a tympanic membrane perforation lasting more than two weeks. The patient presented with right ear discharge for approximately four months, initially serous and later purulent with occasional blood, accompanied by ear pain, fullness, tinnitus, and mild dizziness. Physical examination revealed a central perforation of the tympanic membrane with mucopurulent discharge, supporting the diagnosis of benign-type CSOM. Treatment consisted of aural toilet and systemic antibiotic therapy using levofloxacin 500 mg twice daily combined with cetirizine 10 mg once daily for one week, alongside education to maintain ear hygiene and avoid water exposure. Clinical improvement was observed with resolution of otorrhea and no further complications. Prognosis was good (ad vitam bonam), while functional and curative outcomes were moderately favorable (dubia ad bonam). This report emphasizes the importance of early diagnosis, adequate antimicrobial therapy, and patient compliance in achieving a dry and safe ear condition while preventing recurrence or long-term hearing impairment in CSOM.

Abstract

Ossification of the ligamentum flavum (OLF) is characterized by the replacement of ligamentum flavum tissue with mature laminar bone accompanied by hypertrophy. Although rare, its highest prevalence is reported among East Asian populations, predominantly affecting males under 50 years old, and most commonly found in the lower thoracic region. Clinically, OLF presents as a slowly progressive myelopathy, yet its precise pathogenesis remains unclear. Diagnostic imaging modalities include radiography, computed tomography (CT), and magnetic resonance imaging (MRI). This case report describes a 42-year-old female diagnosed with thoracolumbar OLF based on MRI examination. The patient presented with spastic paraparesis and hypoesthesia from both feet to dermatomes Th10–11 following a history of trauma. Lumbar radiography showed only lumbal spondylosis, while thoracolumbar MRI revealed nodular thickening of the ligamentum flavum from Th6–7 to Th12–L1, resulting in spinal canal stenosis and compression of the spinal cord. Additional degenerative findings included superior endplate degeneration of L4 and multiple intervertebral disc bulging. Neurological deficits indicated myelopathy and radiculopathy due to compression of the lateral corticospinal and spinothalamic tracts. The absence of spinal cord edema on MRI suggested a chronic onset. OLF frequently coexists with vertebral degenerative abnormalities, as observed in this patient. Based on the radiologic characteristics, the lesion represented several morphological types including extended, enlarged, fused, and tuberous forms. Laminectomy performed in this case resulted in gradual neurological recovery. In conclusion, OLF is a rare but important cause of progressive thoracic myelopathy that may lead to irreversible neurological impairment. MRI evaluation plays a crucial role in preoperative assessment of spinal canal stenosis and spinal cord compression.

Abstract

The present study reports a comparative evaluation of the antidiabetic potential of ethanol extracts from Syzygium polyanthum (bay leaves) and Moringa oleifera L. (moringa seeds) based on their inhibitory activity against the α-glucosidase enzyme. Diabetes mellitus, a chronic metabolic disorder characterized by hyperglycemia due to impaired insulin secretion or action, can be managed through enzyme inhibition that delays carbohydrate hydrolysis. The investigation was conducted for five weeks using an in vitro experimental method, in which both plant materials were extracted via maceration with 70% ethanol. The α-glucosidase inhibition assay was performed spectrophotometrically at 410 nm, and the half-maximal inhibitory concentration (IC₅₀) was determined to compare the inhibitory capacities of both extracts. Phytochemical profiling revealed that bay leaf extract contains flavonoids, tannins, saponins, and steroids, while moringa seed extract contains flavonoids, alkaloids, and steroids. Results showed that bay leaf extract exhibited strong α-glucosidase inhibition with an IC₅₀ value below 100 ppm, indicating a significant potential as a natural antidiabetic agent. In contrast, moringa seed extract demonstrated weaker activity with no substantial inhibition observed at the tested concentrations. Based on comparative analysis, bay leaf extract shows a higher inhibitory efficiency, suggesting its potential application in herbal antidiabetic formulations. Further studies are recommended to explore the optimal concentration range and bioactive constituents of moringa seeds.

Abstract

Breast cancer is a malignancy arising from the mammary glands and is the most common cancer found in women. The diagnosis of breast cancer is established using the triple assessment method, which involves physical examination, radiological assessment, and histopathological examination of the breast. This study aims to determine the concordance between physical and radiological examinations with histopathology, which serves as the gold standard. The study utilized an observational analytic cross-sectional design, gathering data from the medical records of breast cancer patients at Royal Prima General Hospital and Dr. Pirngadi General Hospital in Medan. Statistical testing using Chi-Square showed no significant concordance between physical examination and histopathological examination (p>0.05). The radiological examination, however, showed significant concordance but with a negative correlation, suggesting that a higher histopathological grade is associated with a lower BI-RADS level (r=-0.189; p=0.039). Radiological examination demonstrated better concordance with histopathology compared to the physical examination, but it still cannot replace the role of histopathology as the gold standard for breast cancer diagnosis.
