Abstract

When playing sports, athletes are required to always maintain their health to provide maximum results in their performance. Supplements are products intended to complement nutritional needs, maintain, enhance and/or improve health functions and have nutritional value and/or physiological effects. L-Carnitine is a health supplement that is widely consumed by athletes. L-Carnitine has many effects on the body and the main effects are seen on blood pressure and VO2 max. The aim of this review is to look at the effects of L-Carnitine on blood pressure and VO2 max. It was found that L-Carnitine had more effect on a person's diastolic than systolic at doses of more than 2 grams/day. Meanwhile, the effect on VO2 max was seen in long-term use of 2 grams for 24 weeks and found a significant increase in VO2 max (p<0.05). L-Carnitine does influence blood pressure, VO2 max and is also correlated with endurance, but until now there is still a need for more research to show that the effects produced by L-Carnitine are significant both on blood pressure and VO2 max in athletes.

Abstract

Healthcare financing involves the management of various efforts to mobilize, allocate, and spend health funds to ensure the implementation of health development aimed at achieving the highest possible level of public health. Hypertension, a non-communicable disease (NCD) continues to evolve and poses a serious public health issue and leads to an increasing health financing burden. Health Social Security Agency (BPJS) indicated direct medical costs of hypertension IDR 3 trillion in 2017 and 2018. The variety of first-line antihypertensive combinations due to the numerous recommended first-line antihypertensive combinations in various guidelines highlights the importance of pharmacoeconomic studies to determine the most cost-effective combination. This review article aims to further discuss the implementation of the CEA method as a basis for treatment selection or decision-making regarding hypertension treatment. The nifedipine 60 mg and candesartan 32 mg combination is deemed the best, achieving a blood pressure reduction of 23,8/16,5 mmHg (p<0,01) compared to placebo and monotherapy reductions of 5,3/6,7 mmHg. Additionally, the nifedipine GITS - candesartan combination reduces vasodilator side effects incidence to 18,3% compared to 23,6% for nifedipine monotherapy, including a lower incidence of headaches (5,5% vs. 11%).

Abstract

Good control of the supply of drugs and consumables (BMHP) by Hospital Pharmaceutical Installations (IFRS) can avoid shortages or overstocks in pharmaceutical warehouses. The Type C "X" Special Hospital in Semarang City experienced a stockout of IVF drugs and embryo laboratory BMHP which resulted in delays in medical procedures and a decrease in the quality of service. The purpose of this study is to determine the factors that affect the efficiency and effectiveness of pharmaceutical preparations using the MMSL method. This study is a quasi-experimental non-equivalent without control group, samples were taken purposively using retrospective data from October – December 2023 with a stable supply and the research was applied prospectively from January – March 2024. Twelve types of IVF drugs and 56 types of BMHP embryo laboratories were used as research samples. The statistical test used cross-tabulation between efficiency variables (inventory value, inventory turnover ratio [ITOR], stockout value) and effectiveness (number of stockout events). The test results showed that the lower the price of IVF drugs and BMHP in embryo laboratories led to lower inventory values. The more expensive the BMHP price of embryo laboratories results in higher ITOR. The faster the lead time supplier BMHP embryo laboratory, the smaller the stockout will occur. The larger the Smin, Smax, and BMHP safety stock of the embryo laboratory, the lower the incidence of stockout. This study provides an insight into the factors that support the application of MMSL to inventory control in IFRS "X".

Abstract

Hypertension poses a significant global health burden, prompting exploration into alternative treatments. This study investigates the pharmacological network of Orthosiphon aristatus, aiming to uncover its therapeutic potential for hypertension. We compiled data on Orthosiphon aristatus chemical constituents and predicted potential targets for its key components. Through screening processes utilizing the Gencard database, active compounds and protein targets were identified. We examined protein target similarities between Orthosiphon aristatus and hypertension, constructing a network illustrating the relationships between active compounds and target genes. Additionally, we conducted GO function analysis and KEGG pathway enrichment to elucidate Orthosiphon aristatus role in hypertension. Notably, TP53 exhibited the highest degree of centrality, while Scutellarein and aurantiamide acetate displayed the highest affinities in molecular docking with TP53. These findings offer novel insights into Orthosiphon aristatus' potential as an adjunctive therapy for hypertension and contribute to the advancement of pharmacological interventions in this domain.

Abstract

Antimicrobial resistance represents a significant challenge in clinical settings, particularly in intensive care units (ICUs). The incidence of antimicrobial resistance in the ICUs has reached 30% of all hospital patients. Furthermore, over 71% of patients on the unit were using antibiotics. Consequently, infections contribute substantially to the elevated mortality and morbidity rates in patients with critical status. The Antimicrobial Stewardship Program (ASP) is a program that refers to a multidisciplinary approach to optimize the appropriate use of antimicrobials. This systematic review aims to identify ASP policies implemented in hospital ICUs, utilizing various parameters for assessment. This systematic review was conducted using PubMed, Scopus, Google Scholar, and the Cochrane Database of Systematic Reviews, focusing on publications from January 1, 2015, to June 1, 2024. A comprehensive review of 4,419 articles identified 12 studies that met the inclusion and exclusion criteria. The primary goal of ASP is to enhance the appropriateness of antimicrobial use, ensuring accurate prescribing practices in terms of indication, antimicrobial selection, route of administration, and therapy duration. The outcomes of ASP implementation are evaluated based on decreased in antimicrobial use, increased in the number of recommendations, decreased in treatment costs, and a decrease in multidrug-resistant organism (MDRO) incidence. Although numerous articles have provided positive results regarding the implementation of ASP in ICU settings, there remains a need for extensive and structured research to validate these findings comprehensively.
