Abstract

Prevalence of Hypertension increased in line with changes of life style as smoking, obesity, non physical activity, psycho-social stress in many countries. Comprehensive and intensive treatment could be more control of blood pressure. Understood, knowledge and adherence of patient expected will improve along with act of distributed useful and correct information by pharmacist. The aim this study to determine the effect of counseling pharmacist on therapeutic outcomes and adherence and to know the correlation between therapeutic outcomes and adherence of outpatients hypertensive in the internal disease clinic at RSUD Kraton Pekalongan District. This research was experimental intervention with pre-post intervention with randomized without matching. Data was collected by prospective method. The control group given a leaflet at the first month, and the intervention group given a leaflet at the first month and counseling every early month, during 3 months.There was 75 patients, the intervention group (N=39) experienced a significant decrease in blood pressure at the end of the study, on systolic amounted 11.28&plusmn;8.33 mmHg [P=0.000(P<0.05)] and diastolic amounted 7.18&plusmn;6.86 mmHg [P=0.000(P<0.05)], whereas in the control group (N=36) did not experience a significant decrease in blood pressure is systolic amounted 2.22&plusmn;10.45 mmHg [P=0.203(P>0.05)] and diastolic amounted 0.28&plusmn;6.09 mmHg [P=0.782(P>0.05)]. Adherence of patients as measured by MMAS scale in the intervention group better adherence than the control group (33.33% : 2.78%). There was have correlation a means between MMAS score with change of systolic (P=0.019; r=0.270) and diastolic (P=0.001; r=0.372) which positive direction. It was concluded that counseling has an effect on adherence positively in the intervention group better than the control group and decreasing systolic and diastolic blood pressure until reach targets. There was a close correlation between decrease in blood pressure with MMAS score in hypertensive patients.

Abstract

Most of probiotic product as functional food were developed from dairy product. However, milk protein content cause allergy and make it limited to non vegetarians only. To solve these problems, some efforts were tried to produce probiotic drink from fruit juice, such as pineaple, tomato, papaya and aple. The aims of this research were to know the quality of probiotic fruit juice organoleptically and the viability of probiotic in it. Lactobacillus plantarum were used as probiotic and the fermentation process were last for 5 days at 30&deg;C. The results showed that the best quality performed by fermented aple juice, with sour taste, pale brown color, aple fermented aroma and good homogenity. The probiotic viability also showed highest number, 1,7x108 cfu/ml. This research can be developed to produce fermented fruit juice with better quality.

Abstract

Catechins , isolate from green tea GMB4 clones could be developed as a potential preventive agent for atherosclerosis. This activity is thought to be based on the structure epimerisation, hydroxy phenolic and galloil group of these isolates. The purpose of this study is to prove the influence catechins ,isolate from green tea, on the inhibition of the decrease eNOS expression in male wistar rats induced by high fat diet. The method used in this study consisted of ELISA for measurement of eNOS expression. The results showed that catechins inhibits the decreasing expression of eNOS at a dose of 6 mg / day and 24 mg / day was significantly but not dose 3mg / day. This suggests a dose 6mg / day and 24 mg / day is able to repair the damage signaling insulin on PI3K pathway caused by insulin resistance. These results are based on the ability of catechins as antioxidants. Catechins with hydroxy phenolic group is a potential electron donor to bind free radicals. Polyphenol catechins may also modulate signaling cellular processes such as NFk&acirc;, activator protein 1 DNA binding, glutathione biosynthesis, PI3K / Akt, MAPK pathway and the Nrf2 transcription factor that regulates expression of several antioxidant genes. It can be concluded catechins inhibits decreasing expression of eNOS at a dose of 6 mg / day and 24 mg / day and can be developed as an agent and vasoprotective ateroprotective.

Abstract

Nigella sativa seed has a lot of beneficial health effect, such as immune-modulator, anti-histamine, anti-diabetic, anti-hypertension, anti-inflammatory, anti-microbes and antitumor. Numeral antimicrobial effect of Nigella sativa has been published, but only a few number of the information of the antimicrobial activity to the probiotic bacteria such as from the genus of Lactobacillus spp. However this research focused on the selectivity of the antimicrobial activity of N. sativa extract to the probiotic and pathogenic bacteria. This research begins with the extraction of N. sativa seed powder by ethanol 80% 1:3 (w/v). With rotary evaporator the macerates extracted till gained semi solid extract. In the other side probiotic and pathogenic bacteria broth cultures were prepared. Extract resoluted with concentration 5%; 2,5%; 1,25% and 0,625%. Into the extract were submerged some disk paper with 6 mm diameter. After that the disk put on to the solid medium were had cultured by bacteria then incubated overnight. The diameters of the clearing zone that seemed then measured. The result showing that the inhibitory zone occurred at 0,625% concentration. After general analysis there is no significant different inhibitory effect of the extract to the tested bacteria, but from the statistical analysis known that probiotik bacteria seemed more sensitive than pathogenic bacteria were tested.

Abstract

The aim of the research is to know the different release gendarusin A from the external tradisional dosage form named param 60% ethanol and param water phase. The release gendarusin A was detected using Transdermal Delivery System equipment and cellophane membrane. Other conditions: Phosphate buffer pH 6,0 as a diffusion Liquid, temperature 37&deg; C &plusmn; 0,5&deg; C, sampling time 10, 20, 30, 40, 50, 60 minutes and sample were analysis by HPLC to get concentration of gendarusin A. The result shows that the release flux param at 60% ethanol fraction = 0,6065 &micro;g/cm2/minute1/2 and flux param water phase = 0,3895 &micro;g/cm2/minute1/2. It means that the release gendarusin A from param ethanol 60% fraction more rapid than param water phase. Release gendarusin A percentage from param 60% ethanol fraction is bigger than param water phase.

Abstract

Kefir is a probiotic drink act as therapeutic agents in liver damage caused by exposure of formaldehyde. Therapeutic activities were estimate based on the content of kefir like antioxidants, lactic acid bacteria with yeasts, and essential amino acids. The purpose of this study was to prove the influence of kefir on the histological and levels of malondialdehyde (MDA) in liver of white mice (Mus musculus) male Balb C strain that exposure by formalin. The method used is experimental method, with three treatment groups, namely: (1) normal group, (2) the induction of formaline without kefir therapy group, (3) the induction of formaline with kefir therapy group. Histological of liver viewed by HE staining and MDA levels were measured by TBARS method. There are differences in the average number of significant cell damage induced by formalin group (p = 0030, p < 0.05). There are significant differences in MDA levels induced by formalin group (p = 0023, p < 0.05). These results are based on the ability of vitamin C, E and carotene (antioxidant) against Reactive Oxygen Species (ROS) due to exposure of formaldehyde, thus preventing the MDA levels increasing. Lactic acid bacteria and yeasts can preclude the possibility of healthy cells to be attacked by ROS with increasing the body&rsquo;s immunity. Essential amino acids can repairing damaged cells by stimulating cell regeneration. It can be concluded that kefir with a dose of 107 CFU / ml act as therapeutic agents in liver damage due to exposure to formaldehyde.

Abstract

Curcumin, the major yellow pigment of Curcuma sp. has several interesting biological activities, such as anticarcinogenecity, antiinflamatory, and antioxidant. Curcumin naturally occurs as major compound in Curcuminoid substance beside its derivatives, i.e. Bisdemethoxycurcumin (BDC) and Demethoxycurcumin (DC). Quantitative analysis of Curcumin is difficult to carry out unless prior to completely resolved Curcumin from BDC and DC. A good resolution, reproducible, and accurate method for determination Curcumin in pharmaceutical product (capsule formed) by HPLC has been carry out. Using C18 colurnn and mixture of Acetonitrile: Acetic Acid glacial 1% (45:55 v/v) as eluent, BDC, DC, and Curcumin were retained with retention time of 10.79, 12.29, and 13.91 minutes, respectively. Curcumin was simultaneously separated from BDC or DC. The resolution factor (Rs) between Curcumin and DC was 1.73. Good linear relationship (r = 0.9997, Vxo = 0.99%) between peak area and concentrations was found at range of the Curcumin concentration used, i.e. 23.56-62.83 &micro;g/mL. Curcumin recoveries were (93.60-118.17)% with the coefficient of variation (CV) 6.99%. Curcumin content in each capsule sample was found to be (8.89&plusmn;0.06) mg. It showed that samples contained higher amount of Curcumin than that claimed on its label, 5 mg.

Abstract

Utilizing of antibiotics to cure operation infection actually not differ from gift antibiotics in general. The difference was antibiotic treatment be addition therapy after surgery. The aim of antibiotics treatment was prevent or cure infection by reducing or eliminating bacteria until human body can handle this pathogenic organism.

Abstract

Indonesia is one of the developing countries and listed as number seven rank in the world as a country with problems with the disease. Increasing prosperity in developing countries and changes in lifestyle lead to increased prevalence of degenerative diseases one of them with diabetes mellitus (DM). Patients with diabetes mellitus need support from other families also need support from professionals in the health sector such as doctors, nurses, pharmacy. With the support - support that is given so that it can improve the quality of life of people and with diabetes mellitus. This study aimed to profile the compliance in the use of oral hypoglycemic glibenclamide. This research was conducted using descriptive research design. Samples were taken from patients with type 2 diabetes mellitus patients in health centers Kedungkandang Malang. Research done by giving questionnaires and visits from the remaining drug count glubenklamid obtained. From the description of the questions is obtained for 73.3% never received an explanation from the pharmacist and 70% of patients did not request an explanation from the pharmacist. Amounted to 86.7% of patients knew the use of drug glibenclamide. 56.7% of patients returned to the clinic every drug out and amounted to 50% of patients received support from family. Patients using the drug each day with a percentage of 53.3% and 66.7% of information on how to read and drink a patient using the drug in accordance with the percentage dose of 70% and use drug the morning before eating with a percentage of 46.7% and 63.33% and patients who never experienced drug side effects of 76.7%. From the calculation of drug remaining is calculated from the sum of all the samples showed results of 83.3% compliance. So the usage of drug compliance glibenclamide in patients with type 2 Diabetes in Primary Health Care Melllitus Kedungkandang Malang by 83.3%.

Abstract

This research is a modification of controlled release which beneficially to optimize the medicine reaction and to minimize the fluctuation of blood level in a certain period of the release. The aim of research is to develop the matrix system in ethylcellulose basic PVP K30 which applies dispersion solid method using sodium diclofenac as sample. Resulted four formulas consist of PVP K30 percentages as follow 0%, 5%, 10% and 20%. The different percentages of PVP K30 is aimed to know the formula which produces the releasing result of ND more optimal. This research produces in each formula significant differences (p<0,05). The formula whole percentages of PVP K30 10% has completed the requirements of control release better than the other one, shown the better resistance characteristics and release profile.
