Dr. Saiful Anwar Malang is a state hospital has done it is job and function, but in 3rd class of pavilion room, the number of patient decrease dramatically. It is concerned with quality of this hospital. To answer this problem, research was done using Quality Function Deployment (QFD). Quality Function Deployment is a tool which design some needs include customers represented as a voice of customer and including some competitions and also groups some activities that usually called affinity graphic ang getting a benchmarking for it is competition. From the result analysis can be showed that main attribute for patience is a accuracy. And from House Of Quality can be found that getting a periodic meeting to evaluate this hospital and also increase a service can be made 20 concept

Abstract

In practice, the policy of delaying payment periods is prevalent between players in a supply chain system. Generally, payments made at the end of the permitted period. Supply chain management is one of the keys to corporate sustainability that the activities have an impact on the environment.  This paper aims to develop an integrated green supply chain model with a permissible delay in payment consideration. In this research, the author develops a mathematical model to find the effect of delay in payment on emissions costs without ignoring the economic performance of a supply chain. The author develops four different scenarios model. Furthermore, numerical experiments and sensitivity analysis tests were conducted. Result of the study shows that delay in payment is integrated players into the supply chain system.  It has a positive impact on reducing supply chain emissions costs.

Abstract

PT XYZ is company of logistics service of PT CI where this company hold responsible to logistics activity of PT CI that is covering distribution and warehousing. In transportation activity PT XYZ conducted armada outsourcing transport from some transporter every day. A problem faced in this time was truck rent expenses for the delivery of order to store. Hypothesis customer early from this research was if it done consolidation delivery of order customer, hence expected will be yielded to decrease cost rent daily truck which enough significant. At this research, consolidation conducted by developing method of saving heuristic. By using data of customer was same order on February, 10 2006, this research can reduce consumer of truck counted 3 truck and yield cost-saving rent equal to 18.87% from expense of early. This research also design software prototype for daily operational activities in determining used truck.

Abstract

The increase or the Merck and kind of cigarette that revolve in the market had caused  a competition in the cigarette tighter. The new Werck that enter the market hardly fight to get the market segment with looking for another Merck's consumer, while the Merc's that had a name in the market for along time must retained to be dead increase their market section or just keep on the market segment that possess it. In the other hand,  government always suggests the  society not to smoking because of helath reason.  This matter, of course little or more influence the growth of cigarette  consumption that  at the  end will make tighter  the  cigarette product  competition  the market. The enterprises that is involved in the competition condition of course will not released from importance of the strategy utilizing optimally so the pproduct that sold will stand survive The tighten of this competition require the cigarette producer to be creative in looking  for the marketing strategy that fit to the product wich risen. One of them is with  do the  competition analysis.  This competition analysis function as to know the speciorities and weakness that is had by main competitors. Beside that enterprise also must be able to catch consumer preference in the aim to consume the cigarette also their perception to the product is risenProduct grouping is done to know the main competitors Pall Mall product from the product grouping that  is done.  It's  found that  Pall Mall tightly compete with  Sampoerna A Medium, Wismilak,  Diplomat,   234  Filter,   and   Gudang  Garam  Internasional   Merah   Analysis,   the competition is made with that main competitor. It so happens the mathod that is used for analyze that competition is Game Theory Method which is used is Two Person Non Zero Sume  Game and finished with linear program simplex mathod.  The result that is get in this competition analysis is competitions speciorities which is had by product compared with main competitor.The main purpose from the whole calculation and the data processing is business  strategy formulation  optimally  based  on  speciorities that  is  had  by  product  with  consider   about the speciorities of the main competitor also consumer preference. Formulation will be made specially to the Pall  Mall product with the main competitors.  The prime thing that is a grip in the business stratefy formulation is the speciorities that's had by main competitors and consumer preference to the p~oduct attributes.

Abstract

The accident occurred in industry is one of the causes of work disruption or cessation. Work accident should be avoided because of causing risk for company. PT “X” is a tobacco company which has a high level of work accident. Therefore, it is necessary to establish a particular effort in order to prevent and reduce the accident. Hazard Identification and Risk Analysis (HIRA) is the method aims to identify any risk types included in the work operation. Where as Standardized Plant Analysis Risk Human Reliability Assessment (SPAR-H) is the method aims to find out the possibility of human error occurrence in the work operations. Based on the research conducted, it can be concluded that the accident occurred in PT. “X” is caused by environmental condition and insecure workers’ action, and inappropriateness with HSE procedures. PT “X” should enroll some corrective actions so the accident can be reduced. In this study, PT “X” should create improvement through administrative action and the use of safety equipment.

Abstract

This research was aimed to utilize the liquid waste of cacao industry as raw material for nata production. Research was performed in two steps : the first steps was clarification process using active charcoal in different waste dilllutions and the second step was the production of nata. Result of first step showed that active charcoal concentration and dilution treatment showed their effect of all parameters studied. The best result was obtained by treatment of 5% active charcoal concentration and the dilllution of cacao waste : water in 1:3 ratio. The production of nata in the second step of research was performed using this optimal clarification condition. Data showed that the combination treatment of 4% sucrose concentration and 0,4% of (NH4)2SO4 was elected as the best process that gave the best quality of nata. The best treatment showed product characteristic : 83,87% of fermentation yield; 95,23% of moisture content; 4,22% of fiber content; brightness (L*) of 42,87; 0,01 mm/g.dt of textur and 2,42 cm of nata thickness.

Abstract

Along with the development of the tourism in Bali, one of the phenomena occurs was that the land conversion to the non-agricultural land, causing the canal irrigation was not able to function again. In addition, water demand will be increased not only for irrigation, but also for other sectors out of irrigation. This condition can interfere the performance of the subak system. One strategy to improve subak system is to develop agroecotourism based on subak system. Analysis of physical performance subak system done using fuzzy logic approach to quantify the condition of vagueness. Because of the problems associated with subak irrigation systemperformance are often vague and can not be classified with certainty. The results of this study are (i) the values of subak irrigation system performance indicators, the condition and thefunction of subak irrigation facilities, the condition and the function of subak facilities, and the agroecotourism facilities; (ii) find out the physical performance of subak irrigation systems in Bali with agroecotourism oriented. The stages of this research were collecting and analyting the data include: (i) RWS (Relative Water Supply), (ii) RIS (Relative Irrigation Supply),(iii) Ia (Index Area), (iv) analysis of the condition and function of subak irrigation networks, (v) the condition and the function of subak facilities, (vi) the agroecotourism facilities, and (vii) physical performance of subak system using fuzzy logic approach. The utilization rate of water on Subak Anggabaya and Subak Lodtunduh in one year was good. Subak Anggabayaand Subak Lodtunduh have sufficient irrigation water supply with RIS values in one year > 1. Based on Area Index (Ia), it could be seen that the irrigated area in Subak Anggabaya andSubak Lodtunduh in accordance with the plan. The physical performance of Anggabaya subak system and Lodtunduh Subak system was in the middle criteria. The simulation results that development of agroecotourism on subak system could improve the physical performance of subak system.

Abstract

CV. Cool Clean is a company engaged in packing tissue. During this time the company only perform corrective action that causes disruption of the production process due to frequent damage of a sudden the engine components volpack. In this study used a model of preventive replacement that can reduce downtime and costs. The method used to obtain the schedule interval a critical component is age replacement method. Seal heater obtained replacement time interval of 30 days with a 22% reduction in downtime. Replacement interval for knife foil is 26 days with a 27% reduction in downtime. Replacement interval for solenoid valve is30 days with a 29% reduction in downtime. Replacement interval forOring seal is 18 days with a 29% reduction in downtime. Replacement interval for needle bearing is 62 days with a 25% reduction in downtime. Results of this study also showed that by applying age replacement can save costs for seal heater Rp. 974.000 or 23%. Knife foil cost savings of Rp. 1.251.409 or 22%. Solenoid valve cost savings of Rp.546.539 or 24%. Oring seal cost savings of Rp. 350.096 or 26%. And needle bearing cost savings of Rp. 196.712 or 26%.

Abstract

UD. Dhika Putra is a tahu manufacturing company. Currently, the company is having some problems in production facility layout and physical environment. This study aimed to redesign the facility layout of tahu manufacturing company to minimize the path length of material handling and apply the 55 method to enhance work productivity. The approach used to redesign the plant facility layout is Systematic Layout Planning (SLP), which can be categorized in three stages, namely material flow analysis, research, and selection. While the 55 method consist of several steps to organize workplace condition that affect on the effectiveness, eficiency, productivity, and safety. This study concluded that the chosen layout for the final layout showed a significant decrease in path length of material handling, i.e 45 meters or about 19,2%. The decreasing of material handling path length on the choosen layout was also influenced by the arrangement of the work place by using 55 method.

Abstract

Awing & Son represent one of the company which active in effort songkok, handling of inefficient distribution represent main problem in this industry which cause the expense of high product distribution. Of the condition needed by an method to overcome the problem. Alternative of I is Method Transportation represent one of the technique used for the allocation of product to customer by considering the expense of product distribution, capacities of[is source of and request of customer. As for its delivery route is network shipping direct. Especial target of this method is to plan the amount of proposal product allocation so that is later expected by minimization can the expense of product distribution. Alternative of II is Method of Saving Matrix represent one of the method which studying process delivery of goods to customer covering each allocation of retail at vehicle at the same time sort route to pass by vehicle. Especial target of this method is with order allocation every retail at vehicle also minimization can the expense of product distribution. As for its delivery is runs milk with shipping direct. Before we apply transportation method goodness and also method of Saving Matrix, is beforehand done by plotting and forecasting of request data 2 months come. Forecasting of demand by using method matching with result plotting of data, and then selected [by] smallest MAPE. After that result of the forecasting processed by using transportation method and of Saving Matrix. From data processing obtained by result for the allocation of product with transportation method got by retail for the allocation of warehouse of Gresik is to product allocation to Kediri, Ponorogo, Blitar, Banyuwangi, Probolinggo, Situbondo, Sidoarjo, Lamongan, Krian. and Jombang. While for the warehouse of Surabaya product allocation to Malang, Madiun, Jember, Asem Bagus, Madura, Mojokerto, Pasuruan, Krian. By using method of Saving Matrix obtained by cost-saving of distribution equal to Rp130,500.00/period of delivery or equal to 5.17% from Rp2,654,375.00 becoming Rp2,523,875.00.

Abstract

In trade competition for this time, then each factories must be try to completing consumer demand on precise time and quantity. One thing which influence is fluent of production. In order that production able to work smoothly, so it is necessary to conducting work resposibility to perform work operations in production process. There is exact work responsibility could be optimalizing collar worker productivity. Then, this thing have side effect to production cost. One way to create this situation is with using VAM (Vogel' Approximation menthod). On VAM, this case will be finished equivalen with transportation problem that production capacity and demand said on same squad, same total capacity and same demand. If some time this situation is not completed, then the system must be balanced out first with make capacity or demand Dummy.
