Abstract

Background: Mental disorders are conditions that affect a person's thoughts, feelings, and behavior, often disrupting daily activities, including adherence to prescribed treatment. Non-adherence to medication is one of the leading causes of relapse and re-hospitalization in patients with mental disorders. One behavioral approach that can be applied to improve adherence is the token economy method. Objective: The aim of this case study is to apply nursing care for patients with mental disorders who experience medication non-adherence by implementing a token economy intervention. Methods: This study used a descriptive case study design. The researcher explored the real-life experiences of patients through direct observation, interviews, and document analysis. The nursing process was guided by the Indonesian Nursing Diagnosis Standards (SDKI), Nursing Outcomes Standards (SLKI), and Nursing Intervention Standards (SIKI), along with the Morisky Medication Adherence Scale (MMAS-8) as a measurement tool. The subjects were two male clients aged 40–45 years with low adherence scores and no medication supervisors. The intervention was conducted over six sessions in two weeks, providing star-shaped tokens each time the clients took their medication, which could later be exchanged for personal items. Results: The intervention resulted in an increase in MMAS-8 scores from 4 (low) to 7 (moderate) in both clients. Improvements were also observed in all adherence indicators based on SLKI. Clients showed higher motivation, improved behavior, and a reduction in psychiatric symptoms. Conclusion: The application of the token economy method as a behavioral therapy is effective in increasing medication adherence in clients with mental disorders. This intervention helps establish positive routines and provides meaningful motivation for clients.

Abstract

Background: Hypertension is a leading global health concern and a major cause of cardiovascular disease. Its prevalence continues to rise in Indonesia, particularly in rural regions. This study analyzed demographic, dietary, and lifestyle factors associated with hypertension risk in Pulau Morotai Regency. Methods: A cross-sectional study was conducted at Ir. Soekarno Regional Hospital from July to August 2025 among 90 hypertensive patients. Data on demographics, lifestyle, and diet were collected using structured questionnaires and medical records. The dependent variable was hypertension risk (“at risk” vs. “not at risk”), while independent variables included gender, age, smoking history, dietary pattern, salt intake, saturated fat intake, fruit and vegetable intake, and processed food consumption. Data were analyzed using IBM SPSS Statistics version 25 with chi-square tests. Sampling weights accounted for stratification, clustering, and unequal probability of selection to ensure representative estimates. Results: Most participants were older adults (90%) with unhealthy diets (84.4%), high salt intake (84.4%), frequent processed food (58.9%) and saturated fat consumption (58.9%), and low fruit and vegetable intake (53.3%). Hypertension risk was significantly associated with age, diet, salt, saturated fat, fruit and vegetable intake, and processed food (p < 0.001). Gender and smoking were not significant. Conclusion: Hypertension risk in Pulau Morotai is driven by age and modifiable dietary behaviors. Interventions should prioritize salt reduction, healthy eating, and dietary counseling.

Abstract

Latar Belakang: Imunisasi efektif menurunkan morbiditas dan mortalitas anak akibat penyakit yang dapat dicegah dengan vaksin. Namun, cakupan imunisasi lengkap di Indonesia masih di bawah target nasional, terutama di daerah pedesaan. Penelitian ini bertujuan untuk mengidentifikasi determinan imunisasi lengkap pada anak usia 12–23 bulan di UPTD Puskesmas Popayato Barat, Kabupaten Pohuwato, dengan penekanan pada pendidikan orang tua dan kepatuhan vaksin. Metode: Desain analitik potong lintang digunakan dengan data sekunder dari 114 anak usia 12–23 bulan. Imunisasi lengkap didefinisikan sebagai menerima semua vaksin yang direkomendasikan oleh Program Imunisasi Nasional Indonesia. Variabel bebas meliputi jenis kelamin anak, pendidikan orang tua, pekerjaan orang tua, dan penyelesaian vaksin individu. Data dianalisis menggunakan SPSS versi 27.0.1. Asosiasi diuji menggunakan analisis chi-square (p < 0,05). Hasil: Dari 114 anak, 64,9% telah mendapatkan imunisasi lengkap. Pendidikan orang tua berhubungan signifikan dengan imunisasi lengkap (p = 0,021), sementara jenis kelamin anak dan pekerjaan orang tua tidak berhubungan. Penyelesaian vaksin dini sangat memprediksi imunisasi secara keseluruhan (p < 0,001). Kesimpulan: Pendidikan orang tua dan kepatuhan vaksin dini merupakan penentu utama imunisasi lengkap. Penguatan pendidikan kesehatan, keterlibatan orang tua, dan tindak lanjut yang konsisten dapat meningkatkan penyelesaian vaksin dan mendukung tujuan imunisasi Indonesia.

Abstract

Background: A fracture or broken bone is a total or partial loss of bone continuity, which is determined based on the type and extent. Based on data from nurses on duty at the IGD RS X, handling fracture patients remains problematic. One action nurses can take to improve patient satisfaction in health services is to demonstrate caring behaviour. Caring is a behaviour or action that genuinely provides others with a sense of physical and emotional security. Methods: This is a descriptive correlational study with a cross-sectional design. The sample consisted of 55 patients, selected using convenience sampling. Data collection used the nurse caring questionnaire and patient satisfaction questionnaire. The data analysis used in this study includes univariate and bivariate analyses. Results: Characteristics of respondents in the IGD RS X, based on gender: 54,5% were male; education level: 47,3% higher education; and occupation: 29,1% were civil servants. Most respondents were at the satisfaction level 90,0%. The results of the Spearman rank correlation test indicate a strong relationship between nurses' caring behaviour and patient satisfaction in the IGD RS X (P-value = 0.040).

Abstract

Hypertension is one of the most prevalent diseases among elderly. One of the key risk factors contributing to hypertension-related complications is an unhealthy dietary pattern. This study offers an updated approach by utilizing support group therapy as a social–educational intervention aimed at improving dietary habits among the elderly. Unlike conventional one-way educational methods, support group therapy emphasizes emotional support and shared experiences, thereby encouraging sustainable behavioral change. The purpose of this study was to determine the effect of support group therapy on dietary patterns among older adults with hypertension. A pre-experiment study using a one-group pre-test and post-test design was applied. The study population consisted of older adults and a total of 38 hypertensive older adults were selected using purposive sampling. The study collected data using a questionnaire and performed analysis with the parametric paired t-test. The findings revealed an increase in the mean dietary pattern score from 20.61 before the intervention to 31.23 after the intervention. Statistical analysis showed a significant effect of support group therapy on the dietary patterns of older adults (p = 0.000). Support group therapy is recommended to be routinely implemented in elderly posyandu as an educational and supportive strategy to improve adherence to healthy dietary practices and may be expanded on a broader scale to strengthen hypertension control among the elderly population.

Abstract

Abstract Patients with Chronic Kidney Disease (CKD) frequently experience musculoskeletal discomfort, including shoulder and neck pain, due to uremic neuropathy, reduced mobility, and prolonged static posture during hemodialysis. Non-pharmacological interventions such as scapula massage and cervical massage may help reduce muscle tension and improve range of motion. The objective of this study is to report the effects of combined scapula massage and cervical massage on shoulder–neck pain in a CKD patient. The method of this study adheres to the CARE (Case Report) Guidelines. The results showed that the patient's pain score decreased from NRS 6 to NRS 2 after three intervention sessions. In conclusion, scapula and cervical massage may serve as safe and effective complementary therapies for reducing musculoskeletal pain in patients with CKD. Abstract

Latar Belakang: Penyakit ginjal kronik (PGK) merupakan masalah kesehatan yang semakin meningkat di Indonesia, terutama pada lansia yang memiliki risiko lebih tinggi akibat penuaan fisiologis dan gangguan metabolik. Diabetes melitus merupakan penyebab utama PGK secara global, namun bukti berbasis populasi pada lansia Indonesia masih terbatas. Tujuan: Menganalisis pengaruh diabetes melitus serta faktor sosiodemografi dan klinis terhadap PGK pada lansia Indonesia menggunakan data Survei Kesehatan Indonesia (SKI) 2023. Metode: Studi potong lintang ini menganalisis 97.339 responden berusia ≥60 tahun dari dataset SKI 2023. Status PGK ditentukan melalui diagnosis dokter yang dilaporkan sendiri. Analisis survei kompleks pada IBM SPSS Statistics 25 menggunakan pembobotan, stratifikasi, dan pengelompokan untuk menyesuaikan desain survei. Uji chi-square digunakan dengan batas signifikansi p < 0.05. Hasil: Prevalensi PGK sebesar 0,5% (n = 452). Diabetes melitus menunjukkan asosiasi terkuat dengan PGK (p = 0.001). Jenis kelamin, tingkat pendidikan, dan tempat tinggal juga signifikan, sedangkan kelompok umur, status perkawinan, dan pekerjaan tidak signifikan. Kesimpulan: Diabetes melitus merupakan determinan utama PGK pada lansia Indonesia, sehingga perlu penguatan pengendalian glikemik, skrining rutin, dan edukasi kesehatan yang lebih terarah. Abstract

Indonesia as a member of the Association of Southeast Asian Nations (ASEAN), faces a high level of patient visits to the Emergency Room (ER), with the number of patients reaching 4,402,205 in 2017. Triage is a crucial process in handling in the ER that aims to reduce morbidity and mortality. This study aims to identify factors that influence the implementation of triage in the ER of Budi Rahayu Regional Hospital. The method applied is a cross-sectional study with a sample of 10 nurses working in the ER of Budi Rahayu Regional Hospital, Magelang City. The findings of the analysis show that there is no significant relationship between education level and the implementation of triage, because the asymptotic standard error cannot be calculated (zero value), there is no significant relationship between length of service and the implementation of triage with the Chi-Square value obtained is 1.071 and the 2-sided asymptotic significance value is 0.301. However, there was a significant and strong relationship between knowledge and triage implementation, with a lambda value of 1.000 indicating a perfect relationship and strong statistical significance (p = 0.038). In contrast, training did not show a significant relationship with triage implementation, with a two-sided asymptotic significance of 0.778, which is greater than the alpha value of 0.05. In conclusion, knowledge proved to be a significant factor in triage implementation, while education, length of service, and training did not show a significant relationship in this study.
