Dr. Saiful Anwar Malang is a state hospital has done it is job and function, but in 3rd class of pavilion room, the number of patient decrease dramatically. It is concerned with quality of this hospital. To answer this problem, research was done using Quality Function Deployment (QFD). Quality Function Deployment is a tool which design some needs include customers represented as a voice of customer and including some competitions and also groups some activities that usually called affinity graphic ang getting a benchmarking for it is competition. From the result analysis can be showed that main attribute for patience is a accuracy. And from House Of Quality can be found that getting a periodic meeting to evaluate this hospital and also increase a service can be made 20 concept

Abstract

Risiko menjadi bagian yang tidak terpisahkan dalam setiap aktivitas perusahaan sehingga cara terbaik yang dapat dilakukan adalah mengantisipasi dan melindungi diri terhadap risiko. Permasalahan yang terjadi pada PT BALI GRAHA SURYA adalah pada proses welding dan welding inspection, stringing pipa, dan trenching pipa di mana risiko yang terjadi memengaruhi keselamatan dan kesehatan kerja (K3) karyawan dan juga memengaruhi lama waktu penyelesaian proyek. Dalam penelitian ini digunakan konsep manajemen risiko untuk menganalisis risiko operasional, di mana konsep tersebut dilakukan untuk mengidentifikasi, mengawasi, dan mengkomunikasikan kejadian risiko yang berhubungan dengan segala aktivitas yang terjadi di perusahaan. Dari hasil penelitian didapatkan bahwa risiko yang ada pada proses welding dan welding inspection adalah risiko terbakar, bahaya sinar UV dari pengelasan, bahaya panas, bahaya percikan api las, kejatuhan pipa dan tersetrum mesin las dengan total indeks risiko sebesar 9,324. Stringing pipa memiliki risiko tertimpa dan terjepit pipa dengan total indeks risiko sebesar 8,481 dan Trenching pipa memiliki risiko tanah longsor pada bantaran dengan total indeks risiko sebesar 8,092. Tindakan untuk penanganan risiko tersebut adalah dengan mewajibkan pekerja menggunakan APD, memeriksa semua kondisi isolasi untuk mengetahui kondisi alat yang akan digunakan, bekerja sesuai dengan SOP, memasang dinding pengaman galian, dan penempatan tanah bekas galian minimal 1 meter dari bibir galian.Kata Kunci : Risiko, Keselamatan dan Kesehatan Kerja, Manajemen Risiko.ABSTRACT The risk is a common part in every business process of any corporation, so anticipation and protection are the best action to overcome it. In PT BALI GRAHA SURYA, the problems often happen in the welding process and inspection, stringing pipe and pipe trenching where its risk affects the safety & health of labor and the time completion of project. This research adopts risk management concept to analyze the operational risk where that concept aims to identify, monitor, and communicate the risk events associated with all the activities, functions or processes that occur in the company.   The result shows that the risks occurred in the welding process and inspection are fire hazards, UV rays from welding hazards, thermal hazards, welding sparks, pipe collapse and electric shock. Those risks count total risks index 9,324. For stringing-pipe task, the risks such as crushed and wedged by pipelines count total risks index 8,481. Trenching pipelines task counts total risk index 8,092 caused by landslides in the riverbank. The risks management effort can be performed by requiring workers to use appropriate personal protective equipment, inspecting all insulation conditions in order to determine the condition of the tool to be used, working in accordance with the SOP, installing excavation security wall, and placing ex- excavation at least 1 meter away from the edge of excavation.

Abstract

In Indonesia, the increasing area of palm oil plantation has been followed by the increasing number of palm oil mills from 205 units in 1998 to 695 units in 2012. The palm oil mill technology has also been developed, and one of them is fresh bunch sterilizer. The technology of sterilizer changed from horizontal to vertical, and the system improved from batch to continuous. This study analyze the determination of priority in selecting sterilizer technology by using Analytical Hierarchy Process (AHP) method. The advantages of AHP method are easy to understand and useful for decomposing a complex problems into a simple hierarchy of decision problem. According to the evaluation of three alternatives by using AHP, it shows that the priority order of the sterilizer models is respectively vertical, horizontal, and continuous with overall weight 0.366, 0.335, and 0.299 respectively.

Abstract

Small and Medium Enterprises (SMEs) have a major role in the economy of country , both in contribution to the Gross Domestic Product and total employment. In order to develop its business, tempeh SME Sanan Chips has to create an innovation and conduct an improvement in order to be able to compete with other businesses. Due to existence of competition among the competitors with different brand of chips, so SME tempeh chips Sanan should improve properly the determination of marketing strategy. Strategy analysis can be conducted by using element of marketing mix. That triggers the necessity to conduct a research in terms of factors appealing marketing and measure how well a united element of marketing mix is performed according to the marketing strategy. After gaining information related to element of marketing mix from the questionnaire given to the SME tempe chips, then analysis is performed based on SWOT and QSMP to determine the major marketing strategy. According to the result, SME Sanan Chips should adopt the marketing strategy by: focusing on product diversity through producing various chips, launching a brand variant with a various pack and net weight in order to offer different choices to the consumers, developing an agent networks to expand the region of market sale, conduct promotion through exhibitions and major events in the Province of East Java, promoting the product intensively through a free advertisement and social media.

Abstract

UD Sumber Makmur is one of the home industries producing snacks especially cassava chips with various kinds of flavor. Packaging process is still performed manually through utilizing the physical power of workers. This research aims to evaluate manual workload in the particular tasks. The research object includes two groups of task which are group A (associated with production tasks) and group B (associated with packaging tasks). According to the result, it shows that average physical workload in both groups is constants and it is included in light-medium work category. The break time required by workers in group A and B respectively is 31 minutes and 45 minutes.

Abstract

Considering that raw material cost could reach 70% of product cost, supplier selection process was become important consideration to improve the efficiency of procurement department. The supplier selection process was multi-criteria problem involving qualitative and quantitative factors. UD Virgo Bersinar was one of fruit chips manufacturer located in Malang. The company often experienced shortage of jackfruits which interrupt production process. Thus, it was necessary to choose its supplier by considering company’s criteria and order allocation of selected supplier. This research would integrate supplier weight from Analytic Hierarchy Process and Preemptive Goal Programming model to determine priority of supplier and order quantity to the supplier. The model was formulated to achieve two objective functions, namely maximizing total value of purchasing and minimizing total cost of purchasing, by considering capacity of supplier and fulfillment of jackfruit. By running goal programming model, it was concluded that supplier from Semarang would supply 7.5 ton of jackfruit in November 2013-Januari 2014 and deficiency of needs would be supplied by supplier from Tajinan.

Abstract

Dual-Channel Supply Chain (DCSC) is a mixed structure which simultaneously utilizes both offline and online channel to distribute one particular product or service. This structure provides market reach extension in which eventually leads to revenue increase. Whatsoever, aside of this advantage, DCSC also brings some challenging problems. One prominent problem is of about between-channel pricing-problem. In its practice, DCSC actors often apply intuitive offline and online pricing without further analytical evaluation. Yet there is no representative mechanism to address this critical problem. This paper attempts to resolute this challenge by proposing a qualitative estimation mechanism on customer acceptance on online channel relative to offline one. This proposed approach is about to utilize an interplay relationship between offline and online prices. The main parameter to consider is a ratio of customer acceptance parameters. These parameters are elicited from 3 groups of products which are sold under DCSC structure in Indonesian cases to exhibit the different properties in each product group. Numerical experiment result shows that the utilization of this pricing mechanism may increase the financial performance of DCSC under observation.

Abstract

Most of indonesian societies consume rice as staple food and therefore need the amount of rice that meet the demand within the infinite time. Food warehouse is needed in order to maintain supplies. Rice warehouse should be located in the strategic location so that it is easy to reach and to avoid the stagnation of supply. In addition, that can also be useful for financing streamline because it can minimize the logistics costs. Dolog as a unit of the agency Bulog is responsible to maintain the stability and availability of basic foodstuffs in the region of Indonesia and to participate in the procurement and supply of basic foodstuffs especially rice. To carry out its duties, Dolog also must have an ideal rice warehouse which its capacity should meet the demand of the rice according to the market supplied. The result of cluster method used in this research suggests that there are three ideal warehouse locations to supply main markets in Bandung. If the three ideal warehouse locations are compared with the existing warehouse, proposed warehouse location donates saving about IDR 45,032,203 per year or approximately IDR 125,000 per day. So there are two solutions can be taken by Dolog that the first is open new warehouses and the second is using the old warehouse and adding the shortage storage capacity by opening additional warehouse at ± 6,000 tons.

Abstract

Topeng Malangan is one of potential cultural products developed to preserve culture and bring revenue for stakeholder in Malang. This study aims to identify internal and external factors affecting the existence of small medium enterprise (SME) Topeng Malangan and formulate its development strategy. The identification of problem and factors is done by focus group discussions (FGD) involving businessmen, cultural observer, academician, regional government and researchers. The results of FGD is then analyzed and developed by using SWOT matrix in order to determine formula of SME development strategies. To sharpen the strategy more effective in implementation, the analysis using IFAS and EFAS matrix is conducted. The result is final formula for SME Topeng Malangan development strategy that is more focused on strategy SO (Strength - Opportunities); which is the utilization of strength organization of SME Topeng Malangan for taking advantage of opportunities.

Abstract

“One Way Convection” is a garment company which produces many kinds of t-shirt based on the customer’s order. In order to fulfill the order, the company applies role of First-Come First-Serve for scheduling the order production sequence. This role is often disadvantageous for the order with short process time because if it is dispatched after the order with long process time, then it will take longer in queue and causes the completion time of all order to be longer. This research aims to schedule t-shirt’s sequence operation in “One Way Convection” company by using Pour heuristic algorithm so that completion time becomes shorter and order fulfillment can be accomplished on-time. The steps in this methodology consist of collecting data, calculating standard time and total process time according to the order, scheduling using initial method proposed by company, scheduling using Pour heuristic algorithm, and comparing makespan of both approaches. According to the Pour Heuristic method, saving is obtained in makespan as 8.09 hours or 19.25% compared with initial method proposed by company.

Abstract

Final inspection for acceptance or rejection of a product is required by the company before the products are distributed to consumers. Sinar Angkasa Rungkut I, Ltd. Surabaya is a company that produces light. Referring to product quality control, sampling technique is one of methods that can be used to perform inspection in sealex lights process which is seen from bulbs and seals condition. The purpose of this study is to determine the acceptance sampling technique which is suitable for the final inspection of production for the company. From the second sampling which has been used, for sequential sampling result is much better than a single sampling, because the results of sequential sampling calculations give a smaller sample size. In addition, the probability of acceptance (Pa) resulting from sequential sampling produces exactly the same value with the producer and consumer risks defined, namely 0.95 and 0.10.
