Abstract

In anesthesiology, ethical principles such as autonomy, beneficence, non-malfeasance, and justice are critical. This paper investigates these ethical considerations, emphasizing the process of obtaining informed consent. It addresses the unique challenges in anesthesiology, including the effects of anesthesia on patient cognition, the complexity of information, and the urgency often associated with anesthesia in emergency settings. The paper explores the implications of these challenges on obtaining valid consent and offers strategies to improve informed consent practices. These strategies include enhancing communication, providing pre-operative education, and ensuring thorough documentation. By adhering to these practices, anesthesiologists can better respect patient autonomy, mitigate risks, and uphold ethical standards in their practice. The goal is to ensure that patients are fully informed and able to participate meaningfully in decisions about their anesthesia care, even in complex and time-sensitive situations.

Abstract

Spinal anesthesia for cesarean section often causes maternal hypotension in 60-70% of cases, so an effective fluid management strategy is needed. This study aims to analyze the relationship between crystalloid fluid pre-loading and blood pressure changes in patients with spinal anesthesia for cesarean section. A quantitative, analytical, observational study with a cross-sectional design was conducted on 51 patients from a population of 58 at Dr. R. Goeteng Taroenadibrata Purbalingga Regional General Hospital (January-February 2026). Purposive sampling and the Slovin formula were selected. Data were collected through observation sheets and Drager NIBP monitors, and bivariate analysis was carried out using Fisher's Exact Test. The results showed that 76.5% received adequate pre-loading (10-20 ml/kgBW) with a hypotension incidence of 12.8% versus 66.7% in the inadequate group (p=0.001). The findings confirmed a significant relationship between adequate crystalloid pre-loading and blood pressure stability. Pre-loading of 10-20 ml/kgBW is recommended for elective cases to minimize the risk of hypotension.

Abstract

Hyperuricemia is a condition characterized by elevated uric acid levels in the blood, which can cause joint pain and impaired activity in the elderly. Adherence to a low-purine diet and family support are important factors in preventing hyperuricemia. This study aimed to analyze the effect of family support and adherence to a low-purine diet on hyperuricemia based on Lawrence Green's theory. This study used a quantitative correlational design with a cross-sectional approach. The study sample consisted of 48 elderly people aged ≥ 60 years at the Elderly Posyandu in Tlatah Village, Purwosari District, Bojonegoro Regency, selected using a simple random sampling technique. The independent variables were family support and adherence to a low-purine diet, while the dependent variable was hyperuricemia. Data collection used questionnaires and uric acid examination observation sheets. Data analysis used a logistic regression test with a significance level of α <0.05. The results showed that the family support variable had a significant p value of 0.020 (<0.05), which means there is an effect of family support on hyperuricemia in the elderly. The Exp(B) value of 0.134 indicates that elderly people with good family support have a lower chance of experiencing hyperuricemia. The low-purine diet adherence variable showed a significant p value of 0.002 (<0.05), which means there is an effect of low-purine diet adherence on hyperuricemia in the elderly. The Exp (B) value of 0.061 indicates that elderly people who adhere to a low-purine diet have a lower risk of hyperuricemia. The conclusion of this study is that family support and adherence to a low-purine diet influence the incidence of hyperuricemia in elderly people. Good family support and adherence to a low-purine diet can help reduce the risk of hyperuricemia in elderly people

Abstract

Hypertension is a non-communicable disease that continues to increase and is influenced by various lifestyle factors such as physical activity, diet, smoking, coffee consumption, and stress levels. This study aims to analyze the influence of lifestyle on the incidence of hypertension in the community of Campurejo Village, Bojonegoro Regency based on the theory of Lawrence W. Green. The study used a quantitative analytical design with a cross-sectional approach. The study population was 2,062 residents aged 30–50 years with a sample of 92 respondents selected using a random sampling technique. Data were collected through a lifestyle questionnaire, the DASS instrument for stress levels, and blood pressure measurements using a digital sphygmomanometer. Data analysis was performed using binary logistic regression. The results showed that diet (p=0.002), smoking habits (p=0.007), and stress levels (p=0.000) significantly influenced the incidence of hypertension, while physical activity (p=0.684) and coffee consumption (p=0.318) did not significantly influence. Unhealthy diet is the most dominant factor that increases the risk of hypertension. The conclusion of this study is that lifestyle has an important role in the incidence of hypertension, especially diet, smoking habits, and stress levels, so that promotive and preventive efforts based on public health behavior are needed.

Abstract

Pulmonary tuberculosis is an infectious disease caused by the bacterium Mycobacterium tuberculosis and requires long-term treatment for six months. Non-compliance with medication is one of the main causes of therapy failure and increases the risk of Multi Drug Resistance (MDR). Family support is an important factor in improving medication compliance in pulmonary TB patients. This study aims to analyze the relationship between family support and medication compliance in pulmonary TB patients in the Bojonegoro Community Health Center working area based on Lawrence W. Green's theory. This study used a quantitative correlational design with a cross-sectional approach. The study sample was 36 pulmonary TB patients who had undergone treatment for six months using a total sampling technique. The independent variable was family support and the dependent variable was medication compliance. Data collection was carried out using a family support questionnaire and a medical card observation sheet. Data analysis used the Chi-Square test with a significance level of α<0.05. The results showed that most respondents received good family support (32 respondents (88.9%) and the majority of respondents were compliant with taking medication (35 respondents (97.3%). The chi-square test yielded a p-value of 0.004 (<0.05), indicating a relationship between family support and medication adherence in pulmonary TB patients. The study concluded that family support significantly correlates with medication adherence in pulmonary TB patients. Emotional support was the most dominant form of support in improving medication adherence in pulmonary TB patients.
