Abstract

Hematological abnormalities are commonly observed in patients with type 2 diabetes mellitus (T2DM) and may serve as markers of complication risk, thereby playing an important role in T2DM management; however, the determinants and regional variations remain incompletely understood, particularly in Lampung. This cross-sectional observational study was conducted among patients with T2DM attending the Internal Medicine Outpatient Clinic of RSUD Dadi Tjokrodipo, Bandar Lampung, from April to September 2024, enrolling 80 participants who met the inclusion and exclusion criteria. Data were obtained from complete blood count testing, medical records, and structured interviews with direct observation, and were analyzed using bivariable and multivariable logistic regression. The most frequently identified hematological abnormalities were anemia (45%), leukocytosis (32%), and an elevated neutrophil-to-lymphocyte ratio (NLR) (46%). In multivariable analysis, anemia was independently associated with uncontrolled glycemic status (adjusted OR = 15.914; p = 0.001), not following a recommended dietary pattern (adjusted OR = 7.402; p = 0.003), smoking (adjusted OR = 7.920; p = 0.015), and age ≥61 years (adjusted OR = 29.643; p = 0.004), whereas leukocytosis was independently associated only with uncontrolled glycemic status (adjusted OR = 4.310; p = 0.020). High NLR was independently associated with uncontrolled glycemic status (adjusted OR = 18.811; p = 0.001), employment status (adjusted OR = 8.630; p = 0.003), and not following a recommended dietary pattern (adjusted OR = 5.287; p = 0.015). Overall, uncontrolled glycemic status was the most dominant and consistent factor associated with hematological abnormalities among patients with T2DM.

Abstract

Hypertension is a prevalent chronic disease globally and has a substantial impact on patients’ quality of life. Both medication and dietary adherence are critical for effective hypertension management; however, the extent of their respective contributions to quality of life is not well established, especially in community-based settings in Indonesia. This study investigated the associations between medication adherence, dietary adherence, and quality of life among individuals with hypertension. An analytical cross-sectional study was conducted with 172 patients registered at Ngasem Primary Health Center in Kediri, Indonesia. Participants were selected using simple random sampling. Medication adherence was measured with the eight-item Morisky Medication Adherence Scale, dietary adherence with a DASH-based questionnaire, and quality of life with the WHOQOL-BREF. Descriptive statistics summarized participant characteristics. Associations between categorical variables were analyzed using the chi-square test, and ordinal logistic regression was performed to identify the strongest predictor of quality of life. Medication and dietary adherence were both significantly associated with quality of life (p < 0.001). After adjustment for medication adherence, dietary adherence remained the strongest predictor. Participants with low dietary adherence were substantially less likely to report a higher quality of life than those with high adherence (OR = 0.005; 95% CI: 0.002–0.014). These findings highlight the importance of integrating dietary interventions with pharmacological treatment to improve the well-being of patients with hypertension

Abstract

The long duration of pulmonary tuberculosis treatment and the need to take medication regularly can cause treatment fatigue and emotional distress. Limited access to information and emotional support may further affect patients’ quality of life. This study examined whether telenursing-based health education could improve the quality of life of patients with tuberculosis at Jemursari Islamic Hospital, Surabaya. A quasi-experimental pretest–posttest study with a control group was conducted among 38 patients with pulmonary tuberculosis. Participants were randomly divided into an intervention group and a control group. The intervention group received health education through telenursing for four weeks, while the control group received routine care according to hospital standards. Quality of life was measured using a questionnaire. The data were analyzed using paired and independent t-tests, with a significance level of 0.05. Quality of life improved significantly in the intervention group after four weeks of telenursing-based health education (p < 0.001). No significant improvement was observed in the control group. A significant difference was also found between the intervention and control groups after the intervention (p < 0.001).  These findings suggest that telenursing-based health education can improve the quality of life of patients with pulmonary tuberculosis. Telenursing may therefore be a useful approach for providing continuous education and support to patients undergoing long-term treatment. This study used a quasi-experimental pre-post-test control group design. The population consisted of 38 tuberculosis patients at Jemursari Islamic Hospital, Surabaya. A sample size of 38 patients, divided into two groups, was selected using random sampling. Data were collected using a questionnaire. Data were analyzed using a t-test with α = 0.05. The paired t-test results showed a P-value of 0.000, indicating a difference in quality of life before and after the telenursing-based health education intervention. In the control group, there was no difference in quality of life before and after the intervention, according to hospital standards. The results of the independent t-test for the intervention and control groups showed a P=0.000, indicating that telenursing-based health education significantly improved quality of life. Telenursing-based health education can improve quality of life. Hospitals and nurses can utilize telenursing to provide health education to patients with chronic diseases.

Abstract

Malnutrition in children under two years of age remains a public health problem that affects child growth and development. One of the interventions implemented to address this issue is the Growth Monitoring and Promotion (GMP) program accompanied by nutrition counseling for mothers. This study aimed to analyze the effect of GMP program counseling on maternal behavior in monitoring the growth of malnourished under-two children in the working area of the Margototo Public Health Center, East Lampung Regency. This study used a quantitative approach with a one-group pretest-posttest design. The sample consisted of 30 mothers who had malnourished under-two children, selected using a total sampling technique. Quantitative data were analyzed using the paired t-test. The results showed a significant improvement in mothers’ knowledge, attitudes, practices, and behaviors after GMP counseling (p < 0.001). In addition, there was an increase in children’s body weight after the intervention. In conclusion, GMP counseling was effective in improving maternal behavior in monitoring the growth of malnourished under-two children and supporting weight gain in children.

Abstract

Routine child weighing at Posyandu is essential for monitoring growth and early detection of nutritional problems; however, maternal participation remains suboptimal due to inadequate knowledge, negative attitudes, and poor compliance. Although Posyandu cadres play a central role in community-based child health services, evidence regarding the effectiveness of cadre empowerment in improving maternal compliance remains limited. This study aimed to analyze the effectiveness of Posyandu cadre empowerment in improving mothers' knowledge, attitudes, and compliance with routine child weighing in the Pringsewu Community Health Center working area, Indonesia, in 2026. A quasi-experimental pretest–posttest control group design was conducted among 82 mothers of children under five selected using proportional stratified random sampling. Data were collected using a validated and reliable questionnaire and analyzed using paired t-tests and the McNemar test. The intervention significantly increased knowledge (mean difference = 5.67; 95% CI: 5.14–6.21; Cohen's d = 2.33; p < 0.001), attitudes (mean difference = 11.33; 95% CI: 10.93–11.73; Cohen's d = 6.16; p < 0.001), and compliance (p < 0.001; matched OR = 34.00; 95% CI: 4.65–248.38), whereas no significant changes were observed in the control group. These findings indicate that Posyandu cadre empowerment is an effective promotive and preventive strategy for strengthening maternal participation in routine child health monitoring

Abstract

Appropriate complementary feeding (MP-ASI) is an important factor in supporting the growth and development of children aged 6–12 months. This study aimed to analyze the effect of health education on complementary feeding practices among mothers with children aged 6–12 months in the working area of UPT Puskesmas Kampung Sawah. A quasi-experimental pre-test and post-test control group design was employed involving 76 mothers, who were assigned to either an intervention group or a control group. The intervention consisted of health education delivered through lectures, group discussions, question-and-answer sessions, and educational materials on appropriate complementary feeding practices, conducted in two 60-minute sessions. Data were collected using a structured questionnaire and analyzed using the McNemar test and Mantel–Haenszel test. The results showed that health education significantly improved complementary feeding practices in the intervention group. Health education also increased the likelihood of improvement in complementary feeding behavior, with Odds Ratios ranging from 6.00 to 7.00, and these changes were not influenced by maternal education, age, or occupation. The study concluded that health education is an effective health promotion strategy for improving complementary feeding practices among mothers of children aged 6–12 months in primary healthcare settings.

Abstract

Pneumonia is a lower respiratory tract infection that can cause shortness of breath and impact the patient's psychological condition, including anxiety. Unmanaged anxiety can worsen respiratory status and increase the risk of complications. One independent nursing intervention that can be provided to reduce anxiety is mindfulness spiritual therapy. This study aims to apply mindfulness spiritual therapy to pneumonia patients experiencing anxiety and evaluate changes in anxiety levels and respiratory status. This study employed a descriptive case study approach. The sampling technique used was purposive sampling with two respondents who met the inclusion and exclusion criteria. Inclusion criteria included adult patients with a medical diagnosis of pneumonia who were undergoing outpatient care and had a moderate level of anxiety. In contrast, exclusion criteria included patients with severe, uncontrolled anxiety and impaired concentration. The research process covered all stages of nursing care, namely, data collection through interviews, observations, and physical examinations. The instruments used were the nursing care assessment format and the Standard Operating Procedures for mindfulness spiritual therapy. Data were analyzed descriptively and presented narratively. The results of the case study showed that respondents A and B complained of anxiety about their health conditions and felt tired quickly and short of breath, especially when doing activities such as walking and talking too much. Both respondents had anxiety nursing problems, but patient B also experienced ineffective health care nursing problems. Nursing interventions carried out to address anxiety by applying spiritual mindfulness combined with anxiety reduction were carried out in 3 main sessions. Both respondents showed improvements in anxiety levels, followed by improvements in respiratory status. However, more optimal changes were found in respondent A due to more consistent therapy implementation than in respondent B. Spiritual mindfulness therapy has the potential to help reduce anxiety levels and improve respiratory status in pneumonia patients, especially if patients are willing to consistently implement this therapy due to a strong desire to improve their nursing problems.
